Abstract

Tumor necrosis factor-alpha (TNF-α) inhibitors represent an effective treatment for severe psoriasis in hepatitis C virus (HCV) patients. The literature reports mainly on short-term treatment in patients with chronic hepatitis with minimum-to-moderate activity with an acceptable safety profile. We report the first 2 cases of hepatocellular carcinoma (HCC) arising in HCV psoriatic patients with advanced liver disease during long-term treatment with etanercept. Our first patient, known to have had HCV infection for 41 years, developed an HCC after 21 months of therapy with etanercept (50 mg/week). The second patient, HCV+ for 20 years, was treated for 58 months with the same therapy, and despite no signs of liver function impairment was diagnosed with HCC. Both of them presented with cirrhosis, which was diagnosed 9 and 5 years earlier, respectively. It remains to be clarified whether there is any connection between psoriasis treatment with anti-TNF-α agents and the development of HCC in HCV-infected patients. Further long-term, follow-up studies and registries of HCV patients with mild/moderate activity may contribute to clarify this issue. [ABSTRACT FROM AUTHOR]

Inhibitoren des Tumornekrosefaktor alpha (TNF-a-Hemmer) stellen eine wirksame Therapieoption zur Behandlung schwerer Psoriasis bei Hepatitis-C-Virus (HCV)-positiven Patienten dar. In der Literatur wird vorwiegend über die Kurzzeitbehandlung von Patienten mit chronischer Hepatitis von minimaler bis moderater Aktivität berichtet; bei akzeptablem Sicherheitsprofil. Wir berichten über die ersten beiden Fälle der Entstehung eines hepatozellulärem Karzinoms (HCC) bei HCV-positiven Psoriasis-Patienten mit fortgeschrittener Lebererkrankung während einer Langzeitbehandlung mit Etanercept. Beim ersten Patienten war die HCV-Infektion seit 41 Jahren bekannt; er entwickelte nach 21-monatiger Behandlung mit Etanercept (50 mg/Woche) ein HCC. Der zweite Patient war seit 20 Jahren HCV-positiv, wurde 58 Monate lang mit demselben Therapieschema behandelt und schließlich HCC-diagnostiziert, und obwohl keine Anzeichen einer Leberfunktionsstörung vorlagen. Beide Patienten hatten sich mit einer Zirrhose vorgestellt, die 9 bzw. 5 Jahre zuvor diagnostiziert worden war. Es bleibt zu klären, ob zwischen der Psoriasis-Behandlung mit einem Tnf-a-Hemmer und der Entwicklung des HCC bei den HCV-infizierten Patienten ein Zusammenhang besteht. Weitere Langzeit-Follow-up-Studien und Register von HCV-Patienten mit leichter/moderater Aktivität könnten zur Klärung dieser Frage beitragen.

Abstract

The article discusses the diagnostic challenges of Atypical Spitz Tumor (AST), a rare melanocytic lesion that can be mistaken for spitzoid melanoma. The use of Line-field confocal optical coherence tomography (LC-OCT) is introduced as a non-invasive technique to aid in distinguishing AST from benign Spitz nevus, particularly in pediatric patients. LC-OCT provides 3D reconstructions of the skin at cellular resolution, highlighting architectural disorder and cytological atypia within melanocytic nests. Further studies are needed to differentiate AST from other entities, and patient consent was obtained for the publication of case details. [Extracted from the article]

Abstract

Background: The management of paediatric atopic dermatitis (AD) is challenging, mostly relying on emollients and topical corticosteroids. Dupilumab, a fully human monoclonal antibody, has been recently approved for the treatment of children aged 6-11 years with moderate-to-severe AD not adequately controlled with topical therapies or when those therapies are not advisable. Objectives: The aim of this study was to evaluate in real life the effectiveness and safety of dupilumab in the treatment of children aged from 6 to 11 years. Methods: Demographic and clinical data of children aged 6-11 years, affected by moderate-to-severe AD and treated with dupilumab, were retrospectively collected from 24 dermatological and paediatric referral centres. Dupilumab was administered subcutaneously at an induction dose of 300 mg on day (D) 1, followed by 300 mg on D15 and 300 mg every 4 weeks. Disease severity was assessed at baseline and after week 2 (W2), W4 and W16 of dupilumab therapy using Eczema Area Severity Index (EASI), Pruritus Numerical Rating Scale (P-NRS) and Sleep NRS (S-NRS) and Children's Dermatology Life Quality Index (c-DLQI) score. Results: A total of 55 AD children (24 males [43.64%], 31 females [56.36%]; mean age 9.35 ± 1.75 years) were included. A significant improvement in EASI score, P-NRS, S-NRS and c-DLQI was observed from baseline to W16 of treatment with dupilumab. In particular, at W16 the proportion of patients achieving EASI75 was 74.54%. Moreover, at the same timepoint a significant mean percentage reduction for P-NRS, S-NRS and c-DLQI was also observed (68.39%, 70.22% and 79.03%, respectively). Conclusions: Our real-life data seem to confirm the effectiveness of dupilumab in paediatric patients on all disease aspects, including extent and severity of signs, intensity of symptoms, sleep and QoL, with a good safety profile.

Abstract

Objective: The prevalence of anxiety and depression in patients diagnosed with Alopecia Areata (AA) is very high and this significant burden of psychological symptoms threatens the Health-Related Quality of Life (HRQoL) of affected patients. Indeed, AA often does not produce significant physical symptoms, but it nonetheless disrupts many areas of mental health. Clinical assessment of disease severity may not reliably predict patient's HRQoL, nor may it predict the patient's perception of illness. For this reason, considerable effort has been made to apply and develop measures that consider patient's perception and assess the HRQoL of individuals affected by AA. The aim of this multicentric study was to provide the Italian version of the Skindex-16AA and to evaluate its psychometric properties in a clinical sample of consecutive patients with moderate-to-severe AA., Methods: This is a longitudinal, multicenter, observational study. Patients returned for follow-up visits at 4-, 12-, and 24-weeks. The analyses of the current work aimed to confirm the factorial structure of the Skindex-16AA. In the case of non-fit, an alternative structure for the model was proposed, using an Exploratory Graph Analysis and the Bayesian approach., Results: The sample was composed of 106 patients with AA. Alopecia Universalis was the most frequently diagnosed type of alopecia at all time points. The analyses on the Skindex-16AA revealed that a two-factor structure with eight items fit the data best (Bayesian Posterior Predictive Checking using 95% Confidence Interval for the Difference Between the Observed and the Replicated Chi-Square values = -6.246/56.395, Posterior Predictive P -value = 0.06), and reported satisfactory psychometric properties (i.e., internal consistency and convergent validity)., Conclusion: The Skindex-8AA demonstrated optimal psychometric properties (i.e., convergent and construct validity, and test-retest reliability) measured in a sample of patients with AA, that may suggest that it is an appropriate tool to measure the HRQoL in AA patients. Abstract

Colloid milium (CM) is a rare cutaneous condition characterized by translucent papules occurring on sun-exposed regions including the face, neck and dorsal aspects of the hands and back. Clinically, there are two variants of CM: an adult-onset type and a juvenile form. The juvenile form is inherited and presents before puberty. Probably this variant is because of an inherited susceptibility to ultraviolet (UV) light and can be transmitted as both autosomal recessive and autosomal dominant character. In this paper, we report an interesting case of adult CM in a transfused patient affected by beta thalassaemia major. The association of CM with beta thalassaemia, to our knowledge, has not been reported previously, in literature. Thus, this case represents the first case of CM associated with beta thalassaemia major. In our view, the lesion could be related to excess iron, similar to pseudoxanthoma elastic-like lesions, another cutaneous disorder which is present in beta thalassaemia. As our patient is a farmer and was exposed to sun during his work, UV light damage could have have a role in promoting the development of the disease. Other cases of CM associated with beta thalassaemia should be reported to confirm these hypotheses.
