Contains fulltext : 296087.pdf (Publisher’s version ) (Open Access), For biomechanical simulations of orthopaedic interventions, it is imperative to implement a material model that can realistically reproduce the nonlinear behavior of the bone structure. However, a proper material model that adequately combines the trabecular and cortical bone response is not yet widely identified. The current paper aims to investigate the possibility of using an isotropic crushable foam (ICF) model dependent on local bone mineral density (BMD) for simulating the femoral fracture risk. The elastoplastic properties of fifty-nine human femoral trabecular cadaveric bone samples were determined and combined with existing cortical bone properties to characterize two forms of the ICF model, a continuous and discontinuous model. Subsequently, the appropriateness of this combined material model was evaluated by simulating femoral fracture experiments, and a comparison with earlier published results of a softening Von-Mises (sVM) material model was made. The obtained mechanical properties of the trabecular bone specimens were comparable to previous findings. Furthermore, the ultimate failure load predicted by the simulations of femoral fractures was on average 79% and 90% for the continuous and discontinuous forms of the ICF model and 82% of the experimental value for the sVM material model. Also, the fracture locations predicted by ICF models were comparable to the experiments. In conclusion, a nonlinear material model dependent on BMD was characterized for human femoral bone. Our findings indicate that the ICF model could predict the femoral bone strength and reproduce the variable fracture locations in the experiments.

5. Therapie der Cheyne-Stokes-Atmung bei Herzinsuffizienz

Abstract

Zusammenfassung: Schlafbezogene Atmungsstörungen haben eine große prognostische Relevanz bei herzinsuffizienten Patienten. Mit zunehmendem Schweregrad der Herzinsuffizienz findet sich eine steigende Prävalenz der Cheyne-Stokes-Atmung (CSA). Bedingt durch pathophysiologische Interaktionen können CSA und Herzinsuffizienz einen Teufelskreis bilden, den es zu unterbrechen gilt. Grundlage der Therapie der CSA bei herzinsuffizienten Patienten ist die Therapie der Herzinsuffizienz. Hierzu gehört eine maximal mögliche Titration der Herzinsuffizienzmedikation, eine optimale Myokardrevaskularisation sowie, falls nach Leitlinien indiziert, die Einleitung einer kardialen Resynchronisationstherapie. Sollte sich hierunter eine Persistenz der CSA ergeben, erscheint eine spezifische Therapie indiziert. Derzeit stellt die adaptive Servoventilation die vielversprechendste Therapieoption dar. Dabei konnten Verbesserungen sowohl der schlafmedizinischen als auch der kardialen Funktionsparameter gezeigt werden. Randomisierte und kontrollierte Studien zu harten Endpunkten stehen jedoch aus. Daher ist der Stellenwert der adaptiven Servoventilation in der Herzinsuffizienztherapie aktuell noch nicht abschließend einzuordnen.

Abstract

Gallbladder cancer (GBC) is rare in Western populations and data about treatment and outcomes are scarce. This study aims to analyze survival and identify opportunities for improvement using population-based data from a low-incidence country. GBC patients diagnosed between 2005 and 2016 with GBC were identified from the Netherlands Cancer Registry. Patients were grouped according to time period (2005-2009/2010-2016) and disease stage. Trends in treatment and overall survival (OS) were analyzed. In total 1834 patients were included: 661 (36%) patients with resected, 278 (15%) with non-resected non-metastatic, and 895 (49%) with metastatic GBC. Use of radical versus simple cholecystectomy (12% vs. 26%, p < 0.001) in early (pT1b/T2) GBC increased. More patients with metastatic GBC received chemotherapy (11% vs. 29%, p < 0.001). OS improved from 4.8 months (2005-2009) to 6.1 months (2010-2016) (p = 0.012). Median OS increased over time (2005-2009 vs. 2010-2016) in resected (19.4 to 26.8 months, p = 0.038) and metastatic (2.3 vs. 3.4 months, p = 0.001) GBC but not in unresected, non-metastatic GBC. In early GBC, patients with radical cholecystectomy had a median OS of 76.7 compared to 18.4 months for simple cholecystectomy (p < 0.001). 49. A study of the sulphated acid mucopolysaccharides in twenty cases of human amyloidosis and in eighteen controls

Abstract

Amyloidosis is diagnosed and defined by several morphological criteria, most of which can be attributed directly or indirectly to the presence of sulphated acid mucopolysaccharides. However neither uronic acid nor sulphate containing components have been found by several more recent biochemical investigators to be increased. In the spleens of twenty cases with all kinds of generalized amyloidosis and of eighteen normal individuals between the ages of fourteen and ninety-eight years, the acid mucopolysaccharides were extracted quantitatively and separated from neutral aminopolysaccharides. In each case the mixture of sulphated acid mucopolysaccharides was examines by electrophoresis on cellulose acetate and analysed for uronic acid, total hexosamine, glucosamine and galactossmine content and for resistance to testicular hyaluronidase. Existing analytical methods had to be modified because of the small amounts of acid mucopolysaccharides present in the spleens. All but one spleen with amyloid deposits contained increased amounts of sulphated acid mucopolysaccharides. This was almost entirely due to heparan sulphate whose proportion was characteristically increased in all spleens with amyloidosis, even in the organ where the total amount of mucopolysaccharides was normal. Increases in the total amounts of dermatan sulphate and chondroitin sulphates also occured in some of the organs although their relative proportion was always decreased. All but one case showed an electrophoretic slow moving component, which in two cases was isolated and consisted of a low sulphated heparan. In the most acute case of amyloidosis heparan sulphate was the acid mucopolysaccharide present in splenic material insoluble in 8 M urea. In the normal spleens the acid mucopolysaccharides decreased with age without a major change in composition. Whilst it seems unlikely that the protein component of the lesions will be the same in diseases as different as myeloma, macroglobulinaemia, familial lipoproteinaemia and chronic inflammation, it could be shown that in all kinds of generalized human amyloidosis the spleen contains abnormal amounts of heparan sulphate, which might explain most of the morphological characteristics and represent a particular chemical feature of the amyloid lesion.
