Abstract

Methods: The National Enhanced Cancer Surveillance System (NECSS) is a Canadian population-based case-control study conducted in 1994-1997. Incident kidney cancer cases were identified using provincial registries, while the control series was identified through random-digit dialing, or provincial administrative databases. Self-reported questionnaires were used to obtain information on lifetime occupational history and cancer risk factors. Two hygienists, blinded to case status, coded occupational histories for diesel and gasoline exhaust exposures using concentration, frequency, duration, and reliability. Logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals (CIs) separately by exhaust type. The separate and combined impacts of both engine exhausts were also examined. ORs were adjusted for age, province, body mass index, occupational secondhand smoke exposure, and education.Results: Of the kidney cancer cases (n = 712), 372 (52%) had exposure to both exhausts at some point, and 984 (40%) of the controls (n = 2457) were ever exposed. Abstract

Objective: Previous studies considered the role of occupational causes in kidney cancer but were limited by small sample sizes and imprecise exposure assessment. This study examined the relationship between occupational exposure to asbestos and the risk of kidney cancer across a range of jobs in a large, population-based case-control study in Canada. Methods: Data were from the case-control component of the National Enhanced Cancer Surveillance System, a study conducted between 1994 and 1997 in eight Canadian provinces. Male kidney cancer cases, histologically confirmed, and controls completed questionnaires on socio-demographics, anthropometry, diet, smoking, secondhand smoke exposure, and physical activity. 19. 'Failed' contraception in a rural South African population

Abstract

Objective. To investigate whether the free availability of contraception affects the need for termination of pregnancy (TOP).Design. Case-control study.Setting. South African rural hospitaLPopulation. Three thousand and ninety-five TOP seekers and 439 non-pregnant controls.Methods. Structured questionnaire followed by ultrasonography.Main outcome measures. Current use or recent discontinuation of contraception and the reason for discontinuation.Results. Less than one-third (28.6%) of TOP seekers claimed to be using contraception versus 85.0% of controls. Injectable contraception (IC)was preferred by the controls and oral contraception (OC) by TOP seekers (x2 = 48.5, p < 0.0001, OR 0.34 (95% confidence interval 0.25, 0.46)). The percentage of discontinuation of hormonal contraception was higher in controls (x2 = 6.3, p = 0.012, OR 0.51 (0.31, 0.85)). The reason for discontinuation of hormonal contraception was obtained from 31.2% of TOP seekers and 63.3% of controls; no reason for discontinuation was acknowledged by 30.1% of the former and 6.3% of the latter (x2 = 33.4, p < 0.0001, OR 6.40 (3.25, 12.56)). Side-effects of hormonal contraception prompted more discontinuation in the failed-contraception group (x2 = 120.5, p < 0.0001, OR 49.4 (21.6, 112.5)). Poor compliance and absence of an acknowledged reason for discontinuing hormonal contraception resulted in 173 unwanted pregnancies. Conclusion. In South Africa two main components of women's reproductive health and rights are freely available, namely contraception and TOP. Not using contraception is one of the main causes of unwanted pregnancy. Better education of both service providers and users is needed to improve use, compliance and perseverance with contraception.

Abstract

Introduction: Paucity of evidence on good practices in the rehabilitation of patients requiring single-and double level lumbar fusion surgery (LFS), leads to uncertainty and extensive variability in current rehabilitation regimes. Therefore, this study aims to formulate consensus statements on the optimal rehabilitation of LFS, representing the viewpoints of Belgian and Dutch experts on spinal surgery and rehabilitation. Methods: A modified Delphi study consisted of a 3-round online Delphi questionnaire, followed by an in-person consensus meeting. In each round, experts could suggest new items, and received feedback for reconsidered statements. The threshold for consensus agreement was set at =75%. Afterwards, perspectives of nine patients that underwent LFS were solicited through a questionnaire and patient focus group. Results: A total of 31 experts participated in the first online round, of which 27 (87%) completed all online rounds, and 17 (55%) attended the in-person consensus meeting. Consensus was reached on 122 statements relating to the pre-, peri-and postoperative rehabilitation of LFS, including the importance of uniform communication, patient-specific education and specific physiotherapeutic interventions. Patient perspective on optimal rehabilitation was gained from nine patients. Discussion: The final compilation of 122 consensus statements is a solid base for developing and implementing a novel and much-needed rehabilitation pathway for LFS. Benchmarking this consensus against patient perspectives helps understanding patient expectations and potential barriers for implementation. Process evaluation: These results represent the consensus established by Belgian and Dutch experts.
