Treball Final de Grau en Medicina. Codi: MD1158. Curs acadèmic: 2021/2022 Antecedentes: El cáncer de pulmón es la neoplasia que mayor mortalidad causa mundialmente. Para diagnosticarlo contamos con técnicas como la biopsia pulmonar percutánea guiada por tomografía computarizada. No obstante, esta no está exenta de complicaciones. Objetivos: Determinar la tasa de complicaciones de la biopsia pulmonar guiada por TC y sus factores de riesgo. Además, trataremos de averiguar qué tipo de biopsia tiene un mayor porcentaje de complicaciones y precisión diagnóstica. Métodos: Estamos ante un estudio observacional retrospectivo de tipo analítico. Se analizaron 202 biopsias pulmonares realizadas en el servicio de Radiodiagnóstico del Hospital General Universitario de Castellón entre 2008 y 2020. Se llevó a cabo un análisis descriptivo y contraste de hipótesis, estableciendo la significación estadística en un pvalor menor de 0,05. Principales resultados: La tasa global de complicaciones fue 35,6%. El neumotórax fue la principal complicación (32,7%). La tasa de neumotórax y de neumotórax inmediato aumentó en los individuos fumadores (p-valor de 0,025 y 0,014, respectivamente). La tasa de neumotórax tardío se incrementó en las lesiones a más de 40 mm de la pleura (p-valor = 0,014) y en las lesiones localizadas en el lóbulo pulmonar medio derecho (p = 0,039). Un diámetro menor de las lesiones fue un factor de riesgo para desarrollar hemoptisis (p = 0,045). Fueron factores de riesgo para la inserción de tubo torácico el tipo de biopsia (p = 0,008), la distancia de la pleura a la lesión (p = 0,006) y el lóbulo pulmonar (p = 0,009). Conclusiones: El tipo de biopsia con un menor número de complicaciones y con un mayor rendimiento diagnóstico es la BAG, por lo que es preferible a la PAAF o PAAF + BAG. Background: Lung cancer is the neoplasm which causes greater mortality worldwide. There are techniques like the computed tomography guided percutaneous lung biopsy to diagnose it. However, it can cause some complications. Objectives: To determine the rate of complications of CT-guided lung biopsy and its risk factors. Additionally, we aim to find which type of biopsy has a higher percentage of complications and a greater diagnostic yield. Methods: This is an observational, retrospective, analytical study. 202 lung biopsies carried out in the Radiodiagnosis service of the Hospital General Universitario de Castellón between 2008 and 2020 were analysed. Descriptive analysis and hypothesis contrast were made, setting statistical significance at a p-value lower than 0.05. Main outcomes: The overall complication rate was 35.6%. Pneumothorax was the main complication (32.7%). The rate of pneumothorax and immediate pneumothorax was higher in smokers (p-value of 0.025 and 0.014, respectively). The number of patients who suffered late pneumothorax was greater when the lesions were more than 40 mm from the pleura (p-value = 0.014) and in lesions located at the right middle lung lobe (p = 0.039). A smaller diameter of the lesions was a risk factor for developing haemoptysis (p = 0.045). Risk factors for chest tube insertion were the type of biopsy (p = 0.008), the distance between the lesion and the pleura (p = 0.006) and the lung lobe (p = 0.009). Conclusions: The type of biopsy with the fewest number of complications and a higher diagnostic yield is the CNB. This is the reason why the CNB is preferable to the FNAB or the combination of both techniques.
