The study objective was to estimate the long-term risk of skin cancer and lymphoma associated with topical TCI use in adults and children, separately.Patients and Methods: A cohort study in Denmark, Sweden, UK, and the Netherlands was conducted. Adjusted incidence rate ratios (IRRs) and 95% confidence intervals (CIs) were calculated for nonmelanoma skin cancer (NMSC), melanoma, cutaneous T-cell lymphoma (CTCL), non-Hodgkin lymphoma (NHL) excluding CTCL, and Hodgkin lymphoma (HL) in new users of TCIs versus users of moderate/high-potency topical corticosteroids.Results: The study included 126,908/61,841 adults and 32,605/27,961 children initiating treatment with tacrolimus/pimecrolimus, respectively. Follow-up was ≥ 10 years for 19% of adults and 32% of children. Incidence rate ratios and (95% confidence intervals) for tacrolimus versus corticosteroid users in adults were < 1 for melanoma, non-Hodgkin lymphoma, and Hodgkin lymphoma; and 1.80 (1.25– 2.58) for cutaneous T-cell lymphoma. For pimecrolimus, IRRs in adults were < 1 for non-Hodgkin lymphoma, cutaneous T-cell lymphoma, and Hodgkin’s lymphoma; and 1.21 (1.03– 1.41) for melanoma; and 1.28 (1.20– 1.35) for nonmelanoma skin cancer. In children, results were inconclusive due to few events. In adults, incidence rate ratios ≥ 5 years after first topical calcineurin inhibitor exposure were not higher than in overall analyses.Conclusion: Overall, we found little evidence associating use of topical calcineurin inhibitors with skin cancer and lymphoma; confounding by indication, surveillance bias, and reverse causation may have influenced these results. The Dirac equation for an electron in an external electromagnetic field can be regarded as a singular set of linear equations for the vector potential. Radford's method of algebraically solving for the vector potential is reviewed, with attention to the additional constraints arising from non-maximality of the rank. Abstract

Human actions are causing pervasive declines in biodiversity and ecosystem services. Sharks, rays and their cartilaginous relatives (Class Chondrichthyes, herein ‘sharks’) are amongst the most threatened taxa on earth, primarily due to overfishing. Oceanic shark populations have declined by more 70% in the past 50 years, and over one third of shark species and threatened with extinction. Technologies and practices that reduce impacts of fisheries on sharks are relatively well documented, yet little is known about how to incentivise adoption across different types of fisheries while also managing trade-offs between conservation objectives and the important socio-economic functions of fisheries. This thesis seeks to fill this knowledge gap, by exploring approaches which can simultaneously deliver conservation outcomes for sharks and well-being outcomes for people. I begin by critically assessing current approaches for understanding and managing threats to sharks, and highlight gaps relating to socio-economic issues. I then develop a general decision-making framework, based on the mitigation hierarchy, which can integrate socio-economic issues to support least-cost shark conservation. The remainder of the thesis then focuses on research methods and conservation approaches for understanding and addressing socio-economic implementation gaps in shark conservation. I take a dual approach, exploring: 1) fisher behaviour and behavioural interventions in small-scale fisheries (SSFs) in Indonesia, and 2) broader structural interventions, with a focus on market-based approaches. In the SSFs, I empirically investigate the socio-economic drivers of shark fishing, and barriers to pro-conservation behaviour; assess the impact of an existing intervention for manta ray conservation, and document lessons learned; and use predictive methods to explore the cost-effectiveness of hypothetical incentive-based interventions. I find that a range of micro- and macro-level drivers influence shark fishing mortality in SSFs, including: basic needs for food and livelihoods, socio-cultural values, incidental catches, and profit motivations. These drivers interact and vary across fisheries. I also find that incentive-based approaches could tackle these drivers, whilst also maintaining the well-being of coastal communities, provided they are well-designed and implemented as part of an intervention mix. In terms of structural interventions, I explore financing mechanisms for shark conservation, based on operationalising the beneficiary-pays and polluter-pays principles. I demonstrate that bycatch levies could offer a market-based approach for incentivising bycatch reduction in commercial fisheries, and generating revenue for compensatory conservation which could support delivery of no net loss or net gain for marine biodiversity. I also find that marine tourists who benefit from healthy shark populations are willing to pro-actively pay for shark conservation outcomes. Together, these two income streams could generate billions of dollars in ocean finance, and fund direct investments or performance-based payments for marine conservation in SSFs. I conclude by demonstrating how these behavioural and structural interventions could be drawn together as part of an integrated strategy, and offer some recommendations for future research and practice. Overall, my findings highlight the importance of a more holistic and interdisciplinary approach to shark conservation, and illustrate some feasible strategies and next steps for achieving better outcomes for sharks and people. If well implemented, these findings could contribute towards delivering the post-2020 global biodiversity strategy, the sustainable development goals, and a sustainable and equitable ocean economy.

Abstract

ObjectiveTo evaluate whether eculizumab helps patients with anti–acetylcholine receptor–positive (AChR+) refractory generalized myasthenia gravis (gMG) achieve the Myasthenia Gravis Foundation of America (MGFA) post-intervention status of minimal manifestations (MM), we assessed patients' status throughout REGAIN (Safety and Efficacy of Eculizumab in AChR+ Refractory Generalized Myasthenia Gravis) and its open-label extension.MethodsPatients who completed the REGAIN randomized controlled trial and continued into the open-label extension were included in this tertiary endpoint analysis. Patients were assessed for the MGFA post-intervention status of improved, unchanged, worse, MM, and pharmacologic remission at defined time points during REGAIN and through week 130 of the open-label study.ResultsA total of 117 patients completed REGAIN and continued into the open-label study (eculizumab/eculizumab: 56; placebo/eculizumab: 61). At week 26 of REGAIN, more eculizumab-treated patients than placebo-treated patients achieved a status of improved (60.7% vs 41.7%) or MM (25.0% vs 13.3%; common OR: 2.3; 95% CI: 1.1–4.5). After 130 weeks of eculizumab treatment, 88.0% of patients achieved improved status and 57.3% of patients achieved MM status. The safety profile of eculizumab was consistent with its known profile and no new safety signals were detected.ConclusionEculizumab led to rapid and sustained achievement of MM in patients with AChR+ refractory gMG. Abstract

The need for conservation action to be cost-effective is widely accepted, resulting in increased interest and effort to assess effectiveness. Assessing the financial and economic costs of conservation is equally important for assessing cost-effectiveness, yet their measurement and assessment are repeatedly identified as lacking. The healthcare sector, in contrast, has made substantial progress in identifying and including costs in decision-making. Here, we consider what conservation can learn from this experience. We present a three-step framework for identifying and recording the relevant economic costs and benefits of conservation interventions where the user (1) describes the costing context, (2) determines which types of cost and benefit to include, and (3) obtains values for these costs and benefits alongside metadata necessary for others to interpret the data. This framework is designed to help estimate economic costs but can also be used flexibly to record the direct costs of interventions (i.e., financial costs) and calculate financial and economic benefits. Although recording data on economic costs and benefits is deceptively complex, this framework facilitates improved recording, and indicates how collating this data could enhance the assessment of cost-effectiveness across conservation contexts using a range of decision-making tools. © 2022 The Authors. 23. Is children’s education associated with parental health? Evidence from the Philippines

Abstract

This study examines the association between children’s education and parental health using data from the 2007 Philippine Study on Ageing. It employs a broad, more comprehensive, definition of health to capture the different health dimensions. By employing multiple indicators of health, this study is able to examine whether the influence of children's education is consistent across different health indicators. It also investigates whether parental behavior and receipt of support from children serve as pathways that mediate the relationship between children’s education and parental health. Findings show that older women whose children completed tertiary education have lower odds of reporting IADL or ADL difficulty compared with their counterparts whose children attained below tertiary education. These findings contribute to the growing evidence that education is not only an individual resource; rather it could be a household or family resource that could benefit other family members.

25. Assessing the impact of regulations on the use and trade of wildlife: an operational framework, with a case study on manta rays

Overexploitation represents a significant threat to wildlife, with the severest impacts felt by slow growing, economically valuable species. Governments often seek to address this through regulating utilisation and trade of species, which is commonly catalysed by multi-lateral environmental agreements (MEAs) such as the Convention on the International Trade of Endangered Species (CITES). However, it is often unclear to what degree CITES and associated regulations lead to tangible conservation outcomes. Robust impact assessments are needed to understand whether regulations are effective for achieving biodiversity conservation goals, and to learn lessons for future policy interventions. Yet such assessments are hindered by data paucity, bias, complexity and uncertainty. Here we discuss key challenges for assessing the impact of regulations on the use and trade of wildlife, and offer a practical approach to overcome them. Our approach combines an integrated framework for collating and analysing disparate and methodologically inconsistent data with a robust process to establish causal inference (and hence assess impact). This framework and process can be applied to any regulation, species or country context. To demonstrate its utility we apply this approach to the case of manta ray utilisation and trade (Mobula alfredi and M. Birostris) in Indonesia. This case study is particularly important due to the recent increase in the number of commercially important elasmobranchs listed in CITES appendices, with Parties adopting various national-level regulations to implement their CITES commitments. However, it is unclear to what degree these listings lead to meaningful regulatory reform, and much-needed reductions in fishing mortality, utilisation and onward trade. Indonesia is also a priority country for effective regulatory reform, due to its role as a major source country for international elasmobranch trade. Overall, we highlight challenges and opportunities for assessing the impact of wildlife trade regulations, which are generalisable across species and contexts, and provide the first attempt to assess the impact of such regulatory change on manta ray mortality in a source country. We also offer recommendation for future implementation and evaluation, emphasising the importance of mixed-methods, multiple datasets, and explicit acknowledgement of bias and complexity.

Abstract

Among the several models of fertility that have been developed, the Gompertz function has been shown to perform quite well. The fit to cumulative fertility by age of women is better in the middle age range than in the tails of the distribution, especially for high fertility populations. An empirical transformation of the age scale is developed to produce a better fit to the entire reproductive period. The substitution of age by this transformed scale results in the transformed Gompertz model of fertility: F(x)=FPQYs(x) 0 < P, Q < 1 0 < F where F(x) is cumulative fertility by age of women, x, F is completed fertility, P and Q are parameters and Ys(x) is the transformed age scale. The model can also be written as: Y(x)= a+ BYs(x) where Y(x)= -1n(-1n F(x)/F), a=-1n(-1nP) and B=-1nQ. Ys(x) is obtained by averaging over a selection of transformed patterns of high fertility schedules generated by the empirically based Coale-Trussell model of fertility. The model is tested using several sets of good quality fertility rates for birth cohorts of women, including a series of simulated rates developed especially for this purpose. Both the goodness of fit of the model and its projection capabilities from incomplete cohort data are shown to be good. Fitting is by the least squares method with equal weights. Several sets of poorer quality, high fertility, cohort data obtained from maternity histories collected in Bangladesh, Sri Lanka and West New Guinea are used to illustrate the application of the model. Fitting for these data is by least squares with an infinite weight on reported parity at the time of the survey. The estimates of completed fertility are plausible, and the fitted curves provide evidence of reporting errors in the data. There is also evidence of trends over time in the level and pattern of fertility.
