Abstract

Background and purposeClinical outcomes of multiple sclerosis (MS) patients affected by coronavirus disease 2019 (COVID-19) have been thoroughly investigated, but a further analysis on main signs and symptoms and their risk factors still needs attention. The objective of this study was to group together and describe based on similarity the most common signs and symptoms of COVID-19 in MS patients and identify all factors associated with their manifestation.MethodLogistic and linear regression models were run to recognize factors associated with each pooled group of symptoms and their total number.ResultsFrom March 2020 to November 2021, data were collected from 1354 MS patients with confirmed infection of COVID-19. Ageusia and anosmia was less frequent in older people (odds ratio [OR] 0.98; p = 0.005) and more in smoker patients (OR 1.39; p = 0.049). Smoke was also associated with an incremental number of symptoms (OR 1.24; p = 0.031), substance abuse (drugs or alcohol), conjunctivitis and rash (OR 5.20; p = 0.042) and the presence of at least one comorbidity with shortness of breath, tachycardia or chest pain (OR 1.24; p = 0.008). Some disease-modifying therapies were associated with greater frequencies of certain COVID-19 symptoms (association between anti-CD20 therapies and increment in the number of concomitant symptoms: OR 1.29; p = 0.05). Abstract

Background and purposeSome studies have shown that air pollution, often assessed by thin particulate matter with diameter below 2.5 µg/m3 (PM2.5), may contribute to severe COVID-19 courses, as well as play a role in the onset and evolution of multiple sclerosis (MS). However, the impact of air pollution on COVID-19 has never been explored specifically amongst patients with MS (PwMS). This retrospective observational study aims to explore associations between PM2.5 and COVID-19 severity amongst PwMS.MethodsData were retrieved from an Italian web-based platform (MuSC-19) which includes PwMS with COVID-19. PM2.5 2016-2018 average concentrations were provided by the Copernicus Atmospheric Monitoring Service. Italian patients inserted in the platform from 15 January 2020 to 9 April 2021 with a COVID-19 positive test were included. Ordered logistic regression models were used to study associations between PM2.5 and COVID-19 severity.ResultsIn all, 1087 patients, of whom 13% required hospitalization and 2% were admitted to an intensive care unit or died, were included. Based on the multivariate analysis, higher concentrations of PM2.5 increased the risk of worse COVID-19 course (odds ratio 1.90; p = 0.009).ConclusionsEven if several other factors explain the unfavourable course of COVID-19 in PwMS, the role of air pollutants must be considered and further investigated.

8. Quantitative Magnetic Resonance Imaging to investigate the pathophysiology of Alzheimer's disease : implications for clinical trials and non-pharmacological interventions

Abstract

The publications included in this PhD thesis are a small selection of the outputs from Marco Bozzali's research over a 15-year timeframe. The studies are linked to each other by a common theme: using neuroimaging and neurophysiological techniques to investigate the pathophysiology of Alzheimer's disease (AD), with the potential of identifying prognostic measures and targets for intervention. The first work (Bozzali et al., 2002) was the second paper ever published using diffusion-imaging to assess white matter (WM) damage in AD brains. Abnormalities were identified in anatomical locations fitting well with the cognitive impairments observed in AD. The second work (Bozzali et al., 2006) describes an early longitudinal investigation assessing the impact of grey matter (GM) loss on AD progression. A group of mild-cognitive-impairment (MCI) patients were recruited and MRI scanned at baseline, together with AD patients and healthy subjects (HS). MCI patients were followed-up for 2 years, and reclassified as MCIconverters and non-converters. When using voxel based morphometry (VBM) to compare baseline scans of MCI against AD patients and HS, MCI-converters, but not MCI-non-converters revealed a pattern of GM atrophy similar to that observed in AD. This indicates a prognostic value for GM atrophy with implications for patient stratification. The third work (Gili et al., 2011) assessed changes in GM atrophy and brain disconnection across AD evolution, using VBM and resting-state fMRI. Disconnection was observed in the medial-temporal lobes (MTLs) and posterior-cingulate cortex (PCC) of both, AD and MCI patients. GM atrophy was detected in the MTLs of both patient groups and in the PCC of AD but not MC patients. This demonstrates how brain disconnection contributes to AD progression, whose importance is reflected by the contents of publication 4 and 5. The fourth work (Bozzali et al., 2015) investigated how cognitive reserve (CR) may modulate AD symptoms. A detailed questionnaire was validated and used to collect lifestyle information from HS and patients with AD and MCI. Education was found modulating functional brain connectivity (FBC) (assessed by resting-state fMRI) in the PCC, fitting with the pathophysiological model hypothesized in Gili et al. (2011). The fifth work, based on previous own studies, hypothesized that repetitive transcranial-magnetic-stimulation (rTMS) could be used to modulate FBC with improvements in AD symptoms. We set up a clinical-trial (double-blind, sham-controlled) including AD patients who underwent a daily rTMS-/sham-stimulation of their PCC for 2 weeks. rTMS (compared to sham) induced a selective improvement in episodic memory and consistent changes of FBC.

Abstract

Background: Alzheimer's disease (AD) is the most common neurodegenerative dementia, with diagnosis traditionally reliant on clinical criteria. Cerebrospinal fluid (CSF) biomarkers like pTau181 and Aβ42/Aβ40 ratio significantly improve diagnostic accuracy but are invasive. Plasma biomarkers measured by automated assays offer a non-invasive alternative. Objective: To evaluate the diagnostic performance of plasma pTau181, Aβ42/Aβ40, and pTau181/Aβ42 ratios in predicting CSF amyloid status in a real-life clinical setting. Methods: Data from consecutive patients whose plasma and CSF samples sent to our laboratory between March and October 2022, were retrospectively analyzed. Plasma and CSF pTau181, Aβ42, and Aβ40 levels were measured using the Lumipulse G600II platform. CSF amyloid status was classified as amyloid-positive (A+) or amyloid-negative (A-) based on the Aβ42/Aβ40 ratio. Statistical analyses included Spearman correlation, receiver operating characteristic (ROC) curves, and multivariate logistic regression to evaluate biomarker performance. Results: Among 165 individuals (83 females), 29.1% were classified as A+. Significant correlations were found between plasma and CSF biomarkers, with the highest for the pTau181/Aβ42 ratio (ρ=0.620, p < 0.0001). ROC analysis showed the pTau181/Aβ42 ratio had the highest diagnostic performance (AUC 0.818), followed by pTau181 (AUC 0.794) and Aβ42/Aβ40 (AUC 0.775). Combining plasma biomarkers in age-adjusted models improved diagnostic accuracy (AUC up to 0.846). Conclusions: Plasma biomarkers measured by the Lumipulse G600II platform show strong potential in predicting CSF amyloid status and possibly reduces the need for lumbar punctures. These findings support the potential use of plasma assays in the early diagnosis of AD. Anyway, further validations in larger multicenter cohorts are mandatory. [ABSTRACT FROM AUTHOR]

Abstract

Background: Reserves' mechanisms explain inconsistencies between accumulation of neuropathological damage and clinical manifestations. Leisure activities are believed to promote reserves. Objective: This study evaluates whether cognitive, social, and physical leisure activities performed over life-span predict current cognitive functioning in normal aging and Alzheimer's disease (AD) continuum. Methods: 35 AD, 24 amnestic-Mild Cognitive Impairment (a-MCI) patients, 21 individuals with subjective cognitive complaint (SCD), and 25 controls underwent a questionnaire developed to quantify leisure activities in different life periods, the Addenbrooke's Cognitive Examination-Revised (ACE-R), and T1-weighted 3T-MRI scans for brain volumetrics and cortical thickness quantification. Partial/total leisure activities' scores and demographic and brain variables were entered as predictors, while ACE-R scores as dependent variables in linear regression analyses. Results: Current level of cognition was predicted by (i) social and physical activities performed in middle age and current cognitive activity in AD; (ii) cognitive and social activities performed in middle age, current age and cortical thickness in a-MCI; (iii) recreational activities the set of lifetime, current age, and brain features in SCD; (iv) education and the set of lifetime leisure activities over lifespan in controls. Conclusions: This study shows a funnel effect due to gradual reduction of stimulatory activities in the transition from healthy aging to AD. Reserve indices taking into account different types of stimulatory activities allow to capture even smallest residual effects of reserves accumulated over lifespan, until their complete depletion at advanced AD stages. Abstract

The glymphatic system (GS) is a whole‐brain perivascular network, consisting of three compartments: the periarterial and perivenous spaces and the interposed brain parenchyma. GS dysfunction has been implicated in neurodegenerative diseases, particularly Alzheimer's disease (AD). So far, comprehensive research on GS in humans has been limited by the absence of easily accessible biomarkers. Recently, promising non‐invasive methods based on magnetic resonance imaging (MRI) along with aquaporin‐4 (AQP4) quantification in the cerebrospinal fluid (CSF) were introduced for an indirect assessment of each of the three GS compartments. We recruited 111 consecutive subjects presenting with symptoms suggestive of degenerative cognitive decline, who underwent 3 T MRI scanning including multi‐shell diffusion‐weighted images. Forty nine out of 111 also underwent CSF examination with quantification of CSF‐AQP4. CSF‐AQP4 levels and MRI measures—including perivascular spaces (PVS) counts and volume fraction (PVSVF), white matter free water fraction (FW‐WM) and mean kurtosis (MK‐WM), diffusion tensor imaging analysis along the perivascular spaces (DTI‐ALPS) (mean, left and right)—were compared among patients with AD (n = 47) and other neurodegenerative diseases (nAD = 24), patients with stable mild cognitive impairment (MCI = 17) and cognitively unimpaired (CU = 23) elderly people. Two runs of analysis were conducted, the first including all patients; the second after dividing both nAD and AD patients into two subgroups based on gray matter atrophy as a proxy of disease stage. Age, sex, years of education, and scanning time were included as confounding factors in the analyses. Considering the whole cohort, patients with AD showed significantly higher levels of CSF‐AQP4 (exp(b) = 2.05, p =.005) and FW‐WM FW‐WM (exp(b) = 1.06, p =.043) than CU. AQP4 levels were also significantly higher in nAD in respect to CU (exp(b) = 2.98, p <.001). CSF‐AQP4 and FW‐WM were significantly higher in both less atrophic AD (exp(b) = 2.20, p =.006; exp(b) = 1.08, p =.019, respectively) and nAD patients (exp(b) = 2.66, p =.002; exp(b) = 1.10, p =.019, respectively) compared to CU subjects. Higher total (exp(b) = 1.59, p =.013) and centrum semiovale PVS counts (exp(b) = 1.89, p =.016), total (exp(b) = 1.50, p =.036) and WM PVSVF (exp(b) = 1.89, p =.005) together with lower MK‐WM (exp(b) = 0.94, p =.006), mean and left ALPS (exp(b) = 0.91, p =.043; exp(b) = 0.88, p =.010 respectively) were observed in more atrophic AD patients in respect to CU. In addition, more atrophic nAD patients exhibited higher levels of AQP4 (exp(b) = 3.39, p =.002) than CU. Our results indicate significant changes in putative MRI biomarkers of GS and CSF‐AQP4 levels in AD and in other neurodegenerative dementias, suggesting a close interaction between glymphatic dysfunction and neurodegeneration, particularly in the case of AD. However, the usefulness of some of these biomarkers as indirect and standalone indices of glymphatic activity may be hindered by their dependence on disease stage and structural brain damage.
