Child birth is a very significant event in a married woman’s life. She is filled with fear and anxiety mixed with a feeling of joy and expectation of the coming child. Apart from physical care psychological care and assurance are equally important. Present study deals with the women of poor workers of the factories who have to undergo deliveries either at home or ill equipped hospitals causing much suffering and even mortality. Detailed history of such 250 women was taken in a questionnaire proforma prepared for the study. A history of age, caste, education, para, occupation, husband’s income and knowledge about pre and post-natal care was recorded. Thus 21 tables were made of each case and analyzed in detail. It is a practice to marry girls at an early age as they feel it ensures their safety. Husband’s income was quite low and 50% of them were uneducated. 30.8% could only read and only 8.8% were graduate. Illiteracy was a major problem in safe delivery of the child. Old traditional customs were followed some which were not hygienic. Only 19% were employed. Rest were only house wives. Ground delivery was preferred in 74% cases, squatting position in 71.2% and 56% preferred sand in place of mattress. All these are not preferred methods currently. In the second stage of labor birth canal was lubricated in only 8.8% cases. In 76% cases massage of abdomen was done to hasten the delivery. The cord was cut mainly by knife after delivery. Placentas were mainly buried after delivery (74.8%). 56% of women remained lying in for about a week after delivery. 78% mothers took their first bath 3 to 6 days after delivery.

ImportanceOcular trauma terminology should be periodically updated to enable comprehensive capturing and monitoring of ocular trauma in clinical and research settings.ObjectiveTo update terminology for globe and adnexal trauma.Design, Setting, and ParticipantsA 2-round modified Delphi survey was conducted from January 1 to July 31, 2021, using an expert panel, including 69 ophthalmologists identified through their membership in ophthalmology (globe and adnexal trauma) societies. Consensus was defined as at least 67% expert agreement. A steering committee developed questions after identifying gaps in the current terminology via a targeted literature review. Round 1 sought consensus on existing and newly proposed terminology, and round 2 focused on unresolved questions from round 1. Experts included ophthalmologists who had managed, on average, 52 globe or adnexal trauma cases throughout their careers and/or published a total of 5 or more globe or adnexal trauma–related peer-reviewed articles.Main Outcomes and MeasuresExpert consensus on ocular and adnexal terms.ResultsA total of 69 experts participated in and completed round 1 of the survey. All 69 participants who completed round 1 were asked to complete round 2, and 58 responses were received. Consensus was reached for 18 of 25 questions (72%) in round 1 and 4 of 7 questions (57%) in round 2. Existing Birmingham Eye Trauma Terminology system terminology achieved consensus of 84% (58 of 69 experts) in round 1 and 97% (56 of 58 experts) in round 2. Experts agreed on the need for further refinement of the definition of zones of injury (55 of 69 [80%]), as the zone affected can have a substantial effect on visual and functional outcomes. There was consensus that the mechanism of injury (52 of 69 [75%]) and status of the lacrimal canaliculi (54 of 69 [78%]), nasolacrimal ducts (48 of 69 [69%]), lens (46 of 58 [80%]), retina (42 of 58 [73%]), and central and paracentral cornea (47 of 58 [81%]) be included in the revised terminology.Conclusions and RelevanceThere was consensus (defined as at least 67% expert agreement) on continued use of the existing Birmingham Eye Trauma Terminology system definitions and that additional terms are required to update the current ocular trauma terminology.

An article on ‘A model for quantification of ocular trauma’ [1] was published by the author recently. However, there was problem about the construction of ocular traumagram which was on the basis of measuring the magnitude of ocular trauma. Hence a detailed methodology for construction of ocular traumagram is presented for easier comprehension.

In ophthalmology orbit is the main cavity which contains the eyeball and many other structures related to ocular functions. Lacrimal fossa contains the lacrimal sac, nasolacrimal canal contains the nasolacrimal duct whereas the fossa for the lacrimal gland contains the lacrimal gland

Purpose: The current classification of ocular trauma does not incorporate adnexal trauma, injuries that are attributable to a nonmechanical cause and destructive globe injuries. This study proposes a new classification system of ocular trauma which is broader-based to allow for the classification of a wider range of ocular injuries not covered by the current classification. Methods: A clinic-based cross-sectional study to validate the proposed classification. We analyzed 535 cases of ocular injury from January 1, 2012 to February 28, 2012 over a 4-year period in an eye hospital in central India using our proposed classification system and compared it with conventional classification. Results: The new classification system allowed for classification of all 535 cases of ocular injury. The conventional classification was only able to classify 364 of the 535 trauma cases. Injuries involving the adnexa, nonmechanical injuries and destructive globe injuries could not be classified by the conventional classification, thus missing about 33% of cases. Conclusions: Our classification system shows an improvement over existing ocular trauma classification as it allows for the classification of all type of ocular injuries and will allow for better and specific prognostication. This system has the potential to aid communication between physicians and result in better patient care. It can also provide a more authentic, wide spectrum of ocular injuries in correlation with etiology. By including adnexal injuries and nonmechanical injuries, we have been able to classify all 535 cases of trauma. Otherwise, about 30% of cases would have been excluded from the study.

There will be only few clinical conditions in ophthalmology like glaucoma where nationally and internationally intensive and extensive research is going on toward the clinical conditions.

It is human nature not to deviate from accepted norms as we become accustomed to them. Medical science is a fast-growing science which is being practiced at many places and in many different ways. No doubt various societies are formed and many conferences are organized top compare results and to bring a uniformity in coining and defining medical terms.

In general sciences like physics and chemistry it is relatively easy to quantify a value. However in medical science it is not always easy and at times we have to make a compromise. An effort has been made to quantify ocular trauma (eye injuries) based on loss of structure and loss of function. The loss of structure is graded in percentage from a formula and the loss of function is also graded in percentage mainly from loss of vision. Structural loss and functional loss gives total loss. Both are plotted on a graph paper. Recording is done at weekly interval for moderate injuries. For severe injuries it can be done at monthly interval. A horizontal line of 20 cm is drawn at the bottom of graph paper. From the centre of this line a vertical line of 10 cm is drawn. Structural damage is noted on the left side of vertical line and the functional damage on the right side at weekly interval for 8 to 10 weeks. At the end of 10 weeks usually a somewhat triangular area is formed which is labeled as ocular traumagram. Each 1 sq.cm.box enclosed in traumagram indicates one Trauma Unit (T.U.) Thus a quantitative value of ocular trauma is obtained. Further details of construction are given in the section of methods. Different types ocular traumagrams are expected in different types of injury.
