Aim: The present study aimed to detect changes in brain oxygenation associated with the use of surgical and FFP2/N95 masks using the near infrared spectroscopy (NIRS) method.Material and Methods: Volunteers wearing surgical masks were asked to sit upright for 30 minutes. Cerebral oxygen saturation values were measured at the 1st, 5th, and 30th minutes (group 1). The mask wearers were requested to return at the same time on the day following this procedure. In group 2, serial cerebral oxygen saturation values (SpO2) were obtained at the same time intervals as in group 1, but while the subjects were wearing FFP2/N95 masks.Results: A statistically significant difference was found between the groups in the comparison of the values measured using NIRS at the 1st, 5th, and 30th minutes in group 1. According to the post-hoc analysis, this difference was due to discrepancies in NIRS measurements at the 1st minute and 30th minute and at the 5th minute and 30th minute. No statistically significant difference was found between the groups in the comparison of the values measured using NIRS at the 1st, 5th, and 30th minutes in group 2.Conclusion: Neither surgical nor FFP2/N95 masks caused a clinically significant negative difference in brain oxygenation., Amaç: Bu çalışma, Near infrared spektroskopi (NIRS) yöntemini kullanarak cerrahi ve FFP2/N95 maskelerinin kullanımıyla ilişkili beyin oksijenasyonundaki değişiklikleri tespit etmeyi amaçladı.Gereç ve Yöntemler: Cerrahi maske takan gönüllülerden 30 dakika dik oturmaları istendi. 1., 5. ve 30. dakikalarda (grup 1) serebral oksijen satürasyonu değerleri ölçüldü. Maske takanların bu işlemin ertesi günü aynı saatte gelmeleri istendi. Grup 2'de seri serebral oksijen satürasyonu değerleri (SpO2) grup 1 ile aynı zaman aralıklarında ancak denekler FFP2/N95 maskesi takarken elde edildi.Bulgular: Grup 1'de 1., 5. ve 30. dakikalarda NIRS ile ölçülen değerlerin karşılaştırılmasında gruplar arasında istatistiksel olarak anlamlı fark bulundu. AimThis study aimed to compare the Rotterdam and Helsinki computed tomography (CT) scoring systems for predicting the 30-day mortality after traumatic brain injury (TBI) in the geriatric population.Materials and MethodsPatients aged ≥65 years presenting to the emergency department with trauma-related complaints were retrospectively scanned using International Classification of Disease codes, and patients with isolated head trauma examined using brain CT were included. Demographic data including age, gender, trauma mechanisms, Glasgow Coma Scale (GCS) score at the time of admission, light reflex information, intubation, and surgery status, and emergency department outcomes were recorded. Brain CT images were investigated to calculate the Rotterdam and Helsinki CT scores and the relationship between them was examined.ResultsOf the 890 included patients, 403 (45.3%) were male. Hastaların yaş, cinsiyet gibi demografik verileri, travma mekanizmaları, geliş muayenesinde Glasgow Koma Skalası (GKS), ışık refleksi bilgileri, entübe olup olmadığı ve opere olup olmadığı, acil servis sonlanım bilgisi taranarak kaydedildi. Hastaların beyin BT görüntüleri incelenerek Rotterdam ve Helsinki BT skorları hesaplandı ve bunlar arasındaki ilişkiye bakıldı.BulgularÇalışmamıza dahil edilen 890 hastanın 403 (%45.3) erkekti. Çalışmamızda 683 hastanın 1 metreden daha düşük yükseklikten düştüğü, 195 tanesinin çarpma veya direkt darbe alma şeklinde olduğu görüldü. Hastaların bir aylık mortalite bilgisine bakıldığında 22 hastanın öldüğü ve 868 hastanın sağ olduğu saptandı. Erkek hastalarda ölüm oranı %3,7 iken kadın hastalarda bu oran %1,4 olarak bulunmuş olup mortalite açısından cinsiyetler arasında anlamlı fark saptandı. Hastaların Rotterdam ve Helsinki BT Skorları ve bir aylık mortaliteleri ROC analizi ile incelendiğinde Rotterdam BT Skoru için eğri altında kalan alan sırasıyla 0.564 ve 0.603 olarak bulundu. Hastaların Rotterdam BT Skoru ve Helsinki BT skoru 1 aylık mortaliteyi tahmin etmede spesifitesi sırayla %99,08 ve %99,19 olarak hesaplandı. SonuçTBH ile başvuran geriatrik popülasyonda Rotterdam ve Helsinki gibi BT skorlamalarının kullanımı bize 30 günlük mortaliteyi tahmin etmemizi olanak sağlamaktadır.

Amaç: Çalışmamızda acil servise başvuran ve akut renal kolik tanısı alan hastalarda acil tıp uzmanı ve asistanların tanı ve tedavi stratejilerini belirlemeyi amaçladık.Gereç ve Yöntemler: Bu çalışmada acil tıp doktorlarına internet tabanlı kesitsel bir anket yapıldı. Ankette katılımcılara ait demografik verileri, acil serviste çalışma süreleri ve acil tıp uzmanı veya asistanı oldukları soruldu. Katılımcılara acil servise başvuran akut renal kolik hastalarında ilk tercih görüntüleme yöntemleri, tetkikler, hangi durumlarda üroloji konsültasyonu istedikleri, analjezik ve tedavi tercihleri soruldu. Bulgular: Çalışmaya katılan 108 gönüllünün yaş ortancası 31 (27 – 34,8) yıl olarak hesaplandı. Katılımcıların %60,2’i (n=65) acil tıp asistanı, %39,8’i (n=43) acil tıp uzmanı olan çalışma grubunun acil serviste çalıştığı sürenin yıl cinsinden ortancası 4 (2-8) yıl olarak hesaplandı. Acil tıp uzmanı ve asistanlarının renal kolik ön tanısı ile acil servise başvuran hastalardaki yaklaşımları ile ilgili değişkenler açısından farkları incelendi. Non-steroidal antienflamatuvar ilaç (NSAID) verilmesi planlanan hastalarda seçilecek ilk ilaç, diklofenak ve diğer NSAID olarak dikotomize edildiğinde, uzmanların 34 (%79,1)’nün, asistanların ise 60 (%92,3)’ının ilk tercih olarak diklofenak seçtiği görülmüş ve gruplar arasında istatistiksel anlamlı fark saptandı.Sonuç: Çalışmamızda acil tıp uzmanı ve asistanı arasında renal kolik görüntüleme yöntemi açısından benzer tercihler yapmasına rağmen birinci basamak tedavide acil tıp asistanları, acil tıp uzmanlarına kıyasla daha çok Diklofenak’ı tercih etmiştir., Objective: This study was conducted to describe the current diagnosis and treatment strategies of patients admitted to the emergency department and diagnosed with acute renal colic.Material and Methods: In this study, an internet-based cross-sectional survey was administered to emergency medicine physicians. In addition to the participants’ demographic data, such as age and gender, years of experience in the emergency medicine department, and whether they were a specialist or resident, the survey also inquired about their choices of imaging methods, laboratory tests, analgesic medications, and intravenous fluids, and tendency to refer patients to a urologist for a consultation.Results: The median age of the 108 volunteers participating in the study was calculated as 31 (27 - 34.8) years and their years of experience as 4 (2-8) years. More than half (60.2%, n=65) of the participants were emergency medicine residents, while the remaining 39.8% (n=43) were emergency medicine specialists. The emergency medicine specialists and residents were compared in terms of the variables related to their approaches to patients presenting to the emergency department with a preliminary diagnosis of renal colic. The first choice of analgesic drugs was dichotomised as diclofenac and other non-steroidal anti-inflammatory drugs, with 34 (79.1%) of the specialists and 60 (92.3%) of the residents reporting diclofenac as their first choice, and there was a statistically significant difference between the two groups.Conclusion: In this study, the emergency medicine residents preferred diclofenac more than the emergency medicine specialists in primary care, although they had similar renal colic imaging methods preferences.

Study ObjectivesKnow the result of disease severity and clinical results in community-acquired pneumonia (CAP) are preconditions for treatment options and management for health care resources. Various scoring systems as CURB-65 and SMART-COP have been developed to facilitate these awareness. We aimed to investigate the relationship between these two scoring systems with procalcitonin level in the diagnosis of CAP.MethodsStudy included hospitalized patients diagnosis CAP that had been admitted to the emergency department between 01.01.2015 - 12.31.2015. CURB-65 and SMART-COP scores were calculated. We collected measured procalcitonin levels. As described previously during the study, patients who had 2 and over values for CURB-65 and who had values 3 or more for SMART-COP were classified as high risk and groupings were structured according to these values.ResultsThe study was conducted on a total of 124 cases. 72 of the cases had a CURB-65 score of 2 or more and 49 of the cases had a SMART-COP score of 3 or above.
