Medical Imaging, which allows for the non-invasive assessment of biological tissues, is a rapidly growing health care service that has evolved from a diagnostic tool to a platform for personalized precision medicine. Computerized Tomography (CT), which is a commonly obtained imaging study worldwide, utilizes X-ray radiation to differentiate tissues based on density differences. More complex imaging studies are obtained based on the clinical question to supplement the CT. Image overutilization, which is the acquisition of medical images that have minimal impact on patient care and increasing global inequities due to the cost/availability of medical imaging services are significant problems that need to be addressed. One possible solution would be to maximize the amount of clinically relevant information extracted from routine CT images using machine learning approaches. Recent developments in machine and deep learning have provided a powerful set of tools for the automated and complex analysis of medical images. Leveraging these techniques, it is possible to develop algorithms capable of learning from human annotations or paired images. These methods can also be applied to large datasets to minimise human input and to enable the rapid scale up of analyses. In this thesis, I hypothesize that Non-Contrast CT images contain higher-order information to differentiate tissue anatomy or pathology without the need of intravenous contrast agents or radioactive tracers. I further hypothesized that such higher-order information enables stratification of disease progression without the need of additional imaging studies. I focus on two pathologies, abdominal aortic aneurysms (AAA) and Head and Neck Squamous Cell Carcinoma (HNSCC) as demonstration of feasibility. The methods described in this thesis can be applied to other pathologies and will be poised to disrupt clinical pathways in the future.

7. An investigation into the immune landscape of the Abdominal Aortic Aneurysm-Thrombus complex and its systemic effects

Degenerative abdominal aortic aneurysms (AAA) are characterised by aortic smooth muscle apoptosis, elastin degradation and a chronic inflammatory cell infiltrate. The inciting antigen is yet unknown. AAAs share many features with autoimmune conditions whilst overlapping several risk factors with atherosclerosis. The majority of AAAs of an operable size are lined with intraluminal thrombus (ILT), which is in constant contact with the aortic wall and flowing arterial blood. Flow mediated dilatation (FMD) is a surrogate marker for endothelial dysfunction and a predictor of future cardiovascular events. Patients with AAAs have a deteriorating FMD, which improves after surgery, thus implicating the aneurysm-thrombus complex as a potential driver of the systemic endothelial dysfunction. The immunological contribution of ILT to the pathology is under investigated. This thesis aims to comprehensively define the immune landscape of the aortic wall and intraluminal thrombus through deep phenotyping, thus providing new insights into the mechanisms of degeneration in advanced stage aneurysms. Furthermore, it aims to explore a mechanistic link between ILT and systemic endothelial dysfunction. Patients were prospectively recruited on to the Oxford Abdominal Aortic Aneurysm study. Peripheral blood, aortic wall and ILT specimens were collected intraoperatively and optimised for deep phenotyping with Time-of-flight mass cytometry (CyTOF). Cell sorting the aortic complex tissues provided high quality single cell suspensions for discovery analyses using different approaches. The aortic wall was replete with T and B cells demonstrating new evidence of T and B cell crosstalk. In particular, T follicular helper cells were identified, thus providing evidence for class-switching to memory B cells in the aortic wall. The aortic and ILT lymphocytes contained the cytotoxic granule Granzyme B. This is a novel alternate mechanism of tissue destruction in late stage disease and its finding in the peripheral blood suggests a role as a potential biomarker. The myeloid cells had significant overlap in their markers of expression and the ILT also contained active monocyte derived dendritic cells, which is another novel finding. Both the T cells and myeloid cells had no dominant effector phenotype and rather represented a complex continuum aiming to achieve homeostatic balance. Finally, the ILT secretome was found to be cytotoxic to aortic endothelial cells and induced changes in gene expression consistent with endothelial dysfunction. This thesis defines in detail the immune landscape of the aortic aneurysm-thrombus complex. It highlights the involvement of both innate and adaptive immune systems, as well as pro- and anti-inflammatory pathways involved in chronic non-resolving inflammation. The finding that ILT induces gene changes of endothelial dysfunction provides a novel mechanistic link for the changes in FMD of patients with AAA. These observations highlight potential targets for translational therapeutics.

The book “Vascular Surgery: Procedures, Complications and Recovery''was written by a team of vascular, cardiac and neurosurgeons working mainly in the East Slovak Institute of Cardiovascular Diseases, and also Luis Pasteur University Hospital in Kosice, Slovakia. The book is addressed to students of medicine and also as fundamental principles of vascular surgery for residents in different surgical specializations. Vascular surgery has been rapidly developing in recent decades. Although the first successful arterial reconstructions appeared the beginning of the 20th century, the active management of arterial diseases has developed since the 1950s when new techniques of arterial surgery had been established. The first attempts of vascular reconstructions were performed by Alexis Carrel, a French surgeon who was awarded the Nobel Prize in Medicine in 1912 for inventing modern vascular suture, which is used still today. Significant development of vascular surgery brought the introduction of prosthetic grafts by Vorhees in 1952. That enabled De Backey to resect abdominal aortic aneurysm and replace it with synthetic graft in 1955. The first angiography with the catheter was performed by Seldinger in 1952, and Gruntzig in 1974 was the first who did percutaneous transluminal angioplasty and started the modern era of endovascular treatment. Since this time, an essential development in the treatment of vascular diseases has been observed. Nowadays, vascular surgeons solve the most severe cases of arterial diseases, solve complications of endovascular therapy, or are a part of hybrid procedures, which are a combination of surgical and endovascular treatment. In the first chapter, elementary clinical examinations, imagine methods, and also surgical techniques are described. Aortic disorders that are the most severe vascular diseases are analysed in the second, third, and forth chapters. Open aortic surgery is technically the most difficult part of vascular surgery and is associated with the higest mortality. Open aortic surgery is now being replaced by endovascular procedures due to lesser invasivity and mortaliy. The fifth chapter is dedicated maily to surgery of carotid arteries. Stenosis of the internal carotid artery is one of the reasons for stroke. Carotid endarterectomy that removes atherosclerotic plaque from the carotid bifurcations is effective in preventing stroke in patients with stenosis of the internal carotid artery. The sixth chapter analyses the problem of surgery of periferal arteries as periferal arterial occlusive disease or acure limnb ischemia. Both conditions can lead to major amputations and lifelong disability. The seventh capter analyses vascular trauma that may lead to severe bleeding or limb ischemia; both situations require almost immediate diagnosis and treatment. Periferal artery trauma is treated mainly by open surgery, while the treatment of aortic injury is mostly endovascular. Basic types of vascular access and problems of venous disorders are decribed in the eighth chapter. Venous diseases are not so life threatening but much more common compared to arterial. The last chapter deals with complications and recovery that is inevitable from an excellent surgical outcome.
