Choledocholithiasis is a common disease after bile duct surgery, and there may be problems during the diagnosis and treatment of this serious condition. In recent years, ultra-slim endoscopes have been increasingly used in the diagnosis and treatment of choledocholithiasis. We report the case of a 62-year-old woman who was diagnosed and treated successfully with pediatric endoscope, Geçirilmiş safra yolu cerrahileri sonrası sık karşılaşılan koledokolitiazis, tanı ve tedavisinde önemli problemler olabilen ciddi bir durumdur. Son yıllarda ultraslim endoskoplar koledokolitiazis tanı ve tedavisinde giderek artan kullanım alanı bulmuştur. Pediatrik endoskop ile tanı ve tedavisi başarılı bir şekilde yapılmış olan 62 yaşında kadın, koledokolitiazis olgusu sunulmuştur.

ÖZETKARACİĞER SİROZLU HASTALARDA AKUT BÖBREK HASARI GELİŞİMİNİ PREDİKTE EDEN FAKTÖRLER VE HASTANE MORTALİTESİ İLE İLİŞKİSİDr. Murat BIYIKYAN DAL UZMANLIK TEZİKONYA, 2014Amaç: Hastaneye kabul edilen sirozlu hastalarda, KDIGO kriterlerine göre akut böbrek hasarını yaygınlığını belirlemek ve bunun mortalite ile ilişkini değerlendirmek Kapsam: Akut böbrek hasarı (ABH) sirozda kötü prognozla ilişkilidir. ABH nın gelişimi sıklıkla, sirozun, varis kanaması, spontan bakteriyel peritonit gibi diğer komplikasyonlarının başlamasıyla ilişkilidir ve dekompanse siroz tanılı hastane yatışı olan hastalarda % 20 ye kadar görülebilmektedir. Son zamanlarda, KDIGO (Kidney Disease Improving Global Outcomes) yeni ABH kriterleri yayınlamıştır. Bu kriterler içerisinde serum kreatininde 48 saat süresince 0.3 mg/dl artış veya bir haftalık peryod süresince bazal serum kreatininde 1.5 kat veya daha fazla artışlar yer almaktadır. Sonuç olarak KDIGO kriterleri mortaliteyi daha iyi predikte edebilir. Yöntem: 2008-2012 yılları arasında, siroz tanısı almış ve hastanede 48 saatten fazla yatışı olmuş hastaların, hastane elektronik dosya sistemi taranarak, yatış süresince serum kreatinin değerleri incelendi ve akut böbrek hasarının gelişip-gelişmediğine göre hastalar iki gruba ayrıldı. İki grup arasındaki hastane mortalite oranları karşılaştırıldı ve mortaliteyi etkilemesi muhtemel sosyodemografik faktörler, siroz etyolojileri, hastane yatış süreleri, hastane yatış nedenleri, yoğun bakım yatış süreleri, ABH evresi ve progresyonu, CHİLD ve MELD skorlarının mortalite ile ilişkisi incelendi.Bulgular: Çalışmaya toplam 277 servis ya da yoğun bakım yatışı olan hasta alındı. Hastaların yaş ortalaması 62,1, kadın erkek sayısı 117/160 olarak bulundu. KDIGO kriterlerine göre toplam 108 hastada ABH geliştiği izlendi. ABH gelişimiyle ilişkili faktörler arasında, artmış Child Pugh ve MELD skorları, serum kreatinin, AST, ALT, T.Bil. ve INR değerleri, azalmış albumin ve sodyum değerleri, DM varlığı ve diüretik kullanımı bulunmuştur. Toplam 43 hastanın eksitus olduğu ve mortalite ile ilişkili faktörler arasında ise yine artmış Child Pugh ve MELD skorları, serum kreatinin, AST, ALT, T.Bil. ve INR değerleri, azalmış albumin ve sodyum değerleri, peritonit ve HCC varlığı bulunmuştur. Multivaryant analizde, peritonit, artmış MELD skoru ve ABH mevcudiyeti mortalitede bağımsız faktörler olarak bulunmuştur.Sonuç: Karaciğer sirozlu hastalarda ABH sık görülen ve mortalitenin bağımsız prediktörlerindendir. Daha önce tanımlanan RIFLE ve AKIN kriterlerine göre ABH'nın, sirozlu hastalarda mortalite ilişkisini gösteren çalışmalar yapılmıştır. KDIGO kriterleri en son yayınlanan ve diğer kriterleri de içine alması nedeniyle ABH'nı daha duyarlı tespit eden kriterlerdir ve bizim çalışmamız bu yönüyle literatürdeki ilk çalışmadır. ABH tespit edilen hastaların çoğunluğu ilk evrededir ve tespit edildiklerinde, daha dikkatli olunduğu takdirde progresyonun önlenebileceği hastalardır. Bu sayede mortalitenin de azalabileceği kanaatindeyiz.Anahtar Kelimeler: Siroz, Akut Böbrek Hasarı, Mortalite ABSTRACTPREDICTING FACTORS OF ACUTE RENAL INJURY IN CIRRHOTIC PATIENTS AND IT'S ASSOCIATION WITH IN HOSPITAL MORTALITYDr. Murat BIYIKYAN DAL UZMANLIK TEZİKONYA, 2014Aim: The aim of this study was evaluate the presence of acute kidney injury according to KDIGO criteria in cirrhotic patients admitted to the hospital and also to determine its relation with inpatient mortality.Introduction: Acute kidney injury (AKI) is associated with poor prognosis in cirrhotic patients. The development of AKI is frequently associated with development of complications of cirrhosis such as esophageal variceal bleeding and spontaneous bacterial peritonitis and AKI is seen up to 20% of patients with decompansated cirrhosis admitted to the hospital. New criteria for AKI are published recently by KDIGO (Kidney Disease Improving Global outcomes). This criteria includes a rise in serum creatinine of 0,3 mg/dl in 48 hours of admission or increase of basal serum creatinine by 1,5 fold in one week. KDIGO criteria may predict mortality better.Method: Cirrhotic patients who were hospitalised for more than 48 hours in Necmettin Erbakan University, Faculty of Medicine between 2008-2012 were taken into the study. Serum creatinine values after admission were investigated from electronic archives of the hospital and patients were divided into two groups according to the development of AKI. Two groups were compared in terms of inpatient mortality and factors such as cirrhosis etiology, duration of hospitalisation, primary hospitalisation reason, duration of intensive care unit, sociodemografical factors, AKI stage and progression, CHILD and MELD scores were evaluated in terms of a potential association with mortality.Results: 277 patients hospitalisated in intensive care unit or gastroenterology clinic were taken into the study. The mean age of the patients were 62,1 years. 117 of the patients were females and 160 were males.108 of the patients were found to develop AKI according to KDIGO criteria. Factors associated with the development of AKI were found to be; hihg Child Pugh and MELD scores, high serum creatinine, AST, ALT, total bilirubine and INR levels, low serum sodium and albumine levels and the presence of DM and diuretic usage. 43 patients died during hospitalisation. Factors associated with mortality were high serum creatinine, AST, ALT, total bilirubine and INR levels, low serum sodium and albumine levels and the presence of peritonitis and HCC. Multivariant analyses yielded peritonitis, high MELD scores and the presence of AKI as independent factors associated with mortality.Conclusion: AKI is frequently seen in hospitalisated cirrhotic patients and is an independent factor predicting mortality in these patients. Prior studies showed that AKI determined with RIFLE or AKIN criteria are associated with mortality in cirrhotic patients. KDIGO criteria are the most recently published and science it is more sensitive to determine of AKI in these patients. Our study is the first study to use KDIGO criteria to determine of AKI. The AKI detected in these patients are mostly in early stage and we believe that it may be possible to prevent AKI progression and thus mortality if early preventive measures are taken.Key Words: Cirrhosis, Acute Kidney Injury, Mortality 61

Variceal bleeding is the major complication of portal hypertension in patients with liver cirrhosis. Hemorrhage mainly occurs in gastrointestinal lumen. Extraluminal hemorrhages are quite rare, such as intraperitoneal hemorrhages. We aimed to present a variceal bleeding case from the anastomosis on the anterior abdominal wall, as an extraordinary bleeding location, in a patient with portal hypertension in whom there were no esophageal and gastric varices.
