Abstract

Aims Patients with acute coronary syndromes (ACSs) who are managed without coronary revascularization represent a mixed and understudied population that seems to receive suboptimal pharmacological treatment. Methods and results We assessed patterns of antithrombotic therapies employed during the hospitalization and in-hospital clinical events of medically managed patients with ACS enrolled in the prospective, multicentre, nationwide EYESHOT (EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units) registry. Among the 2585 consecutive ACS patients enrolled in EYESHOT, 783 (30.3%) did not receive any revascularization during hospital admission. Of these, 478 (61.0%) underwent coronary angiography (CA), whereas 305 (39.0%) did not. The median GRACE and CRUSADE risk scores were significantly higher among patients who did not undergo CA compared with those who did (180 vs. 145, P < 0.0001 and 50 vs. 33, P < 0.0001, respectively). Antithrombotic therapies employed during hospitalization significantly differ between patients who received CA and those who did not with unfractioned heparin and novel P2Y12 inhibitors more frequently used in the first group, and low-molecular-weight heparins and clopidogrel in the latter group. During the index hospitalization, patients who did not receive CA presented a higher incidence of ischaemic cerebrovascular events and of mortality compared with those who underwent CA (1.6 vs. 0.2%, P = 0.04 and 7.9 vs. 2.7%, P = 0.0009, respectively). Conclusion Almost one-third of ACS patients are managed without revascularization during the index hospitalization. In this population, a lower use of recommended antiplatelet therapy and worse clinical outcome were observed in those who did not undergo CA when compared with those who did. Abstract

Aims In the last decades, several new therapies have emerged for the treatment of acute coronary syndromes (ACS). We sought to describe real-world patterns of use of antithrombotic treatments in the catheterization laboratory for ACS patients undergoing percutaneous coronary interventions (PCI). Methods EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units was a nationwide, prospective registry aimed to evaluate antithrombotic strategies employed in ACS patients in Italy. Results Over a 3-week period, a total of 2585 consecutive ACS patients have been enrolled in 203 cardiac care units across Italy. Among these patients, 1755 underwent PCI (923 with ST-elevation myocardial infarction and 832 with non-ST-elevation ACS). In the catheterization laboratory, unfractioned heparin was the most used antithrombotic drug in both ST-elevation myocardial infarction (64.7%) and non-ST-elevation ACS (77.5%) undergoing PCI and, as aspirin, bivalirudin and glycoprotein IIb/IIIa inhibitors (GPIs) more frequently employed before or during PCI compared with the postprocedural period. Any crossover of heparin therapy occurred in 36.0% of cases, whereas switching from one P2Y12 inhibitor to another occurred in 3.7% of patients. Multivariable analysis yielded several independent predictors of GPIs and of bivalirudin use in the catheterization laboratory, mainly related to clinical presentation, PCI complexity and presence of complications during the procedure. Conclusion In our contemporary, nationwide, all-comers cohort of ACS patients undergoing PCI, antithrombotic therapies were commonly initiated before the catheterization laboratory. In the periprocedural period, the most frequently employed drugs were unfractioned heparin, leading to a high rate of crossover, followed by GPIs and bivalirudin, mainly used during complex PCI. Abstract

Considering the consistent reduction in battery range due to the operation of the Heating Ventilation and Air Conditioning (HVAC) system, this study deals with the CO2 measurement inside the cabin of an electric crane and aims to reduce the energy consumption through the control of the air recirculation. A control strategy was defined and tested through an experimental set-up where the presence of a driver was simulated as a source of CO2. The cabin was placed inside a climatic wind tunnel and the benefits of this control strategy on the HVAC system energy consumption were assessed, both in the heating and the cooling modes. In addition, we discussed the optimal position of the CO2 sensor inside the cabin by comparing the results obtained from some sensors placed around the cabin occupant with the ones logged by three sensors in the breathing zone. Abstract

This work deals with the assessment of an experimental setup for the CO 2 measurement inside the cabin of an electric crane. Different operating and ventilation conditions are compared and the optimal positioning of the CO 2 sensors are discussed. In particular, the results obtained by some sensors placed around the cabin occupant are compared with three sensors positioned in the breathing zone. In all the ventilation modes, a flux of 66 g/h of pure CO 2 is introduced inside the cabin, to simulate the breathing of a driver. We studied three different operating conditions: heating mode, ventilation mode and cooling mode. In each condition the CO 2 distribution is obtained for the cases of internal ventilation in fresh air mode and on/off control of the recirculation mode. Two different ventilation distributions were considered: one for the heating mode and one for ventilation and cooling modes. A stratification of the CO 2 along the vertical axis is shown in both the cases. While in heating mode the CO 2 concentration decreased going from the bottom to the top of the cabin, in the other two modes the CO 2 level was maximum at medium height and minimum at the extremities. The optimal positioning of the CO 2 sensors is obtained for the three operating modes.

Abstract

This work deals with the assessment of the benefits, in terms of energy saving, given by the measurement of the CO 2 concentration inside the cabin of an electric crane to control the air recirculation. An experimental set-up, where a flux of 66 g/h of pure CO 2 is introduced inside the cabin, to simulate the breathing of a driver, is used to compare two different ventilation strategies, in heating and cooling conditions. With the first control strategy the air recirculation is always off. With the second one the air recirculation on/off system is controlled on the bases of the CO 2 levels in the cabin air. The power consumed by the positive temperature coefficient (PTC) heater (in heating mode) and by the compressor of the refrigeration cycle (in cooling mode) is measured in the two cases. The value of the energy consumption along the test duration is used as parameter to assess the energy saving effect of the air recirculation. The results show that the air recirculation based on the CO 2 concentration inside the cabin allowed an energy saving of the 53.8% and 72.6% in the cases studied in heating mode and of the 0.8% and 9.0% in the cases considered in cooling mode.

We report an unusual case of impending paradoxical embolization in a 69-year-old woman heterozygote carrier of factor V Leiden mutation. The patient presented to the emergency room with the clinical scenario of massive pulmonary embolism. Serial echocardiographic examinations revealed a large thrombus in the right atrium floating via a patent foramen ovale into the left atrium. Anticoagulation therapy was started. After 72 h, due to the unresolved thrombus, the patient underwent surgical treatment consisting of complete excision of the thrombus, closure of the foramen ovale, and pulmonary embolectomy. No in-hospital complications were noted. At 1-year follow-up, the patient is doing well on long-term anticoagulation treatment free of thromboembolic events.

Abstract

Background: Over the past few decades, percutaneous coronary intervention (PCI) has undergone significant advancements as a result of the combination of device-based and drug-based therapies. These iterations have led to the development of polymer-free drug-eluting stents. However, there is a scarcity of data regarding their clinical performance. Furthermore, while various risk scores have been proposed to determine the optimal duration of dual antiplatelet therapy (DAPT), none of them have undergone prospective validation within the context of randomized trials., Design: The PARTHENOPE trial is a phase IV, prospective, randomized, multicenter, investigator-initiated, assessor-blind study being conducted at 14 centers in Italy (NCT04135989). It includes 2,107 all-comers patients with minimal exclusion criteria, randomly assigned in a 2-by-2 design to receive either the Cre8 amphilimus-eluting stent or the SYNERGY everolimus-eluting stent, along with either a personalized or standard duration of DAPT. Personalized DAPT duration is determined by the DAPT score, which accounts for both bleeding and ischemic risks. Patients with a DAPT score <2 (indicating higher bleeding than ischemic risk) receive DAPT for 3 or 6 months for chronic or acute coronary syndrome, respectively, while patients with a DAPT score ≥2 (indicating higher ischemic than bleeding risk) receive DAPT for 24 months. Patients in the standard DAPT group receive DAPT for 12 months. The trial aims to establish the noninferiority between stents with respect to a device-oriented composite end point of cardiovascular death, target-vessel myocardial infarction, or clinically-driven target-lesion revascularization at 12 months after PCI. Additionally, the trial aims to demonstrate the superiority of personalized DAPT compared to a standard approach with respect to a net clinical composite of all-cause death, any myocardial infarction, stroke, urgent target-vessel revascularization, or type 2 to 5 bleeding according to the Bleeding Academic Research Consortium criteria at 24-months after PCI., Summary: The PARTHENOPE trial is the largest randomized trial investigating the efficacy and safety of a polymer-free DES with a reservoir technology for drug-release and the first trial evaluating a personalized duration of DAPT based on the DAPT score. Abstract

Considering the consistent reduction in battery range due to the operation of the Heating Ventilation and Air Conditioning (HVAC) system, this study deals with the CO 2 measurement inside the cabin of an electric crane and aims to reduce the energy consumption through the control of the air recirculation. A control strategy was defined and tested through an experimental set-up where the presence of a driver was simulated as a source of CO 2 . The cabin was placed inside a climatic wind tunnel and the benefits of this control strategy on the HVAC system energy consumption were assessed, both in the heating and the cooling modes. In addition, we discussed the optimal position of the CO 2 sensor inside the cabin by comparing the results obtained from some sensors placed around the cabin occupant with the ones logged by three sensors in the breathing zone. Finally, an investigation of the uncertainty of the indirect measurement of the leakage flow and its dependence on the number of CO 2 sensors installed in the cabin was made through the Monte Carlo method.

Abstract

On the basis of a clinical review of a consecutive series of 27 medial meniscectomies of the knee, the authors report the results obtained by the systematic use of the Van Unen elastic bandage ii the post-operative period. The use of this technique would appear to be advantageous both as a means of speeding up functional recovery of the joint operated on and as a means of preventing complications of an arthrosynovitic type which are often secondary to early rehabilitation treatments.
