BACKGROUND Since its outbreak in early 2020, the COVID-19 pandemic has diverted resources from non-urgent and elective procedures, leading to diagnosis and treatment delays, with an increased number of neoplasms at advanced stages worldwide. The aims of this study were to quantify the reduction in surgical activity for indeterminate thyroid nodules during the COVID-19 pandemic; and to evaluate whether delays in surgery led to an increased occurrence of aggressive tumours. METHODS In this retrospective, international, cross-sectional study, centres were invited to participate in June 22, 2022; each centre joining the study was asked to provide data from medical records on all surgical thyroidectomies consecutively performed from Jan 1, 2019, to Dec 31, 2021. Patients with indeterminate thyroid nodules were divided into three groups according to when they underwent surgery: from Jan 1, 2019, to Feb 29, 2020 (global prepandemic phase), from March 1, 2020, to May 31, 2021 (pandemic escalation phase), and from June 1 to Dec 31, 2021 (pandemic decrease phase). The main outcomes were, for each phase, the number of surgeries for indeterminate thyroid nodules, and in patients with a postoperative diagnosis of thyroid cancers, the occurrence of tumours larger than 10 mm, extrathyroidal extension, lymph node metastases, vascular invasion, distant metastases, and tumours at high risk of structural disease recurrence. Univariate analysis was used to compare the probability of aggressive thyroid features between the first and third study phases. The study was registered on ClinicalTrials.gov, NCT05178186. FINDINGS Data from 157 centres (n=49 countries) on 87 467 patients who underwent surgery for benign and malignant thyroid disease were collected, of whom 22 974 patients (18 052 [78·6%] female patients and 4922 [21·4%] male patients) received surgery for indeterminate thyroid nodules. Objective Mastalgia is the most common complaint of patients who are admitted for breast examination. Breast pain may originate breast tissue pathologies or non-breast pathologies adjacent organs. One of the causes of mastalgia is the varicella-zoster infection of the thoracic nerve ganglions. The zona zoster infection is painful, and vesicular lesions in the infected dermatomal regions can be observed because of the reactivation of the latent varicella (Herpes)-zoster virus (VZV) in the dorsal radix of medulla spinalis. There are no reviews of the zona cases of mastalgia patients in the literature. We aimed to investigate and represent the characteristics of these patients. Materials and methods Patients complaining of mastalgia and who were diagnosed with zona zoster infection after physical examination and clinical evaluation in the outpatients department of General Surgery were investigated retrospectively between January 2010 and January 2015. Results The study included 12 patients. All of them were female, and the mean age of patients was 51.66 (36-72) years. Eight of the zona cases were seen in the right breast (66.6%), and four of them were seen in the left breast (33.4%). Complaints of patients were pain (100%), eruption (70%), and burning sensation (60%). Underlying pathology was seen in one of the cases. Physical examination at admission revealed that four of the patients did not have any physical abnormality (33.3%). On the contrary of vesicular lesions, typical physical findings of zona, were seen in eight patients (66.7%). Conclusion Detailed history analysis and physical examination of the breast should be performed, particularly in older patients with unilateral severe mastalgia, and zona should be considered.

Introduction: This study aims to review thyroidectomy cases performed by a single surgeon in a single centre during the course of 1 year, to determine if the malignancy rate increased, and to compare the results with the literature. Material and methods: We retrospectively evaluated data of patients who underwent thyroidectomy between August 2018 and August 2019 in the General Surgery Clinic of our hospital. The malignancy rate was calculated and analysed according to the demographic data of the patients (gender and age), preoperative thyroid nodule sizes, and postoperative pathological mass sizes. Also, age, preoperative nodule size, and tumour size were all evaluated according to gender. Results: There were a total of 60 patients (14 (23%) male and 46 (77%) female). The pathology result revealed 16 (26.7%) malignant and 44 (73.3%) benign biopsies. The mean age was 46.72 ± 12.08 years (20-74). We found that age, preoperative nodule size (cm), pathological tumour size (cm), and thyroid function tests were not significantly associated with malignancy. However, when age, preoperative nodule size, and tumour sizes were evaluated according to gender, it was found that tumour sizes were significantly larger in men than in women (p < 0.001). In addition, the mean age of male patients was higher than that of women (p = 0.025). Conclusions: The malignancy rate has increased in the thyroidectomies performed in our centre. This finding is consistent with relevant studies from the last 30 years. Therefore, it is likely that clinicians will continue to handle an increased number of thyroid malignancies. Giriş: Gastrointestinal sistem kanserlerinde özellikle CEA, CA-19.9 ve AFP düzeyleri yardımcı olan tümör belirteçleridir.Yöntem ve Gereç: Kliniğimizde Ocak 2008–Aralık 2013 yılları arasında mide kanseri tanısı ile opere edilen 54 hastanın preoperatif serum tümör belirteci düzeyleri, cinsiyet, yaş, evre ve tümör yayılım göstergeleri (karaciğer metastazı, omental tutulum, serozal tutulum, lenf nodu tutulumu) incelendi.Bulgular: TNM evrelemesine göre evre-I 9 (%16.7), evre-II 6 (%11.1), evre-III 35 (%64.8) ve evre-IV 4 (%7.4) hasta izlendi. CEA, 37’sinde (%68.5) normal, 17’sinde (%31.5) yüksekti. Tümör evresi arterken CEA düzeylerindeki artışın anlamlı olmadığı (p=0.161) ancak erken evre mide kanseri (Evre-I) hastalarında ileri evre mide kanseri (Evre-IV) hastalarına oranla preoperatif CEA düzeylerinin anlamlı derecede düşük olduğu (p=0.027) izlendi. Karaciğer metastazı, omental tutulum, lenf nodu tutulumu ve serozal invazyon ile CEA yüksekliği arasında ilişki izlenmedi.CA-19.9,43 hastada (%80) normal, 11 hastada (%20) yüksekti. Tümör evresi ile serum CA-19.9 arasında anlamlı bir ilişki görülmedi (p=0.88). Evre-I hastalar ile Evre-IV hastalar arasında preoperatif CA19.9 düzeyleri açısından anlamlı farklılık izlenmedi (p=0.538). Lenf nodu tutulumu, karaciğer metastazı, omental tutulum ve serozal invazyon açısından CA19.9 yüksekliği anlamlı değildi.AFP, 41 hastada (%76) normal, 13 hastada (%24) yüksek izlendi. Tümör evresi ile anlamlı bir ilişki göstermediği izlendi (p=0.478). Evre-I hastalar ile Evre-IV hastalar arasında preoperatif AFP düzeyleri açısından anlamlı farklılık izlenmedi (p=0.14). Karaciğer metastazı, lenf nodu tutulumu, omental tutulum ve serozal invazyon ile serum AFP düzeyleri arasından anlamlı bir ilişki izlenmemiştir. Cinsiyet ve yaş açısından CEA, CA19.9 ve AFP yüksekliğinde anlamlı farklılık izlenmedi.Sonuç: CEA düzeyleri ileri evre mide kanseri hastalarında anlamlı derecede yüksekti. Bu nedenle CEA düzeylerindeki yüksekliğin mide kanserlerinde ileri tümör evresinin habercisi olabileceğini ancak düşük değerlerde olmasının tümörün olmadığını ya da erken evre olduğunu düşündürmemesi gerektiği kanısındayız.

Bu çalışmanın amacı obstrüksiyon yapmış kolorektal kanserler nedeniyle kliniğimizde acil şartlarda ameliyat edilen hastalardaki morbidite ve mortalite oranlarını ve bunlara etki eden faktörleri sunmaktır. Materyal ve Metod Ocak 2008 - Temmuz 2012 yılları arasında mekanik bağırsak tıkanıklığına neden olmuş kolorektal kanser nedeniyle ameliyat edilen 10 erkek ve 8 kadın olmak üzere 18 hasta retrospektif olarak değerlendirildi. Hastalar yaş, cinsiyet, komplikasyon, yapılan ameliyat, morbidite, mortalite açısından incelendi. Yaşın, cinsiyetin ve tümör yerleşiminin morbidite üzerine olan etkileri incelendi. Bulgular Hastaların yaş ortalaması 66 ± 8,6 idi. 70 yaş üzerinde 10 (% 56,6) vardı. Hastaların 16’sında komplet bir obstrüksiyon izlendi (% 88,8). Bir hastada obstrüksiyonla beraber perforasyon mevcuttu (% 5,6). On iki hastaya Hartmann prosedürü (% 66,7) uygulanırken, 2 hastaya loop kolostomi (% 11,2), 2 hastaya sağ hemikolektomi (% 11,2), 1 hastaya total kolektomi ileorektal anastomoz (% 5,6), 1 hastaya sağ hemikolektomi uç ileostomi (% 5,6) uygulandı. Beş hastada morbidite izlendi (% 27,8). İki hastada mortalite izlendi (% 11,2). Morbidite görülen tüm hastalar 70 yaş üzerindeydi (p=0.21). Cinsiyete göre ve tümörün yerleşim yerine göre ise morbidite oranları açısından anlamlı fark yoktu. Sonuç Obstrüktif kolorektal kanser nedeniyle acil operasyona alınan hastalarda mortalite ve morbidite izlenme olasılığı artmaktadır. Bunun nedenlerinden birisi hastaların daha çok ileri yaşlarda olmalarıdır. Ayrıca hastalarda perforasyon gibi eşlik eden bir komplikasyon varlığı mortalite ve morbiditeyi ciddi ölçüde arttırmaktadır. Aim The aim of this study is sharing the morbidity and mortality rates and risc factors efecting the morbidity and mortality rates of patients underwent emergency operation in our clinic. Materials and Methods Between January 2008 - July 2012 eighteen patients, 10 men and 8 women, who operated because of obstructive colorectal cancer, were reviewed. Patients were examined fora ge, sex, complication, operation type, morbidity and mortality. The effects of age, sex and tumor location on morbidity and mortality were examined. Results Mean age of patients was 66 ± 8,6. Ten patients were elder than 70 (% 56,6). Complet obstruction was seen in 16 patients (% 88,8). In one patient perforation was seen wtih obstrucion (% 5,6). Hartmann’s procedure was performed for 12 patients (% 66,7), loop colostomy was performed for two patients (% 11,2), right hemicolectomy was performed for two patients (% 11,2), total colectomy with ileorectal anastomosis was performed for 1 patient (% 5,6), right hemicolectomy with end ileostomy was performed for 1 patient (% 5,6). Morbidity was seen in 5 patients (% 27,8). Mortality was seen in two patients (% 11,2). Both patients with mortality were elder than 70 (p=0.21). There weren’t any significant difference for sex and tumor’s location. Conclusion Morbidity and mortality rates increases in patients whom underwent emergency surgery for obstructive colorectal cancer. Being elder patient is one of the reasons. And also accompanied complications like perforation searously increase mortality and morbidity.

Introduction: Iliac artery aneurisms are generally observed associated with abdominal aortic aneurisms. Iliac artery dissection happens to be one of the rarely encountered cases and very few cases have been reported in literature. We aimed to present a patient with both iliac artery aneurisms associated with left iliac artery dissection in this study. Case: A male, 34-years old patient applied to emergency service with complaints of abdominal pain and paresthesia in his left leg. There was no pain or loss of strength in the left leg of the patient, but there existed a numbness described by himself. The brain computed tomography (CT) of the patient was considered as normal but his abdominal ultrasonography was reported as "an aneurismal dilatation in both iliac artery at bifurcation level on a nearly 3 cm-segment, reaching up to 3 cm at the widest section and a mural thrombus reaching up to 70 % stenosis within the vein lumen were observed". Thereon, the patient went through a CT-angiogram and as a result dissection was observed in the left iliac artery together with aneurismal dilatation and mural thrombus in both iliac arteries The patient was hospitalized in the intensive care unit of cardiovascular surgery clinic. Conclusion: In regards to the patients who apply to emergency services with complaints of abdominal pain, numbness in extremities, acute abdomen or neurologic symptoms atypically seen like loss of strength; we should also keep in mind aortic dissection and/or iliac artery dissection among our preliminary diagnoses. AMAÇ: Safra taşları hastalığında Laparoskopik kolesistektomi kesin altın standart haline gelmiş bir tedavidir. Ancak akut kolesistitli hastalarda yaklaşımın ne olacağı bu kadar net olmamakla beraber, her geçen gün soğutma ve elektif operasyon planlama modalitesi değerini kaybedecek gibi gözükmektedir. Bu çalışmanın amacı, akut kolesistitli hastalara uygulanan erken kolesistektominin sonuçlarını irdeleyerek ortaya koymaktır. YÖNTEMLER: Şubat 2012 ile Aralık 2012 tarihleri arasında akut kolesistit tanısı alarak erken Laparoskopik kolesistektomi uygulanan 39 hasta retrospektif olarak incelendi. BULGULAR: 39 akut kolesistitli vakanın 35’inde başarılı laparoskopik kolesistektomi gerçekleştirildi, 4 hastada açık ameliyatla devam edildi. Toplam 24 (%61.5) hastada safra kesesi kolesistektomi öncesi aspire edildi. Otuz hastaya (%76.9) subhepatik alana dren konuldu. Üç hastada (%7.7) komplikasyon gelişti. SONUÇ: Akut kolesistitli hastaların tedavi algoritmasında erken dönemde yakalanmış olan vakalarda cerrahi düşünmek fikri ön plana çıkmaktadır. Gerek operasyon süresi ve gerekse postoperatif morbidite ve mortalite oranları ile erken akut kolesistitte Laparoskopik kolesistektomi etkin bir tedavi şekli olmaya devam etmektedir., OBJECTIVE: Laparoscopic cholecystectomy has become the gold standard for definitive treatment. of gallstones disease. However, approach to patients with acute cholecystitis is less clear and delayed surgery for acute cholesystitis seems to lose it’s value day by day. The purpose of this study, is to examine and reveal the results of Laparoscopic cholecystectomy in acute cholecystitis. METHODS: Between February 2012 and December 2012 thirty-nine patients with the diagnosis of acute cholecystitis who were treated laparoscopically were retrospectively analyzed.RESULTS: Laparoscopic cholecystectomy was successful in 35 of 39 cases of acute cholecystitis. Open surgery was continued in 4 patients. The gallbladder was aspirated total of 24 (61.5%) patients prior to cholecystectomy. Subhepatic space was drained in thirty-one patients (76.9%). Complications were developed in three patients (7.7%). CONCLUSION: In the treatment algorithm for patients with acute cholecystitis, early surgery come to the fore. Both operative time and postoperative morbidity and mortality rates of laparoscopic cholecystectomy in early acute cholecystitis continues to be an effective form of treatment.

Crohn's disease is an inflammatory disease of the gastrointestinal system which is characterized with transmural inflamation and remission - activatication periods. Meckel's diverticulum is the most common congenital abnormality of the gastrontestinal tract. In the Crohn's disease patients, Meckel's diverticula is seen two - three times above normal population. We are presenting a case admitting with ileus and operated in emergently because of the obstructive Crohn's disease with Meckel's diverticulum., Crohn hastalığı gastrointestinal sistemin, transmural inflamasyonla karakterize, remisyon ve alevlenme periyodları ile seyreden yangısal hastalığıdır. Gastrointestinal sistemin en sık rastlanan konjenital anomalisi Meckel divertikülüdür. Crohn hastalığı olanlarda Meckel divertikülü normal popülasyonun iki - üç katı oranında fazla izlenmektedir. Bu olgu sunumunda hastanemize ileus nedeniyle başvuran ve obstruktif Crohn hastalığı ile Meckel divertikülünün beraber görüldüğü acil şartlarda opere edilen nadir bir vakayı sunuyoruz.

Objective: In this study, we aimed to present patients who have developed suture reaction and were treated in our clinic following thyroidectomy operation. Material and Methods: Patients who had been treated for suture reaction following thyroidectomy between January 2012 and December 2014 were retrospectively evaluated. The patients were analyzed in terms of their age, gender, duration of the symptoms, type of previous operation and treatment modality. Results: Between January 2012 and December 2014, 559 thyroid/parathyroid operations were performed in our clinic. A total of 12 patients were admitted with suture reaction within this period thus yielding a suture reaction incidence of 2.1%. The mean age of these patients was 42±7.65 years, 75% of them were female while 25% of them were male. The types of previous operations were bilateral total thyroidectomy in 83.3%, lobectomy in 8.3% and near total thyroidectomy in 8.3% of the patients. The mean symptom duration was 7.2±4.3 (2-16) months. Two patients (16.7%) underwent a second surgical operation for suture reaction, while 10 patients (83.3%) were treated conservatively. None of the patients developed complications. Conclusion: One of the most common complications that develop after thyroidectomy is bleeding. Ligation must be performed in order to prevent this complication. As it is known, surgical ligation with sutures may cause tissue reaction. Sutures that are absorbable and have a low risk for reaction formation should be chosen if suturing is preferred. [ABSTRACT FROM AUTHOR]

- Abstract
Purpose: Colonic pseudo obstruction disease commonly seen in the elderly, immobile patient group can cause serious mortality and morbidity. Our objective in this retrospective study is to share our clinical experience by evaluating patients with Ogilvie syndrome who were followed and treated in our clinic. Methods: Eleven cases with the diagnosis of Ogilvie syndrome followed up and treated between September 2010 and April 2013 were evaluated retrospectively. All the patients that had no symptoms of acute abdominal pain were initiated conservative treatment. Colonoscopic decompression was attempted for patients whose clinical pictures were not recovered. Patients underwent operation if they developed peritoneal irritation symptoms during these procedures and of their number of white blood cells seriously increased during the follow-up period. Results: A total of 11 patients were treated for Ogilvie syndrome. 6 of the patients underwent surgical treatment, and 5 were treated medically. Mortality developed in two patients. The main cause of mortality was a delay in diagnosis and additional severe underlying diseases. Seven patients were given Neostigmine. Of these, 2 patients required surgery and 3 patients responded to Neostigmine. Conclusion: Ogilvie syndrome is a rare cause of ileus of the colon. It is more common particularly in old patients with additional problems. If the disease is suspected and diagnosed early, unnecessary surgical interventions can be prevented with medical treatment choices. [ABSTRACT FROM AUTHOR]

Spontaneous rupture of a splenic artery aneurysm (SAA) during pregnancy is a rare but life-threatening situation typically presenting with sudden and unexpected fetal and maternal death. The etiology is unclear; however, there is a strong association between pregnancy and the rupture of a SAA. Maternal and fetal prognosis is poor and mortality rates remain at 70% and 90%, respectively. 48. Are the immature granulocyte count and percentage important in continue medical treatment in acute appendicitis? A prospective, randomized, and controlled study.

Background: Although appendectomy is still a curative therapy for acute appendicitis, medical treatment has come to the fore in uncomplicated cases. This study aimed to determine the importance of immature granulocyte (IG) count and percentage for the role of medical treatment success in uncomplicated acute appendicitis., Methods: Acute appendicitis cases were prospectively registered between July 2019 and April 2020. Using ball drawing, patients were divided into two groups as medical treatment (Group M) and undergo appendectomy (Group A). Group M was divided into two subgroups as those who responded to medical treatment medically responded (MR) and failed medical treatment (MF) within 24 h of follow-up. Changes in IG count and percentage, C-reactive protein levels, neutrophil-lymphocyte ratio, and white blood cell count between initial administration and 24th h of follow-up were examined., Results: Sixty-four patients who met the inclusion criteria were followed as 31 patients in Group A and 33 in Group M. At Sub-group MF 11 patients and Subgroup MR 22 patients were followed up. At the 24th h of the follow-up, the IG count and percentage were higher in the Group MF (for IG count: Between Group A and MF, p=0.002; between Group A and Group MR, p=0.111; and between Group MR and MF, p<0.001) (for IG percentage: Between Group A and MF, p=0.001; between Group A and MR, p=0.809; and between Group MF and MR, p=0.001). This decrease in the IG count and percentage suggests that the response to medical treatment was effective [for IG count: F (148.862) = 61, p≤0.001, η2=0.707] [for IG percentage: F (10.157) = 0.252, p≤0.001, η2=0.504]., Conclusion: IG count and percentage are effective for evaluating the success of medical treatment of uncomplicated acute ap-pendicitis and they guide in the decision to continue medical treatment of uncomplicated acute appendicitis.

Background: This study aims to examine the factors affecting the selection of anaesthesia method in Fournier's gangrene., Methods: A retrospective evaluation was made of 113 patients operated on because of Fournier's gangrene between January-May 2019. The operations were performed under spinal anaesthesia in 78 cases (Group S) and under general anaesthesia in 35 cases (Group G). The patients were evaluated regarding age, gender, the anaesthesia method used (spinal, general) anaesthetic agent applied, presence of sepsis, and biochemical, hematological and inflammatory parameters., Results: When the patients were evaluated regarding the Fournier Gangrene Severity Index (FGSI), patients in Group S had lower scores (p=0.001). Examination of the tomography images revealed that in 13 (37.1%) patients, air values were seen in the right or left gluteal area, or both, extending to the subcutaneous tissue. In the evaluation of the factors affecting the selection of general anaesthesia, a positive correlation was determined between an increase in FGSI (r=0.482, p=0.001) and the presence of sepsis (r=0.485, p=0.001) and gluteal region involvement (r=0.628, p<0.001)., Conclusion: The selection of anaesthesia method in Fournier gangrene patients is a complex process affected by factors, such as the patients' general condition, sepsis, and whether or not there is bleeding diathesis. The risk -benefit balance in the selection of anaesthesia method should be evaluated individually for patients.

Background: Morbid obesity surgery and related complications have increased with time., Aim: To evaluate the relationship between perioperative complications before discharge and preoperative body mass index and obesity surgery mortality score in laparoscopic sleeve gastrectomy., Method: 1617 patients who met the inclusion criteria were evaluated retrospectively. The patients were examined in terms of demographic data, presence of comorbidities, whether there were complications or not, type of complications and obesity surgery mortality score., Results: Complications were seen in 40 patients (2.5%) and mortality wasn't seen in the early postoperative period before discharge. The mean age of patients with complications was 36.3±10.02 years (19-57) and without complications 34.12±9.54 (15-64) years. The preoperative mean BMI values of patients with and without complications were 45.05±3.93 (40-57) kg/m2 and 44.8±3.49 (35-67) kg/m2 respectively. According to BMI groups 40-45 kg/m2, 45-50 kg/m2 and 50 and over, there was not any statistical significance seen in three groups in terms of complication positivity and major-minor complication rates. There was not any statistical significance seen between the patients with and without major-minor complications and obesity surgery mortality score., Conclusion: There was not any relation between perioperative laparoscopic sleeve gastrectomy complication rates before discharge and BMI and obesity surgery mortality scores.
