Cilj rada bio je utvrditi mogu li standardna automatizirana perimetrija (SAP) i perimetrija udvostručene frekvencije (frequency doubling technology, FDT) otkriti učinke dijabetesa na funkciju retine u osoba s dijabetesom u ranoj fazi bolesti i ocijeniti koja je metoda bolja, jer se dosadašnji radovi spore oko činjenice može li FDT otkriti funkcionalne promjene u bolesnika s dijabetesom prije SAP-a. Svim ispitanicima koji su zadovoljili uključne kriterije testirano je vidno polje pomoću SAP-a i FDT perimetrije. Dobiveni parametri statistički su obrađeni i doneseni su zaključci o valjanosti otkrivanja oštećenja vidnog polja upotrebom svakog od testova. Vidna funkcija istražena je u tri skupine od po 60 ispitanika. Prvu skupinu činili su zdravi ispitanici, drugu bolesnici s dijabetesom bez klinički vidljive dijabetičke retinopatije, a treću skupinu bolesnici s dijabetesom i klinički vidljivom početnom neproliferacijskom dijabetičkom retinopatijom. Postojanje dijabetičke retinopatije utvrđeno je analizom dviju fotografija očne pozadine širine 50°, jedna s centrom u makuli i druga s centrom na papili vidnoga živca. Objectives: Intravitreal injections of anti- VEGF agents have been widely used and intensively developed as a treatment option for many ophthalmological indications. The most frequently used anti-VEGF agent in Croatia is bevacizumab. This type of therapy is mostly indicated in patients with exudative age- related macular degeneration and diabetic macular edema. If, in addition to these two conditions, patients have a diagnosis of primary open angle glaucoma (POAG), they also present with optic nerve head (ONH) retinal nerve fibre layer (RNFL) thinning. The aim of this prospective study was to establish whether administering bevacizumab to patients with primary open angle glaucoma leads to additional thinning of the RNFL. Subjects and methods: The study included 60 patients divided into two groups. The first group comprised the eyes of 30 patients with exudative ARMD and POAG, whereas the second group comprised the eyes of 30 patients with diabetic macular edema (DME) and POAG, all treated with bevacizumab. The control group comprised the fellow eye of each involved patient, which was not treated with bevacizumab. In a period of one year, all patients underwent OCT (optical coherence tomography) measurements of ONH retinal nerve fibre layer thickness (RNFL). We compared the results of all patients and subsequently compared them with control group results. Results: The results showed decrease of RNFL in both groups of patients. First group of patients showed no statistically significant difference between treated and untreated eye (p=0, 758). In the second group there was statistically significant difference between treated and untreated eye in RNFL thickness ( p= 0, 023).A comparison of these two groups of patients after one year revealed a statistically more significant RNFL thickness decrease in the second group ( p=0, 023). Conclusion: Multiple intravitreal bevacizumab injections have possible toxic effect on RNFL thickness in group of patients with DME and POAG during follow up period of one year, while in the group of patients with exudative age- related macular degeneration, bevacizumab doesn't have that effect.

Cilj rada bio je odrediti ulogu blefarospazma kao zaštitnog čimbenika prednjeg segmenta oka usporedbom stupnja blefarospazma i promjenama prednjeg segmenta oka. Rad je uključivao 60 žena starijih od 40 godina s kliničkom slikom blefarospazma podijeljenih u dvije skupine. Prvu skupinu činile su bolesnice s I. i II. stupnjem blefarospazma kod kojih su dominirali simptomi suhog oka, a drugu skupinu činile su bolesnice III. i IV. stupnja blefarospazma kod kojih je bila potrebna interventna terapija (sve bolesnice su bile liječene botulin toksinom tipa A). Za bojanje okularne površine radi otkrivanja kornealnih epitelnih defekata rabio se fluorescein. Bojanje kornealnih epitelnih defekata fluoresceinom bilo je statistički značajno manje u intervencijskoj skupini. Usporedba rezultata između promatranih skupina ukazala je na to da izraženi blefarospazam ima zaštitnu ulogu na površinu oka.

Cilj istraživanja bio je usporediti dijagnostičke testove za konjunktivitis sicca (Schirmerov test, test raskidanja suznog filma (engl. tear break-up time test, TBUT) i testove bojanja rožnice fluoresceinom i zelenom bojom lisamin) kod bolesnika s blefarospazmom. Prospektivnom studijom obuhvaćeno je šezdeset ispitanica starijih od četrdeset godina kojima je dijagnosticiran blefarospazam. Ispitanice su podijeljene u dvije skupine ovisno o kliničkim simptomima. Površina ispod krivulje ROC za fluoresceinski test iznosila je 1, 0 uz standardnu grešku (SE) 0 i interval pouzdanosti (95% CI) 0, 940-1, 0 ; za Schirmerov test je površina ispod krivulje ROC iznosila 0, 817 uz SE 0, 0555 i 95% CI 0, 696-0, 905 ; za test sa zelenom bojom lisamin površina ispod krivulje ROC bila je 0, 813 uz SE 0, 056 i 95% CI 0, 691- 0, 902 ; te za test TBUT je površina ispod krivulje ROC bila 0, 772 uz SE 0, 061 i 95% CI 0, 645-0, 870. U studiji je prema rezultatima krivulje ROC, kojom se utvrđuje osjetljivost i specifičnost normalnih vrijednosti, fluoresceinski test imao najbolju osjetljivost i specifičnost u usporedbi s drugim testovima koji se primjenjuju u dijagnosticiranju suhog keratokonjunkitivitisa. Izvođenje Schirmerova testa ne preporuča se u bolesnika s keratokonjunktivitisom sicca i blefarospazmom zbog čestog treptanja, što može utjecati na rezultate testa i njegovu pravilnu interpretaciju. Unatoč patološkim brojčanim vrijednostima TBUT test je prihvatljiv za kliničku primjenu u bolesnika s keratokonjunktivitisom i blefarospazmom, jer je interval raskidanja suznog filma duži od vremenskog interval između dva treptaja.

The aim of this prospective study was to detect primary open angle glaucoma (POAG) in its early stage in patients at a higher risk of its development, and to identify the risk group with the highest prevalence of POAG. The study was conducted at Department of Ophthalmology, Osijek University Hospital Centre, and included 250 patients divided into five groups, as follows: group 1, patients with diabetes type 1 and type 2; group 2, patients with arterial hypertension (blood pressure >140/90 mm Hg); group 3, patients with positive family history of POAG; group 4, patients with myopia between -3.0 and -8.0 diopters; and group 5, control group including patients aged 40 with no risk factors for POAG development. Study results showed that distribution of glaucoma patients was not equal across the groups. The prevalence of POAG in all patients was 5.6%, whereas in patients with positive family history of POAG it was 14%, which was statistically significantly higher than in patients with diabetes and myopia (4% both), as well as in control group. The difference was greatest in comparison to control group. There was no statistically significant difference in glaucoma incidence between the group of patients with positive family history (14%) and patients with systemic hypertension (6%). The results obtained suggest that of all risk factors analyzed, positive family history of POAG is the most important risk factor for glaucoma development in all risk groups., Cilj ove prospektivne studije bio je dijagnosticirati primarni glaukom otvorenog kuta (POAG) u ranom stadiju kod bolesnika s visokim rizikom od razvoja POAG te identificirati skupinu bolesnika s najvećom učestalošću POAG. Studija je provedena na Zavodu za oftalmologiju Kliničkog bolničkog centra Osijek i uključila je 250 bolesnika podijeljenih u pet skupina: skupina 1: bolesnici s dijabetes melitusom tip 1 i 2; skupina 2: bolesnici s arterijskom hipertenzijom (krvni tlak >140/90 mm Hg); skupina 3: bolesnici s pozitivnom obiteljskom anamnezom POAG; skupina 4: bolesnici s miopijom između -3 i -8 dioptrije sfere; skupina 5: kontrolna skupina koja uključuje bolesnike starije od 40 godina i bez rizičnih čimbenika za razvoj POAG. Rezultati studije pokazuju da distribucija bolesnika s glaukomom nije jednaka u svim skupinama. Učestalost POAG u svih bolesnika bila je 5,6%, dok je u bolesnika s pozitivnom obiteljskom anamnezom bila 14%, što je statistički zančajno više nego u bolesnika s dijabetesom i miopijom (4%) i u kontrolnoj skupini. Razlika je bila najveća u usporedbi s kontrolnom skupinom. Nije pronađena statistički značajna razlika u učestalosti POAG između bolesnika s pozitivnom obiteljskom anamnezom (14%) i bolesnika s arterijskom hipertenzijom (6%). Dobiveni rezultati navode na zaključak da je za pojavu POAG od svih analiziranih rizičnih čimbenika najvažnija pozitivna obiteljska anamneza.

Rad daje znanstvenu analizu pojave pseudocističnih intraokularnih tvorbi identificiranih kod 27. godišnje bolesnice. Detaljan vitreoretinalni pregled obuhvaćao je i optičku koherentnu tomografiju i fluoresceinsku angiografiju. Ultrazvučnim pregledom (B sken), prikazane su pseudocistične tvorbe s hipoehogenim sadržajem. Optička koherentna tomografija u razini veće lezije prikazala je eleviranu retinalnu konturu i hiporeflektivnu sjenu uzrokovanu tvorbom. Fluoresceinska angiografija detektirala je dvije okrugle hiporeflektivne strukture koje nisu nakupljale contrast. Učinjena je opsežna laboratorijska obrada kako bi se isključili mogući infektivni i upalni uzroci promjena. Neurološkom obradom isključeno je moguće postojanje neuroloških uzroka navedenih promjena. Krvarenje ispod membrane limitans interne može biti idiopatsko ali je češće udruženo s Valsalvinim manevrom, Tersonovim sindromom, makroaneurizmom, traumom ili krvnim diskrazijama. Pretpostavljena patogeneza krvarenja ispod interne membrane limitans u odsutnosti retinalnih arteriolarnih makroanuerizmi i trauma jest Valsalvin manevar i akutni porast intraokularnog venskog tlaka što dovodi do rupture retinalnih kapilara. Kod Tersonovog sindroma krvarenje ispod membrane limitans interne može biti udruženo s intrakranijalnim subarahnoidalnim krvarenjem koje dovodi do povišenog tlaka u retinalnim kapilarama. Prema podacima dostupnim u literaturi, vrlo rijetko je opisana elevacija membrane limitans interne i razdvajanje unutrašnjih retinalnih slojeva nakon rezolucije krvarenja.

The aim of this case study was to evaluate the clinical symptoms in patients with basal ganglia calcifications and compare the neuroimaging methods used in confirming this state. The clinical status and performed transcranial sonography of basal ganglia structures in patients with brain calcifications found by computed brain tomography was examined. In one of these patients DaTSCAN was performed. A large spectrum of different symptoms was found. Transcranial sonography of basal ganglia showed the hyperechogenicity of nucleus lenticularis in eight out of 10 patients. DaTSCAN, which was performed to one patient with parkinsonian signs and the hyperechogenicity of substantia nigra found by transcranial sonography, was normal. Transcranial sonography is a newly neuroimaging method which can contribute to diagnosing basal ganglia calcifications in patients with different neurological signs. Computed tomography of brain remains the most adequate technique in visualising calcifications.

A 29-year-old woman presented with acute visual loss on both eyes. Ophthalmologic examination revealed intermediate uveitis bilaterally and after systemic workup oral corticosteroids were started as well as local corticosteroid, cycloplegic and non steroid anti-inflammatory drops. HLA tissue typing showed increased relative risk for developing uveitis, reactive arthritis and subacute thyroiditis. The initial optical tomograms of the macula were normal. Combined therapy lead to improvement in her clinical status after month and a half with gradual tapering of systemic corticosteroids. Her visual acuity returned to normal (1.0 both eyes on Snellen charts) and vitreous inflammation diminished. After initial improvement she presented again with loss of vision on both eyes and optical coherence tomography revealed bilateral macular edema which did not improve after periocular corticosteroid injections. She received intravitreal application of triamcinolone acetonide in both eyes and regained normal visual acuity as well as normal appearance of the macular region documented on optical coherence tomograms. The patient remains stabile for the past six months and further follow up is anticipated.

Cilj studije je bio testirati pacijente koje je zadesio nagli gubitak vida na najčešće mutacije mitohondrijske DNK koji se pojavljuju kod LEBEROVE bolesti. Leberova nasljedna optička neuropatija je bolest koju karakterizira akutni ili subakutni gubitak centralne vidne oštrine oba oka uslijed atrofije vidnog živca. Unazad dvadesetak godina uočeno je da je povezana sa točkastim mutacijama mitohondrijske DNK i to na pozicijama G3460A, G11778A i T14484C uzrokujući defekt u dijelovima kompleksa I (NADH-dehidrogenasa-ubiquinone reduktasa) u mitohondrijima. Prenosi se ženskom nasljednom linijom pri čemu muški nasljednici obolijevaju dok su ženski slijednici uglavnom prenosioci. Sedamnaest odraslih pacijenata iz pet obitelji su testirani, 10 muškaraca i 7 žena. Kriteriji uključenja u studiju su bili oboljeli pacijenti kojima je postavljena klinički sumnja na LHON te njihovi obiteljski srodnici po ženskoj liniji kao mogući prenosioci mutiranog gena. DNK je izolirana iz limfocita periferne krvi i mitohondrijska DNK mutacija je detektirana PCR i RFLP metodom. Točkasta mutacija mitohondrijske DNK G11778A nađena je kod šest ispitanika u dvije različite obitelji. Četiri od pet pozitivnih uzoraka su pripadnici iste obitelji dvije sestrične po ženskoj liniji i njihovi sinovi koji su zahvaćeni bolešću. Peti pozitivni uzorak je pripadao mladiću iz druge obitelji, a šesti njegovoj majci. Ostalih 11 uzoraka su bili negativni na testirane mutacije i pripadali su trima različitim obiteljima. U zaključku možemo istaknuti da je ovo prva studija u Hrvatskoj koja ispituje povezanost LEBEROVE bolesti sa točkastim mutacijama mitohondrijske DNK te da molekularna dijagnostika uveliko pomaže u dijagnosticiranju LEBEROVE bolesti.

Optički neuritis udružen je s brojnim patološkim stanjima.Autori ove epidemiološke studije žele istaknuti akutni optički neuritis(AON) kao inicijalnu manifestaiju multiple skleroze(MS), i samim tim usmjeravanje dijagnostike u pacijenata sa AON u smislu otkrivanja udruženog demijelinizirajućeg procesa.Retrospektivna studija obuhvatila je 24 pacijenta sa dg.AON hospitaliziranih na Odjelu za Očne bolesti Kliničke bolnice Osijek u periodu od 01.01.1998.g. do 31.12.2003.g.Uvidom u kartoteku ambulantno i hospitalno liječenih bolesnika sa dg.MS Klinike za neurologiju KBO učinili smo statističku analizu podataka.Rezultati su pokazali da je AON i MS bolest koja ugrožava zdravlje radno aktivne relativno mlađe i to pretežno ženske populacije te bi u tom smislu trebalo obratiti pažnju na te entitete.

Specifičan je cilj studije bilo precizirati prevalenciju Parkinsonove bolesti u našoj regiji u odnosu na dob i spol, te precizirati tip parkinsonizma, najčešći komorbiditet osoba oboljelih od Parkinsonove bolesti sa drugim bolestima. Retrospektivnom analizom obuhvaćeno je 147 pacijenata ; 76 pacijenata ženskog spola i 71 muškog. Zaključili smo da je od analiziranih aktualno liječenih pacijenata veći postotak muškaraca oboljelih od Parkinsonove bolesti u dobi većoj od 80. g. u odnosu na žene iste dobi. Vrijeme pojave bolesti ima najveći pik u dobi od 60. - 69. g. u oba spola jednako. Gotovo je jednaka zastupljenost primarnog oblika bolesti kod oba spola, dok je posttraumatski oblik zastupljeniji kod muškaraca. S obzirom na komorbiditet signifikantna je povezanost sa kardiovaskularnim bolestima i depresijom te relativno visokom učestalosti malignih oboljenja.

35. Detection of mitochondrial DNA point mutations in Lebert's optical neurophaty (LOHN)

Mitochondrial DNA (mtDNA) is a small circular molecule of 16.5 kB encoding enzymes of the respiratory chain in mitochondria. It is inherited maternally. Point mutations, deletions and insertions of mtDNA may cause various diseases. Leber's hereditary optic neuroretinopathy (LHON) causes bilateral acute or subacute loss of central vision due to optic atrophy. Most common mtDNA point mutations associated with this hereditary disease are G3460A, G11778A and T14484C causing defects of subunits of complex I (NADH-dehydrogenase-ubiquinone reductase) in mitochondria. G11778A causes most unfavourable visual outcome (blindness), while T14484C is associated with a mild clinical course. We present results of a study of Leber's disease in Croatia. 16 adult patients from five families, 10 males and 6 females were tested. Each family has had a member with visual impairment indicating possible diagnosis of Leber's disease. DNA was isolated from peripheral blood lymphocytes and mtDNA point mutations were detected by means of PCR and RFLP. Point mutation G11778A was detected in 5 DNA samples from two families. Four of these 5 were members of the same family, i. e. 2 female cousins and their 2 sons. The fifth case was from another family. None of the three common mtDNK point mutations for the Leber's defect were found in other 11 DNA samples from the other three families. Molecular diagnosis helps recognize Leber's disease as the mtDNK point mutation and contributes to prognosis and treatment.

Autori prikazuju dva slučaja pravodobne ultrazvučne dijagnoze endoftalmitisa. 1.Pacijent sa perforacijskom ozljedom lijevog oka koja je zbrinuta stavljanjem pojedinačnih kornealnih šavova. Postoperacijski se registriraju u staklovini niskoreflektivni difuzni eho odjeci uz proširenje horioretinalnog sloja. Bolesnik trpi bolove i jak podražajni sindrom.Ultrazvučnim praćenjem zamijeti se postupno skraćivanje očne jabučice uz pojavu uveitičkih membrana i konglomerata ispred PNO u stražnjoj staklovini oka. Bolovi se pojačavaju te se učini evisceracija sadržaja lijevog bulbusa. 2. U bolesnice kojoj je operacijski odstranjena leća, postoperativno se razvije bulozna keratopatija i ulkus rožnice s jakim podražajem i bolovima. Učini se UZV pregled i u staklovini se registriraju nisko reflektivni eho odjeci uz skraćenje očne jabučice. Nekoliko dana kasnije dijagnosticira se eksudativna ablacija retine u donjim kvadrantima, uz permanentnu terapiju antibioticima i lokalnu terapiju ubrzo dolazi do povlačenja eksudata, retina prileži, staklovina je manje zamućena, ulkus rožnice se reparira, upalase smiruje i pacijentica otpušta kući

Most frequently, eye injuries entailed premanent alterations of the anterior eye segment (29,1%), followed by eye bulb phthisis and eviscerations (11,24%), cataract (8,99%) and damage to the eye protection apparatus (6,74%). Blindness of the injured eye was recorded in 28,10%, and practical blindness of one eye in 7,86% of the cases. Total visual field loss in the injured eye was found in 30,34%, and central and paracentral scotoms in 22,47% of the eyes. Severe functional and esthetic defects were found in 40,43% of the injured eyes. Permanent working disability was found in 11,42% of the patients. Re-education was required in 27,14% and professional orientation in 12,86% of them. Capable for previous occupation were 30,00% and less capable 18,57% of our patients.

The paper presents the results of the medical treatment with adjuvant hyperbaric oxygenation in eight patients with central or branch retinal artery occlusion admitted at the Ophthalmology Department of the Clinical Hospital in Osijek (Croatia) in the period 2004.-2006. Initial visual acuity ranged from light perception to 0.05. On discharge all patients reported a subjective visual improvement which could be confirmed considerably in four patients - ranged from 0.2 to 1.0 (more than 3 lines) as a result of hyperbaric oxygenations treatment. The aim of this case study was to evaluate the clinical symptoms in patients with basal ganglia calcifications and compare the neuroimaging methods used in confirming this state. The clinical status and performed transcranial sonography of basal ganglia structures in patients with brain calcifications found by computed brain tomography was examined. In one of these patients DaTSCAN was performed. A large spectrum of different symptoms was found. Transcranial sonography of basal ganglia showed the hyperechogenicity of nucleus lenticularis in eight out of 10 patients. DaTSCAN, which was performed to one patient with parkinsonian signs and the hyperechogenicity of substantia nigra found by transcranial sonography, was normal. Transcranial sonography is a newly neuroimaging method which can contribute to diagnosing basal ganglia calcifications in patients with different neurological signs. Computed tomography of brain remains the most adequate technique in visualising calcifications. [ABSTRACT FROM AUTHOR]

- Abstract
Background/aims: To evaluate efficacy, safety, pharmacokinetics (PK) and immunogenicity of SB15 versus reference aflibercept (AFL), and switching from AFL to SB15 in neovascular age-related macular degeneration (nAMD)., Design: Prospective, double-masked, randomised, phase 3 trial., Methods: Participants with nAMD were randomised 1:1 to receive SB15 (N=224 participants) or AFL (N=225). At week 32, participants either continued on SB15 (SB15/SB15, N=219) or AFL (AFL/AFL, N=108), or switched from AFL to SB15 (AFL/SB15, N=111). This manuscript reports 1-year and switching results of secondary efficacy endpoints such as changes from baseline to week 56 in best-corrected visual acuity (BCVA), central subfield thickness (CST, from internal limiting membrane (ILM) to retinal pigment epithelium), and total retinal thickness (TRT, from ILM to Bruch's membrane). Additional endpoints included safety, PK and immunogenicity., Results: Efficacy results were comparable between groups. The least squares mean (LSmean) change in BCVA from baseline to week 56 was 7.4 letters for SB15/SB15 and 7.0 letters for AFL/AFL (difference (95% CI)=0.4 (-2.5 to 3.2)). The LSmean changes from baseline to week 56 in CST and TRT were -119.2 µm and -132.4 µm for SB15/SB15 and -126.6 µm and -136.3 µm for AFL/AFL, respectively (CST: difference (95% CI)=7.4 µm (-6.11 to 20.96); TRT: difference (95% CI)=3.9 µm (-18.35 to 26.10)). Switched and non-switched participants showed similar LSmean changes in BCVA from baseline to week 56 (AFL/SB15, 7.9 letters vs AFL/AFL, 7.8 letters; difference (95% CI)=0.0 (-2.8 to 2.8)). - Abstract
The aim of this study was to find out to which extent the instilled silicone oil affects the changes of lens, cornea, and the ocular pressure with regard to its time kept in the operated eye with a lens. The study was carried out on the sample of 40 patients, divided in three groups. The first group of patients with the silicone oil kept in the eye during 3 months, the second one during 3-6 months, and the third one during 6-9 months. Statistically important changes were observed in the progression of the lens opacification with the patients, progressively with the time of keeping the silicone oil in the eye. The followed cornea opacifications and increased ocular pressure were not statistically significantly linked with the length of keeping the silicone oil in the eye, so that it can be concluded that the silicone oil is the best substitute for the vitreous body but it should be removed from the eye, most frequently, in the period of 3-6 months from the date of its instillation, and after 3 weeks at the earliest.
