Abstract

BackgroundFluid resuscitation is the standard treatment to restore circulating blood volume and pressure after massive haemorrhage and shock. Packed red blood cells (PRBC) are transfused to restore haemoglobin levels. Restoration of microcirculatory flow and tissue oxygen delivery is critical for organ and patient survival, but these parameters are infrequently measured. Patient Blood Management is a multidisciplinary approach to manage and conserve a patient’s own blood, directing treatment options based on broad clinical assessment beyond haemoglobin alone, for which tissue perfusion and oxygenation could be useful. Our aim was to assess utility of non-invasive tissue-specific measures to compare PRBC transfusion with novel crystalloid treatments for haemorrhagic shock.MethodsA model of severe haemorrhagic shock was developed in an intensive care setting, with controlled haemorrhage in sheep according to pressure (mean arterial pressure 30–40 mmHg) and oxygen debt (lactate 4 mM) targets. We compared PRBC transfusion to fluid resuscitation with either PlasmaLyte or a novel crystalloid. Efficacy was assessed according to recovery of haemodynamic parameters and non-invasive measures of sublingual microcirculatory flow, regional tissue oxygen saturation, repayment of oxygen debt (arterial lactate), and a panel of inflammatory and organ function markers. Invasive measurements of tissue perfusion, oxygen tension and lactate levels were performed in brain, kidney, liver, and skeletal muscle. Outcomes were assessed during 4 h treatment and post-mortem, and analysed by one- and two-way ANOVA.ResultsEach treatment restored haemodynamic and tissue oxygen delivery parameters equivalently (p 0.05), despite haemodilution after crystalloid infusion to haemoglobin concentrations below 70 g/L (p 0.001). Abstract

The fractional abundance of branched glycerol dialkyl glycerol tetraether (brGDGT) membrane lipids in coastal marine sediments has been posited as a proxy for the reconstruction of terrestrial temperatures on the nearby land, based on the assumption that they are produced in soils and delivered to the marine realm by rivers following erosion. Here, we test the suitability of brGDGTs as a continental paleothermometer in Pliocene age sediments from the northern Gulf of Mexico (GoM; speculated Mississippi River input) and the Ceará Rise (speculated Amazon River input). Low branched to isoprenoid tetraether (BIT) index values of 0.00–0.13 and the near absence of pollen and long-chain plant waxes in the GoM sediments suggest that the Mississippi River did not have a strong influence on the delivery of terrestrial organic matter to the site during the Pliocene and soil input was limited. Indeed, the high weighted average of cyclopentane-containing tetramethylated brGDGTs (#ringstetra) in the GoM sediments (0.50 ± 0.09) relative to that of modern soils from the Mississippi catchment (0.25 ± 0.16) indicates that the brGDGTs in the GoM sediments were mostly produced in situ in the marine realm, hampering reliable land temperature reconstruction using the global soil transferfunction. In contrast, high BIT index values (0.46 ± 0.21) and low #rings tetra (0.25 ± 0.15) in sediments from the Ceará Rise suggest that these brGDGTs are primarily derived from soils. However, reconstructed temperatures were 11–18 °C lower than modern Amazon catchment temperatures. The relative abundance of 6-methylated brGDGTs (Isomerisation Ratio; IR) in the sediments is 0.82 ± 0.10, which resembles that of suspended particulate matter (SPM) in the modern Amazon River more than that of catchment soils (IR = 0.18 ± 0.18). Abstract

PURPOSE: Brain death (BD) causes metabolic and energetic imbalances leading to cardiac dysfunction, and predisposes the donor heart to further injury following heart transplantation (HTx). The metabolic mechanisms required for myocardial energy production during BD and subsequent HTx are poorly understood. Our aim was to determine the myocardial metabolic profile and mitochondrial function following donor BD and HTx. METHODS: Donor BD in sheep was induced by inflation of a catheter placed through the skull (catheter placement, but no inflation for SHAM), followed by 24 hrs monitoring, and heart procurement (n=6/group, BD vs. SHAM). Additional donor hearts exposed to BD/SHAM were flushed with cold St Thomas cardioplegia, and stored via cold static storage (CSS) for ∼2 hrs. Following standard orthotopic HTx, recipients were weaned off bypass and monitored for ≤6 hrs prior to heart procurement (n=4/group, BD-Tx vs. Sh-Tx). Cardiac mitochondrial function was assessed using high resolution respirometry. Metabolic profiles were determined in hearts using metabolomics. Cardiac mitochondrial function was also determined in two sheep that underwent HTx following BD and 8 hr hypothermic ex vivo perfusion (HEVP) preservation. RESULTS: BD caused significant right ventricular (RV) mitochondrial uncoupling (vs. SHAM). HTx following CSS also impaired RV mitochondrial function, with these effects more pronounced in hearts exposed to both donor BD and HTx. Early findings show that HEVP improved cardiac mitochondrial function post-HTx (vs. CSS). Metabolically, BD increased myocardial amino-acid utilisation and accumulation of glucose metabolites. Post-HTx, particularly in those exposed to donor BD, there was a significant decrease in metabolites involved in mitochondrial respiration (eg. Abstract

Routine mode and quantitative measurements of main hydrocarbon families in petroleum cuts have managed the settinq-up of our methodology, spread over the ten last years in our lab on a high resolution MS 50 Kratos spectrometer.The technique is devoted to petroleum cuts ranging from 250 to 650°C, without any previous separation, and provides thirty three data on the whole sample, in relation with the hydrocarbon families composition, balancing From "saturates" (paraffins, cyclanes, …) to "sulfur compounds" (benzo, dibenzo, naphtalenobenzothiophenes, etc.) and including "aroroatics" scattered From mono to hepta fused structures.Checkings of the method and examples are illustrated with the help of applications picked up on petroleum processing and hydra-carbon product research topics.
