8. Canagliflozin, serum magnesium and cardiovascular outcomes—Analysis from the CANVAS Program

Abstract

Background: Patients with type 2 diabetes (T2D) are predisposed to derangements in serum Magnesium (Mg), which may have implications for cardiometabolic events and outcomes. In clinical trials, participants with T2D randomized to sodium-glucose co-transporter 2 (SGLT2) inhibitors have shown mild to moderate increases in serum Mg from baseline levels. This post hoc analysis assesses the relation between serum Mg with cardiovascular outcomes in 10,140 participants of the Canagliflozin Cardiovascular Assessment Study (CANVAS) Program. Methods: We evaluated the association of baseline serum Mg with the primary composite end point of death from cardiovascular causes, non-fatal myocardial infarction, and non-fatal stroke, and tested whether this association is modified by baseline serum Mg. Using mediation analysis, we determined whether change in serum Mg post-randomization mediates the beneficial effect of canagliflozin on cardiovascular outcomes. Results: Mean serum Mg levels at baseline were 0.77 ± 0.09 mmol/L in both canagliflozin group and placebo groups. The canagliflozin group experienced an average increase in serum Mg by 0.07 mmol/L (95% CI, 0.065–0.072 mmol/L; p <.001) for the duration of the trial. We found no association between baseline serum Mg levels and the primary composite end point, and no evidence of effect modification by baseline Mg levels. Change in serum Mg post-randomization was not a mediator of the effects of canagliflozin on cardiovascular outcomes. Conclusions: In participants of the CANVAS Program, baseline and post-randomization serum Mg levels are not associated with cardiovascular outcomes.

Abstract

Introduction:An accurate interocclusal record is necessary for the delivery of fixed prosthodontic restorations. There are various materials and techniques used to obtain an interocclusal record in order to facilitate mounting of the dental casts on an articulator. The interocclusal record refers to the vertical and horizontal relationship of the maxillary and mandibular teeth. The clinician should have an understanding of various interocclusal recording materials, their modifications, shortcomings and awareness of its use in different clinical conditions. Materials and Methods:An electronic search was conducted using the PubMed Database from 1960 to 2020 using the keywords “bite registration materials” OR “occlusal registration materials” OR “interocclusal recording materials.” The articles pertaining to these keywords were further analysed. Results:The search identified 574 articles, out of which 563 articles did not fulfil the inclusion criteria and were excluded from the study. Hence 11 research articles were analysed for the review. Conclusion:Accurate interocclusal records are necessary to deliver high quality, predictable and precise fixed prosthodontic restorations. Establishing a tripod of opposing tooth contact is a fundamental principle to dictate accurate mounting of casts, and thus delivery of a well-fitting restoration. Furthermore, the review outlines the clinical guidelines and recommendations for accurate use of interocclusal recording materials in different clinical circumtances.

Large amplitude free vibrations of a clamped shallow spherical shell on a Pasternak foundation are studied using a new approach by Banerjee, Datta, and Sinharay. Numerical results are obtained for movable as well as immovable clamped edges. The effects of geometric, material, and foundation parameters on relation between nondimensional frequency and amplitude have been investigated and plotted.

Abstract

At high flux conditions, surface absorption based solar thermal systems underperform owing to high overheat temperatures of surface (temperature difference between the surface and the working fluid). One of the ways by which this drawback could be addressed is by allowing direct interaction of solar radiation with the working fluid. In this context, nanoparticles (NP) laden fluid (nanofluids) have been shown to be promising candidates as direct absorption volumetric solar energy absorbers owing to their enhanced thermo-physical properties and high solar weighted absorptivity. However, the nanofluids inherently have high emissivity in the mid- infrared region thus not satisfying the conditions of solar selectivity. In this study a layer of liquid (silicone oil) is placed above the nanofluid, which acts as a barrier against the infrared emissions from the nanofluid. Selection criterion for this liquid layer is high transparency in the visible solar spectrum and high absorption in the mid-infrared wavelength range. Thus the two conditions of solar selectively have been met by using two different liquid layers in direct thermal contact. On comparison of two results, it is seen that with silicone oil layer, the temperature rise is about 17% more as compare to without silicone oil layer.

Abstract

The conventional approach for solar collector design often requires performing large-scale experimentations or computationally intensive simulations which hinders the comprehensive screening and optimization of process design. This creates a strong rationale for developing a computationally efficient framework capable of leveraging a relatively small number of samples to generate a machine-learning model with sufficiently high fidelity. In this regard, the present study aims to integrate the concepts of random sampling, Gaussian process(GP), and Bayesian optimization for developing a computationally efficient data-driven framework for capturing the complete continuous domain of the parametric variation and predicting the desired performance measure. The proposed framework is rigorously tested at different stages with the help of unknown samples (out-of-fold test samples) to ensure the sound generalization capability of the constructed model. The model assessment revealed that the increase in sample size for training the GP model from 35 samples to 105 samples resulted in ≈ 56% reduction in root mean square error (RMSE), which further reduces to ≈ 96.5% after performing Bayesian optimization based hyperparameter tuning. The proposed framework will be extremely helpful in designing the highly efficient nanofluid-based solar thermal collector, by preventing the need of performing large-scale experimentations/simulations for screening purpose.

Abstract

Background: Although inhibitors of the renin–angiotensin–aldosterone system can slow the progression of diabetic kidney disease, the residual risk is high. Whether nuclear 1 factor (erythroid-derived 2)–related factor 2 activators further reduce this risk is unknown. Methods: We randomly assigned 2185 patients with type 2 diabetes mellitus and stage 4 chronic kidney disease (estimated glomerular filtration rate [GFR], 15 to < 30 ml per minute per 1.73 m2 of body-surface area) to bardoxolone methyl, at a daily dose of 20 mg, or placebo. The primary composite outcome was end-stage renal disease (ESRD) or death from cardiovascular causes. Results: The sponsor and the steering committee terminated the trial on the recommendation of the independent data and safety monitoring committee; the median follow-up was 9 months. A total of 69 of 1088 patients (6%) randomly assigned to bardoxolone methyl and 69 of 1097 (6%) randomly assigned to placebo had a primary composite outcome (hazard ratio in the bardoxolone methyl group vs. the placebo group, 0.98; 95% confidence interval [CI], 0.70 to 1.37; P=0.92). In the bardoxolone methyl group, ESRD developed in 43 patients, and 27 patients died from cardiovascular causes; in the placebo group, ESRD developed in 51 patients, and 19 patients died from cardiovascular causes. A total of 96 patients in the bardoxolone methyl group were hospitalized for heart failure or died from heart failure, as compared with 55 in the placebo group (hazard ratio, 1.83; 95% CI, 1.32 to 2.55; P < 0.001). Estimated GFR, blood pressure, and the urinary albumin-to-creatinine ratio increased significantly and body weight decreased significantly in the bardoxolone methyl group, as compared with the placebo group. Conclusions: Among patients with type 2 diabetes mellitus and stage 4 chronic kidney disease, bardoxolone methyl did not reduce the risk of ESRD or death from cardiovascular causes. Abstract

Bariatric surgery is a well-established approach to improve metabolic disease in morbidly obese patients with high cardiovascular risk. The post-operative normalization of lipid metabolism has a central role in the prevention of future cardiovascular events. The aim of the present study therefore was to characterize changes of plasma lipidomic patterns, consisting of 229 lipid species of 13 lipid classes, 3 months after Roux-en-Y gastric bypass (RYGB) in morbidly obese patients with and without diabetes. RYGB resulted in a 15-32% decrease of body mass index, which was associated with a significant reduction of total cholesterol (TC, -28.3%; P=0.02), LDL-cholesterol (LDL-C, -26.8%; P=0.03) and triglycerides (TGs, -63.0%; P=0.05) measured by routine clinical chemistry. HDL-cholesterol remained unchanged. The effect of RYGB on the plasma lipidomic profile was characterized by significant decreases of 87 lipid species from triacylglycerides (TAGs), cholesterol esters (CholEs), lysophosphatidylcholines (LPCs), phosphatidylcholines (PCs), phosphatidylethanolamine ethers (PEOs), phosphatidylinositols (PIs) and ceramides (Cers). The total of plasma lipid components exhibited a substantial decline of 32.6% and 66 lipid species showed a decrease by over 50%. A direct correlation with HbA 1C values could be demonstrated for 24 individual lipid species (10 TAG, three CholE, two LPC, one lysophosphatidylcholine ethers (LPCO) (LPC ether), one PC, two phosphatidylcholine ethers (PCO) and five Cer). Notably, two lipid species (TAG 58:5 and PEO 40:5) were inversely correlated with HbA 1C. LPCO, as single whole lipid class, was directly related to HbA 1C. These data indicate that RYGB-induced modulation of lipidomic profiles provides important information about post-operative metabolic adaptations and might substantially contribute to improvements of glycemic control.
