8. How Diverse Is Your Reading List? Exploring Issues of Representation and Decolonisation in the UK

Abstract

There is a growing impetus, from university students and administrations, to decolonise the curriculum and develop diverse reading lists. Yet, there is limited theoretical or empirical analysis of the authorship of current reading lists to justify this imperative. The present study developed and applied a method for auditing the authorship on reading lists of two modules, one from science and one from social science, in a research-intensive British university. The paper explores whether these reading lists can be considered descriptively representative of the student body or scholarly community. The analysis of reading lists found empirical basis for concerns that university curricula are dominated by white, male and Eurocentric authors, with some exceptions. The reading lists did not represent the diverse local student body but came closer to representing the demographic profile of academic staff. To interpret these findings, the paper argues that reading lists have a role to play in decolonising our universities, and offer opportunities to critique and deconstruct disciplinary boundaries. Further research is required to explore student and staff views of reading lists and the meaning of diversity, to evaluate existing tools, and address barriers to decolonising our curricula on a global scale.

33. Continuing Education Survey.

Abstract

In 1978, a national survey was conducted in New Zealand to determine the extent of participation in continuing education and the level of unmet need for these activities. A questionnaire was developed dealing with respondent characteristics, spare time and interests, agency-directed learning activities (ADLAS), and unmet needs, and administered to a random sample of 1500 persons past the age of 15 who were not attending a formal education program. Both rural and urban dwellers were represented. It was found that 30 percent of the sample were participating in ADLAS conducted by a wide variety of educational institutions, community organizations, clubs, sports organizations, and special interest groups. Participants were more likely to be younger people; more likely to be in paid employment and in professional or technical occupations; more likely to own or have the use of private transport; and more likely to have lived in their location for less than five years. Sports, educational and vocational ADLAS were most popular among those aged 30-49 years. Programs were mostly close to home, and were more often attended by women with dependent children than by women without children. Women who worked full time often participated in educational and vocational ADLAS, as did their male counterparts. It was also found that a high proportion of the population has unmet needs for adult learning activities, and that these needs are proportionately greater in craft and art areas than in educational and vocational areas. Finally, it was found, that people were very satisfied with their adult learning experiences. Policy implications are evident from the need to continue to provide ADLAS to meet the needs of the people of New Zealand. (KC)

41. Acute sensory and motor response to 45-s heavy isometric holds for the plantar flexors in patients with Achilles tendinopathy.

Abstract

Purpose: The purpose of this study was to explore the immediate effects of heavy isometric plantar flexor exercise on sensory output (pain during a functional task and mechanical pain sensitivity) and motor output (plantar flexor torque) in individuals with Achilles tendinopathy.Methods: Sixteen subjects with Achilles tendinopathy participated in the study, mean (SD) age 48.6 (8.9) years and Victorian institute assessment-Achilles (VISA-A) score 61.3 (23.0). Sensory testing assessing pain during a functional task, mechanical pain sensitivity and motor output, and plantar flexor peak torque was completed prior to the intervention. All subjects completed a 45-s heavy isometric plantar flexor contraction and were then re-tested using the same sensory and motor tests. Motor output was assessed using isokinetic dynamometry at speeds previously identified as of interest in subjects with Achilles tendinopathy.Results: Only 9 of the 16 subjects experienced pain during a functional task, self-reported pain was 4.2 (1.9) numerical rating scale (NRS) pre-intervention and 4.9 (3.2) NRS postintervention (n.s.). Mechanical pressure sensitivity was 446.5 (± 248.5) g/mm2 pre-intervention and 411.8 (± 211.8) g/mm2 post-intervention (n.s.). Mean concentric plantar flexor torque at 90 and 225°/s was 47.1 (14.5) and 33.6 (11.6) Nm, respectively, pre-intervention and 53.0 (18.5) and 33.4 (6.6) Nm post-intervention (p = 0.039 and n.s.). Eccentric torque at 90°/s was 98.5 (34.2) Nm preintervention versus 106.0 (41.4) Nm post-intervention (n.s.).Conclusion: In this exploratory study, patients with Achilles tendinopathy had a varied sensory and motor output response to heavy isometric contractions. Using the recommended approach of heavy 45-s isometric contractions did not offer a meaningful acute benefit for sensory or motor output for subjects with Achilles tendinopathy. Based on this study, heavy 45-s isometric contractions cannot be recommended for immediate pain relief or improved motor output for patients with Achilles tendinopathy.Level Of Evidence: IV, prospective cohort study. Abstract

Aims\ud \ud To comprehensively investigate the incidence, nature and risk factors of adverse drug reactions (ADRs) in a hospital-based population of children, with rigorous assessment of causality, severity and avoidability, and to assess the consequent impact on children and families. We aimed to improve the assessment of ADRs by development of new tools to assess causality and avoidability, and to minimise the impact on families by developing better strategies for communication.\ud Review methods\ud \ud Two prospective observational studies, each over 1 year, were conducted to assess ADRs in children associated with admission to hospital, and those occurring in children who were in hospital for longer than 48 hours. We conducted a comprehensive systematic review of ADRs in children. We used the findings from these studies to develop and validate tools to assess causality and avoidability of ADRs, and conducted interviews with parents and children who had experienced ADRs, using these findings to develop a leaflet for parents to inform a communication strategy about ADRs.\ud Results\ud \ud The estimated incidence of ADRs detected in children on admission to hospital was 2.9% [95% confidence interval (CI) 2.5% to 3.3%]. Of the reactions, 22.1% (95% CI 17% to 28%) were either definitely or possibly avoidable. Prescriptions originating in the community accounted for 44 out of 249 (17.7%) of ADRs, the remainder originating from hospital. A total of 120 out of 249 (48.2%) reactions resulted from treatment for malignancies. Off-label and/or unlicensed (OLUL) medicines were more likely to be implicated in an ADR than authorised medicines [relative risk (RR) 1.67, 95% CI 1.38 to 2.02; p 48 hours, the overall incidence of definite and probable ADRs based on all admissions was 15.9% (95% CI 15.0 to 16.8). Opiate analgesic drugs and drugs used in general anaesthesia (GA) accounted for > 50% of all drugs implicated in ADRs. Abstract

Our experience with high flow CAVH (1-2 L/hr) and intermittent hemodialysis in our combined medical-surgical tertiary care intensive care unit was reviewed. During a 16 month period, 12 patients received CAVH and 23 patients received hemodialysis (3-7 days/week). Mean CAVH treatment duration was 5.5 days (0.5-18 days). Hemodialysis patients received a mean of 4.3 treatments (1-16). CAVH patients had a mean age of 44.5 +/- 16.4 years (19-68), whereas the hemodialysis patients had a mean age of 60.9 +/- 14 years (34-82) (p = 0.004). Mean admission APACHE II score was 21.3 (11-29) for the CAVH group and 25.5 (10-39) for the hemodialysis group (p = NS). Peak ICU APACHE II score did not differ between groups (CAVH 26.2 [11-37], HD 28 [10-40]; p = NS). There were 4 deaths in the CAVH group (25%) and 19 deaths in the HD group (82%) (p = 0.006: chi-squared test). Although age was significantly different between groups, APACHE II scores were similar. Mortality rate was significantly higher in the hemodialysis group. This supports the hypothesis that CAVH may be the renal replacement therapy of choice in the ICU.
