Abstract

The survival of pediatric patients with multiply relapsed and/or refractory (R/R) B-cell acute lymphoblastic leukemia has historically been very poor; however, data are limited in the current era. We conducted a retrospective study to determine the outcome of multiply R/R childhood B-ALL treated at 24 TACL institutions between 2005 and 2013. Patient information, treatment, and response were collected. Prognostic factors influencing the complete remission (CR) rate and event-free survival (EFS) were analyzed. The analytic set included 578 salvage treatment attempts among 325 patients. CR rates (mean ± SE) were 51 ± 4% for patients with bone marrow R/R B-ALL who underwent a second salvage attempt, 37 ± 6% for a third attempt, and 31 ± 6% for the fourth through eighth attempts combined. For patients achieving a CR after their second, third, and fourth through eighth attempts, the 2 year EFS was 41 ± 6%, 13 ± 7%, and 27 ± 13% respectively. Our results showed slight improvement when compared to previous studies. This is the largest and most recent study to date that evaluates the outcome of this patient population. Our data will provide detailed reference for the evaluation of new agents being developed for childhood B-ALL.

Abstract

Introduction: Chimeric antigen receptor (CAR) T-cells have revolutionized cancer treatment, showing significant success, including treatment of pediatric B-cell acute lymphoblastic leukemia (B-ALL). Despite their efficacy, cytokine release syndrome (CRS) emerges as a common early adverse effect that can be life threatening in severe cases, resulting from the immune system's targeted activation against tumors. Areas covered: This review concentrates on CRS in children and young adults undergoing CAR T-cell therapy for B-ALL. It explores CRS pathophysiology, clinical presentation, and incidence, emphasizing the importance of a consensus definition and grading to homogenize the treatment according to the severity of symptoms. We will discuss the standard of care treatment of CRS but also novel approaches. We will highlight the importance of managing CRS without compromising the efficacy of immune cell activation against tumors. Expert opinion: As CAR T-cell therapy in pediatric B-ALL become increasingly available and used, optimal management of CRS becomes increasingly important. Early recognition and timely management has improved. Further information will aid us to identify optimal timing of tocilizumab and corticosteroids. Continued bench research coupled with clinical studies and biomarker discovery will allow for valuable insights into CRS pathophysiology and patient and/or cell-targeted treatments. Abstract

BackgroundIntravenous busulfan combined with therapeutic drug monitoring to guide dosing improves outcomes after allogeneic haemopoietic cell transplantation (HCT). The best method to estimate busulfan exposure and optimum exposure in children or young adults remains unclear. We therefore assessed three approaches to estimate intravenous busulfan exposure (expressed as cumulative area under the curve [AUC]) and associated busulfan AUC with clinical outcomes in children or young adults undergoing allogeneic HCT.MethodsIn this retrospective analysis, patients from 15 centres in the Netherlands, USA, Canada, Switzerland, UK, Italy, Germany, and Australia who received a busulfan-based conditioning regimen between March 18, 2001, and Feb 12, 2015, were included. Cumulative AUC was calculated by numerical integration using non-linear mixed effect modelling (AUCNONMEM), non-compartmental analysis (AUC from 0 to infinity [AUC0-∞] and to the next dose [AUC0-τ]), and by individual centres using various approaches (AUCcentre). The main outcome of interest was event-free survival. Other outcomes of interest were graft failure or relapse, or both; transplantation-related mortality; acute toxicity (veno-occlusive disease or acute graft versus-host disease [GvHD]); chronic GvHD; overall survival; and chronic-GvHD-free event-free survival. We used propensity-score-adjusted Cox proportional hazard models, Weibull models, and Fine-Gray competing risk regressions for statistical analyses.Findings790 patients were enrolled, 674 of whom were included: 274 (41%) with malignant and 400 (59%) with non-malignant disease. Median age was 4·5 years (IQR 1·4-10·7). The median busulfan AUCNONMEM was 74·4 mg × h/L (95% CI 31·1-104·6), which correlated with the standardised method AUC0-∞ (r2=0·74), but the latter correlated poorly with AUCcentre (r2=0·35). Estimated 2-year event-free survival was 69·7% (95% CI 66·2-73·0). Event-free survival at 2 years was 77·0% (95% CI 72·1-82·9) in the 257 patients with an optimum intravenous busulfan AUC of 78-101 mg × h/L compared with 66·1% (60·9-71·4) in the 235 patients at the low historical target of 58-86 mg × h/L and 49·5% (29·2-66·0) in the 44 patients with a high (>101 mg × h/L) busulfan AUC (p=0·011). Compared with the low AUC group, graft failure or relapse occurred less frequently in the optimum AUC group (hazard ratio [HR] 0·57, 95% CI 0·39-0·84; p=0·0041). Abstract

Digitaria insularisis a Poaceaedisseminated mainly by seeds that can cause problems in agroecosystems. The objective of this work was to evaluate the influence of the age of D. insularissubmitted to mowing on morphophysiological characteristics. Plants were grown in a greenhouse and cut close to the substrate at 21, 35, 49 and 77 days after emergence (DAE) or uncut, with evaluation of the variables up to 105 DAE. Except for chlorophyll b, the other variables were influenced by the age of the plants when cutting (p<0.05). Plant height and number of leaves decreased with increasing cutting age. The highest chlorophyll acontent was observed in the cut at 51 DAE. Epicuticular wax mass was greater in the 49 DAE (0.4 mg cm-2) cut. The shoot dry mass decreased with increasing age at the cut, reaching zero (0 g) after 54 DAE. The root dry mass reached the minimum point when the aerial part was cut at 57 DAE. Shoot water content decreased with increasing cutting age. The root water content showed a biphasic behavior, with the lowest water content (18.8%) in the cut at 49 DAE. These results suggest that mowing close to 21 DAE favors the development of D. insularis. Therefore, D. insularisplants in optimal development conditions should be managed by mowing priority between 49 and 60 DAE., Digitaria insularis é uma Poaceae disseminada majoritariamente por sementes que pode ocasionar problemas em agroecossistemas. O objetivo deste trabalho foi avaliar a influência da idade de D. insularis submetida a roçada sobre características morfofisiológicas. Plantas foram cultivadas em casa de vegetação e cortadas rente ao substrato aos 21, 35, 49 e 77 dias após a emergência (DAE) ou não cortadas, com avaliação das variáveis até 105 DAE. Com exceção da clorofila b, as demais variáveis foram influenciadas pela idade das plantas no corte (p<0,05). A estatura das plantas e o número de folhas diminuiu com o aumento da idade no corte. O maior teor de clorofila a foi observado no corte aos 51 DAE. A massa de cera epicuticular foi maior no corte aos 49 DAE (0,4 mg cm-2). A massa seca de parte aérea diminuiu com o aumento da idade no corte, zerando (0 g) a partir dos 54 DAE. A massa seca de raiz atingiu ponto mínimo quando a parte aérea foi cortada aos 57 DAE. O teor de água de parte aérea diminuiu com o aumento da idade no corte. O teor de água da raiz apresentou comportamento bifásico, com o menor teor de água (18,8%) no corte aos 49 DAE. Estes resultados sugerem que a roçada próxima aos 21 DAE favorece o desenvolvimento de D. insularis. Portando, plantas de D. insularis em condições ótimas de desenvolvimento devem ser manejadas com roçada prioritariamente entre os 49 e 60 DAE.

Abstract

Grasslands in southern Brazil have inherent characteristics favorable to forage production for domestic herbivores, which can be impaired or enhanced by management. The aim of this study was to evaluate the influence of management on plant characteristics,comparing commonly used techniques. The experimental design was bifactorial, the first factor consisting of four managements: burning, oversowing with annual winter forages, mowing and absence of interference, and the second factor was the time elapsed between management and evaluation. The experiment was conducted under field conditions, in a complete randomized block design with four replications. The LAI (leaf area index), dry mass production and crude protein content of the pasture were evaluated. Pasture management with oversowing showed higher LAI growth rate, as well as higher pasture dry mass production than the other treatments. There was no effect of the different managements on the crude protein content of the pasture, but of the time, with the highest value on the 53rd day. The results suggest that the adoption of management with oversowing of annual winter forages should be emphasized for the productive improvement of native grasslands in southern Brazil., Os campos do Sul do Brasil possuem características inerentes favoráveis à produção de forragem para herbívoros domésticos, que podem ser prejudicadas ou potencializadas pelo manejo. O objetivo do trabalho foi avaliar a influência do manejo nas características das plantas, comparando técnicas comumente utilizadas. O delineamento experimental foi bifatorial, sendo o primeiro fator composto por quatro manejos: queimada, sobressemeaduracom forrageiras anuais de inverno, roçada e ausência de interferência, e o segundo fator foi o tempo decorrido entre manejo e avaliação. O experimento foi conduzido em condições de campo, em delineamento em blocos completos casualizados com quatro repetições. Foram avaliados o IAF (índice de área foliar), produção de massa seca e teor de proteína bruta da pastagem. O manejo da pastagem com sobressemeadura apresentou maior velocidade de crescimento do IAF, bem como maior produção de massa seca da pastagem que os demais tratamentos. Não houve efeito dos diferentes manejos sobre o teor de proteína bruta da pastagem, mas sim do tempo, com o maior valor no 53° dia. Os resultados sugerem que a sobressemeadura de forrageiras anuais de inverno deve ser enfatizada para a melhoria produtiva dos campos nativos do Sul do Brasil.

Abstract

Clinical trials frequently include multiple end points that mature at different times. The initial report, typically based on the primary end point, may be published when key planned co-primary or secondary analyses are not yet available. Clinical Trial Updates provide an opportunity to disseminate additional results from studies, published in JCO or elsewhere, for which the primary end point has already been reported.In the primary analysis of the global phase II ELIANA trial (ClinicalTrials.gov identifier: NCT02435849), tisagenlecleucel provided an overall remission rate of 81% in pediatric and young adult patients with relapsed or refractory B-cell acute lymphoblastic leukemia (R/R B-ALL), with 59% of responders remaining relapse-free at 12 months. Here, we report an update on efficacy, safety, and patient-reported quality of life in 79 pediatric and young adult patients with R/R B-ALL following a median follow-up of 38.8 months. The overall remission rate was 82%. The median event-free survival was 24 months, and the median overall survival was not reached. Event-free survival was 44% (95% CI, 31 to 57) and overall survival was 63% (95% CI, 51 to 73) at 3 years overall (most events occur within the first 2 years). The estimated 3-year relapse-free survival with and without censoring for subsequent therapy was 52% (95% CI, 37 to 66) and 48% (95% CI, 34 to 60), respectively. No new or unexpected long-term adverse events were reported. Grade 3/4 adverse events were reported in 29% of patients > 1 year after infusion; grade 3/4 infection rate did not increase > 1 year after infusion. Patients reported improvements in quality of life up to 36 months after infusion. These findings demonstrate favorable long-term safety and suggest tisagenlecleucel as a curative treatment option for heavily pretreated pediatric and young adult patients with R/R B-ALL.

Abstract

Eragrostis tenuifolia is a weed species that is gaining ground in Brazil. This weed occurs in pastures, grasslands, crop fields, and roadsides. The objective of this study was to examine the effects of different environmental factors on E. tenuifolia seed germination and seedling emergence. The optimum constant temperature for germination was around 35-30°C. It was also found that 85% of seeds germinated under a 30/20°C alternate temperature regime. Light appears to have a positive effect on seed germination. No seedlings emerged when seeds were buried 3 cm or deeper. The results suggested that E. tenuifolia has the potential to spread into pastures and in no-tillage crop systems in Brazil. Measures such as the use of cover crops and/or soil cultivation can be used to limit germination and seedling emergence, respectively.
