Objectives: Bone mineral density abnormalities can cause falls due to imbalance and movement limitation. Morbidities that result from a fall mean a loss of functionality and quality of life. The aim of this study was to evaluate the relationship between bone mineral density and gait, balance, and fear of a fall in epileptic patients. Methods: A total of 104 patients were separated into 3 groups according to T-score. A metabolic serum profile analysis was performed, and the Tinetti Gait and Balance Scale (TGBS), the Falls Efficacy Scale (FES), the Short Cognitive Examination (SCE), the Beck Depression Scale, and the Neurocom Balance Master tests were administered. Results: In the osteoporosis group, the duration of medication use and the FES scores were high, the TGBS score and level of vitamin D were low, eyes-closed balance was more affected, and the degree of sway and turning to the left was greater, and tandem-walk speed was reduced. Positive correlations were found between age of onset and the TGBS and SCC scores, eyes-closed balance and duration of medication use, and FES score and eyes-open balance. Negative correlations were determined between the number of drugs used and the FES, TGBS, and CCM scores, as well as tandem-walk speed. Conclusion: Simple clinical tests can be administered in outpatient clinics to address the risk of falling due to gait and balance abnormalities in osteoporotic epileptics. Brucellosis is a multisystemic disease that may present with a broad spectrum of clinical manifestations and complications. Neurological symptoms are common in brucellosis. Neurobrucellosis associated with central nervous system involvement is a rare clinical form of disease, (1.7-7% of prevalence). A 50-year-old man presented with confusion, bilateral vestibulocochlear nerve paralysis, symptoms of cerebellar dysfunction, atrophy and paralysis of multiple muscles. Involvement of both central and peripheral nervous system as well as the production of Brucella melitensis in the cerebro-spinal fluid culture was thought to be eligible for presentation. In laboratory tests; Rose-Bengal test in blood serum was positive; brucella tube agglutination test was 1/160 positive and Brucella melitensis was reproduced in cerebro-spinal fluid culture. He was treated with rifampicin (600 mg/day), doxycycline (200 mg/day) and ceftriaxone (2000 mg/ day) combination. After one month, the patient was hospitalized for a second time as symptoms and findings of prior clinical picture were again detected. Rose-Bengal in blood serum, Brucella tube agglutination (1/160 titration) and Brucella tube agglutination with coombs (1/160 titration) tests were positive. In conclusion; in a case of subacute progressive neurobrucellosis, successful results may be attained by application of a proper antibacterial combination in a sufficient dose and for a sufficient time period. Primary central nervous system lymphoma is an uncommon intracranial tumor which belongs to the group of B cell non-Hodgkin lymphomas. Infrequently contrast enhancing transient lesions may precede the diagnosis. They are named as "sentinel lesions" after Alderson's paper. Due to the transient nature of these lesions, lymphomas may be confused with demyelinating or infectious central nervous system disorders. In those cases an accurate diagnosis is only possible with a brain biopsy. Here we present a patient followed for more than two years without a clear distinction of lymphoma or a relapsing remitting demyelinating disease. Ultimately after development of an isolated mass lesion, a brain biopsy was performed and primary central nervous system lymphoma was histopathologically diagnosed. [ABSTRACT FROM AUTHOR]

Orbital myositis (OM) is an inflammatory disorder of the extraocular muscles. The signs and symptoms of OM are periorbital pain, eyelid swelling and redness, restricted ocular motility, and strabismus. There are at least two major forms, described by Benedikt GH Schoser, a limited oligosymptomatic ocular myositis (LOOM), which is associated with conjunctival injection only, and severe exophthalmic ocular myositis (SEOM), which presents with additional ptosis, chemosis, and proptosis. We report the clinical and radiological features of five patients with OM who were recently followed in our clinic. Three patients, one man and two women, were placed in the LOOM group, and the other two patients, both women, were in the SEOM group. In both groups, the initial complaints were pain worsening with eye movements and double vision, with only one patient in the SEOM group having pain worsening secondary to Crohn's disease. The most affected muscles were the medial and lateral recti. All the patients were treated with corticosteroids, resulting in rapid improvement. Only one patient in the SEOM group experienced a relapse. Orbital magnetic resonance imaging of all the patients revealed enlargement and contrast enhancement of the involved muscles.
