Abstract In the past two decades, Europe has witnessed a significant transition in the design codes used for assessing foundation structures, with the widespread adoption of the Eurocodes (EC). This shift remains a pertinent topic within the engineering community, particularly concerning the transition from traditional design methodologies to those prescribed by the Eurocodes, as well as the potential for fully probabilistic design. While the Eurocodes’ methodology is described as probabilistic, it is crucial to recognize that the achievement of the target reliability level is predominantly facilitated through a system of partial safety factors. These factors are integrated into the calculation algorithm as fixed values, rendering the process essentially deterministic. To refine these calculations for more accurate reliability estimates—expressed in terms of failure probability—a genuinely probabilistic framework is required, termed as fully probabilistic computation. This paper aims to elucidate the fully probabilistic calculation approach for the broader professional community, using the geotechnical application of shallow foundations as an illustrative example. We present a comparative analysis of this advanced approach with the standard foundation design according to EC7 and ČSN 731001, the latter being a precursor in Europe for implementing the partial safety factor method. The discussion extends to a practical demonstration of full probabilistic design juxtaposed against the conventional partial safety factor method, using a shallow foundation case study. Furthermore, the paper delves into the impact of the tail behavior of uncertain or spatially varying soil parameters on the theoretical probability of failure, underscoring its significance in foundation design.

In this paper, we give a comparative study of several planar antenna concepts for reliable long range links in a tactical environment. The antenna elements are studied in terms of their electrical properties (bandwidth, reflection coefficient and radiation characteristics) and construction (robustness and material consumption). First, we model single antenna elements to investigate if they meet the requirements. Second, we arrange the elements with the best features into 2x2 arrays. Computer simulations of the arrays are verified by measurements. Finally, we formulate recommendations for large array (8x8 or 16x16 elements) synthesis to achieve the required properties. Although Francisella (F.) tularensis is a well-described and understood zoonotic pathogen, its importance in Central Europe is relatively minor and, as such, tularaemia may be missed in the differential diagnosis. The annual incidence of tularaemia in the Czech Republic is relatively stable with up to 100 reported cases per year, except in the epidemic years 1998 and 1999 with 225 and 222 reported cases, respectively. It is, however, higher in comparison with the neighbouring countries. The common route of transmission in Central Europe is handling infected animals. Tularaemia is not commonly recognized as a tick-borne disease. Here we report two rare cases of a tick bite-associated ulceroglandular form of tularaemia in 2.5-year-old and 6.5-year-old children presenting with cervical lymphadenopathy. The unusual and interesting features of those cases are the young age and relatively uncommon route of transmission suggesting possible changes in the epidemiology of tularaemia in the Czech Republic. Therefore, the infection with F. tularensis should be considered in the differential diagnosis after a tick bite even in infants. [ABSTRACT FROM AUTHOR]

Bacterial ocular infections represent a common public health problem affecting people of all age groups. These infections can lead to damage of ocular structures or even a loss of vision. The spectrum of isolated bacteria and their susceptibilities to antibiotics, however, shows geographical variabilities, which can affect the success of most empirically-administered antimicrobial therapies. The aim of this study was thus to analyse bacterial aetiology in culture-positive acute and chronic ocular infections and its antimicrobial susceptibility profile in a large cohort of patients in the Czech Republic. The study also focused on corynebacteria identification, particularly on the prevalence of Corynebacterium macginleyi. A total of 2500 bacterial isolates obtained from 2015 to 2020 in University Hospital Hradec Kralove were included in the study. A total of 2320 (92.8%) bacterial isolates were Gram-positive and 180 (7.2%) were Gram-negative. Staphylococcus aureus was the predominant pathogen, isolated from 15.3% of ocular infections, followed by Enterobacterales, Streptococcus pneumoniae and Haemophilus influenzae, isolated in 2.9%, 1.6% and 1.0%, respectively. Corynebacterium macginleyi was confirmed as the most prevalent species of corynebacteria. Most bacteria showed good susceptibility to fluoroquinolones, chloramphenicol, and aminoglycosides. Gram-positive bacteria were also susceptible to tetracycline. To conclude, this study presents a 5-year assessment of bacterial aetiology of ocular infections in the East Bohemian region. The survey showed clear differences in the susceptibilities of several bacteria to select antibiotics compared to studies from other geographical regions in Europe. This clearly shows that local surveillance of the aetiology and antimicrobial susceptibility of bacteria is essential for adequate empirical therapy of ocular infections.

Natural killer (NK) cells represent an important component of the human body´s defense against viruses, bacteria and other intra-cellular pathogens as well as against tumor cells. NK cells are capable of very fast response without prior sensitization. They produce a wide range of cytokines and chemokines regulating both innate and acquired immune responses. Similar to excessive physical activity, stress can lead to a reduction in the numbers of NK cells, consequently increasing the susceptibility of the organism to viral, bacterial or parasitic diseases. Historically, NK cells were viewed as natural killers with relatively simply regulated functions. But new findings have shown that NK cells play roles in many processes and regulation of their functions in many infectious diseases represents a complex issue. This review presents new potential roles of NK cells in viral infections, autoimmunity and their specific functions in some organs.

Aims: Mycoplasma hominis and Ureaplasma urealyticum are potentially pathogenic bacterial species that are frequently isolated from the urogenital tract of women. These pathogens could be responsible for various genitourinary diseases and have been associated with adverse pregnancy outcomes and female fertility problems. The aim of this study was to analyse the presence of M. hominis and U. urealyticum in the cervical canal of uterus of women with and without fertility problems., Methods: Endocervical swabs obtained from women with reproductive problems and fertile women were tested by both cultivation and polymerase chain reaction. The antimicrobial susceptibility to the azithromycin, ciprofloxacin, doxycycline and erythromycine of the isolated strains of M. hominis and U. urealyticum was also tested by the microdilution broth method., Results: A total of 111 women with fertile problems were examined. U. urealyticum was detected in samples from 44 (39.6%) women. M. hominis was detected in significantly fewer samples, i.e. only from 9 (8.1%) samples. From these, 6 (5.4%) women were positive for both microorganisms. The fertile group consisted from 23 women. The presence of U. urealyticum was detected in 8 (34.7%) of them. M. hominis was detected only in the mixture with U. urealyticum in 3 (13.0%) cases. The most effective antibiotic against both species in our study was doxycycline., Conclusion: The results show slightly higher incidence of M. hominis and U. urealyticum in the genitourinary tract of women with fertility problems compare with control group. The potential negative effect of these species on the reproduction ability of women was not observed.

The aim was to analyze T-regulatory cells (Tregs), activated CD8(+) T cells, and transforming growth factor-beta (TGF)-β in hepatitis C patients. We enrolled 31 patients with chronic genotype 1 hepatitis C virus (HCV) infection, 30 seropositive persons with spontaneous HCV elimination, and 23 healthy volunteers. The patients were examined at the beginning of the interferon-alpha (IFN-α)-based therapy (baseline) and at weeks 4 (W4) and 12 (W12) of the therapy. The percentage of Tregs and the expression of activation markers CD38 and HLA-DR on CD8(+) T cells were analyzed in the peripheral blood by flow cytometry. Serum levels of TGF-β were measured in a multiplex assay using flow cytometry. The percentage of Tregs in patients was higher than in controls and seropositive persons. Similarly, the percentage of CD8(+) T cells expressing CD38 and HLA-DR was higher in patients compared with controls and seropositive persons. Chronic HCV infection is associated with elevated circulating Tregs and activated CD8(+) T cells. Studies focused on arbovirus diseases transmitted by invasive species of mosquitoes have become increasingly significant in recent years, due to the fact that these vectors have successfully migrated to Europe and become established in the region. Mosquitoes, represented by more than 3 200 species, occur naturally worldwide, except in Antarctica. They feed on the blood of warm-blooded animals and by this route, they are capable of transmitting dangerous diseases. Some species can travel a distance of 10 km per night and can fly continuously for up to 4 hours at a speed of 1-2 km/h. Most species are active at night, in the evening or morning. It usually takes a mosquito female about 50 seconds to penetrate the skin of mammals and the subsequent blood meal usually takes about 2.5 minutes. Mosquitoes live for several weeks or months, depending on the environmental conditions. The VectorNet project is a European network of information exchange and sharing of data relating to the geographical distribution of arthropod vectors and transmission of infectious agents between human populations and animals. It aims at the development of strategic plans and vaccination policies which are the main tasks of this time, as well as the development and application of new disinfectants to control vector populations.
