The aim of this study was to compare physical and hormonal responses of seventeen elite rugby sevens players over a 6-week intense training block (IT) and a consecutive 2-week tapering period (TAP), using a fatigue cut-off score of 20 as a potential moderating variable. Training was monitored by daily training load (TL) and strain (TS) (using the session rating of perceived exertion [sRPE]) and also the weekly total score of fatigue (TSF; 8-item questionnaire tool). Testing and 24 h urinary cortisol (CL), cortisone (CN), adrenaline (AD) and noradrenalin (NAD) concentrations were also analysed before (T0) and after IT (T1) and after the TAP (T2). Players were assigned to group 1 with a TSF above 20 (G1 > 20, n = 9) and group 2 with a TSF below 20 (G2 < 20, n = 8) according to the French Society for Sports Medicine guidelines. TSF (effect size [ES] from 1.17 to 1.75), TL (ES from 0.81 to 1.06) and TS (ES from 1.23 to 1.40) were higher in G1 > 20 than in G2 < 20 over IT. Likewise, performance standards (ES from 1.58 to 2.61) and AD levels were lower (ES = 3.20), whereas CL and CL/CN ratio (ES from 1.60 to 3.47) were higher in G1 > 20 than in G2 < 20. After the TAP, TSF, TL and TS returned to baseline values for both groups, with an increase in performance standards and normalization in hormone levels. We suggest that a TSF greater than or equal to 20 could be considered as a fatigue threshold generating hormone disturbance and performance decrement, making it a potentially useful preventive and complementary training monitoring tool., Competing Interests: None., (Copyright © Biology of Sport 2023.)

Ovarian adrenal rest tumors (OART) are tumors that develop in females with congenital adrenal hyperplasia (CAH). In contrast to their counterpart in testicles, they are exceptional and few cases have been reported in the literature. In this report, we present clinicopathological findings of a female patient with CAH due to 21-hydroxylase deficiency who was incidentally diagnosed with OART with a review of the literature. The 14-year-old patient, who was raised as a boy, developed a virilizing syndrome with high testosterone levels that were attributed to non adherence to her replacement corticosteroid therapy. She consulted for sex reassignment surgery. Pelvic ultrasound was normal. She underwent hysterectomy and bilateral adnexectomy. No abnormalities were noticed during the operation. Grossly, both ovaries were variegated with well circumscribed and lobulated, brownish-yellow nodules. Histologically, the nodules were composed of nests of large polygonal cells with centrally located nuclei and prominent nucleoli. There was mild atypia and no crystals of Reinke. Thus, the findings of the histopathological examination were consistent with bilateral OART. Histological differential diagnosis of OART can be challenging particularly with leydig cell tumor, stromal luteoma and steroid cell tumors, not otherwise specified. OART must be considered in women with CAH and persistent virilizing symptoms despite negative imaging results.

Purpose: Trainers and physical fitness coaches' need a useful tool to assess training loads to avoid overtraining. However, perceived scales or questionnaires were required. Therefore, the purpose of this study was to assess whether a short 8-item questionnaire of fatigue could be a useful tool for monitoring changes in perceived training load and strain among elite rugby Sevens (7s) players during preparation for a major competition., Methods: Sixteen elite rugby 7s players completed an 8-week training program composed of 6-week intense training (IT) and 2-week reduced training (RT). They were tested before (T0), after the IT (T1) and after the RT (T2). The quantification of the perceived training load and strain were performed by the session-RPE (rating of perceived exertion) method and concomitantly the 8-item questionnaire of fatigue was administered., Results: Training load (TL) and strain (TS) and total score of fatigue (TSF from the 8-item questionnaire) increased during IT and decreased during RT. Simultaneously, physical performances decreased during IT and were improved after LT. The changes in TL, TS and TSF correlated significantly over the training period (r=0.63-0.83)., Conclusions: These findings suggest that the short questionnaire of fatigue could be a practical and a sensitive tool for monitoring changes in training load and strain in team-sport athletes. Accordingly, the simultaneous use of the short questionnaire of fatigue along with the session-RPE method for perceived changes in training load and strain during training could provide additional information on the athletes' status, allowing coaches to prevent eventual states of overreaching or overtraining.

A 68-year-old woman presented with a painless inflammation of the right superior eyelid that had started several weeks before. The clinical diagnosis concluded in canaliculitis and the solid concretions were surgically extracted from the superior canalicula. The anaerobic bacteria Fusobacterium nucleatum sp. nucleatum was isolated. Signs dramatically regressed two weeks after surgery followed by one course of oral amoxicillin and clavulanic acid associated with topical tobramycin. Aim: To assess functional impairment in patients with rheumatoid pelvispondylite., Methods: cross-sectional study conducted on patients with rheumatoid pelvispondylite. Data are collected in a form specifying the epidemiological and demographic data, disease activity by Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), metrological static and dynamic examination. Evaluation of functional impairment by Bath Ankylosing Spondylitis Functional Index (BASFI), and likert scale assessing social, economic and physical disability., Results: 30 patients (22 men), the mean age is 37.2 years old; twenty one patients are without work. The mean duration of disease is 13.36 years, 50% are annoyed by morning stiffness than one hour, spinal pain and fatigue. The average value of BASDAI is 43.72/100; the average value of EVA fatigue is 56.16/100. The average value of BASFI is 56.36/100. An important or very important social, economic and physical disability was felt among 67% of the patients, which is correlated with duration of the disease, the BASDAI index, work, and the BASFI index., Conclusion: The functional impairment caused is statistically correlated with the intensity of the pain, the fatigue, the morning stiffness and BASDAI index, This impact on the quality of life is increased by the low level of education and poor professional integration.

A case of a bilateral necrotizing scleritis following strabismus surgery for thyroid ophthalmopathy is described. A 71-year-old man was operated on for esotropia due to prior and equilibrated Grave's disease. Two weeks after surgery, he had a scleral necrosis that was located on the surgery sites (insertion of the two medial rectus muscles). There was no infectious or autoimmune etiology. The pathogenic mechanism was ischemic and inflammatory, doubtless facilitated by the thyroid ophthalmopathy. The outcome was favorable after 3 weeks of antibiotic and anti-inflammatory treatment. To the authors' knowledge, this is the first bilateral case of this complication.
