Electromyography (EMG) is widely used in human–machine interfaces (HMIs) to measure muscle contraction by computing the EMG envelope. However, EMG is largely affected by powerline interference and motion artifacts. Boards that directly provide EMG envelope, without denoising the raw signal, are often unreliable and hinder HMIs performance. Sophisticated filtering provides high performance but is not viable when power and computational resources must be optimized. This study investigates the application of feed-forward comb (FFC) filters to remove both powerline interferences and motion artifacts from raw EMG. FFC filter and EMG envelope extractor can be implemented without computing any multiplication. This approach is particularly suitable for very low-cost, low-power platforms. The performance of the FFC filter was first demonstrated offline by corrupting clean EMG signals with powerline noise and motion artifacts. The correlation coefficients of the filtered signals envelopes and the true envelopes were greater than 0.98 and 0.94 for EMG corrupted by powerline noise and motion artifacts, respectively. Further tests on real, highly noisy EMG signals confirmed these achievements. Background In many telemedicine applications, the correct use of medical device at the point of need is essential to provide an appropriate service. Some applications may require untrained people to interact with medical devices and patients: care delivery in transportation, military actions, home care and telemedicine training. Appropriate operation of medical device and correct connection with patient's body are crucial. In these scenarios, tailored applications of Augmented Reality can offer a valid support by guiding untrained people at the point of need. This study aims to explore the feasibility of using Augmented Reality in telemedicine applications, by facilitating an acceptable use of biomedical equipment by any unskilled person. In particular, a prototype system was built in order to estimate how untrained users, with limited or no knowledge, can effectively interact with an ECG device and properly placing ECG electrodes on patient's chest. Methods An Augmented Reality application was built to support untrained users in performing an ECG test. Simple markers attached to the ECG device and onto patient's thorax allow camera calibration. Once objects and their pose in the space are recognized, the video of the current scene is enriched, in real-time, with additional pointers, text boxes and audio that help the untrained operator to perform the appropriate sequence of operations. All the buttons, switches, ports of the ECG device together with the location of precordial leads were coded and indicated. Some user's voice commands were also included to improve usability. Results Ten untrained volunteers, supported by the augmented reality, were able to carry out a complete ECG test first on a mannequin and then on a real patient in a reasonable time (about 8 minutes on average). Average positioning errors of precordial electrodes resulted less than 3 mm for the mannequin and less than 7 mm for the real patient. These preliminary findings suggest the effectiveness of the developed application and the validity of clinical ECG recordings. Conclusion This application can be adapted to support the use of other medical equipment as well as other telemedicine tasks and it could be performed with a Tablet or a Smartphone. Abstract: This study aims to reproduce the effect of motor-unit synchronization on surface EMG recordings during vibratory stimulation to highlight vibration evoked muscle activity. The authors intended to evaluate, through numerical simulations, the changes in surface EMG spectrum in muscles undergoing whole body vibration stimulation. In some specific bands, in fact, vibration induced motion artifacts are also typically present. In addition, authors meant to compare the simulated EMGs with respect to real recordings in order to discriminate the effect of synchronization of motor units discharges with vibration frequencies from motion artifacts. Computations were performed using a model derived from previous studies and modified to consider the effect of vibratory stimulus, the motor unit synchronization and the endplates-electrodes relative position on the EMG signal. Results revealed that, in particular conditions, synchronization of MUs’ discharge generates visible peaks at stimulation frequency and its harmonics. However, only a part of the total power of surface EMGs might be enclosed within artifacts related bands (±1Hz centered at the stimulation frequency and its superior harmonics) even in case of strong synchronization of motor units discharges with the vibratory stimulus. Recently, in the context of telemedicine, telemonitoring services are gaining attention. They are offered, for example, to patientswith implantable cardioverter defibrillators (ICDs). A major problem associated with ICD therapy is the occurrence of inappropriate shocks which impair patients' quality of life and may also be arrhythmogenic. The telemonitoring can provide a valid support to intensify followup visits, in order to improve the prevention of inappropriate defibrillator shock, thus enhancing patient safety. Inappropriate shock generally depends on atrial fibrillation, supraventricular tachycardia, and abnormal sensing (such as those caused by electromagnetic interferences). As a practical example, an unusual case of an ICD patient who risked an inappropriate shockwhile taking a shower is reported. Continuous remote telemonitoring was able to timelywarn cardiologist via GSM-SMS,who were able to detect improper sensing examining the intracardiac electrogramviaWeb. Patient was promptly contacted and warned to not further come in contact with the hydraulic systemand any electrical appliance to prevent an inappropriate defibrillator shock. Background: Electrosurgery units are widely employed in modern surgery. Advances in technology have enhanced the safety of these devices, nevertheless, accidental burns are still regularly reported. This study focuses on possible causes of sacral burns as complication of the use of electrosurgery. Burns are caused by local densifications of the current, but the actual pathway of current within patient's body is unknown. Numerical electromagnetic analysis can help in understanding the issue.Methods: To this aim, an accurate heterogeneous model of human body (including seventy-seven different tissues), electrosurgery electrodes, operating table and mattress was build to resemble a typical surgery condition. The patient lays supine on the mattress with the active electrode placed onto the thorax and the return electrode on his back. Common operating frequencies of electrosurgery units were considered. Finite Difference Time Domain electromagnetic analysis was carried out to compute the spatial distribution of current density within the patient's body. A differential analysis by changing the electrical properties of the operating table from a conductor to an insulator was also performed.Results: Results revealed that distributed capacitive coupling between patient body and the conductive operating table offers an alternative path to the electrosurgery current. The patient's anatomy, the positioning and the different electromagnetic properties of tissues promote a densification of the current at the head and sacral region. In particular, high values of current density were located behind the sacral bone and beneath the skin. This did not occur in the case of non-conductive operating table.Conclusion: Results of the simulation highlight the role played from capacitive couplings between the return electrode and the conductive operating table. The concentration of current density may result in an undesired rise in temperature, originating burns in body region far from the electrodes. This outcome is concordant with the type of surgery-related sacral burns reported in literature. Such burns cannot be immediately detected after surgery, but appear later and can be confused with bedsores. In addition, the dosimetric analysis suggests that reducing the capacity coupling between the return electrode and the operating table can decrease or avoid this problem. [ABSTRACT FROM AUTHOR]

Accurate measurement of intervertebral kinematics of the cervical spine can support the diagnosis of widespread diseases related to neck pain, such as chronic whiplash dysfunction, arthritis, and segmental degeneration.The natural inaccessibility of the spine, its complex anatomy, and the small range of motion only permit concise measurement in vivo. Low dose X-ray fluoroscopy allows time-continuous screening of cervical spine during patient's spontaneous motion. To obtain accurate motion measurements, each vertebra was tracked by means of image processing along a sequence of radiographic images. To obtain a time-continuous representation of motion and to reduce noise in the experimental data, smoothing spline interpolation was used. Estimation of intervertebral motion for cervical segments was obtained by processing patient's fluoroscopic sequence; intervertebral angle and displacement and the instantaneous centre of rotation were computed. Abstract: In vivo analysis of intervertebral kinematics provides useful information about spinal disorders and performance of disk prostheses. Diagnosis of intervertebral instability is based on measurement of abnormal range of segmental motion in sagittal plane through functional flexion–extension radiography; however, this concise measure does not take into account the progression of segmental motion in between flexion and extension extremes. Fluoroscopy can support analysis of intervertebral kinematics during patient''s motion with an acceptable X-ray dose. A spline-based method designed for a continuous-time description of intervertebral motion extracted by videofluoroscopy is proposed. Fluoroscopic sagittal sequences of lumbar spine were processed by an automated method based on template matching to track vertebrae. A smoothing spline interpolation of the estimated intervertebral kinematic data was performed and a continuous-time description of segmental rotation and translation was obtained; the smoothing parameter was chosen both to preserve motion and to reduce noise. Concise measurements were extracted by the continuous-time kinematics and compared with standard clinical measurements of intervertebral sagittal rotation and translation. The trajectory of instantaneous center of rotation, never presented before for in vivo spinal segments, was provided and compared with standard measurements of the finite center of rotation. Results showed a good agreement with standard clinical measurements: on average, absolute differences resulted 0.74degree for sagittal rotation, 0.59mm for translation and 1.02mm for the x- and y-position of center of rotation. The proposed method offers an effective technique for the continuous-time description of intervertebral motion, maintaining standard clinical measurements for diagnosis of lumbar instability. This study extends previous research concerning in vivo intervertebral motion by means of single-plane fluoroscopy in an attempt to overcome 2D analysis limitations. Knowledge of out-of-plane vertebra rotations will extend the results provided by planar kinematic studies, which is particularly important for lateral bending investigation where axial rotation accompanies side bending, but is also valuable in sagittal analysis (e.g. indicating an absence of coupled axial rotation). Combining a fluoroscopic projection of a vertebra with volumetric information provided by CT data, vertebra 3D position can be estimated. Out-of-plane vertebral rotations are estimated by comparing Digitally Reconstructed Radiographs (DRRs) in different orientations with a reference fluoroscopic projection, maximising the image cross-correlation index. DRRs have been computed from CT-data using a ray-casting algorithm. In this work a feasibility study of the method was performed by means of a computer simulation. To this end the CT volume (vertebra L4, segmented) provided by the Visible Human Project was utilised and reference fluoroscopic projections were simulated in different orientations adding various levels of noise. Accuracy and precision of the proposed method was determined. Error analysis reveals that an accuracy of less than 1 degree can be achieved in computation of out-of-plane vertebral angles. During surgical procedures, real-time estimation of the current position of a metal lead within the patient's body is obtained by radiographic imaging. The inherent opacity of metal objects allows their visualization using X-ray fluoroscopic devices. Although fluoroscopy uses reduced radiation intensities, the overall X-ray dose delivered during prolonged exposure times poses risks to the safety of patients and physicians. This study proposes a potential alternative to real-time visualization of a lead inside the human body. In principle, by making a weak current flow through the lead and measuring the related magnetic field generated outside the body, it is possible to trace the position of the lead. This hypothesis was verified experimentally via two tests: one carried out on a curved copper wire in air and one carried out on a real pacemaker lead in a saline solution. In the second test, a pacemaker lead and a large return electrode were placed in a tank filled with a saline solution that reproduced the mean resistivity of the human torso. In both tests, a current flowed through the lead, which consisted of square pulses with short duration, to avoid any neuro-muscular stimulation effects in a real scenario. A small coil with a ferrite core was moved along a grid of points over a plastic sheet and placed just above the lead to sample the spatial amplitude distribution of the magnetic induction field produced by the lead. For each measurement point, the main coil axis was oriented along the x and y axes of the plane to estimate the related components of the magnetic induction field. The two matrices of measurements along the x and y axes were further processed to obtain an estimate of lead positioning. The preliminary results of this study support the scientific hypothesis since the positions of the leads were accurately estimated. The aim of this study is to characterise the transient mechanical response and the neuromuscular activation of lower limb muscles in subjects undergoing Whole Body Vibration (WBV) at different frequencies while holding two static postures, with focus on muscles involved in shaping postural responses. Twenty-five participants underwent WBV at 15, 20, 25 and 30 Hz while in hack squat or on fore feet. Surface electromyography and soft tissue accelerations were collected from Gastrocnemius Lateralis (GL), Soleus (SOL) and Tibialis Anterior (TA) muscles. Estimated displacement at muscle bellies revealed a pattern never highlighted before that differed across frequencies and postures (p < 0.001). After stimulation starts, muscle oscillation peaks, drops and further stabilises, suggesting the occurrence of a neuromuscular activation to reduce the vibration-induced oscillation. The oscillation attenuation at the SOL muscle correlated with its increased activation (rho = 0.29, p < 0.001). Furthermore, only specific WBV settings led to a significant increase in muscle contraction: WBV-induced activation of SOL and GL was maximal in fore-feet (p < 0.05) and in response to higher frequencies (30 Hz vs 15 Hz, p < 0.001). The analysis of the mechanical dynamics of lower leg muscles highlights a resonant response to WBVs, that for the SOL correlates to the increased muscle activation. Despite differing across frequencies and postures, this resonant behaviour seems to discourage the use of dynamic exercises on vibrating platforms. As for the most efficient WBV combination, calf muscle response to WBVs is maximised if those muscles are already pre-contracted and the stimulation frequencies are in the 25–30 Hz range. The Open-electroencephalography (EEG) framework is a popular platform to enable EEG measurements and general purposes Brain Computer Interface experimentations. However, the current platform is limited by the number of available channels and electrode compatibility. In this paper we present a fully configurable platform with up to 32 EEG channels and compatibility with virtually any kind of passive electrodes including textile, rubber and contactless electrodes. Together with the full hardware details, results and performance on a single volunteer participant (limited to alpha wave elicitation), we present the brain computer interface (BCI)2000 EEG source driver together with source code as well as the compiled (.exe). In addition, all the necessary device firmware, gerbers and bill of materials for the full reproducibility of the presented hardware is included. Furthermore, the end user can vary the dry-electrode reference circuitry, circuit bandwidth as well as sample rate to adapt the device to other generalized biopotential measurements. Although, not implemented in the tested prototype, the Biomedical Analogue to Digital Converter BIOADC naturally supports SPI communication for an additional 32 channels including the gain controller. Objective: The Wilson Central Terminal (WCT) is an artificially constructed reference for surface electrocardiography, which is assumed to be near zero and steady during the cardiac cycle; namely it is the simple average of the three recorded limbs (right arm, left arm and left leg) composing the Einthoven triangle and considered to be electrically equidistant from the electrical center of the heart. This assumption has been challenged and disproved in 1954 with an experiment designed just to measure and minimize WCT. Minimization was attempted varying in real time the weight resistors connected to the limbs. Unfortunately, the experiment required a very cumbersome setup and showed that WCT amplitude could not be universally minimized, in other words, the weight resistors change for each person. Taking advantage of modern computation techniques as well as of a special ECG device that aside of the standard 12-lead Electrocardiogram (ECG) can measure WCT components, we propose a software minimization (genetic algorithm) method using data recorded from 72 volunteers. Result: We show that while the WCT presents average amplitude relative to lead II of 58.85% (standard deviation of 30.84%), our minimization method yields an amplitude as small as 7.45% of lead II (standard deviation of 9.04%). Since its inception, electrocardiography has been based on the simplifying hypothesis that cardinal limb leads form an equilateral triangle of which, at the center/centroid, the electrical equivalent of the cardiac activity rotates during the cardiac cycle. Therefore, it is thought that the three limbs (right arm, left arm, and left leg) which enclose the heart into a circuit, where each branch directly implies current circulation through the heart, can be averaged together to form a stationary reference (central terminal) for precordials/chest-leads. Our hypothesis is that cardinal limbs do not form a triangle for the majority of the duration of the cardiac cycle. As a corollary, the central point may not lie in the plane identified by the limb leads. Using a simple and efficient algorithm, we demonstrate that the portion of the cardiac cycle where the three limb leads form a triangle is, on average less, than 50%. [ABSTRACT FROM AUTHOR]

Cardio-mechanical monitoring techniques, such as Seismocardiography (SCG) and Gyrocardiography (GCG), have received an ever-growing interest in recent years as potential alternatives to Electrocardiography (ECG) for heart rate monitoring. Wearable SCG and GCG devices based on lightweight accelerometers and gyroscopes are particularly appealing for continuous, long-term monitoring of heart rate and its variability (HRV). Heartbeat detection in cardio-mechanical signals is usually performed with the support of a concurrent ECG lead, which, however, limits their applicability in standalone cardio-mechanical monitoring applications. The complex and variable morphology of SCG and GCG signals makes the ECG-free heartbeat detection task quite challenging; therefore, only a few methods have been proposed. Very recently, a template matching method based on normalized cross-correlation (NCC) has been demonstrated to provide very accurate detection of heartbeats and estimation of inter-beat intervals in SCG and GCG signals of pathological subjects. In this study, the accuracy of HRV indices obtained with this template matching method is evaluated by comparison with ECG. Tests were performed on two public datasets of SCG and GCG signals from healthy and pathological subjects. Linear regression, correlation, and Bland-Altman analyses were carried out to evaluate the agreement of 24 HRV indices obtained from SCG and GCG signals with those obtained from ECG signals, simultaneously acquired from the same subjects. The results of this study show that the NCC-based template matching method allowed estimating HRV indices from SCG and GCG signals of healthy subjects with acceptable accuracy. On healthy subjects, the relative errors on time-domain indices ranged from 0.25% to 15%, on frequency-domain indices ranged from 10% to 20%, and on non-linear indices were within 8%. The estimates obtained on signals from pathological subjects were affected by larger errors. Overall, GCG provided slightly better performances as compared to SCG, both on healthy and pathological subjects. These findings provide, for the first time, clear evidence that monitoring HRV via SCG and GCG sensors without concurrent ECG is feasible with the NCC-based template matching method for heartbeat detection. In developing countries, due to the high cost involved, amputees have limited access to prosthetic limbs. This constitutes a barrier for this people to live a normal life. To break this barrier, we are developing ultra-low-cost closed-loop myoactivated prostheses that are easy to maintain manufacture and that do not require electrodes in contact with the skin to work effectively. In this paper, we present the implementation for a simple but functional hand prosthesis. Our simple design consists of a low-cost embedded microcontroller (Arduino), a wearable stretch sensor (adapted from electroresistive bands normally used for “insulation of gaskets” against EM fields), to detect residual muscle contraction as direct muscle volumetric shifts and a handful of common, not critical electronic components. The physical prosthesis is a 3D printed claw-style two-fingered hand (PLA plastic) directly geared to an inexpensive servomotor. To make our design easier to maintain, the gears and mechanical parts can be crafted from recovered materials. To implement a closed loop, the amount of closure of prosthesis is fed back to the user via a second stretch sensor directly connected to claw under the form of haptic feedback. Our concept design comprised of all the parts has an overall cost below AUD 30 and can be easily scaled up to more complicated devices suitable for other uses, i.e., multiple individual fingers and wrist rotation. A heartbeat generates tiny mechanical vibrations, mainly due to the opening and closing of heart valves. These vibrations can be recorded by accelerometers and gyroscopes applied on a subject's chest. In particular, the local 3D linear accelerations and 3D angular velocities of the chest wall are referred to as seismocardiograms (SCG) and gyrocardiograms (GCG), respectively. These signals usually exhibit a low signal-to-noise ratio, as well as non-negligible amplitude and morphological changes due to changes in posture and the sensors' location, respiratory activity, as well as other sources of intra-subject and inter-subject variability. These factors make heartbeat detection a complex task; therefore, a reference electrocardiogram (ECG) lead is usually acquired in SCG and GCG studies to ensure correct localization of heartbeats. Recently, a template matching technique based on cross correlation has proven to be particularly effective in recognizing individual heartbeats in SCG signals. This study aims to verify the performance of this technique when applied on GCG signals. Tests were conducted on a public database consisting of SCG, GCG, and ECG signals recorded synchronously on 100 patients with valvular heart diseases. The results show that the template matching technique identified heartbeats in GCG signals with a sensitivity and positive predictive value (PPV) of 87% and 92%, respectively. Regression, correlation, and Bland–Altman analyses carried out on inter-beat intervals obtained from GCG and ECG (assumed as reference) reported a slope of 0.995, an intercept of 4.06 ms (R2 > 0.99), a Pearson's correlation coefficient of 0.9993, and limits of agreement of about ±13 ms with a negligible bias. A comparison with the results of a previous study obtained on SCG signals from the same database revealed that GCG enabled effective cardiac monitoring in significantly more patients than SCG (95 vs. 77). This result suggests that GCG could ensure more robust and reliable cardiac monitoring in patients with heart diseases with respect to SCG. [ABSTRACT FROM AUTHOR]

Cardiac monitoring can be performed by means of an accelerometer attached to a subject's chest, which produces the Seismocardiography (SCG) signal. Detection of SCG heartbeats is commonly carried out by taking advantage of a simultaneous electrocardiogram (ECG). SCG-based long-term monitoring would certainly be less obtrusive and easier to implement without an ECG. Few studies have addressed this issue using a variety of complex approaches. This study proposes a novel approach to ECG-free heartbeat detection in SCG signals via template matching, based on normalized cross-correlation as heartbeats similarity measure. The algorithm was tested on the SCG signals acquired from 77 patients with valvular heart diseases, available from a public database. The performance of the proposed approach was assessed in terms of sensitivity and positive predictive value (PPV) of the heartbeat detection and accuracy of inter-beat intervals measurement. Sensitivity and PPV of 96% and 97%, respectively, were obtained by considering templates that included both systolic and diastolic complexes. Regression, correlation, and Bland–Altman analyses carried out on inter-beat intervals reported slope and intercept of 0.997 and 2.8 ms (R2 > 0.999), as well as non-significant bias and limits of agreement of ±7.8 ms. The results are comparable or superior to those achieved by far more complex algorithms, also based on artificial intelligence. Pulse waves (PWs) are mechanical waves that propagate from the ventricles through the whole vascular system as brisk enlargements of the blood vessels' lumens, caused by sudden increases in local blood pressure. Photoplethysmography (PPG) is one of the most widespread techniques employed for PW sensing due to its ability to measure blood oxygen saturation. Other sensors and techniques have been proposed to record PWs, and include applanation tonometers, piezoelectric sensors, force sensors of different kinds, and accelerometers. The performances of these sensors have been analyzed individually, and their results have been found not to be in good agreement (e.g., in terms of PW morphology and the physiological parameters extracted). Such a comparison has led to a deeper comprehension of their strengths and weaknesses, and ultimately, to the consideration that a multimodal approach accomplished via sensor fusion would lead to a more robust, reliable, and potentially more informative methodology for PW monitoring. However, apart from various multichannel and multi-site systems proposed in the literature, no true multimodal sensors for PW recording have been proposed yet that acquire PW signals simultaneously from the same measurement site. In this study, a true multimodal PW sensor is presented, which was obtained by integrating a piezoelectric forcecardiography (FCG) sensor and a PPG sensor, thus enabling simultaneous mechanical–optical measurements of PWs from the same site on the body. The novel sensor performance was assessed by measuring the finger PWs of five healthy subjects at rest. The preliminary results of this study showed, for the first time, that a delay exists between the PWs recorded simultaneously by the PPG and FCG sensors. Despite such a delay, the pulse waveforms acquired by the PPG and FCG sensors, along with their first and second derivatives, had very high normalized cross-correlation indices in excess of 0.98. Six well-established morphological parameters of the PWs were compared via linear regression, correlation, and Bland–Altman analyses, which showed that some of these parameters were not in good agreement for all subjects. The preliminary results of this proof-of-concept study must be confirmed in a much larger cohort of subjects. Further investigation is also necessary to shed light on the physical origin of the observed delay between optical and mechanical PW signals. This research paves the way for the development of true multimodal, wearable, integrated sensors and for potential sensor fusion approaches to improve the performance of PW monitoring at various body sites. Conducting polymers are materials displaying high electrical conductivity, easy of fabrication, flexibility and biocompatibility, for this, they are routinely employed in organic electronics, printed electronics, and bioelectronics. Organic electrochemical transistors (OECTs) are a second generation of organic thin transistors, in which the insulator layer is an electrolyte medium and the conductive polymer is electrochemically active. OECT devices have been demonstrated in chemical and biological sensing: while accurate in determining the size of individual ions in solution, similar devices break down if challenged with complex mixtures. Here, we combine a conductive PEODOT:PSS polymer with a super-hydrophobic scheme to obtain a family of advanced devices, in which the ability to manipulate a biological solution couples to a precise texture of the substrate (which incorporates five micro-electrodes in a line, and each is a site specific measurement point), and this permits to realize time and space resolved analysis of a solution. The objective of this work was to study the use of electrical impedance spectroscopy (EIS) measurement to characterize muscle electrical properties in different conditions. In vivo EIS measurements were carried out in the range 1–60 kHz on 32 forearm flexor muscles of healthy volunteers. Each subject underwent to a protocol consisting of three trials: rest condition, isometric contraction, 4 min relaxation. Measured data were fitted with Cole–Cole equivalent electrical circuit using the complex nonlinear least square method. Finally, the relative variations from the steady state of the electrical parameters of the equivalent electrical circuit were estimated. Results obtained are in agreement with the hypothesis that, in the frequency range considered, the current preferentially flows through the extracellular fluid, being the component of intracellular resistance negligible. Among circuital parameters, in fact, the relative variations from steady state of R 0 changed highly significant ( p < 0.001) both during contraction and after 4 min relaxation. Seismocardiography (SCG) is largely regarded as the state-of-the-art technique for continuous, long-term monitoring of cardiac mechanical activity in wearable applications. SCG signals are acquired via small, lightweight accelerometers fixed on the chest. They provide timings of important cardiac events, such as heart valves openings and closures, thus allowing the estimation of cardiac time intervals of clinical relevance. Forcecardiography (FCG) is a novel technique that records the cardiac-induced vibrations of the chest wall by means of specific force sensors, which proved capable of monitoring respiration, heart sounds and infrasonic cardiac vibrations, simultaneously from a single contact point on the chest. A specific infrasonic component captures the heart walls displacements and looks very similar to the Apexcardiogram. This low-frequency component is not visible in SCG recordings, nor it can be extracted by simple filtering. In this study, a feasible way to extract this information from SCG signals is presented. The proposed approach is based on double integration of SCG. Numerical double integration is usually very prone to large errors, therefore a specific numerical procedure was devised. This procedure yields a new displacement signal (DSCG) that features a low-frequency component (LF-DSCG) very similar to that of the FCG (LF-FCG). Experimental tests were carried out using an FCG sensor and an off-the-shelf accelerometer firmly attached to each other and placed onto the precordial region. Simultaneous recordings were acquired from both sensors, together with an electrocardiogram lead (used as a reference). Quantitative morphological comparison confirmed the high similarity between LF-FCG and LF-DSCG (normalized cross-correlation index >0.9). Statistical analyses suggested that LF-DSCG, although achieving a fair sensitivity in heartbeat detection (about 90%), has not a very high consistency within the cardiac cycle, leading to inaccuracies in inter-beat intervals estimation. Future experiments with high-performance accelerometers and improved processing methods are envisioned to investigate the potential enhancement of the accuracy and reliability of the proposed method. Forcecardiography (FCG) is a novel technique that measures the local forces induced on the chest wall by the mechanical activity of the heart. Specific piezoresistive or piezoelectric force sensors are placed on subjects' thorax to measure these very small forces. The FCG signal can be divided into three components: low-frequency FCG, high-frequency FCG (HF-FCG) and heart sound FCG. HF-FCG has been shown to share a high similarity with the Seismocardiogram (SCG), which is commonly acquired via small accelerometers and is mainly used to locate specific fiducial markers corresponding to essential events of the cardiac cycle (e.g., heart valves opening and closure, peaks of blood flow). However, HF-FCG has not yet been demonstrated to provide the timings of these markers with reasonable accuracy. This study addresses the detection of the aortic valve opening (AO) marker in FCG signals. To this aim, simultaneous recordings from FCG and SCG sensors were acquired, together with Electrocardiogram (ECG) recordings, from a few healthy subjects at rest, both during quiet breathing and apnea. The AO markers were located in both SCG and FCG signals to obtain pre-ejection periods (PEP) estimates, which were compared via statistical analyses. The PEPs estimated from FCG and SCG showed a strong linear relationship (r > 0.95) with a practically unit slope, and 95% of their differences were found to be distributed within ± 4.6 ms around small biases of approximately 1 ms, corresponding to percentage differences lower than 5% of the mean measured PEP. Background: Perfusion CT is a technology which allows functional evaluation of tissue vascularity. Due to this potential, it is finding increasing utility in oncology. Although since its introduction continuous advances have interested CT technique, some issues have to be still defined, concerning both clinical and technical aspects. In this study, we dealt with the comparison of two widely employed mathematical models (dual input one compartment model – DOCM - and maximum slope – SM -) analyzing their robustness to the noise. Methods: We carried out a computer simulation process to quantify effect of noise on the evaluation of an important perfusion parameter (Arterial Blood Flow – BFa) in liver tumours. A total of 4500 liver TAC, corresponding to 3 fixed BFa values, were simulated using different arterial and portal TAC (computed from 5 real CT images) at 10 values of signal to noise ratio (SNR). BFa values were calculated by applying four different algorithms, specifically developed, to these noisy simulated curves. Three algorithms were developed to implement SM (one semiautomatic, one automatic and one automatic with filtering) and the last for the DOCM method. Results: In all the simulations, DOCM provided the best results, i.e., those with the lowest percentage error compared to the reference value of BFa. Concerning SM, the results are variable. Results obtained with the automatic algorithm with filtering are close to the reference value, but only if SNR is higher than 50. Vice versa, results obtained by means of the semiautomatic algorithm gave, in all simulations, the lowest results with the lowest standard deviation of the percentage error. Conclusions: Since the use of DOCM is limited by the necessity that portal vein is visible in CT scans, significant restriction for patients’ follow-up, we concluded that SM can be reliably employed. Abstract: Objective: We present a new, low power EEG recording system with an ultra-high input impedance that enables the use of long-lasting, passive dry electrodes. It incorporates Bluetooth wireless connectivity and is designed to be suitable for long-term monitoring during daily activities. Methods: The new EEG system is compared to a standard and clinically available reference EEG system using wet electrodes in three separate sets of experiments. In the first two experiments, each dry electrode was surrounded by four standard wet electrodes and the alpha and mu-rhythms were recorded. In the third experiment, serial monopolar (referred to the left ear) recordings of flash visual evoked potential were performed using the new EEG system and a reference system. Results: These experiments showed that the signal recorded using the new EEG system is almost identical to that recorded with standard clinical EEG equipment; our measurements showed that the correlation coefficient between the dry electrode recordings and the average of the four standard electrodes surrounding each dry electrode is greater than 0.85. Conclusion: We conclude that the new EEG system performs similarly to reference EEG systems, while providing the advantages of portability, ease of application and minimal scalp preparation. Significance: The proposed system using passive dry electrodes suitable for single use while performing as good as standard EEG equipment provides ease of application and minimal scalp preparation. Abstract: The aim of this study is to highlight the relationship between muscle motion, generated by whole body vibration, and the correspondent electromyographic (EMG) activity and to suggest a new method to customize the stimulation frequency. Simultaneous recordings of EMG and tri-axial accelerations of quadriceps rectus femoris from fifteen subjects undergoing vibration treatments were collected. Vibrations were delivered via a sinusoidal oscillating platform at different frequencies (10–45Hz). Muscle motion was estimated by processing the accelerometer data. Large EMG motion artifacts were removed using sharp notch filters centred at the vibration frequency and its superior harmonics. EMG–RMS values were computed and analyzed before and after artifact suppression to assess muscular activity. Muscles acceleration amplitude increased with frequency. Muscle displacements revealed a mechanical resonant-like behaviour of the muscle. Resonance frequencies and dumping factors depended on subject. Moreover, RMS of artifact-free EMG was found well correlated (R 2 =0.82) to the actual muscle displacement, while the maximum of the EMG response was found related to the mechanical resonance frequency of muscle. Results showed that maximum muscular activity was found in correspondence to the mechanical resonance of the muscle itself. Assuming the hypothesis that muscle activation is proportional to muscle displacement, treatment optimization (i.e. to choose the best stimulation frequency) could be obtained by simply monitoring local acceleration (resonance), leading to a more effective muscle stimulation. Abstract: Electromyography readings (EMGs) from quadriceps of fifteen subjects were recorded during whole body vibration treatment at different frequencies (10–50Hz). Additional electrodes were placed on the patella to monitor the occurrence of motion artifact, triaxial accelerometers were placed onto quadriceps to monitor motion. Signal spectra revealed sharp peaks corresponding to vibration frequency and its harmonics, in accordance with the accelerometer data. EMG total power was compared to that associated with vibration harmonics narrow bands, before and during vibration. On average, vibration associated power resulted in only 3% (±0.9%) of the total power prior to vibration and 29% (±13.4%) during vibration. Often, studies employ surface EMG to quantitatively evaluate vibration evoked muscular activity and to set stimulation frequency. However, previous research has not accounted for motion artifacts. The data presented in this study emphasize the need for the removal of motion artifacts, as they consistently affect RMS estimation, which is often used as a concise muscle activity index during vibrations. Such artifacts, rather unpredictable in amplitude, might be the cause of large inter-study differences and must be eliminated before analysis. Hand prostheses partially restore hand appearance and functionalities. In particular, 3D printers have provided great opportunities by simplifying the manufacturing process and reducing costs. The "Federica" hand is 3D-printed and equipped with a single servomotor, which synergically actuates its five fingers by inextensible tendons; no springs are used for hand opening. A differential mechanical system simultaneously distributes the motor force on each finger in predefined portions. The proportional control of hand closure/opening is achieved by monitoring muscle contraction by means of a thin force sensor, as an alternative to EMG. The electrical current of the servomotor is monitored to provide sensory feedback of the grip force, through a small vibration motor. A simple Arduino board was adopted as the processing unit. A closed-chain, differential mechanism guarantees efficient transfer of mechanical energy and a secure grasp of any object, regardless of its shape and deformability. The force sensor offers some advantages over the EMG: it does not require any electrical contact or signal processing to monitor muscle contraction intensity. The activation speed (about half a second) is high enough to allow the user to grab objects on the fly. The cost of the device is less then 100 USD. The "Federica" hand has proved to be a lightweight, low-cost and extremely efficient prosthesis. It is now available as an open-source project (CAD files and software can be downloaded from a public repository), thus allowing everyone to use the "Federica" hand and customize or improve it. Background: Low-dose X-ray images have become increasingly popular in the last decades, due to the need to guarantee the lowest reasonable patient's exposure. Dose reduction causes a substantial increase of quantum noise, which needs to be suitably suppressed. In particular, real-time denoising is required to support common interventional fluoroscopy procedures. The knowledge of noise statistics provides precious information that helps to improve denoising performances, thus making noise estimation a crucial task for effective denoising strategies. Noise statistics depend on different factors, but are mainly influenced by the X-ray tube settings, which may vary even within the same procedure. This complicates real-time denoising, because noise estimation should be repeated after any changes in tube settings, which would be hardly feasible in practice. This work investigates the feasibility of an a priori characterization of noise for a single fluoroscopic device, which would obviate the need for inferring noise statics prior to each new images acquisition. The noise estimation algorithm used in this study was tested in silico to assess its accuracy and reliability. Then, real sequences were acquired by imaging two different X-ray phantoms via a commercial fluoroscopic device at various X-ray tube settings. Finally, noise estimation was performed to assess the matching of noise statistics inferred from two different sequences, acquired independently in the same operating conditions.Results: The noise estimation algorithm proved capable of retrieving noise statistics, regardless of the particular imaged scene, also achieving good results even by using only 10 frames (mean percentage error lower than 2%). The tests performed on the real fluoroscopic sequences confirmed that the estimated noise statistics are independent of the particular informational content of the scene from which they have been inferred, as they turned out to be consistent in sequences of the two different phantoms acquired independently with the same X-ray tube settings.Conclusions: The encouraging results suggest that an a priori characterization of noise for a single fluoroscopic device is feasible and could improve the actual implementation of real-time denoising strategies that take advantage of noise statistics to improve the trade-off between noise reduction and details preservation. With this paper we communicated the existence of a surface electrocardiography (ECG) recordings dataset, named WCTECGdb, that aside from the standard 12-lead signals includes the raw electrode biopotential for each of the nine exploring electrodes refereed directly to the right leg. This dataset, comprises of 540 ten second segments recorded from 92 patients at Campbelltown Hospital, NSW Australia, and is now available for download from the Physionet platform. The data included in the dataset confirm that the Wilson's Central Terminal (WCT) has a relatively large amplitude (up to 247% of lead II) with standard ECG characteristics such as a p-wave and a t-wave, and is highly variable during the cardiac cycle. As further examples of application for our data, we assess: (1) the presence of a conductive pathway between the legs and the heart concluding that in some cases is electrically significant and (2) the initial assumption about the limbs potential stating the dominance of the left arm concluding that this is not always the case and that might requires case to case assessment. Upper limb amputation is a condition that significantly restricts the amputees from performing their daily activities. The myoelectric prosthesis, using signals from residual stump muscles, is aimed at restoring the function of such lost limbs seamlessly. Unfortunately, the acquisition and use of such myosignals are cumbersome and complicated. Furthermore, once acquired, it usually requires heavy computational power to turn it into a user control signal. Its transition to a practical prosthesis solution is still being challenged by various factors particularly those related to the fact that each amputee has different mobility, muscle contraction forces, limb positional variations and electrode placements. Thus, a solution that can adapt or otherwise tailor itself to each individual is required for maximum utility across amputees. Modified machine learning schemes for pattern recognition have the potential to significantly reduce the factors (movement of users and contraction of the muscle) affecting the traditional electromyography (EMG)-pattern recognition methods. Although recent developments of intelligent pattern recognition techniques could discriminate multiple degrees of freedom with high-level accuracy, their efficiency level was less accessible and revealed in real-world (amputee) applications. This review paper examined the suitability of upper limb prosthesis (ULP) inventions in the healthcare sector from their technical control perspective. More focus was given to the review of real-world applications and the use of pattern recognition control on amputees. We first reviewed the overall structure of pattern recognition schemes for myo-control prosthetic systems and then discussed their real-time use on amputee upper limbs. Continuous monitoring of pacemaker activity can provide valuable information to improve patients’ follow-up. Concise information is stored in some types of pacemakers, whereas ECG can provide more detailed information, but requires electrodes and cannot be used for continuous monitoring. This study highlights the possibility of a continuous monitoring of pacemaker pulses by sensing magnetic field variations due to the current pulses. This can be achieved by means of a sensor coil positioned near the patient’s thorax without any need for physical contact. A simplified model of coil response to pacemaker pulses is presented in this paper, along with circuits suitable for pulse detection. In vitro tests were carried out using real pacemakers immersed in saline solution; experimental data were used to assess the accuracy of the model and to evaluate the sensor performance. It was found that the coil signal amplitude decreases with increasing distance from the pacemaker lead wire. The sensor was able to easily perform pacemaker spike detection up to a distance of 12 cm from the pacemaker leads. The stimulation rate can be measured in real time with high accuracy. Since any electromagnetic pulse triggers the same coil response, EMI may corrupt sensor measurements and thus should be discriminated. [ABSTRACT FROM AUTHOR]

We developed a scanner for micro computed tomography dedicated to the breast (BµCT) with a high resolution flat-panel detector and a microfocus X-ray tube. We evaluated the system spatial resolution via the 3D modulation transfer function (MTF). In addition to conventional absorption-based X-ray imaging, such a prototype showed capabilities for propagation-based phase-contrast and related edge enhancement effects in 3D imaging. The system limiting spatial resolution is 6.2 mm −1 (MTF at 10%) in the vertical direction and 3.8 mm −1 in the radial direction, values which compare favorably with the spatial resolution reached by mini focus breast CT scanners of other groups. The BµCT scanner was able to detect both microcalcification clusters and masses in an anthropomorphic breast phantom at a dose comparable to that of two-view mammography. Monitoring of foetal heart rate and its variability (FHRV) covers an important role in assessing health of foetus. Many analysis methods have been used to get quantitative measures of FHRV. FHRV has been studied in time and in frequency domain and interesting clinical results have been obtained. Nevertheless, a standardized definition of FHRV and a precise methodology to be used for its evaluation are lacking. We carried out a literature overview about both frequency domain analysis (FDA) and time domain analysis (TDA). Then, by using simulated FHR signals, we defined the methodology for FDA. Further, employing more than 400 real FHR signals, we analysed some of the most common indexes, Short Term Variability for TDA and power content of the spectrum bands and sympathovagal balance for FDA, and evaluated their ranges of values, which in many cases are a novelty. Finally, we verified the relationship between these indexes and two important parameters: week of gestation, indicator of foetal growth, and foetal state, classified as active or at rest. Our results indicate that, according to literature, it is necessary to standardize the procedure for FHRV evaluation and to consider week of gestation and foetal state before FHR analysis. Rationale, aims and objectives In 2012, health care spending in Italy reached €114.5 billion, accounting for 7.2% of the Gross Domestic Product (GDP) and 14.2% of total public spending. Therefore, reducing waste in health facilities could generate substantial cost savings. The objective of this study is to show that Lean Six Sigma represents an appropriate methodology for the development of a clinical pathway which allows to improve quality and to reduce costs in prosthetic hip replacement surgery. Methods The methodology used for the development of a new clinical pathway was Lean Six Sigma. Problem solving in Lean Six Sigma is the DMAIC ( Define, Measure, Analyse, Improve, Control) roadmap, characterized by five operational phases which make possible to reach fixed goals through a rigorous process of defining, measuring, analysing, improving and controlling business problems. Results The following project indicated several variables influencing the inappropriate prolongation of the length of stay for inpatient treatment and corrective actions were performed to improve the effectiveness and efficiency of the process of care. The average length of stay was reduced from 18.9 to 10.6 days (−44%). This assessment, usually performed at Emergency departments, is a highly dynamic process and there are international grading systems that according to the patient condition initiate the patient journey. Triage requires an initial rapid assessment followed by routine checks of the patients' vitals, including respiratory rate, temperature, and pulse rate. Ideally, these checks should be performed continuously and remotely to reduce the workload on triage nurses; optimizing tools and monitoring systems can be introduced and include a wearable patient monitoring system that is not at the expense of the patient's comfort and can be remotely monitored through wireless connectivity. In this study, we assessed the suitability of a small ceramic piezoelectric disk submerged in a skin-safe silicone dome that enhances contact with skin, to detect wirelessly both respiration and cardiac events at several positions on the human body. For the purposes of this evaluation, we fitted the sensor with a respiratory belt as well as a single lead ECG, all acquired simultaneously. To complete Triage parameter collection, we also included a medical-grade contact thermometer. Performances of cardiac and respiratory events detection were assessed. The instantaneous heart and respiratory rates provided by the proposed sensor, the ECG and the respiratory belt were compared via statistical analyses. In all considered sensor positions, very high performances were achieved for the detection of both cardiac and respiratory events, except for the wrist, which provided lower performances for respiratory rates. These promising yet preliminary results suggest the proposed wireless sensor could be used as a wearable, hands-free monitoring device for triage assessment within emergency departments. We present a bio-potential front-end capable of recording unipolar ECG leads without making use of the Wilson central terminal (WCT). The information contained in the new unipolar recordings may yield unique diagnostic information as it avoids the need to essentially subtract data or make use of the averaging effect imposed by the WCT. The system also allows a direct, real-time software calculation of signals corresponding to standard ECG leads for standard diagnosis. These calculated standard ECG leads have a correlation in excess of 92% with a gold standard ECG recorded in parallel. The circuit is wideband, compatible with both the standard and the dry electrodes, and of low power (requiring less than 20 mW powered at 12 V). Total hip arthroplasty ( THA) can be achieved by using a cemented or noncemented prosthesis. Besides patient's age, weight, and other clinical signs, the evaluation of the quality of the bones is a crucial parameter on which orthopedic surgeons base the choice between cemented and noncemented THA. Although bone density generally decreases with age and a cemented THA is preferred for older subjects, the bone quality of a particular patient should be quantitatively evaluated. This study proposes a new method to quantitatively measure bone density and fracture risk by using 3 D models extracted by a preoperative computed tomography ( CT) scan of the patient. Also, the anatomical structure and compactness of the quadriceps muscle is computed to provide a more complete view. A spatial reconstruction of the tissues is obtained by means of CT image processing, then a detailed 3 D model of bone mineral density of the femur is provided by including quantitative CT density information ( CT must be precalibrated). A finite element analysis will provide a map of the strains around the proximal femur socket when solicited by typical stresses caused by an implant. The risk for structural failure due to press-fitting and compressive stress during noncemented THA surgery was estimated by calculating a bone fracture risk index (ratio between actual compressive stress and estimated failure stress of the bone). A clinical trial was carried out including 36 volunteer patients (ages 22-77) who underwent unilateral THA surgery for the first time: 18 received a cemented implant and 18 received a noncemented implant. CT scans were acquired before surgery, immediately after, and after 12 months. Bone and quadriceps density results were higher in the healthy leg in about 80% of the cases. Bone and quadriceps density generally decrease with age but mineral density may vary significantly between patients. Preliminary results indicate the highest fracture risk at the calcar and the lowest at the intertrocanteric line, with some difference between patients. An analysis of the results suggest that this methodology can be a valid noninvasive decision support tool for THA planning; however, further analyses are needed to tune the technique and to allow clinical applications. Background: Pregnancy testing in cattle is commonly invasive requiring manual rectal palpation of the reproductive tract that presents risks to the operator and pregnancy. Alternative non-invasive tests have been developed but have not gained popularity due to poor specificity, sensitivity and the inconvenience of sample handling. Our aim is to present the pilot study and proof of concept of a new non invasive technique to sense the presence and age (limited to the closest trimester of pregnancy) of the foetus by recording the electrical and audio signals produced by the foetus heartbeat using an array of specialized sensors embedded in a stand alone handheld prototype device. The device was applied to the right flank (approximately at the intercept of a horizontal line drawn through the right mid femur region of the cow and a vertical line drawn anywhere between lumbar vertebrae 3 to 5) of more than 2000 cattle from 13 different farms, including pregnant and not pregnant, a diversity of breeds, and both dairy and beef herds. Pregnancy status response is given "on the spot" from an optimized machine learning algorithm running on the device within seconds after data collection. Results: Using combined electrical and audio foetal signals we detected pregnancy with a sensitivity of 87.6% and a specificity of 74.6% for all recorded data. Those values increase to 91% and 81% respectively by removing files with excessive noise (19%). Foetus ageing was achieved by comparing the detected foetus heart-rate with published tables. However, given the challenging farm environment of a restless cow, correct foetus ageing was achieved for only 21% of the correctly diagnosed pregnant cows. Conclusions: In conclusion we have found that combining ECG and PCG measurements on the right flank of cattle provides a reliable and rapid method of pregnancy testing. The device has potential to be applied by unskilled operators. This will generate more efficient and productive management of farms. There is potential for the device to be applied to large endangered quadrupeds in captive breeding programs where early, safe and reliable pregnancy diagnosis can be imperative but currently difficult to achieve. Abstract: Congenital nystagmus is an ocular–motor disorder characterised by involuntary, conjugated and bilateral to and fro ocular oscillations. In this study a method to recognise automatically jerk waveform inside a congenital nystagmus recording and to compute foveation time and foveation position variability is presented. The recordings were performed with subjects looking at visual targets, presented in nine eye gaze positions; data were segmented into blocks corresponding to each gaze position. The nystagmus cycles were identified searching for local minima and maxima (SpEp sequence) in intervals centred on each slope change of the eye position signal (position criterion). The SpEp sequence was then refined using an adaptive threshold applied to the eye velocity signal; the outcome is a robust detection of each slow phase start point, fundamental to accurately compute some nystagmus parameters. A total of 1206 slow phases was used to compute the specificity in waveform recognition applying only the position criterion or adding the adaptive threshold; results showed an increase in negative predictive value of 25.1% using both features. The duration of each foveation window was measured on raw data or using an interpolating function of the congenital nystagmus slow phases; foveation time estimation less sensitive to noise was obtained in the second case. As a definition, Human–Machine Interface (HMI) enables a person to interact with a device. Starting from elementary equipment, the recent development of novel techniques and unobtrusive devices for biosignals monitoring paved the way for a new class of HMIs, which take such biosignals as inputs to control various applications. The current survey aims to review the large literature of the last two decades regarding biosignal-based HMIs for assistance and rehabilitation to outline state-of-the-art and identify emerging technologies and potential future research trends. PubMed and other databases were surveyed by using specific keywords. The found studies were further screened in three levels (title, abstract, full-text), and eventually, 144 journal papers and 37 conference papers were included. Four macrocategories were considered to classify the different biosignals used for HMI control: biopotential, muscle mechanical motion, body motion, and their combinations (hybrid systems). The HMIs were also classified according to their target application by considering six categories: prosthetic control, robotic control, virtual reality control, gesture recognition, communication, and smart environment control. An ever-growing number of publications has been observed over the last years. Most of the studies (about 67%) pertain to the assistive field, while 20% relate to rehabilitation and 13% to assistance and rehabilitation. A moderate increase can be observed in studies focusing on robotic control, prosthetic control, and gesture recognition in the last decade. In contrast, studies on the other targets experienced only a small increase. Biopotentials are no longer the leading control signals, and the use of muscle mechanical motion signals has experienced a considerable rise, especially in prosthetic control. Hybrid technologies are promising, as they could lead to higher performances. Abstract: Cardiotocography (CTG) is the most widely used diagnostic technique in clinical practice to monitor fetal health. Cardiotocographic recording also permits to assess maturation of the fetal autonomous nervous system (ANS): fetal heart rate (FHR) modifications may reveal ANS’ reactions to stimuli. To assess fetal reactivity, physicians evaluate specific clinical CTG parameters, generally, by means of visual inspection, thus depending on observer''s expertise, with lack of reproducibility. Still nowadays, there is a very high intra- and inter-observer variation in the assessment of FHR patterns. More objective methods for CTG interpretation are of crucial importance. For adults, frequency analysis of heart rate variability (HRV) is a non-invasive and powerful method to investigate ANS activity. This frequency analysis can also be a valid support for a better knowledge of fetal ANS functional state and reactions. Indeed, fetal HRV is a good indicator of fetal well-being in non-stress conditions. Fetal reactivity is a very important CTG characteristic used to diagnose fetal distress, but its interpretation is still uncertain. In the last few decades, a number of wearable systems for respiration monitoring that help to significantly reduce patients' discomfort and improve the reliability of measurements have been presented. A recent research trend in biosignal acquisition is focusing on the development of monolithic sensors for monitoring multiple vital signs, which could improve the simultaneous recording of different physiological data. This study presents a performance analysis of respiration monitoring performed via forcecardiography (FCG) sensors, as compared to ECG-derived respiration (EDR) and electroresistive respiration band (ERB), which was assumed as the reference. FCG is a novel technique that records the cardiac-induced vibrations of the chest wall via specific force sensors, which provide seismocardiogram-like information, along with a novel component that seems to be related to the ventricular volume variations. Simultaneous acquisitions were obtained from seven healthy subjects at rest, during both quiet breathing and forced respiration at higher and lower rates. The raw FCG sensor signals featured a large, low-frequency, respiratory component (R-FCG), in addition to the common FCG signal. Statistical analyses of R-FCG, EDR and ERB signals showed that FCG sensors ensure a more sensitive and precise detection of respiratory acts than EDR (sensitivity: 100% vs. 95.8%, positive predictive value: 98.9% vs. 92.5%), as well as a superior accuracy and precision in interbreath interval measurement (linear regression slopes and intercepts: 0.99, 0.026 s (R2 = 0.98) vs. 0.98, 0.11 s (R2 = 0.88), Bland–Altman limits of agreement: ±0.61 s vs. ±1.5 s). This study represents a first proof of concept for the simultaneous recording of respiration signals and forcecardiograms with a single, local, small, unobtrusive, cheap sensor. This would extend the scope of FCG to monitoring multiple vital signs, as well as to the analysis of cardiorespiratory interactions, also paving the way for the continuous, long-term monitoring of patients with heart and pulmonary diseases. There are two surgical approaches to performing total hip arthroplasty (THA): a cemented or uncemented type of prosthesis. The choice is usually based on the experience of the orthopaedic surgeon and on parameters such as the age and gender of the patient. Using machine learning (ML) techniques on quantitative biomechanical and bone quality data extracted from computed tomography, electromyography and gait analysis, the aim of this paper was, firstly, to help clinicians use patient-specific biomarkers from diagnostic exams in the prosthetic decision-making process. The second aim was to evaluate patient long-term outcomes by predicting the bone mineral density (BMD) of the proximal and distal parts of the femur using advanced image processing analysis techniques and ML. The ML analyses were performed on diagnostic patient data extracted from a national database of 51 THA patients using the Knime analytics platform. The classification analysis achieved 93% accuracy in choosing the type of prosthesis; the regression analysis on the BMD data showed a coefficient of determination of about 0.6. The start and stop of the electromyographic signals were identified as the best predictors. This study shows a patient-specific approach could be helpful in the decision-making process and provide clinicians with information regarding the follow up of patients. [ABSTRACT FROM AUTHOR]
