9. Iron Deficiency in Preschool Children with Autistic Spectrum Disorders

Abstract

Iron deficiency (ID) causes negative outcomes on psychomotor and behavioral development of infants and young children. Children with autistic spectrum disorders (ASD) are under risk for ID and this condition may increase the severity of psychomotor and behavioral problems, some of which already inherently exist in these children. In the present study, the frequency of ID and the association between ID and autistic symptoms, developmental level, and behavioral problems in preschool children attending a clinic for ASD (N = 31) were evaluated. No association was observed between ID and the severity of autistic symptoms, developmental level and behavioral problems. ID was detected in 32.3% (N = 10) of the children based on serum ferritin level. In this study, the negative impact of low serum ferritin in ASD has not been confirmed. On the other hand, the rate of ID was considerably high in this sample of children with ASD compared to normative data of preschool children. Further studies with larger samples are needed to clarify the relationship between ID and clinical variables associated with ASD. (Contains 2 tables.)

WOS: 000325969200008, PubMed: 28360549, Introduction: This study examined the prevalence of the use of different complementary and alternative medicine (CAM) strategies, families' attitudes and belief systems about the use of these strategies, and the economic burden of these strategies placed on family income in families of children with autism spectrum disorders (ASD). Method: A questionnaire survey concerning the use of CAM in children with ASD was administered to parents in the five different geographic locations in Turkey. Results: Of the 172 respondents, 56% had used at least one CAM therapy. The most frequently used CAM intervention was spiritual healing. Among the most reported reasons for seeking CAM were dissatisfaction with conventional interventions and a search for ways to enhance the effectiveness of conventional treatments. The most frequently reported source of recommendation for CAM was advice from family members. The mean economic burden of the CAM methods was a total of 4,005 Turkish lira ($2,670) in the sample using CAM. The CAM usage rate was lower in parents who suspected genetic/congenital factors for the development of ASD. Conclusion: This study observed the importance of socioeconomic and cultural factors as well as parents' beliefs about the etiology of ASD in treatment decisions about CAM.

Abstract

Objective: Attention deficit hyperactivity disorder (ADHD) has recently been reported to be related to elevated rates of acne vulgaris in two different epidemiological studies. However, the association between ADHD and acne vulgaris has not been investigated in a clinical setting. ADHD is a childhood-onset neurodevelopmental disorder; therefore, it is not expected to develop secondarily to the psychosocial burden of having acne vulgaris. It has been considered that androgen exposure in early life may be related to the development of both ADHD and acne vulgaris. Therefore, elucidating the putative association between ADHD and acne vulgaris may help in identifying common etiological factors between these conditions, such as elevated androgen exposure in early life. The aim of this study was to assess the symptoms of childhood and current ADHD symptoms in a clinical sample of women with acne vulgaris. Methods: The study sample consisted of 91 women with acne vulgaris referred to the Dermatology Clinic of Selcuk University Faculty of Medicine and 53 control subjects. Childhood and current ADHD symptoms of the participants were assessed using the Wender Utah Rating Scale (WURS) and the Adult ADHD Self-Report Scale (ASRS), respectively. Acne severity was measured by the International Consensus Conference on Acne Classification System, which classifies three grades (mild, moderate, and severe acne). While the normal distribution of variables was acceptable, Student's t-test and, in other cases, Mann-Whitney U-test were used to analyze the differences between patient and control groups. Pearson or Spearman correlation coefficients were calculated to examine the relationship between the ADHD scores and severity of acne. Results: The mean age did not differ significantly between the patient (22.2±4.3 years) and control (22.4±4.4 years) groups. The education level was also similar in the patients with acne vulgaris (13.3±3.0 years) and controls (13.7±2.9 years) (range: 5-17 years). There were no significant differences between patients and controls on any of the ADHD scales. Furthermore, except for childhood irritability, ADHD symptoms did not show a relationship with acne severity. Conclusion: Contrary to the results of past epidemiological studies, this study did not show a relationship between ADHD and acne vulgaris in the clinical sample. 30. Management of the antidepressants usage risks in children and adolescents.

Abstract

Antidepressants are nowadays used not only in the treatment of depressive symptoms, but in diverse child psychiatric disorders, such as obsessive compulsive disorder, social phobia, generalized anxiety disorder, posttraumatic stress disorder and eating disorder. Although these drugs are frequently used "off-label" in children and adolescents, increased amount of studies has supported the positive effects of selective serotonin reuptake inhibitors (SSRIs) on depression and anxiety disorders in this age group. Therefore, the use of antidepressants in young patients has increased markedly. SSRIs are the most preferred antidepressants in children and adolescents. Because, they are generally well-tolerated and adverse effects usually subside after approximately a week of treatment. However, clinicians should take into account some rare side effects of these drugs including suicidality, increasing bleeding, manic symptoms, serotonin syndrome, discontinuation syndrome and behavioral activation. Drug interactions should also always be considered in patients taking SSRIs, because some of them are moderate to potent inhibitors of various CYP enzymes. Additionally, there is lack of long-term safety data about the effects of antidepressant exposure on physical growth and the developing brain. In conclusion, clinician who prescribe antidepressants should keep in mind their potential risks. Abstract

Objective: This study aimed to determine the prevalence and comorbidities of attention-deficit hyperactivity disorder (ADHD) by evaluating a large-scale nation-wide sample of children., Method: The inclusion criterion was being enrolled as a 2nd, 3rd, or 4th-grade student. A semi-structured diagnostic interview (K-SADS-PL), DSM-IV-Based Screening Scale for Disruptive Behavior Disorders, and assessment of impairment (by both parents and teachers) were applied to 5,842 participants., Results: The prevalence of ADHD was 19.5% without impairment and 12.4% with impairment. Both ADHD with and without impairment groups had similar psychiatric comorbidity rates except for oppositional defiant disorder (ODD) and conduct disorder (CD) diagnoses. Impairment in the ADHD group resulted in significantly higher ODD and CD diagnoses., Conclusion: Even when impairment is not described, other psychiatric disorders accompany the diagnosis of ADHD and may cause impairment in the future. Impairment in the diagnosis of ADHD significantly increases the likelihood of ODD and CD.
