Cancer is a major cause of death in patients with spinal cord injury (SCI). Preventive strategies, such as colonoscopy, deal with higher burdens that may lead to lower quality. The primary objective was to evaluate the adenoma detection rate. Secondary objectives were to investigate other quality indicators regarding bowel preparation, sedation, and endoscopy. Consecutive SCI patients who had undergone colonoscopy from 2003 to 2014 were assigned to a control group matched for age, gender, and year of procedure and reviewed retrospectively. Bowel preparation lasted longer (3.6 ± 1.5 vs. 1.2 ± 0.6 days, p = 0.001), achieved unsatisfactory cleansing results more often (23.7 vs. 3.6%) and caused more adverse events in 236 SCI compared to 414 control patients. Colonoscopy needed a longer time (36.9 vs. 25.0 min) and remained incomplete more often (24.6 vs. 4.6%), resulting in more re-colonoscopies (14.8 vs. 4.3%). Endoscopy- and sedation-related adverse events were equal. However, neither overall nor size-dependent polyp (30.9 vs. 34.8%), adenoma (21.2 vs. 21.0%), advanced adenoma (6.8 vs. 7.2%), or cancer (1.7 vs. 2.0%) detection rates differed. Despite intensified protocols, bowel preparation shows inferior results in SCI patients; colonoscopy needs more effort to succeed but achieves a comparable quality.

Hintergrund: Die neurogene Darmstörung von Rückenmarksverletzten (RMV) könnte die Ergebnisqualität einer Koloskopie herabsetzen. Methode: Retrospektive Analyse von 236 RMV-Patienten und 414 Kontrollgruppen-Patienten (KG), gepaart nach Alter, Geschlecht und Untersuchungsjahr. Zielparameter: Adenom-, Polypen- und Karzinomdetektionsraten (ADR, PDR, KDR), Qualität der Darmvorbereitung, Häufigkeit von Re-Endoskopie, Komplikationen, Zusammenhang zwischen RMV und Ergebnisqualität. Ergebnisse: Die Darmreinigung dauerte bei RMV-Patienten länger, war häufiger unzureichend und häufiger mit Komplikationen assoziiert. Die Koloskopie dauerte länger, blieb häufiger inkomplett, erforderte mehr Re-Endoskopien. PDR, ADR und KDR waren gleich. Das RMV-Muster hatte keinen Einfluss. Schlussfolgerung: Die Darmreinigung führt bei RMV-Patienten zu schlechteren Ergebnissen. Durch relevanten Mehraufwand konnte dennoch eine vergleichbare Untersuchungsqualität der Koloskopie erreicht werden.

Son muchas las acciones que se han puesto en marcha para favorecer las Igualdad efectiva entre ambos sexos, pero muy pocas las investigaciones que existen sobre la ética de las relaciones amorosas y sexuales. Actualmente todavía no podemos afirmar que exista una igualdad real en el trato entre hombres y mujeres, y como muestra basta remitirnos a los nuevos casos de violencia de género que salen a la luz a diario, y que no son más que la prueba definitiva de que aún existen conductas discriminatorias hacia la mujer. La intención de esta investigación es conocer cómo es, cuál es y cómo debe ser la ética de las relaciones amorosas, con el objetivo final de contribuir a la superación de la doble moral sexual, la discriminación y la violencia de género. Consideramos éste un tema central de la educación sexual, emocional y social porque nuestra sociedad ha pasado de una moral sexual religiosa represiva y discriminatoria con la mujeres a una situación en la que no se da ningún tipo de educación ética, como si las relaciones sexuales y amorosas no estuvieran sujetas a valores y deberes. Nuestro objetivo final es proponer una ética sexual y amorosa básica, que pueda ser aceptada por creyentes y no creyentes, conservadores y liberales, porque es una ética de principios fundamentada en la dignidad, igualdad y libertad de los hombres y mujeres. Estos contenidos deben ser incluidos en la educación sexual, emocional y social de niños y niñas, considerándolos fundamentales para evitar la violencia y conseguir la libertad, la igualdad y la responsabilidad en las relaciones sexuales y amorosas y las relaciones en general entre mujeres y hombres., [EN]Equality between men and women is not only a right, is also a constitutional duty, we all need to implement. Our ultimate goal is to provide a loving sexual ethics and basic that can be accepted by believers and non-believers, conservatives and liberals, because it is an ethic of principles based on human dignity, equality and freedom of men and women.
