To influence school achievement, researchers investigate student academic help-seeking. However, a growing population of students of color combined with the risk of poorer academic performance yields attention toward African American student academic help-seeking. In this review, 13 studies analyzed sampling characteristics, theoretical frameworks, and designing methodologies. Results illustrated that samples varied in terms of data collection, settings, participant characteristics, and research purpose. Also, frameworks differed with the majority of the studies referencing culture or motivation-informed theory. The methodology focused on quantitative measures of help-seeking behavior, attitude, and influencing variables. Qualitative measures showed African American students experienced help-seeking challenges, family involvement, and assumed responsibility for seeking help. Implications for research and practice are provided.

BackgroundProper functioning of high-density lipoprotein (HDL) is necessary for protection against sepsis. However, previous work has demonstrated that HDL becomes oxidized and dysfunctional (Dys-HDL) during sepsis. Older (aged >65 years) patients are at particularly high risk of sepsis and poor outcomes from sepsis.Study objectiveThe aim of the study was to compare functional properties of HDL (cholesterol efflux capacity and paraoxonase enzyme 1 [PON-1] activity) and Dys-HDL between older (aged >65 years) sepsis patients and older healthy volunteers.MethodsThis was a subanalysis of a prospective study in which patients with sepsis were prospectively enrolled from the emergency department within the first 24 h. Serum and plasma samples were drawn from septic patients and age- and sex-matched control subjects. Percent cholesterol efflux, HDL inflammatory index, and PON1 activity were measured. Data were analyzed using Student t test or Wilcoxon rank-sum test.ResultsTen sepsis and 10 healthy controls were analyzed. Mean age of sepsis patients (80 ± 2 years [SD]) and control subjects (77 ± 2 years) was similar (P = 0.31). Mean systolic blood pressures were significantly different in sepsis patients (113 ± 8 mmHg) compared with controls (133 ± 6 mmHg) (P = 0.049). Median SOFA scores for sepsis patients were 5.5 (interquartile range [IQR] 4-9). ObjectiveHigh density lipoprotein (HDL) is important for defense against sepsis but becomes dysfunctional (Dys-HDL) during inflammation. We hypothesize that Dys-HDL correlates with organ dysfunction (sequential organ failure assessment (SOFA) score) early sepsis.MethodsA prospective cohort study of adult ED sepsis patients enrolled within 24 hours.ResultsEighty eight patients were analyzed. Dys-HDL (expressed as HDL inflammatory index (HII)) correlated with SOFA at enrollment (r = 0.23, p = 0.024) and at 48 hours (r = 0.24, p = 0.026) but HII change over the first 48 hours did not correlate with change in SOFA (r = 0.06, p = 0.56). Enrollment HII was significantly different in patients with most severe organ failure (2.31, IQR 1.33-5.2) compared to less severe organ failure (1.81, IQR 1.23-2.64, p = 0.043). Change in HII over 48 hours was significantly different for in-hospital non-survivors (-0.45, IQR-2.6, -0.14 p = 0.015) and for 28-day non-survivors (-1.12, IQR -1.52, 0.12, p = 0.044). In a multivariable linear regression equation (R2 = 0.13), for each unit HII increase, 48-hour SOFA increased by 0.72 (p = 0.009).ConclusionHII correlated with SOFA and predicted 48-hour SOFA score in early sepsis. BackgroundHigh density lipoprotein (HDL) can be readily oxidized in inflammatory conditions and exhibit pro-inflammatory and dysfunctional (Dys-HDL) characteristics. We hypothesize that Dys-HDL may predict adverse outcomes and correlate with inflammatory cytokines in sepsis.MethodsEmergency department (ED) patients with sepsis were enrolled. Blood was drawn at enrollment and after 48 h. Dys-HDL, expressed as HDL inflammatory index (HII), and cytokines were measured. Multivariable logistic regression was used to determine the predictive ability of Dys-HDL for adverse outcomes (death, discharge to hospice, or nursing home).ResultsThirty-five patients were included in the study. HII was not significantly different at baseline or 48 h between patients with adverse outcomes versus those without. However, there was a significant difference in change in HII over the first 48 h between those with adverse outcomes (+0.21, 95% CI -0.13 to 0.31) versus those without (-0.11, 95% CI -1 to 0.11) (P = 0.025). Logistic regression revealed increasing HII to be an independent predictor of adverse outcomes (OR 5.2, 95% CI 1.1-25.1 P = 0.040). Of the 24 patents with cytokine measurements at both time points, significant inverse correlations between change in HII and change in GRO (rs = -0.52, P = 0.0088) and monocyte chemotactic protein-1 (rs = -0.61, P = 0.0014) concentrations over 48 h were observed.ConclusionIncreasing Dys-HDL concentrations in the first 48 h of sepsis are associated with an ongoing inflammatory response and adverse clinical outcomes. Early changes in HII may be a potential biomarker in ED patients admitted with sepsis.

Introduction: Percussive stunning is a widely used and ethically supported method of stunning fish per welfare standards as part of a one- or two-step slaughter process. In this study, the use of a non-penetrative captive bolt (NPCB) gun was evaluated as an effective one-step method of improving welfare for juvenile and adult farmed white sturgeon (Acipenser transmontanus) at slaughter. Methods: A Jarvis HPS-1 NPCB was operated at pressures of 120, 135, and 145 PSI (827.37, 930.79, and 999.74 kPa, respectively) for juvenile sturgeon (n = 3 sturgeon per operating pressure) and 175, 200, and 225 PSI (1206.58, 1378.95, and 1551.32 kPa, respectively) for adult sturgeon (n = 3 sturgeon per operating pressure). Following a single strike, fish were assessed for jaw relaxation and a somatic response before being exsanguinated and transferred to an ice slurry. An hour after slaughter, fish heads were collected, and a section of cartilage containing the brain was removed and fixed in formalin for histological analysis of brain death. To evaluate fish recovery, juveniles (n = 100) and adult female sturgeon (n = 65) were monitored for two hoursafter a single exposure to 145 and 225 PSI, respectively. Results: Histology results showed there was an effect between operating pressure and intracranial hemorrhage in juvenile sturgeon (p = 0.024). There was a greater meningeal-to-cerebral hemorrhage at 135 PSI compared to the 120 PSI group (p = 0.020) and a trend towards increased tissue damage from 120 PSI to 145 PSI (p = 0.056). Adults showed no significant difference in meningeal hemorrhage at any operating pressure. When investigating recovery rates, NPCB successfully stunned 100% of juvenile sturgeon at 145 PSI, and 225 PSI stunned 90% of adult sturgeon without recovery Discussion: These results demonstrate that the use of an NPCB gun is an improvement in animal welfare compared to repeated strikes, but a single application did not produce histological brain death; further research should be conducted to determine optimal pressures that result in immediate brain death. There is increasing evidence for the benefits of providing neuropsychological assessment feedback to patients and caregivers, but the current practices of clinical neuropsychologists are unclear. This survey study aimed to determine the provision of neuropsychological feedback practices by Australian neuropsychologists across a range of settings and patient groups. An online survey was circulated to eligible practising clinicians via an Australian Google Group of clinical neuropsychologists. The survey comprised demographic, education, and practice-related questions, followed by questions relating to the participants' training, experience and confidence in providing neuropsychological feedback. Participants were also asked about the usefulness and barriers to feedback. Of the 102 clinicians who participated, 60% reported that they routinely provide neuropsychological assessment feedback and a further 16% provide it frequently. Participants reported high confidence in their capacity to provide feedback. However, there was variability across different patient groups, with the highest frequency of routine feedback seen in the paediatric setting and the lowest in the areas of mild cognitive impairment and dementia. The duration and aims of feedback were relatively consistent, with most sessions lasting 30–60 minutes and covering similar themes. In summary, Australian neuropsychologists who participated are providing regular neuropsychological feedback services across a range of patient groups and are confident to do so. However, there were substantial differences in the frequency of feedback provided across patient groups. Further research is needed to determine the effectiveness of neuropsychological feedback across patient groups and the barriers to providing routine feedback, particularly for older adults. Background Exposure to secondhand smoke (SHS) is a significant contributor to ill health in children. A study was undertaken to determine the effectiveness of two brief multi-strategic child health nurse delivered interventions in: decreasing the prevalence of infants exposed to SHS; decreasing the prevalence of smoking amongst parent/carers of infants and increasing the prevalence of household smoking bans. Methods This study was a 3 arm, cluster randomised controlled trial. Clusters were 39 community based well child health clinics in one local area health service. Clinics were stratified according to annual number of client appointments and then randomly assigned in a 1:1:1 ratio, (Intervention 1: Intervention 2: Control), with 13 clinics in each cluster. Parents/carers of infants in the intervention groups received a brief multi-strategic intervention from child health nurses during clinic consultations. Treatment condition 1 included computer delivered risk assessment and feedback and nurse brief advice. Treatment condition 2 included all elements of Treatment condition 1 with the addition of biochemical feedback of infant SHS exposure. Results When compared to the Control group at 12 months, no significant differences in the prevalence of infant exposure to SHS were detected from baseline to follow-up for Treatment condition 1 (OR 1.16, 95 % CI 0.73-1.85, p = 0.53) or Treatment condition 2 (OR 1.30, 95 % CI 0.88-1.92, p = 0.19) Similarly, no significant differences were detected in the proportion of parent/carers who reported that they were smokers (T1:OR 0.95, 95 % CI 0.78-1.15, p = 0.58 and T2:OR 0.97, 95 % CI 0.80-1.18, p = 0.77), or in the proportion of households reported to have a complete smoking ban (T1:OR 1.21, 95 % CI 0.89-1.64, p = 0.23 and T2:OR 1.06, 95 % CI 0.79-1.43, p = 0.68). Conclusions Further research is required to identify effective interventions that can be consistently provided by child health nurses if the potential of such settings to contribute to reductions in child SHS exposure is to be realised., Author(s): Justine B.
