For a clinical center it is increasingly important to successfully use information technology (IT). However, IT systems in a clinical environment are part of a complex socio-technical system. They only can be enduringly successful if they closely follow their users’ needs and if they are able to rapidly react on volatile requirements. Since 1997, an IT concept for the introduction of major parts of the Marburg health information system (HIS) has been elaborated. The concept is described in the first part of this thesis. Initially, due to time and cost restrictions, a so called "big bang" approach had to be chosen. Important base functionality had to be introduced rapidly throughout the clinical center. The "big bang" approach is associated with a high project risk. In order to minimize this risk, the "big bang" phase was limited as far as possible. For the further development of the HIS an incremental approach for demand-driven software evolution has been used. This approach shall allow to closely involve the users and rapidly and effectively implement changed requirements. The aim of this thesis is to assess the described approach concerning its success and sustainability when applied in practice. For this purpose an evaluation concept was elaborated starting from the fundamental IT success factors. It combines different qualitative and quantitative methods: measuring indicators of HIS development (extent of infrastructure, development and support times, system usage, and system response times), quantitatively comparing user satisfaction before and after using the incremental approach, interviewing users, as well as continuously monitoring process and documentation quality, and usability as part of software development. These studies were performed from 2000 to 2005, accompanying clinical parts of the HIS project. To evaluate IT interventions within an organization is closely intertwined with application development and user training. Therefore, a number of social and organizational factors aggravated the evaluation of the IT concept. These obstacles and possible weaknesses of the deployed methods are described and discussed. The evaluation results largely confirm the hypotheses. The "big bang" approach showed the expected drawbacks: functional deficiencies, insufficient alignment to processes and users’ needs, and moderate user satisfaction. The situation improved measurably when the incremental approach was used for the further development of the HIS. Combining these approaches proved to be feasible. With limited resources it was possible to build a HIS that offers broad hospital-wide clinical functionality and that has the advantages of an integrated system as well as enough flexibility and adaptability. This is also reflected in an acceptable user satisfaction, which increased significantly when using the incremental approach. Moreover, only the scores of those satisfaction factors increased that are assumed to be causally influenced by a participatory demand-driven software engineering process. The comparison of results from different analyses (user satisfaction, system response times, interviews, and user feedback) seems to confirm the assumption that these positive effects can be explained to a large extent by the applied model of software engineering and are not caused by possible confounders like higher system performance or higher commitment of IT staff. The experiences from clinical parts of the HIS project also show that effective and demand-driven IT support for clinical processes can be achieved on the basis of the Marburg IT concept. Furthermore, potential negative impacts of an IT intervention can be rapidly identified and eliminated by continuously monitoring quality indicators as part of an iterative participative software engineering process that is embedded into a comprehensive change management process. Finally, shortcomings of the applied tool are discussed and international research activities on multi-layered service-oriented architectures are described briefly with respect to the Marburg IT concept. Such architectures promise to enable adaptable and flexible IT support for interorganizational processes in health care networks., Der erfolgreiche Einsatz von Informationstechnologie (IT) ist von wachsender Bedeutung für den Erfolg eines Unternehmens wie einem Universitätsklinikum. IT-Systeme im klinischen Umfeld sind jedoch Teil eines komplexen soziotechnischen Systems und haben nur nachhaltigen Erfolg, wenn sie sich eng an den Bedürfnissen der Endanwender orientieren und schnell auf geänderte Anforderungen reagieren können. Für die Neueinführung wesentlicher Komponenten des Marburger Krankenhausinformationssystems (KIS) wurde ab 1997 ein IT-Konzept erarbeitet. Dieses Konzept wird im ersten Teil der vorliegenden Arbeit vorgestellt. Aus Zeit- und Kostengründen musste zunächst eine sogenannte "Big-Bang"-Vorgehensweise für die schnelle klinikumsweite Einführung wichtiger Basisfunktionen gewählt werden. Um das damit verbundene hohe Projektrisiko zu minimieren wurde die "Big-Bang"-Phase auf das Nötigste begrenzt und für den weiteren Ausbau des KIS ein inkrementeller Ansatz zur bedarfsorientierten Softwareevolution verfolgt. Er soll die Möglichkeit bieten, neue und geänderte Anforderungen schnell und effektiv unter enger Einbeziehung der Endanwender umzusetzen. Ziel der vorliegenden Arbeit ist es, den Erfolg und die Tragfähigkeit dieses Ansatzes im praktischen Einsatz zu bewerten. Dazu wurde ausgehend von den wesentlichen Faktoren für den Erfolg eines IT-Systems ein Evaluationskonzept erarbeitet, das verschiedene qualitative und quantitative Methoden kombiniert. Dazu gehören: die Messung von Kennzahlen zum Ausbau des KIS (Umfang von Infrastruktur, Funktionalität und Nutzung, Projektierungsaufwand sowie Systemantwortzeiten), quantitative Vergleiche der Anwenderzufriedenheit vor und nach der Verwendung des inkrementellen Vorgehensmodells, Interviews mit Anwendern sowie kontinuierliche Analysen zu Bedienbarkeit, Prozess- und Dokumentationsqualität als Teil der Softwareentwicklung. Diese Untersuchungen wurden in den Jahren 2000 bis 2005 begleitend zu klinischen Teilprojekten der KIS-Einführung vorgenommen. Da die Evaluierung von IT-Maßnahmen innerhalb einer Organisation eng mit der Anwendungsentwicklung und der Schulung der Benutzer verwoben ist, wurde die Evaluierung des IT-Konzepts durch eine Reihe sozialer und organisatorischer Faktoren erschwert. Diese Hindernisse und mögliche Schwächen der einzelnen Methoden werden dargestellt und diskutiert. Die Evaluierungsergebnisse bestätigen weitgehend die Hypothesen. Der "Big-Bang"-Ansatz zeigte die erwarteten Nachteile: funktionale Defizite, eine unzureichende Anpassung an die Prozesse und die persönliche Arbeitsweise der Nutzer sowie eine mäßige Anwenderzufriedenheit. Der für den weiteren Ausbau des KIS verfolgte inkrementelle Ansatz zur bedarfsorientierten Softwareevolution führte zu messbaren Verbesserungen. Die Kombination dieser Ansätze erwies sich als praktikabel. Es war mit relativ geringen Ressourcen möglich, ein KIS aufzubauen, das flächendeckend ein breites Spektrum an klinischer Funktionalität bereitstellt und dabei sowohl die Vorteile eines integrierten Systems als auch eine hohe Flexibilität und Anpassbarkeit aufweist. Dies spiegelt sich auch in einer signifikant gesteigerten Anwenderzufriedenheit nach Verwendung des inkrementellen Vorgehensmodells wider. Dabei erhöhten sich nur die Werte derjenigen Zufriedenheitsfaktoren, bei denen ein direkter kausaler Zusammenhang zur partizipativen, bedarfsorientierten Softwareentwicklung vermutet wird. Auch der Abgleich mit den gemessenen Systemantwortzeiten und den Ergebnissen aus den Interviews und Anwenderrückmeldungen scheint die Annahme zu bestätigen, dass diese positiven Effekte weitgehend durch das verwendete Vorgehensmodell und nicht durch mögliche Störgrößen wie eine erhöhte Systemperformanz oder eine intensivere Betreuung durch das IT-Personal zu erklären sind. Die Erfahrungen aus klinischen Teilprojekten zeigen außerdem, dass es auf der Basis des Marburger IT-Konzepts möglich ist, klinische Prozesse effektiv und bedarfsorientiert zu unterstützen. Mittels einer kontinuierlichen, projektbezogenen Beobachtung von Qualitätsindikatoren als Teil eines iterativen, partizipativen Softwareentwicklungsprozesses, der in einen umfassenden "Change-Management"-Prozess eingebettet ist, können zudem mögliche negative Auswirkungen einer IT-Intervention frühzeitig erkannt und schnell beseitigt werden. Abschließend werden Defizite des zum Einsatz gekommenen Werkzeugs diskutiert und es werden ausgehend vom Marburger IT-Konzept internationale Forschungsbestrebungen hin zu mehrschichtigen Service-orientierten Architekturen skizziert. Solche Architekturen und die konsequente Trennung verschiedener Belange der Softwareentwicklung stellen einen vielversprechenden Ansatz zur entwicklungsfähigen, organisationsübergreifenden IT-Unterstützung von Prozessen in "Gesundheitsnetzen" dar.

Context: Successful tumor resection in endogenous Cushing's syndrome (CS) results in tertiary adrenal insufficiency requiring hydrocortisone replacement therapy. Objective: The aim was to analyze the postsurgical duration of adrenal insufficiency of patients with Cushing's disease (CD), adrenal CS, and ectopic CS. Design: We performed a retrospective analysis based on the case records of 230 patients with CS in our tertiary referral center treated from 1983-2014. The mean follow-up time was 8 years. Patients: Weincluded 91 patients of the three subtypes of CS undergoing curative intended surgery and documented followup after excluding cases with persistent disease, pituitary radiation, concurrent adrenostatic or somatostatin analog treatment, and malignant adrenal disease. Results: The probability of recovering adrenal function within a 5 years followup differed significantly between subtypes (P =.001). It was 82% in ectopic CS, 58% in CD and 38% in adrenal CS. In the total cohort with restored adrenal function (n = 52) the median time to recovery differed between subtypes: 0.6 years (interquartile range [IQR], 0.03-1.1 y) in ectopic CS, 1.4 years (IQR, 0.9-3.4 y) in CD, and 2.5 years (IQR, 1.6-5.4 y) in adrenal CS (P =.002). In CD the Cox proportionalhazards model showed that the probability of recovery was associated with younger age (hazard ratio, 0.896; 95% confidence interval, 0.822-0.976; P =.012), independently of sex, body mass index, duration of symptoms, and basal ACTH and cortisol levels. There was no correlation with length and extend of hypercortisolism or postoperative glucocorticoid replacement doses. Conclusions: Time to recovery of adrenal function is dependent on the underlying etiology of CS.
