Commotio cordis or cardiac concussion is a rare and fatal mechano-electric arrhythmogenic syndrome. It is the second most common cause of sudden cardiac death in young athletes. It is most commonly associated with a sports-related injury, wherein, there is a high-velocity impact between a projectile and the precordium, causing arrhythmia that leads to the immediate death of the individual without cardiac resuscitation. On autopsy, the heart is structurally normal. With increasing awareness of this condition and community training in cardiopulmonary resuscitation, survival rates have been improving. The objective of this study is to describe the case of a patient who arrived at our hospital with commotio cordis and his course, emphasizing the importance of prevention and training of the population in cardiopulmonary resuscitation techniques and the use of the automated external defibrillator for the survival of patients suffering from commotio cordis., (Sociedad Argentina de Pediatría.)

Background: COVID19 is the novel respiratory illness caused by SARS-CoV-2. The presence of other potentially pathogenic microorganisms could worsen the prognosis of these patients., Aim: The study aims to describe coinfections in COVID-19 patients and contrast it between standard ward and critical care patients at Hospital Central de la Defensa Gómez Ulla (HCDGU)., Methods: A retrospective study was carried out of patients with COVID-19 confirmed with RTPCR admitted to the HCDGU from March 5, 2020 to May 7 of 2020., Findings: Of a total of 703 patients with COVID-19, 75(10.7%) had other microbiologically confirmed infections: 9% (58/648) in standard ward patients and 31.5%(17/54) in critical care patients. In total 86 samples of the 75 patients presented some microorganism; clinically relevant bacteraemias, 50%, respiratory cultures, 32.6% and pneumococcal positive antigens, 17.4%., Conclusions: We found a low frequency of microorganism coinfection in COVID-19 patients, however in critical care these coinfections increased considerably., (Published by Elsevier Inc.)

An empirical approach through remote sensing generally produces a robust data model of water quality for inland and coastal water. Traditional regressions in water quality mapping fail because the bio-optical relationship of turbid water exhibits nonlinear and heterogeneous patterns. In addition, in situ data are generally insufficient in the water quality mapping. Mapping based on a relatively small amount of water quality samples is considered a practical issue in environmental monitoring. Learning-based algorithms that require a large amount of data are inapplicable in this case. According to the concept of Nadaraya-Watson estimator, the kernel model can estimate nonlinear and spatially varying water quality maps effectively in turbid water.Experiments indicate that the kernel estimator provides better goodness-of-fit between the observed and derived concentrations of water quality parameter, e.g., chlorophyll-a in turbid water. The kernel estimator is feasible for a relatively small size of ground observations. Approximately 30% improvement of cross-validation error was identified in this approach when compared with traditional regressions. The model offers a robust approach without further calibrations for estimating the spatial patterns of water quality by using remote sensing reflectance and a small set of observations, considering spatial and spectral information, e.g., multiple bands and band ratios.

The fate of plastic waste and a sustainable use of synthetic polymers is one of the major challenges of the twenty first century. Waste valorization strategies can contribute to the solution of this problem. Besides chemical recycling, biological degradation could be a promising tool. Among the high diversity of synthetic polymers, polyurethanes are widely used as foams and insulation materials. In order to examine bacterial biodegradability of polyurethanes, a soil bacterium was isolated from a site rich in brittle plastic waste. The strain, identified as Pseudomonas sp. by 16S rRNA gene sequencing and membrane fatty acid profile, was able to grow on a PU-diol solution, a polyurethane oligomer, as the sole source of carbon and energy. In addition, the strain was able to use 2,4-diaminotoluene, a common precursor and putative degradation intermediate of polyurethanes, respectively, as sole source of energy, carbon, and nitrogen. Whole genome sequencing of the strain revealed the presence of numerus catabolic genes for aromatic compounds. This study examined the presence of neurological soft signs, an accessible diagnostic instrument, in patients with anxiety. Individuals with anxiety were more likely to manifest hypoesthesia than healthy controls, and patients who showed hypoesthesia exhibited greater symptoms of anxiety and depression.

Background: The aim of choosing a mood-stabilizing drug (lithium or anticonvulsants) or a combination of them with minimal neurocognitive effects is to stimulate the development of criteria for a therapeutic adequacy, particularly in Bipolar Disorder (BD) patients who are clinically stabilized., Method: Three groups of BD patients were established according to their treatment: (i) lithium monotherapy (n=29); (ii) lithium together with one or more anticonvulsants (n=28); and (iii) one or more anticonvulsants (n=16). A group of healthy controls served as the control (n=25). The following tests were applied: Wechsler Adult Intelligence Scale, Trail Making Test, Wechsler Memory Scale, Rey Complex Figure Test, Stroop color-word test, Wisconsin Card Sorting Test, Tower of Hanoi, Frontal Assessment Battery, and Reading the Mind in the Eyes Test., Results: Relative to healthy controls, BD patients showed the following: (i) those on lithium monotherapy, but not other BD groups, had preserved short-term auditory memory, long-term memory, and attention; (ii) those who took only anticonvulsants showed worse findings in short-term visual memory, working memory, and several executive functions; and (iii) all BD patients showed worse performance in processing speed, resistance to interference, and emotion recognition., Limitations: Medication alone cannot explain why all BD patients showed common cognitive deficits despite different pharmacological treatment., Conclusion: The impairment on some executive functions and emotion recognition is an inherent trait in BD patients, regardless of their pharmacological treatment. However, while memory, attention, and most of the executive functions are preserved in long-term stable BD patients, these cognitive functions are impaired in those who take anticonvulsants., (Copyright © 2015. Background: Physical signs often are present in many psychiatric conditions, making up a fundamental part of them and accompanying the psychiatric manifestations themselves. Identifying minor neurological signs is especially of interest due to they are easily accessible through simple neurological examination and could be a useful if underused tool for the diagnostic process and patient therapy., Method: A group of depressed patients (n=85) and group of healthy individuals (n=101) that served as control were examined using the Wartenberg wheel, a medical device for neurological use, in order to determine the presence of hypoesthesia on both sides of their ankles., Results: The data revealed: (i) patients with depression are generally more likely to present malleolar hypoesthesia than healthy participants; and (ii) participants who presented malleolar hypoesthesia presented greater depressive symptomatology as well as greater anxiety symptomatology at the time of assessment., Limitations: Although all patients in this study were taking psychotropic medication, anxiolytics and antidepressants are not associated with skin sensitivity. As is usual, the categorization of hypoesthesia is based on participant subjectivity. However, this subjectivity cannot explain the differences between depressed patients and healthy individuals., Conclusions: The present findings corroborate that localized tactile sensitivity is altered in depression and correlates with anxiety-depressive symptomatology, even on a subclinical level. The observation of neurological soft signs such as the detection of malleolar hypoesthesia in patients with depressive symptomatology is easily accessible using a simple neurological examination, and it could became a powerful tool that could provide objective information on affective disorders., (Copyright © 2014 Elsevier B.V. An antisaccade experiment, using happy, sad, and neutral faces, was conducted to examine the effect of mood-congruent information on inhibitory control (antisaccade task) and attentional orienting (prosaccade task) during the different episodes of bipolar disorder (BD) - manic (n=22), depressive (n=25), and euthymic (n=24). A group of 28 healthy controls was also included. Results revealed that symptomatic patients committed more antisaccade errors than healthy individuals, especially with mood-congruent faces. The manic group committed more antisaccade errors in response to happy faces, while the depressed group tended to commit more antisaccade errors in response to sad faces. Additionally, antisaccade latencies were slower in BD patients than in healthy individuals, whereas prosaccade latencies were slower in symptomatic patients. Taken together, these findings revealed the following: (a) slow inhibitory control in BD patients, regardless of their episode (i.e., a trait), and (b) impaired inhibitory control restricted to symptomatic patients (i.e., a state)., (Copyright © 2013 Elsevier B.V. An "affective" go/no-go task was used in the different episodes of bipolar patients (euthymic, depressed, and manic) to examine (1) the presence of a mood-congruent attentional bias; and (2) the patients' ability to inhibit and invert associations between stimuli and responses through blocks. A group of healthy individuals served as controls. Results revealed a mood-congruent attentional bias: patients in the manic episode processed positive information faster, whereas those in the depressive episode processed negative information faster. In contrast, neither euthymic patients nor healthy individuals showed any mood-congruent biases. Furthermore, there was a shift cost across blocks for healthy individuals, but not for the patients. This may reflect a general impairment at selecting relevant information (e.g., in terms of disability to inhibit and invert associations between stimuli and responses) in bipolar participants, regardless of their episode. This state/trait dissociation in an episodic and chronic disorder such as bipolar disorder is important for its appropriate characterisation.

Radon (²²²Rn) measurements were conducted in Shiraho Reef (Okinawa, Japan) to investigate nearshore submarine groundwater discharge (SGD(nearshore)) dynamics. Estimated average groundwater flux was 2-3 cm/h (maximum 7-8 cm/h). End-member radon concentration and gas transfer coefficient were identified as major factors influencing flux estimation accuracy. For the 7-km long reef, SGD(nearshore) was 0.39-0.58 m³/s, less than 30% of Todoroki River's baseflow discharge. SGD(nearshore) was spatially and temporally variable, reflecting the strong influence of subsurface geology, tidal pumping, groundwater recharge, and hydraulic gradient. SGD(nearshore) elevated nearshore nitrate concentrations (0.8-2.2 mg/l) to half of Todoroki River's baseflow NO₃⁻-N (2-4 mg/L). This increased nearshore Chl-α from 0.5-2 μg/l compared to the typically low Chl-α (< 0.1-0.4 μg/l) in the moat. Diatoms and cyanobacteria concentrations exhibited an increasing trend. However, the percentage contributions of diatoms and cyanobacteria significantly decreased and increased, respectively. Introduction: The cardiopulmonary resuscitation (CPR) is a common setting in the pediatric intensive care unit (PICU). There are very few reports or publications that evaluate the form of CPR administered in children., Objectives: 1) Identify the etiology and epidemiology of the CPR in the PICU. 2) Describe how to conduct CPR. 3) Describe the drugs used. 4) Knowing the patient outcomes. 5) Knowing CPR training of physicians in the PICU., Patients and Methods: All children with cardiopulmonary arrest who were resuscitated in the PICU between 01/04/2004- 31/03/2005. A prospective cohort study., Results: There were 2065 admissions in nine 9 PICU and CPR was conducted in 132 patients (6.39%). Most common etiologies of PCR were hypoxia and hypotension (65.9% of total). The initial rates of PCR were 43.94% bradycardia, asystolia 43.18%, 9.85% ventricular arrhythmias. The average beginning of time of CPR was 0.08 minutes (SD 0.25) and the average total duration was 25.91 minutes (SD 18.56). Patients who require drugs were 131. The number (mean) dose of adrenaline was 4.03 (SD 2.78). Increased number of doses of adrenaline was associated with less chance of recovery of spontaneous circulation (ROSC), OR 0.48 95% CI 0.37-0.63. (p= 0.000). The duration of CPR was inversely associated and significantly related to achieve ROSC, OR 0.93 95% CI 0.87-0.99. One hundred and four (78.79%) patients died and 28 survived. All survived were discharged from PICU, but 26 from hospital. In 22 patients there were not evidence of severe neurological damage when discharged home., Conclusions: Most common etiologies of PCR were hypoxia and shock. Most children received drugs. The drugs most used were the association adrenaline-bicarbonate and adrenaline alone. The chances of recovery were not favorable when CPR was conducted for over sixty minutes or more than six doses of adrenaline were given, without response. CPR in PICU children has a high mortality. Most patients discharged from hospital, had neurological normal state or slight disability. PICU physicians were highly trained in CPR with resuscitation courses.

Microalgal-based indicators were used to assess terrestrial influence on Shiraho coral reef of Ishigaki Island (Okinawa, Japan). A typhoon occurred on 4-5 August 2005 and sampling were made on three occasions thereafter (6, 8, and 11 August). Pre-typhoon sampling was conducted on 26 July. The typhoon-enhanced terrestrial discharges increased reef nutrient levels (e.g. average NO(3)(-)-N: 0.088mg/L pre-typhoon to 0.817mg/L post-typhoon). This elevated chlorophyll-a concentrations by four times and shifted phytoplankton composition (spectral class-based) from an initial dominance of diatoms and green microalgae to the dominance of bluegreen microalgae (cyanobacteria) and cryptophytes. Cyanobacteria later increased by more than 200% and accounted for as much as 80% of total chl-a ( approximately 0.29microg/L), possibly assisted by favorable nutrient availability. In outer reef waters, diatoms and green microalgae predominated whereas cyanobacteria and cryptophytes were nearly undetectable. Due to detrital decomposition and river discharge, the CDOM was much higher in the inner reef than in the outer reef. Benthic bluegreen microalgae were relatively more abundant in areas close to the river mouth and coastal agricultural fields. At these locations, nutrient concentrations were much higher due to river discharge and potentially significant groundwater discharge. Thus, phytoplankton and benthic microalgae can serve as indicators of terrestrial influence on coral reefs.

The presence of E. coli producer of shiga toxin and rotavirus was investigated in 90 stool samples from children less than 3 years old with diarrhea. Three aliquots were separated from each sample: the first one underwent previous enrichment for E. coli O157, the second one was plated on agar MacConkey-Sorbitol and Red Eosine with MUG, and the last one was frozen at -70 degrees C for the later analysis of rotavirus. The search of the antigen O157 of E. coli was carried out by immunochromatography in vitro of Coris Bioconcept (Belgium). The presence of the antigen O157 (rbf O157) and the genes that code for the shiga toxin (stx1 and stx2) were determined by PCR. Rotavirus were detected by electrophoresis in polyacrylamide gels. The 90 samples analyzed by immunochromatography were negative for the antigen O157. The isolates were STEC strains non O157 and contained the gene sx1, showing a 10% of positivity. The electrophoresis for the viral RNA detected rotavirus in 21 (23.33%) samples. This result confirms the rotavirus prevalence and suggests, that the circulation of STEC strains non O157 is an indication of the involvement of these strains in the ethiology of acute diarrheas.
