In most cases, these patients are treated with anticoagulant drugs for at least 3 months. The American College of Physicians recommend treatment of these patients as patients with symptomatic pulmonary embolism. In this study, we evaluated and compared the efficacy and safety of fondaparinux vs warfarin in the prevention of unsuspected pulmonary embolism in patients with active cancer. Materials and methods: A prospective and parallel group study was performed on 64 cancer patients (29 males and 35 females) with APE. A multidetector CT angiography with high spatial and temporal resolution and quality of arterial opacification was used to make the diagnosis. Lung scintigraphy was reserved to selected patients only. Patients were randomized to either the warfarin (Group A) or the fondaparinux (Group B) for 90 days. The first end point of efficacy was the persistence, reduction, or disappearance of thrombosis after 90 days. The second end point was the reappearance of thrombosis after 1 year. The first end point of safety was the development of major bleeding. Results: We enrolled 32 patients into each treatment group. We reached the first end point of efficacy and safety in Group B which showed that fondaparinux was able to induce the disappearance of thrombotic pulmonary with a lower incidence of major bleeding events compared with warfarin. No difference in the secondary end point was recorded. Conclusion: We suggest that the treatment of cancer patients with APE can be oriented with the administration of a standard dose of fondaparinux until the next CT lung control (3 months). However, the lack of a randomized clinical trial, including a larger patient cohort, does not allow formulation of final recommendations in these patients. Contains fulltext : 288240.pdf (Publisher’s version ) (Open Access) BACKGROUND: Social attention affords learning opportunities across development and may contribute to individual differences in developmental trajectories, such as between male and female individuals, and in neurodevelopmental conditions, such as autism. METHODS: Using eye-tracking, we measured social attention in a large cohort of autistic (n = 123) and nonautistic females (n = 107), and autistic (n = 330) and nonautistic males (n = 204), aged 6-30 years. Using mixed Growth Curve Analysis, we modelled sex and diagnostic effects on the temporal dynamics of proportional looking time to three types of social stimuli (lean-static, naturalistic-static, and naturalistic-dynamic) and examined the link between individual differences and dimensional social and nonsocial autistic traits in autistic females and males. RESULTS: In the lean-static stimulus, average face-looking was higher in females than in males of both autistic and nonautistic groups. Differences in the dynamic pattern of face-looking were seen in autistic vs. nonautistic females, but not males, with face-looking peaking later in the trial in autistic females. In the naturalistic-dynamic stimulus, average face-looking was higher in females than in males of both groups; changes in the dynamic pattern of face looking were seen in autistic vs. nonautistic males, but not in females, with a steeper peak in nonautistic males. Lower average face-looking was associated with higher observer-measured autistic characteristics in autistic females, but not in males. CONCLUSIONS: Overall, we found stronger social attention in females to a similar degree in both autistic and nonautistic groups. Nonetheless, the dynamic profiles of social attention differed in different ways in autistic females and males compared to their nonautistic peers, and autistic traits predicted trends of average face-looking in autistic females. These findings support the role of social attention in the emergence of sex-related differences in autistic characteristics, suggesting an avenue to phenotypic stratification.

The QDF (Quality of life, Depression and cognitive Function) is a project elaborated and implemented by a Research Network of the nursing personnel of the cardiological services which promoted and run the GISSI-HF trial. It is a "companion study" which included up to 1/3 of all the GISSI-HF centres.To describe the distribution and evaluation of QDF variables measured with standardised instruments (Kansas City Cardiomiopathy Quality of Life questionnaire, KCCQ; Geriaric Depression scale GDS, Mini Mental State Examination MMSE), to verify whether and how far their knowledge could influence clinical decisions; to assess the prognostic impact of QDF values measured at entrance on mortality and morbidity. The study protocol included also a comprehensive evaluation of QDF variables independent from the questionnaires, by the nurses in charge of the study patients (the results of this part of the protocol are not included in this report).The QDF data were collected at baseline, 6 months, 1, 2 and 3 years through two self administered questionnaires (KCCQ, GDS), while the MMSE only for patients70 years. The questionnaires were administered to the patients who accepted to participate. A specific goal of the project focuses on the degree of independence between the results of the questionnaires and the judgement of the nurses. This contribution presents the operational implementation of the study protocol and general framework.

To document the degree of reliability of validated and mainly qualitative questionnaires most often used in research settings and/or projects, in a broad network of unselected cardiological centres; to assess whether the quality of data could be compatible with their use not only as descriptive but also in prognostic scores including mainly "hard" clinical variables.Eighty-three patients were included. Compliance with the self-administration of the Kansas City Cardiomiopathy Questionnaire (KCCQ) and the Geriatric Depression Scale (GDS) was very high throughout the study (KCCQ 97.7% baseline and 85.8% at three years). As expected more difficulties were encountered with the Mini Mental State Examination (administered to 80.6% patients baseline and to 54.3% at three years). Main reasons for not administering questionnaires was ascribed to organizational and patients problems.It is worth noticing that the study was conducted by nurses without economical incentives. The questionnaires were well completed with minimal missing data. If questionnaires are well presented to patients, a routine assessment of quality of life and depression could be feasible in everyday care.

Aim of this paper is to explore and quanti-quantitatively assess whether QoL as measured with the Kansas City Cardiomiopathy Questionnaire (KCCQ) summary score could be considered as an independent relevant component of clinical prognostic score of morbidity and mortality and identify patients at risk for death or admissions.Sixty-three per cent NYHA II and 39.4% NYHA III-IV patients experience a good Qol (score75). Risk factors for "not good" (75) QoL are age, NYHA class III-IV, diabetes, COPD and previous hospitalizations. NYHA II and III-IV patients with not good QoL experience an higher mortality than patients with a good QoL in the same classes. NYHA II patients with not good QoL experience the same 1 year readmission rates as NYHA III-IV patients with good QoL.Quality of life scores identify patients with different risk of mortality and readmissions within the same NYHA class. The prognostic value of KCCQ summary scores could identify candidates for disease management in whom better targeted care strategies may reduce hospitalizations and prevent deaths.

This contribution describes the patients included for the QDF (Quality of life, Depression and Cognitive functions) study and explores the distribution and time trends of scores of the three dimensions.The large majority of patients are males (78.3%), and mean age is 67.2 +/- 10.7 years (9.2%80 years). Patients in NYHA class III-IV experience more depression and a worse QoL as compared to the less severly clinically affected counterparts in NYHA class. Although QoL and depression are influenced by the same variables, not all patients with bad QoL are depressed and viceversa. Mean scores of cognitive functioning are not influenced by the same variables that influence QoL and depression.The regular measurement of QoL, depression and cognitive function is recommended by some authors. It seems to be confirmed that the three dimensions need to be explored to disentangle the specific informative value that each of them could add to clinical measures.

The prevalence of cognitive impairment (CI) is reported to be higher in heart failure patients than in the general population.To describe the prevalence of CI and to assess its prognostic value on the increased risk of hospitalizations and death over the medium term (3 years).MMSE was administered to 620/745 heart failure patients70 years. Up to 35% of the tested population were classified as mild CI and 12% as moderate. The severity of baseline clinical conditions, as expressed as NYHA classes, do not appear to be associated to different degrees of CI. The subgroup of patients with moderate CI includes higher proportion of people with the lowest educational level, of feminine gender, of higher age (80). Even in a population closely monitored as the one included in the GISSI-HLF trial, it is worth to be underlined that the presence of moderate CI appears to be strongly prognostic of an increased burden of care (hospitalizations), and of death over the whole observation period of the study (3 years).The presence of even mild or moderate CI is prognostic for increased mortality and readmissions for patients of the same NYHA class. In the absence of effective intervention strategies, research should concentrate on the yield, if any, of a systematic screening for CI in heart failure patients.

Depression is common in patients with heart failure but its causal and/or prognostic role with respect to the cardiac condition in still a matter of research.To describe the prevalence of depression and to assess its specific prognostic value in terms of increased risk of hospitalizations or death.Overall, 27.5% of 1495 patients experience symptoms of depression. Risk factors for depression are female sex, age70, higher NYHA class, previous hospitalizations and comorbidities. Depressed patients of the same NYHA class are at higher risk for 1 and 3 years mortality and all causes hospital admissions.Depression, assessed with a simple and easy to administer instrument, can add information to clinical variables in the identification of patients at increased risk of mortality and readmissions within the same NYHA class.

Gastric bicarbonate secretion has been evaluated by Feldman's method in 48 duodenal-ulcer patients. The relationship between smoking, clinical ulcer outcome (healing and recurrence) and bicarbonate secretion has been analysed. Heavy smokers secreted higher bicarbonate ions than did non-smokers. High-relapsing patients produced lower bicarbonate output. These preliminary data suggest that an impaired gastric bicarbonate secretion is associated with smoking, a well-known ulcer-associated factor; further-more, it may single out high-relapsing duodenal ulcer patients.

Numerous studies have shown that alcohol causes both acute and chronic damage to gastroduodenal mucosa. The methods of damage differ however, and experimental studies in animals have shown that the degranulation of mast cells in gastric mucosa causes acute hemorrhagic lesions after the consumption of alcohol. It is not known whether this mechanism also operates in man. The aim of the present study was therefore to evaluate whether there is a correlation between mast-cell activation, determined by assaying tryptase levels in gastric mucosa, and the consumption of alcohol in patients with ulcerative diseases. Thirty-one patients with cicatrized ulcerative lesions (13 gastric ulcers, 18 duodenal ulcers) were included in the study. Biopsies at the level of the gastric fundus and antrum and the duodenal bulb were performed in all patients to determine tryptase levels. Biopsy material was frozen and subsequently homogenized; the enzyme was assayed in the supernatant using a radioimmunometric method. The mean daily alcohol consumption was calculated in clinical terms for each patient over the past 5 years and patients were subdivided into non-drinkers and moderate (60 g alcohol/day) and excessive (60 g alcohol/day) drinkers. It was found that tryptase concentrations were higher in the fundus compared to the gastric antrum and duodenal bulb, irrespective of alcohol consumption both in patients with gastric ulcer and duodenal ulcer. The importance of mast cells in provoking alcohol-dependent damage was studied at a gastric level. Alcohol leads to their degranulation and therefore contributes to the formation of gastric lesions.(ABSTRACT TRUNCATED AT 250 WORDS)

Expression of the apoptosis-promoting Fas gene is frequently reduced or lost during the development of colorectal carcinoma. However, loss of heterozygosity at the Fas locus or Fas gene rearrangements do not account for the loss of expression of Fas, raising the possibility that methylation of the Fas promoter may inhibit gene expression in colorectal carcinomas. We have examined the Fas promoter region CpG island for evidence of hypermethylation in colorectal tumours. Forty-seven specimens of colorectal adenoma and carcinoma, as well as six samples of normal colonic mucosa, were examined by Southern blotting for methylation at HpaII and Cfol sites in this region. No methylation was detected in any of the specimens, suggesting that hypermethylation is not primarily responsible for the loss of expression of the Fas gene during colorectal tumorigenesis.

The pharmacokinetics of gentamicin in 34 febrile neutropenic patients (40 courses) were compared with those in 40 nonneutropenic patients receiving the drug. No pharmacokinetic differences were seen in half-life, volume of distribution (liter per kilogram; total and ideal body weight), or clearance (milliliter per minute per 1.73 m2). The incidences of nephrotoxicity in the two groups were not statistically different. Because of the small number of patients with positive cultures, no relationship between initial peak serum gentamicin concentration and mortality could be determined. Mortality risk factors that were determined to be statistically important included presence of pneumonia, persistent fever in the presence of anti-infective therapy of more than 1 week duration, and peak serum creatinine of greater than 1.2 mg/dl. Initial aminoglycoside dosing in the febrile neutropenic patient should be similar to that in the nonneutropenic patient, with concentrations in serum monitored and doses adjusted accordingly.

Carotid body tumor (CBT) is a rare neoplasm, although it represents about 65% of head and neck paragangliomas. Surgical excision is considered the appropriate therapy for CBTs. The aim of this study was to evaluate surgical outcomes on a large scale. We reviewed 19 studies between 2004 to 2014 with a total of 625 procedures. We observed a higher number of cases in women (62%). Only 3 (0,48%) deaths were reported as surgical complication. Total cranial nerve injuries were 302 (48,32%) of which 194 (31,04%) were transient and 108 (17,28%) were permanent. We found a total of 174 (27,84%) arterial injuries, most of which are external carotid artery (ECA) injuries. Cerebrovascular accident due to surgery were 15 (2,4%). We concluded that surgical resection remains the treatment of choice for these disease despite the related morbidity.

To study the effect of Esaprazole, a new antiulcer drug, on acid, peptic and alkaline secretion a modified gastric acid test was performed in 18 duodenal ulcer patients. Pentagastrin was administered as bolus 30' and 75' after the beginning of the test, followed by Esaprazole 300 mg i.v. at 90'. Gastric juice was collected every 15' for determination of: total volume, volume of non parietal secretion, acid, bicarbonate and pepsin output. Serum pepsinogen group I was determined by radioimmunoassay. Esaprazole had a significant inhibitory effect on the total volume of gastric secretion and on volume of non parietal secretion. Pepsin output and serum pepsinogen group I were not affected by Esaprazole, while bicarbonate secretion was reduced. Antiulcer activity of Esaprazole seems to be due to the reduction of total volume of gastric secretion.
