Epidermoid cysts are rare, slow-growing, benign, developmental cysts that are derived from abnormally situated ectodermal tissue. Epidermoid cysts may grow anywhere on the body and about 7% of them are located in the head and neck. In literature, very few epidermoid cysts have been reported in the temporal region. Histopathologically, they are lined with plain stratified keratin-producing squamous epithelium, although in some cases part of the lining is made up of mucous secreting and ciliated epithelium. This may suggest an endodermal rather than an ectodermal origin. We present and discuss the management of a patient presenting a posttraumatic epidermoid cyst of the temporal region. Background: The aim of this paper was to report our experience and to review the literature of arthrocentesis focusing on protocols and results., Methods: Arthrocentesis with supplemental hyaluronic acid was performed in patients with TMDs between January 2017 and December 2020 at the Division of Maxillofacial Surgery. The maximum interincisal opening (MIO) and pain score were recorded preoperatively (T0), 2 months postoperatively (T1), and 6 months postoperatively (T2). A literature search was performed to analyze the same parameters in patients with TMDs. Patient demographic, characteristics and treatment protocols used were also recorded., Results: This retrospective analysis enrolled 45 patients. Twenty-two patients (20 females, 2 males) with mean age of 37.13 years with internal derangement were included in study group A. Study group B included 23 patients with degenerative joint disease (19 females and 4 males) with mean age of 55.73 years. The outcomes trend of MIO and pain during the follow-up period showed a gradual improvement. Fifty articles meeting the proposed scientific criteria were selected for the literature revision. A range of clinical and procedural variables were analyzed by grouping the studies into two broad categories based on the diagnosis of TMD., Conclusions: Based on our experience and on the basis of the most accredited scientific studies in the literature, intra-articular injections of HA are beneficial for the improvement of the pain and/or functional symptoms of TMDs.

Infection in patients supported with left ventricular assist devices (LVAD) is common and it can limit widespread implantation of mechanical assistance as destination therapy (DT). The infection-resistance power delivery system could improve longevity and quality of life. The Jarvik 2000 (Jarvik Heart, New York, NY, USA) driveline design showed a prolonged infection-free survival and a better quality of life compared to those patients supported with traditional LVAD with an abdominal cable. We report a singular driveline complication in a 71-year-old patient supported with a Jarvik 2000. A new retroauricular pedestal was successfully repositioned after the detachement of the first one.

Synovial chondromatosis (SC) of the temporomandibular joint (TMJ) is an uncommon disease, which is characterized by chondrometaplasia of the synovial membrane. It is characterized by cartilaginous metaplasia of the mesenchymal remnants of the synovial tissue of the joints. Loose bodies can be found within the joint space. The cause of SC in the TMJ remains unclear. The nonspecific signs of SC often lead to a delay in diagnosis or a misdiagnosis. Removal of the loose bodies and of the synovial membrane involved is sufficient to control the disease in most cases. We present and discuss the diagnosis and surgical management of a new case of unilateral SC of the TMJ.

Purpose: Achieving functional and anatomic restoration is remarkably difficult in patients with symphyseal mandibular fractures associated with bicondylar fractures. Our objective was to evaluate the functional and esthetic outcomes of a group of patients who underwent surgery using rigid fixation., Patients and Methods: In our study, 25 patients with symphyseal mandibular fractures associated with bicondylar fractures surgically treated from 1998 to 2008 at the Division of Maxillofacial Surgery of the University of Turin were retrospectively analyzed. The standardized evaluation consisted of an evaluation of occlusion, assessment of mandibular motion, and esthetic evaluation of facial width., Results: The patients showed good mandibular motion and good occlusion. After esthetic evaluation of the facial width of the 25 patients, 23 patients were rated as "excellent" or "good.", Conclusion: Successful treatment of multiple mandibular fractures may be achieved by different techniques. The key point is the re-establishment of the transversal bigonial dimension by a correct reconstruction of the mandibular arch.

Granuloma gangraenescens or "midline granuloma" (Stewart's malignant granuloma, idiopathic midline destructive syndrome) is a rare clinical syndrome. In clinical practice, the destructive process of the facial midline may appear as a symptom of various infective, malignant or autoimmune diseases. In addition to these cases, a number of cases have been reported in the literature since 1896 with the diagnosis of lethal midline granuloma. These subjects underwent a progressive and often fatal destructive process of unknown cause which generally initiated in a nasal cavity (or sometimes on the palate) in the form of a granulomatous lesion with a rapid tendency to necrosis. The lesion affects the nose, paranasal sinuses, palate and soft facial tissue. As it progresses, it involves soft tissue, cartilage and the bone structures of the aforesaid and adjacent areas. A number of systemic symptoms may be present. The age of patients ranges from a minimum of 15 to a maximum of 59 years old. Repeated biopsies are usually necessary before the syndrome is diagnosed. From a histological point of view, it has been reported that the tissues in the nasal cavities are affected by diffuse infiltration of lymphocytes and leukocytes and histiocytic-type cells. The disease lasts between 3 and 20 months and causes death due to secondary infection, hemorrhage caused by the erosion of major vessels in the head and neck, and cachexia. With regard to therapy, the majority of authors appear to agree that local high-dose radiations in the area of the lesion are the best method of treatment for this pathology.(ABSTRACT TRUNCATED AT 250 WORDS)
