Abstract

Bristol Myers Squibb announced on 19 May 2022 the presentation of scientific research across cancers and blood disorders at the 2022 American Society of Clinical Oncology (ASCO) Annual Meeting and the European Hematology Association (EHA) Congress that underscores the company's commitment to delivering transformational therapies for patients. Data from more than 140 company-sponsored studies, investigator-sponsored studies and collaborations evaluating compounds across 28 cancer types and blood disorders will be featured at the two meetings. Key data being presented by Bristol Myers Squibb at ASCO and EHA 2022 include: Landmark five-year analysis of CheckMate -227, the longest reported follow-up of a Phase 3 immunotherapy combination study in first-line metastatic non-small cell lung cancer (NSCLC), demonstrate long-term, durable survival outcomes with Opdivo (nivolumab) plus Yervoy (ipilimumab). Three-year follow-up data from CheckMate -9LA reinforce the long-term, durable survival outcomes of Opdivo plus Yervoy with two cycles of chemotherapy, in the first-line treatment of patients with metastatic NSCLC, including those with PD-L1 expression less than 1%. Analyses from the CheckMate -816 trial highlight the association between pathological response and improved event-free survival in patients with resectable NSCLC treated with Opdivo plus chemotherapy in the neoadjuvant setting. Overall survival and overall response rate data from the RELATIVITY-047 trial evaluating Opdualag (nivolumab and relatlimab-rmbw), the combination of nivolumab, a PD-1 blocking antibody, and relatlimab, a LAG-3 blocking antibody, demonstrate the clinical benefit of the company's third distinct checkpoint inhibitor in patients with advanced melanoma. These data were first disclosed at the Mar 2022 ASCO Plenary Series. With the longest reported follow-up of median overall survival from a Phase 3 advanced melanoma trial, seven and a half-year survival results will be presented on the combination of Opdivo plus Yervoy from CheckMate -067 in patients with advanced melanoma. First disclosure of data from the PILOT study of Breyanzi (liso-cel), in patients with relapsed or refractory large B-cell lymphoma after one line of therapy who were not intended for transplant, show substantial durable responses. A patient-reported outcomes analysis from PILOT also showed treatment with Breyanzi improved health-related quality of life measures for patients. Correlative analysis of characteristics of patients treated with Abecma (ide-cel) in the KarMMa and KarMMa-2 clinical trials, with CAR T product quality attributes, informs further insights into potentially optimizing patient selection for CAR T manufacturing and improving clinical outcomes. Long-term data from pivotal MEDALIST and BELIEVE studies highlight continued benefit of Reblozyl (luspatercept-aamt) for patients with lower-risk myelodysplastic syndromes and transfusion-dependent beta thalassemia (ASCO/EHA). Long-term survival results from the pivotal Phase 3 QUAZAR AML-001 study, highlight the survival benefit of Onureg (azacitidine tablets) following intensive chemotherapy (EHA). First dose expansion data for Eisai and Bristol Myers Squibb's co-developed antibody-drug conjugate MORAb-202, demonstrate anti-tumour activity in platinum-resistant ovarian cancer patients at two dose levels as well as benefits of exploration of body surface area dosing to optimize safety. Design and setting Prospective, randomized, controlled animal study in the experimental laboratory of a university. Subjects 42 male Wistar rats. Interventions The animals were divided into three groups. One half of the animals of each group underwent studies of intestinal functional capillary density and leukocyte adherence on venular endothelium by intravital fluorescence microscopy. In the other half of the animals mesenteric plasma extravasation (FITC albumin) was determined by intravital fluorescence microscopy. Treatment groups received endotoxin infusion of 2.5 mg/kg per hour (group 2 and 3) and 100 U/kg b.w. C1-INH (group 3) during the 2 h of endotoxemia. Measurements and results Endotoxemia resulted in a significant decrease in mucosal functional capillary density (18.5% vs. controls), which was reduced by C1-INH administration (9.5%). Treatment with C1-INH also significantly attenuated intestinal leukocyte adherence in submucosal venules (35% vs. endotoxin group) and mesenteric plasma extravasation (44% vs. endotoxin group). Conclusions C1-INH administration diminishes endotoxin-induced changes in the intestinal microcirculation during experimental endotoxemia. Background: Pure red-cell aplasia (PRCA) is a rare hematologic disease characterized by anemia, reticulocytopenia, and absence of bone marrow erythroid precursors. Most patients respond to some form of immunosuppressive treatment, but few prospective clinical trials have been performed. Objective: To examine the efficacy of a humanized monoclonal antibody to the interleukin-2 receptor (daclizumab) for treating PRCA. Design: Pilot study. Setting: Federal government research hospital. Patients: 15 patients with idiopathic PRCA. Intervention: Daclizumab, 1 mg/kg of body weight, every 2 weeks for a total of 5 infusions. Pure red-cell aplasia was defined as transfusion-dependent anemia with a reticulocyte count of 60 x [10.sup.9] cells/L or less and bone marrow showing absent or diminished erythroid precursors. Measurements: Response to therapy was assessed by transfusion independence, increments in reticulocyte count, and nontransfused hemoglobin. Results: Daclizumab had little toxicity. Of the 15 patients, 6 patients (40%) responded to treatment. Ali responders became transfusion-independent and achieved normal or near-normal hemoglobin values and normal reticulocyte counts. Limitations: The study was unblinded, and the number of patients was too small to assess drug safety reliably. Conclusions: Daclizumab seems safe. Its efficacy in this pilot protocol suggests that expanded study in PRCA and other bone marrow failure syndromes is warranted.

Introduction The American Academy of Pediatrics (AAP) Red Book Committee states that a 10-day course of a narrow-spectrum (first-generation) cephalosporin is an acceptable alternative to penicillin for treatment of group [...], The purpose of this study was to compare the bacteriologic and clinical efficacy of oral cephalexin twice vs. three times daily vs. cefadroxil once daily as therapy for group A beta-hemolytic streptococcal (GABHS) tonsillopharyngitis. A prospective open-label, observational cohort study was conducted over 18 months (January 2000-June 2001). Children enrolled had an acute onset of symptoms and signs of a tonsillopharyngeal illness and a laboratory-documented GABHS infection. Follow-up examination and laboratory testing occurred 21 ± 4 days following enrollment. Two hundred seventy-one patients were enrolled (intent to treat group): 63 received cephalexin twice daily, 124 received cephalexin three times daily, and 84 received cefadroxil once daily. Fifty-three children did not return for the follow-up visit, leaving 218 patients in the per-protocol group: 54 cephalexin twice-daily treated, 94 cephalexin 3-times daily treated, and 70 cefadroxil once-daily treated. In the per-protocol group, bacteriologic cure for those treated with cephalexin twice daily was 87%, for cephalexin 3 times daily, it was 81% and for cefadroxil once daily it was 81% (p=0.61). The clinical cure rate for cephalexin twice-daily treatment was 91%; for three-times daily, it was 86%; and for cefadroxil once daily, it was 84% (p=0.56). Because treatment allocation was not randomized, logistic regression analysis was used to adjust for treatment group differences. Younger age of patient was significantly associated with bacteriologic (p=0.04) and clinical (p=0.01) failure independent of treatment group but in the adjusted logistic model no differences were found among the 3 treatment regimens. Cephalexin dosed twice daily or three times daily and cefadroxil dosed once daily appear equivalent in bacteriologic and clinical cure of GABHS tonsillopharyngitis. Clin Pediatr. 2003;42:519-526

Hyperhomocysteinemia is an independent risk factor for cardiovascular diseases. Folic acid lowers the homocysteine concentrations in plasma in hyperhomocysteinemia. Whether this results in a reduced risk for cardiovascular diseases remains to be proven by intervention studies. In the present study we investigated: (1) if homocysteine concentrations are elevated in the plasma of children with renal failure and (2) the influence of folic acid administration on the plasma homocysteine concentration. The plasma homocysteine concentration was measured in 21 children, 9 on hemodialysis and 12 on peritoneal dialysis, before and 4 weeks after treatment with 2.5 mg folic acid daily. Healthy children (234) constituted the control group. In controls the median homocysteine concentration was 9.1 Aumol/l (range 4.3--20.0 Aumol/l). The median plasma homocysteine concentration in patients before folic acid treatment was 20.0 Aumol/l (Q1-Q3 13.7--26.0 Q, quartile). After 4 weeks of folic acid treatment the median plasma homocysteine concentration was 12.0 Aumol/l [Q1-Q3 9.8--14.3 (P&lt 0.0001 Wilcoxon signed rank test)]. There was no significant difference between hemodialysis and peritoneal dialysis patients. Children with renal failure treated with hemodialysis or peritoneal dialysis have elevated plasma homocysteine concentrations, but this is significantly reduced after administration of 2.5 mg folic acid daily for 4 weeks. It is suggested that folic acid be added to the treatment of children with renal failure, although a beneficial effect still has to be proven. The required dose needs further study. Abstract

Recombinant (genetically engineered) human granulocyte-macrophage colony-stimulating factor (GM-CSF) may be an effective treatment for patients with myelokathexis. Myelokathexis is a disorder characterized by the retention of cells in the bone marrow, leading to abnormally low blood levels of neutrophils, a type of white blood cell (WBC). Neutrophils of patients with myelokathexis are abnormal in appearance. Two sisters with myelokathexis were treated with GM-CSF. Both sisters experienced an increase in blood levels of neutrophils, and they no longer had neutrophils that were abnormal in appearance. Blood levels of other types of WBCs also increased, as did blood levels of immunoglobulins, or antibodies. GM-CSF may stimulate the release of neutrophils and other types of WBCs from bone marrow into circulating blood. The cause of myelokathexis is not known, but myelokathexis patients may not produce adequate amounts GM-CSF. Both sisters with myelokathexis had undetectable levels of GM-CSF before treatment.
