Abstract

Beroepsbeoefenaren in de gezondheidszorg worden hoe langer hoe meer geconfronteerd met structuren waarin ze moeten functioneren, regelingen waaraan ze zich moeten houden en partijen waarmee ze moeten overleggen en tot overeenstemming dienen te komen. In een instelling moeten ze marktgericht werken, voldoen aan prestatie-indicatoren en met patiënten/ cliënten-vertegenwoordigers rond de tafel. De organisatie van de gezondheidszorg is complex en veeleisend en voortdurend aan verandering onderhevig. Om als (toekomstig) beroepsbeoefenaar volwaardig en met succes en voldoening binnen de gezondheidszorg te kunnen werken, moet die organisatie wel gekend en doorzien worden. Dit boek wil daarvoor de kennis en het inzicht bieden. In deze vierde druk zijn de ontwikkelingen in de organisatie van de gezondheidszorg van de afgelopen jaren verwerkt en de meest recente cijfers opgenomen. Het boek geeft daarmee weer een goed en samenhangend overzicht van de vele facetten binnen organisatie en beleid in de gezondheidszorg in Nederland. Als vanouds biedt dit boek inzicht in de structuur en het functioneren van de gezondheidszorg, waarbij de actualiteit een logische plaats krijgt in het licht van de beschreven ontwikkelingen door de jaren heen. Zo blijven bijvoorbeeld nieuwe wetgevingen en veranderende rollen van nationale en gemeentelijke overheid niet onbesproken in deze vierde herziene druk. Organisatie van de gezondheidszorg kan als (studie)boek in zijn geheel gebruikt worden, maar de hoofdstukken zijn ook zo geschreven, dat ze los van elkaar gebruikt kunnen worden. Jan Maarten Boot was tot 1 februari 2014 als universitair hoofddocent beleid en management gezondheidszorg verbonden aan het UMC Utrecht (Julius Centrum voor Gezondheidswetenschappen en Eerstelijnsgeneeskunde). Hij is actief als docent en consultant beleid en management gezondheidszorg.

Abstract

In recent days, the products and services provided by banks have moved to the digital domain more and more. Since most people rely on these services in their day to day lives, availability and reliability are very important in the success of a bank. In an effort to increase automation in this regard, ING Bank wants to employ machine learning to predict failure and alert a response system which will then try to put the system back into normal operation. To achieve this a better understanding of the intricacies of the interaction between system prior to and during failure is required. By creating an interactive way of visually playing back the event logs that describe the interaction between these systems we provide a first step in this understanding. Currently a lot of ING systems log their events to a database called TracING. These logs include a lot of information which is described in more detail in subsection 4.2. Logs extracted from this database are transformed into a report of metrics in the format of an interactive HTML page, where they can be visualised through time. Furthermore, some static metrics are also generated such as charts that showcase event success and failure for various durations. The initial focus of the project was to implement the machine learning prediction, but due to lack of classifiers in the provided data, the focus shifted towards visualising the data. Because of this, simple feature vector creation and a support vector machine have also been implemented as a basis for future work. Item does not contain fulltext The Netherlands has approximately 20,000 registered HIV-infected patients. The current HIV prevention policy consisting of condom use and active HIV testing does not effectively mitigate the HIV epidemic in all risk groups. In July of 2012, tenofovir/emtricitabine (TDF/FTC) was approved by the American Food and Drug Administration (FDA) for pre-exposure prophylaxis (PrEP) for long-term use in persons who exhibit frequent risky and unsafe sexual behaviour. With once-daily use and good therapy compliance, TDF/FTC has proved to be effective as PrEP, and few side effects are reported. Drawbacks in the use of TDF/FTC as PrEP are the potential risk of viral resistance and reduced condom use, the relatively high cost and the intensive counselling required. In special cases, long-term PrEP could enhance the current Dutch preventive policy. Further research is needed into the practical feasibility and protective efficacy of the ad hoc use of TDF/FTC as PrEP before a high-risk contact occurs.
