To establish the prevalence of oral mucosal lesions and normal variations in a population of older adults. This observational study was conducted from 2015 to 2016 and involved 156 participants from Los Guido (San José, Costa Rica). The participants’ sociodemographic information, risk factors, comorbidity, and clinical examination were evaluated and analyzed descriptively and qualitatively. Findings in oral mucosa were found to preferentially affect women (n=111; 71%) and individuals between the ages of 60 and 69 years (n=65; 42%), having smoking as a risk factor (n=67; 43%), and suffering from arterial hypertension (n=101; 65%). Intraoral findings were mainly presented together with variations from normal and pathology (n=71; 46%), predominantly one variation from normal (n=67; 33%) and one pathology (n=51; 43%). The most prevalent normal variations were the saburral tongue, lingual varicose veins, and cryptic tonsils. Infectious diseases (n=33; 21%), potentially malignant disorders (n=30; 19%), and physical and chemical injuries (n=18; 12%) were the most prevalent groups. The four most predominant lesions were prosthetic stomatitis, actinic cheilitis, frictional keratosis, and atrophic glossitis. The differences in pathological groups were not statistically significant (p=0.392) when comparing the frequency ratio by sex. Variations of the normal anatomy are the predominant ones, the saburral tongue being the most prevalent individually; among the pathologies, the most remarkable group was the infectious diseases, and prosthetic stomatitis was the most frequent. The information about this subject in Costa Rica is limited. So it is important to conduct more studies that contribute to the oral health of the elderly population in this area., Establecer la prevalencia de lesiones de la mucosa oral y variaciones de lo normal en una población de adultos mayores. Estudio observacional en 156 participantes de Los Guido (San José, Costa Rica) del 2015 a 2016. Se evaluaron y analizaron de forma descriptiva y cualitativa información sociodemográfica, factores de riesgo, comorbilidad y exploración clínica. Los hallazgos en mucosa oral afectaron preferentemente a las mujeres (n=111; 71%), el rango de edad entre 60 y 69 años (n=65; 42%) y con el tabaco como factor de riesgo (n=67; 43%) e hipertensión arterial (n=101; 65%). Los hallazgos intraorales se presentaron preferentemente de forma conjunta variaciones de lo normal y patologías (n=71; 46%), predominantemente una variación de lo normal (n=67; 33%) y una patología (n=51; 43%). Las variaciones de lo normal más prevalentes fueron lengua saburral, várices linguales y amígdalas criptodónticas. Las enfermedades infecciosas (n=33; 21%), los desórdenes potencialmente malignos (n=30; 19 %) e injurias físicas y químicas (n=18; 12%) fueron los grupos más prevalentes. Las cuatro lesiones más predominantes fueron estomatitis protésica, queilitis actínica, queratosis friccional y glositis atrófica. No hubo diferencia estadísticamente significativa (p=0.392) al comparar la proporción de frecuencia por sexo. Las variaciones de la anatomía normal predominaron; siendo en forma individual la lengua saburral la más frecuente; de las patologías destacaron las enfermedades infecciosas principalmente la estomatitis protésica. Existe poca información en esta área en Costa Rica, es importante generar más estudios que permitan contribuir con la salud oral de la población adulta mayor.

The torus are benign bony prominences that generally do not require treatment, however, they can occasionally present osteomyelitis (OM). The objective of this article is to describe the timely management of torus osteomyelitis through a series of clinical cases. Three patients with OM are reported. Two cases of men aged 66 and 69, diabetics, as well as a 57-year-old woman with a history of basal cell carcinoma of the nose. All had torus ulcers associated with trauma and without resolution of the same. In the three cases, a biopsy was performed to confirm the diagnosis and they were treated with anti-inflammatory drugs and antibiotic therapy, achieving complete resolution. Uncontrolled diabetes mellitus and trauma can favor the development of OM in patients with bone exostoses, therefore, when there are signs of infection in these structures, it is imperative to suspect the condition and offer treatment., Los torus son prominencias óseas benignas que generalmente no requieren tratamiento, sin embargo, en ocasiones pueden presentar osteomielitis (OM). El objetivo de este artículo es describir por medio de una serie de casos clínicos el manejo oportuno de osteomielitis en torus. Se informa de tres pacientes con OM. Dos casos de masculinos de 66 y 69 años, diabéticos, así como una mujer de 57 años con antecedente de carcinoma basocelular de nariz. Todos presentaban úlceras en los torus asociadas a traumas y sin resolución de las mismas. En los tres casos se realizó biopsia para confirmar el diagnóstico y fueron tratados con antiinflamatorios y antibioticoterapia, logrando la resolución completa. La diabetes mellitus no controlada y el trauma pueden favorecer el desarrollo de OM en pacientes con exostosis óseas, por lo tanto, ante signos de infección en estas estructuras es imperativo sospechar de la condición y ofrecer tratamiento.

Current research highlighted the importance to recognize feasible biomarkers for early diagnoses and treatment in oral cancer. Our study analyzed the expression and spatial distribution of ALDH1A1, FGFR2, caspase-3, and CD44 in Oral Squamous Cell Carcinoma (OSCC) and leukoplakia with and without oral mucosal dysplasia. Paraffin-embedded samples of OSCC (n=5), leukoplakia with (n=5) and without (n=5) dysplasia obtained by incisional biopsies were processed using conventional histochemical techniques. Immunohistochemistry was performed using antibodies against ALDH1A1, FGFR2, caspase-3, and CD44. Images of the immunohistochemically stained tissue sections were analyzed according to the intensity of the immunostaining of each marker and classified in Scores. The Kruskal-Wallis test was performed (p≤0.05). Our results demonstrated a statically difference in the expression of all immunomarkers between OSCC and leukoplakia without dysplasia, being more significant in FGFR2 and ALDH1A1. Within the limitations of this study, our data showed that all biomarkers were overexpressed in OSCC and leukoplakia with oral mucosa dysplasia, suggesting that the presence of dysplasia is a significant clinic-pathologic predictor for malignant transformation., La actual evidencia científica enfatiza la importancia de reconocer biomarcadores viables para el diagnóstico y tratamiento temprano del cáncer oral. Nuestro estudio piloto analizó la expresión y distribución espacial de ALDH1A1, FGFR2, caspasa-3 y CD44 en carcinoma oral de células escamosas (COCE) y en leucoplasia con o sin displasia de la mucosa oral. Las muestras incluidas en parafina de COCE (n=5), con (n=5) y sin (n=5) displasia fueron obtenidas mediante biopsias incisionales, las cuales se procesaron utilizando técnicas histoquímicas convencionales. El análisis inmunohistoquímico se realizó utilizando anticuerpos contra ALDH1A1, FGFR2, caspasa-3 y CD44. Las imágenes de las secciones de cada muestra fueron analizadas según la intensidad de inmunoexpresión de cada marcador y se clasificaron en diferentes escalas (scores). Se realizó la prueba de Kruskal-Wallis (valores de p<0,05). Nuestros resultados demostraron una diferencia estadística en la expresión de todos los inmunomarcadores entre COCE y las muestras con leucoplasia sin displasia, siendo más significativa en FGFR2 y ALDH1A1. Considerando las limitaciones de este estudio, los datos sugieren que la presencia de displasia en la mucosa oral es un importante predictor clínico-patológico de transformación maligna.

Exfoliative cheilitis (EC) is a rare and chronic inflammatory condition. It is characterized by a continuous peeling of the vermilion of the lips and abnormal production of keratin scales. Treatment is difficult due to its chronic nature and it is often refractory. The objective of this article is to describe the management of EC and its impact on quality of life through two clinical cases. Two patients with EC are reported: one 21 years old with allergic rhinitis and gastric reflux, the other 18 years old, with a history of anxiety symptoms, without medication or psychiatric control. They had chronic scaly and crusty areas on both lip vermilions. In one of the patients, a biopsy was prescribed and he was treated with antifungals and topical vitamin E. Both patients were prescribed hydration, balanced diet, the application of moisturizing lip balm with sunscreen and modification of parafunctional habits, behavioral aid. The treatment healed the condition and eased their daily life and improved personal perception. Nibbling and lip licking favor dryness and superinfection in patients with EC; for its part, hydration, topical vitamin E and behavioral aid, improved this condition., La queilitis exfoliativa (QE) es una afección inflamatoria crónica y poco común. Se caracteriza por una descamación continua del bermellón de los labios y producción anormal de escamas de queratina. El tratamiento es difícil debido a su naturaleza crónica y suele ser refractaria. El objetivo de este artículo es describir por medio de dos casos clínicos el manejo de la QE y su impacto en la calidad de vida. Se informa de dos pacientes con QE, uno de 21 con rinitis alérgica y reflujo gástrico, el otro de 18 años, con antecedente de cuadros de ansiedad, sin medicación ni control psiquiátrico. Presentaban áreas descamadas y costrosas en ambos bermellones labiales de forma crónica y sin resolución. En un caso se realizó biopsia y fue tratado con antifúngicos y vitamina E tópica. En ambos pacientes se insistió en la hidratación, alimentación balanceada, uso de bálsamo labial humectante con protector solar y modificación de hábitos parafuncionales, ayuda conductual logrando una resolución que facilitaba sus labores cotidianas y mejorando la percepción personal. El mordisqueo y lamer los labios favorecen la resequedad y sobreinfección en pacientes con QE; por su parte, la hidratación, la vitamina E tópica y la ayuda conductual favorecen en mejorar esta condición.

In the oral cavity, gingiva lesions may occur that are not associated with bacterial plaque, which require adequate diagnosis and treatment. Desquamative gingivitis is usually related to mucocutaneous disorders, such as pemphigus vulgaris (PV), where oral lesions, including gingival lesions, may precede skin lesions. Management is multidisciplinary and treatment includes topical and systemic pharmacological therapy, require adequate control of dental plaque by the patient and strict supervision over time by the professional for the maintenance and stability of the gingival tissues. Two clinical cases of patients with a diagnosis of PV are presented, which were managed in a timely and integral way to control and stabilize the systemic and local factor., En la cavidad oral se pueden presentar lesiones en gíngiva que no están asociadas a placa bacteriana, las cuales requieren de un adecuado diagnóstico y tratamiento. La gingivitis descamativa está usualmente relacionada con desórdenes mucocutáneos, como el pénfigo vulgar (PV), donde las lesiones orales incluyendo las lesiones gingivales, pueden preceder las lesiones cutáneas. El manejo es multidisciplinario y el tratamiento incluye terapia farmacológica tópica y sistémica, se requiere un adecuado control de la placa bacteriana por parte del paciente y una estricta supervisión en el tiempo por parte del profesional para el mantenimiento y estabilidad de los tejidos gingivales. Se presentan dos casos clínicos de pacientes con diagnóstico de PV los cuales fueron manejados de manera oportuna e integral para controlar y estabilizar el factor sistémico y local.

The objective of this study is to determine the existing concordance between the clinical and histopathological diagnosis of lesions in the oral mucosa at the Faculty of Dentistry of the University of Costa Rica (UCR). A retrospective study of 261 reports of oral lesions recovered from the biopsy archive of the UCR Faculty of Dentistry from 2008 to 2015. Of the total reports 165 were included in the analysis, 96 (36.8%) did not have any diagnostic hypotheses. The concordance between the clinical and histopathological diagnosis was verified by the Kappa test. Concordance with the first diagnostic hypothesis was presented in 114 (69.1%) cases, the value of kappa was 0.663 (substantial agreement) and 36.8% without clinical diagnosis only a description of the lesion. The premalignant lesions presented an excellent concordance (kappa=0.902). The concordance of the group of non-neoplastic proliferative lesions was moderate (kappa=0.504) and the dermatological and autoimmune conditions with negligible concordance (0.157). Concordance occurred in the majority of the patients investigated with a value corresponding to a substantial agreement, however, the percentage of reports that did not have a clinical hypothesis should be improved., El objetivo de este estudio es determinar la concordancia existente entre el diagnóstico clínico e histopatológico de las lesiones en la mucosa oral en la Facultad de Odontología de la Universidad de Costa Rica (UCR). Es un estudio retrospectivo de 261 informes de lesiones orales recuperados del archivo de biopsias de la Facultad de Odontología de la UCR de 2008 a 2015, fueron analizados 165 reportes que cumplian con los criterios de inclusión. La concordancia entre el diagnóstico clínico e histopatológico fue verificada mediante el test Kappa. Del total de los informes, 96 (36.8%) no contaban con alguna hipótesis diagnóstica. La concordancia con la primera hipótesis diagnóstica se presentó en 114 (69.1%) casos, el valor de kappa fue de 0.663 (concordancia sustancial). Las lesiones premalignas presentaron una concordancia excelente (kappa=0.902). La concordancia del grupo de lesiones proliferativas no neoplásicas fue moderada (kappa=0.504) y las condiciones dermatológicas y autoinmunes con una concordancia insignificante (0.157). La concordancia se produjo en la mayoría de los pacientes investigados con un valor correspondiente a un acuerdo sustancial, sin embargo, se debe mejorar el porcentaje de informes que no contaban con hipótesis clínica.

Oral lichen planus (OLP) is a chronic inflammatory and immune-mediated disease that presents as bilateral, symmetrical oral lesions with three main clinical presentations: reticular, atrophic, and erosive. It is clinically characterized by pain, burning sensation, and discomfort for sufferers. Currently OLP is classified as a potentially malignant oral disorder. It has been associated with numerous systemic connotations such as diabetes mellitus, hypertension, metabolic syndrome, thyroid diseases, psychosomatic ailments, chronic liver disease, and gastrointestinal diseases. Diagnosis is made by clinical and histological examinations. It is a non-curable disease, the treatments are applied seeking the reduction or disappearance of the associated pain, as well as improving the clinical appearance of erosive-ulcerative lesions. The objective of this work was to present through a clinical case the difficulties in the treatment of OLP in a systemically compromised patient, highlighting the comprehensive assessment of the same, failed pharmacological therapies, until the symptomatology diminished and the patient's quality of life improved., El liquen plano oral (LPO) es una enfermedad crónica inflamatoria e inmunomediada, que se presenta como lesiones orales bilaterales, simétricas con tres presentaciones clínicas principales: reticular, atrófico y erosivo. Se caracteriza clínicamente por dolor, sensación de ardor y malestar para las personas que lo padecen. Actualmente LPO se clasifica como un desorden oral potencialmente maligno. Se ha relacionado con numerosas connotaciones sistémicas como diabetes mellitus, hipertensión, síndrome metabólico, enfermedades de la tiroides, dolencias psicosomáticas, enfermedad hepática crónica y enfermedades gastrointestinales. El diagnóstico se realiza mediante exámenes clínicos e histológicos. Es una enfermedad no curable, los tratamientos se aplican buscando la reducción o desaparición del dolor asociado, así como mejorar el aspecto clínico de las lesiones de carácter erosivo-ulcerativo. El objetivo de este trabajo fue presentar por medio de un caso clínico las dificultades en el tratamiento de LPO en un paciente sistémicamente comprometido, resaltando la valoración integral del mismo, terapias farmacológicas fallidas, hasta disminuir la sintomatología y mejorar la calidad de vida del paciente.

Bone lesions of the jaws have their origin from odontogenic and non-odontogenic structures. They can be benign or malignant, asymptomatic, they can be located around the root of the tooth, around the crown and in the interradicular area or they may not be related to the teeth. OBJECTIVE: to determine the frequency of the different bone lesions and the concordance between the clinical and histopathological diagnosis, in the clinical internship of the Faculty of Dentistry of the University of Costa Rica (UCR). METHODOLOGY: retrospective study of bone lesions recovered from the biopsy archive of the Faculty of Dentistry of the UCR from 2008 to 2015. Information on sex, age, location of the lesion, clinical diagnosis and diagnosis were evaluated and described. The agreement between the clinical and histopathological diagnosis was verified by the Kappa test. RESULTS: The 77 cases of oral bone lesions preferentially affected men 53.8% (n=41), the average age was 34.7 years (s.d.±19.6) and with lesions predominantly located in the posterior jaw 36.4% (n=28) and anterior maxilla 35.1% (n=27). Odontogenic cysts (OC) 42.9% (n=33), non-specific or unclassified diagnosis 28.6% (n=22) and inflammatory lesions of pulp and periapical origin 14.2% (n=11). TOs accounted for 7.8% (n=6) of the lesions. The four most predominant lesions were the radicular cyst, nonspecific diagnosis, dentigerous cyst and periapical granuloma. Concordance with the first diagnostic hypothesis was presented in 24 (31.2%) cases, the value of Kappa was 0.274 (discrete concordance) and 20.8% without clinical diagnosis only a description of the lesion. CONCLUSIONS: The OC were the predominant; being individually the radicular cyst the most frequent lesion. The clinical and histopathological concordance was discrete., Las lesiones óseas de los maxilares tienen su origen a partir de estructuras odontogénicas y no odontogénicas. Pueden ser benignas o malignas, asintomáticas, pueden ubicarse alrededor de la raíz del diente, alrededor de la corona y en el área interradicular o pueden no tener relación con los dientes. OBJETIVO: determinar la frecuencia de las diferentes lesiones óseas y la concordancia existente entre el diagnóstico clínico e histopatológico, en el internado clínico de la Facultad de Odontología de la Universidad de Costa Rica (UCR). METODOLOGÍA: estudio retrospectivo de lesiones óseas recuperados del archivo de biopsias de la Facultad de Odontología de la UCR del 2008 a 2015. Se evaluaron y analizaron de forma descriptiva y cualitativa información sobre el sexo, edad, localización de la lesión, diagnóstico clínico y diagnóstico histopatológico. La concordancia entre el diagnóstico clínico e histopatológico fue verificada mediante el test Kappa. RESULTADOS: los 77 casos de lesiones óseas orales afectaron preferentemente a los hombres 53.8 % (n=41), el promedio de edad fue 34.7±19.6 años y con lesiones predominantemente localizadas en mandíbula posterior 36.4 % (n=28) y maxila anterior 35.1% (n=27). Los quistes odontogénicos (QO) 42.9% (n=33), diagnóstico no específico o sin clasificación 28.6% (n=22) y lesiones inflamatorias de origen pulpar y periapical 14.2% (n=11). Los TO representaron el 7.8% (n=6) de las lesiones. Las cuatro lesiones más predominantes fueron el quiste radicular, diagnóstico inespecífico, quiste dentígero y granuloma periapical. La concordancia con la primera hipótesis diagnóstica se presentó en 24 (31.2%) casos, el valor de Kappa fue de 0.274 (concordancia discreta) y un 20,8 % sin diagnóstico clínico solo una descripción de la lesión. CONCLUSIONES: Los QO fueron los predominantes; siendo en forma individual el quiste radicular la lesión más frecuente. La concordancia clínica e histopatológica fue discreta.

Celiac Disease (CD) is an immune-mediated systemic disease, caused by gluten and related prolamins, in genetically susceptible individuals. It is characterized by gluten-dependent clinical manifestations, CD-specific antibodies, HLA DQ2 or DQ8 haplotypes, and enteropathy. Most patients are asymptomatic or with mild manifestations. It mainly affects the gastrointestinal system causing symptoms and signs such as diarrhea, recurrent abdominal pain and abdominal distension; also extradigestive manifestations. Patients with CD may have oral manifestations that include recurrent aphthous stomatitis (RAS), enamel hypoplasia, atrophic glossitis, among others. The diagnosis of CD is based on clinical history, serology, endoscopy and histology. There are other entities that should be differentiated from CD, such as non-celiac gluten sensitivity and wheat allergy. The aim of this study is to present three patients with a history of RAS and other clinical manifestations associated with the early diagnosis of CD and food allergy., La Enfermedad Celíaca (EC) es una enfermedad sistémica inmunomediada, provocada por el gluten y prolaminas relacionadas, en individuos genéticamente susceptibles. Se caracteriza por manifestaciones clínicas dependientes del gluten, anticuerpos específicos de EC, haplotipos HLA DQ2 o DQ8 y enteropatía. La mayoría de los pacientes son asintomáticos o con manifestaciones leves. Afecta principalmente el sistema gastrointestinal causando síntomas y signos como diarrea, dolor abdominal recurrente y distensión abdominal; además manifestaciones extradigestivas. Los pacientes con EC pueden tener manifestaciones bucales que incluyen estomatitis aftosa recurrente (EAR), hipoplasia del esmalte, glositis atrófica, entre otras. El diagnóstico de EC se basa en la historia clínica, serología, endoscopía e histología. Existen otras entidades que deben diferenciarse de la EC, como la sensibilidad al gluten no celíaca y la alergia al trigo. El objetivo de este trabajo es presentar tres pacientes con antecedentes de EAR y otras manifestaciones clínicas asociadas al diagnóstico precoz de EC y alergia alimentaria.

Thymomas are the most common mediastinal neoplasms in adults. Patients with thymoma are frequently diagnosed with paraneoplastic syndromes and are generally associated with several autoimmune conditions. Sjögren's syndrome (SS) is an autoimmune inflammatory disorder, characterized by the lymphocyte-mediated destruction of the exocrine glands, which leads to the absence of glandular secretion, particularly salivary and lacrimal; hyposalivation can vary from a slight reduction in the flow of saliva with transient discomfort, to a serious deterioration of oral health and psychological distress. Mucous membrane pemphigoid (MMP) is a chronic, autoimmune disease that shows predominant mucosal involvement, most commonly affecting the oral cavity, followed by the conjunctiva, the nasal cavity and the esophagus; intraorally includes scaly gingivitis, vesicles, erosions covered by pseudomembranes and ulcers. The objective of this work is to present the first case of a patient with a history of thymoma, which, jointly presented SS and MMP; also highlight the importance of multidisciplinary management for an accurate and early diagnosis, as this increases the efficiency and efficacy of the treatment strategy., Los timomas son las neoplasias mediastínicas más comunes en adultos. Los pacientes con timoma son frecuentemente diagnosticados con síndromes paraneoplásicos y generalmente están asociados con varias condiciones autoinmunes. El síndrome de Sjögren (SS) es un trastorno inflamatorio autoinmune, que se caracteriza por la destrucción mediada por linfocitos de las glándulas exocrinas, que conduce a la ausencia de secreción glandular, en particular las salivales y las lagrimales; la hiposalivación puede variar desde una ligera reducción en el flujo de saliva con molestias transitorias, a un grave deterioro de la salud oral y malestar psicológico. El penfigoide de membranas mucosas (PMM) es una enfermedad autoinmune crónica, muestra predominante afectación de la mucosa, afectando con mayor frecuencia la cavidad oral, seguida de la conjuntiva, la cavidad nasal y el esófago; intraoralmente incluye gingivitis descamativa, vesículas, erosiones cubiertas por pseudomembranas y úlceras. El objetivo de este trabajo es presentar el primer caso de una paciente con antecedente de timoma, la cual, conjuntamente presentó SS y PMM; además evidenciar la importancia del manejo multidiciplinario para un diagnóstico preciso y temprano, ya que así, aumenta la eficiencia y la eficacia de la estrategia de tratamiento.

Introduction: Recurrent aphthous stomatitis (RAS) is the most common ulcerative disease that affects the non-keratinized oral mucosa. The etiology is still unknown, but several local and systemic factors have been proposed as causal agents. Case description: three patients are reported with RAS associated with nutritional deficiencies, due to dietary restriction diets. The first case, a 19-year-old woman with iron deficiency, reported taking green tea daily and consuming a small amount of carbohydrates, fats and meat. The second case, a 32-year-old man with iron deficiency and vitamin B12 associated with vegan diet, although sometimes if he consumed meat. The third case, man of 50 years, vegan and ex-smoker for a year, presented deficiency of iron, vitamin B12 and folic acid. In all three cases a detailed questioning, clinical examination and complete blood analysis allowed to establish an adequate diagnosis, dental management and referral with a nutritionist, in order to treat the underlying pathology and not only provide a symptomatic treatment of the injuries. Discussion: In the setting of RAS, it is advisable to perform an analysis that includes a blood count, folic acid, iron and vitamin B12, to rule out possible systemic causes and eventually treat them. Clinical management aims to improve patient function and quality of life through topical and systemic therapies; however, it is essential to identify and control the causal factors that contribute; as well as, the exclusion or treatment of the underlying systemic disease., Introducción: La estomatitis aftosa recurrente (EAR) es la enfermedad ulcerativa más común que afecta la mucosa oral no queratinizada. La etiología aún se desconoce, pero se han propuesto varios factores locales y sistémicos como agentes causales. Descripción del caso: se informa tres pacientes con presencia de EAR asociada a deficiencias nutricionales, debidas a dietas de restricción alimentaria. El primer caso, una mujer de 19 años con deficiencia de hierro, refirió tomar té verde a diario y consumir poca cantidad de carbohidratos, grasas y carne. El segundo caso, un hombre de 32 años con deficiencia de hierro y vitamina B12 asociada a dieta tipo vegana, aunque a veces si consumía carne. El tercer caso, hombre de 50 años, vegano y exfumador desde hace un año, presentó deficiencia de hierro, vitamina B12 y ácido fólico. En los tres casos un detallado interrogatorio, examen clínico y análisis sanguíneo completo, permitió establecer un adecuado diagnóstico, manejo odontológico y derivación con un nutricionista, con el fin de tratar la patología de base y no sólo brindarles un tratamiento sintomático de las lesiones. Discusión: Ante un cuadro de EAR es recomendable realizar un análisis que incluya hemograma, ácido fólico, hierro y vitamina B12, para descartar posibles causas sistémicas y eventualmente tratarlas. El manejo clínico tiene como objetivo mejorar la función del paciente y la calidad de vida mediante terapias tópicas y sistémicas; sin embargo, es fundamental identificar y controlar los factores causales que contribuyen; así como, la exclusión o tratamiento de la enfermedad sistémica subyacente.

Actinic cheilitis (AC) is a potentially malignant lip disorder induced by chronic exposure to ultraviolet light, which mainly affects the vermilion of the lower lip of adults and fair skin. If it is not treated in a timely manner it can become a squamous cell carcinoma of the lip. Histologically, AC presents a series of alterations at the epithelial level ranging from hyperplasia to dysplasia; Connective tissue presents different degrees of actinic elastosis accompanied by inflammatory infiltrate. The objective of this work is to present three cases of patients with AC and analyze their clinical and histopathological characteristics., La queilitis actínica (QA) es un desorden potencialmente maligno del labio inducido por la exposición crónica a la luz ultravioleta, que afecta principalmente al bermellón del labio inferior de personas adultas y de piel clara. Si no es tratada en forma oportuna puede transformarse en un carcinoma de células escamosas del labio. Histológicamente la QA presenta una serie de alteraciones a nivel epitelial que van desde hiperplasia a displasia; en el tejido conectivo se presentan distintos grados de elastosis actínica acompañada por infiltrado inflamatorio. El objetivo de este trabajo es presentar tres casos de pacientes con QA y analizar sus características clínicas e histopatológicas.

The extracellular matrix (ECM) plays an important role in the regulation of biological events, such as cell migration, proliferation and differentiation. Chronic exposure to ultraviolet (UV) light causes elastosis (to varying degrees), which corresponds to a basophilic degeneration of the ECM. Actinic cheilitis (AC) is a potentially malignant lip lesion induced by regular and prolonged exposure to UV light, which mainly affects the vermilion. AC lesions have a complex stroma characterized by the presence of elastosis, chronic inflammatory infiltrate of different intensity and the appearance of telangiectatic blood vessels. Within this inflammatory infiltrate a significant increase of mast cells (MCs) has been described, located especially around areas of elastosis and at the subepithelial zone. It has been proposed that actinic elastosis is produced both, by degenerative processes and by abnormal synthesis of elastic fibers by photodamaged fibroblasts, which is accompanied by morphological changes in collagen. Although the fibroblast would play a major role in actinic elastosis formation, several studies suggest that other cell types such as MCs also contribute significantly to actinic ECM damage. The purpose of this review is to discuss the characteristics of elastosis in AC., La matriz extracelular (MEC) juega un papel importante en la regulación de los eventos biológicos, tales como, el desarrollo de la migración celular, proliferación y diferenciación. La exposición crónica a la luz ultravioleta (UV) provoca elastosis (en distintos grados), que corresponde a una degeneración basófila de la MEC. La queilitis actínica (QA) es una lesión potencialmente maligna del labio inducida por la exposición regular y prolongada a la luz UV, que afecta principalmente al bermellón del labio inferior. Las lesiones de QA tienen un estroma complejo, se observa siempre la presencia de elastosis, infiltrado inflamatorio crónico de distinta intensidad y la aparición de vasos sanguíneos telangiectásicos. Dentro de este infiltrado inflamatorio se ha descrito un aumento significativo de mastocitos (MCs), localizados especialmente alrededor de las zonas de elastosis y en la zona subepitelial. Se ha propuesto que la elastosis actínica se produce tanto por procesos degenerativos como de síntesis anormal de fibras elásticas por parte de fibroblastos con daño solar, lo que va acompañado de cambios morfológicos del colágeno. A pesar de que el fibroblasto tendría un rol preponderante en la formación de la elastosis actínica, diversos estudios sugieren que otros tipos celulares como el MC también contribuirían en forma significativa al daño actínico de la MEC. El propósito de esta revisión es analizar las características de la elastosis en la QA.

Oral squamous cell carcinoma (OSCC) is the most common malignant neoplasm of epithelial origin in the oral cavity, usually affecting men over 40 years of age. The most frequent localization sites are the tongue and the floor of the mouth. The etiology is multifactorial being tobacco and alcohol the most important risk factors. The clinical presentation is variable and although it is preceded by visible changes in the oral mucosa, the diagnosis is usually made late. An early diagnosis is critically important to improve survival and treatment outcomes. Oral cancer screening is effective in reducing the mortality and morbidity of oral cancer. The purpose of this study was to present a case of OSCC and to highlight the importance of an early diagnosis using oral cancer screening, and the review of the relevant literature as for this pathology.

Basaloid Squamous Cell Carcinoma (BSCC) is a rare and aggressive variant of squamous cell carcinoma (SCC), with a predilection for the upper aerodigestive tract, the palate being a very rare site of development. It is constituted by a basaloid and a squamous components in variable proportions, however, the histopathological characteristics of the BSCC overlap with other neoplasias. Diagnosis with various immunohistochemical markers such as p63, Ki67, p16, and AE1 / AE3 cytokeratins have become a necessity for the specialist. The objective of this study is to present a case of BSCC in soft palate and to show the importance of the support in the immunohistochemistry for an early histopathological diagnosis. A review of the literature on this pathology is also presented.
