The clinical prognostic value of the Spinal Instability Neoplastic Score (SINS), in the context of vertebrectomy for neoplasia, has not yet been established. This retrospective study of 134 patients aims to evaluate the efficacy of the SINS to predict outcomes and survival after vertebrectomy for malignancy.The patients were classified into 2 groups: indeterminate stability (SINS 7-12) and unstable (SINS 13-18). Outcomes assessed included survival days after procedure, neurological function (modified Frankel grade), operative time, blood loss, complications, construct failure, and length of inpatient stay.The indeterminate group included 68 patients, whereas the unstable group included 66 patients. No patients were classified as stable (SINS 0-6). The median survival was 225 days (interquartile range, 81-522 days). There was a statistically significant difference (P0.001) in survival days after vertebrectomy between the indeterminate group (435 days) and the unstable group (126 days). The majority of patients (119) had a favourable Frankel grade after procedure with no significant difference between SINS groups (P = 0.534). There were no differences in the operative time (234 vs. 210; P = 0.130), inpatient hospital length of stay (10 days vs. 11 days; P = 0.152), complications, or need for intensive care admission (intensive care unit) between the 2 cohorts. There was a statistically significant difference (P = 0.006) for intraoperative blood loss between the indeterminate group (1400 mL) and the unstable group (850 mL).This study demonstrates a statistically significant increased survival in the indeterminate cohort. These results demonstrate the potential ability of the SINS to act as a clinical prognostic tool with regard to survival time.

Fine-structure $\alpha$ decays to excited levels in the daughter $^{176}\mathrm{Au}$ were identified and an $\alpha$-decay scheme of $^{180}\mathrm{Tl}$ was constructed based on an analysis of $\alpha$-$\gamma$ and $\alpha$-$\gamma$-$\gamma$ coincidences. Multipolarities of several $\gamma$-ray transitions deexciting levels in $^{176}\mathrm{Au}$ were determined. Based on the analysis of reduced $\alpha$-decay widths, it was found that all $\alpha$ decays are hindered, which signifies a change of configuration between the parent and all daughter states.

A rich set of transitional and diagonal E2 matrix elements, including those for non-yrast structures, has been extracted from the differential Coulomb-excitation cross sections. The results support the scenario of a shape transition at N=60, giving rise to the coexistence of a highly deformed prolate and a spherical configuration in Sr98, and are compared to predictions from several theoretical calculations. The experimental data suggest a significant contribution of the triaxal degree of freedom in the ground state of both isotopes. In addition, experimental information on low-lying states in Rb98 has been obtained.

The neutron-rich isotopes Fe65,67 and Co65 have been produced at the LISOL facility, Louvain-La-Neuve, in the proton-induced fission of U238. Beams of these isotopes have been extracted with high selectivity by means of resonant laser ionization combined with mass separation. Yrast and near-yrast levels of Co65 have also been populated in the Ni64+U238 reaction at Argonne National Laboratory. The level structure of Co65 could be investigated by combining all the information from both the Fe65 and Co65β decay and the deep-inelastic reaction. The Fe65, Co65, and Fe67 decay schemes and the Co65 yrast structure are fully established. The Co65,67 level structures can be interpreted as resulting from the coexistence of core-coupled states with levels based on a low-energy proton-intruder configuration. © 2009 The American Physical Society.

A 54-year-old Caucasian female presented with a 1 year history of intermittent numbness of the left leg progressing to bilateral, lower extremity sensory loss that advanced to include impaired vibration and proprioception. The subsequent thoracic spine magnetic resonance imaging (MRI) scan revealed a heterogeneous, avidly enhancing, centrally situated spinal cord mass involving T7 through T10 in association with thick linear enhancement of the anterior and posterior cord surfaces extending both superiorly and inferiorly. Both the cervical and lumbar spine MRI demonstrated diffuse leptomeningeal disease as well. A brain MRI revealed focal leptomeningeal enhancement in the left and right sylvian fissures, the suprasellar cistern, and the posterior fossa; a pattern consistent with metastatic disease. The patient underwent a T6-T10 laminectomy for tumor biopsy and debulking. Histology revealed a WHO grade III glioneuronal tumor with rosetted neuropil-like islands. Synaptophysin and neurofilament (NF) positive staining was noted within the neural appearing component, whereas, glial fibrillary acidic protein (GFAP) immunopositivity was evident in the fibrillary astrocytoma component of the tumor. The Ki-67 labeling index was 7%. This tumor pattern, now included in the 2007 World Health Organization (WHO) classification of central nervous system tumours as a pattern variation of anaplastic astrocytoma (Kleihues et al. In: Louis et al. (eds) WHO classification of tumours of the central nervous system, 2007), was first described in a four-case series by Teo et al. in 1999. The majority of subsequently reported cases described them as primary tumors of the cerebrum. Herein, we report a unique example of a spinal glioneuronal tumor with neuropil-like islands with associated leptomeningeal dissemination involving the entire craniospinal axis.

To compare toxicities, disease control, and survival outcomes for patients treated with either cisplatin/irinotecan versus carboplatin/paclitaxel concurrent chemoradiotherapy for locally advanced esophageal cancer.Single-institution retrospective comparison between treatment groups: the cisplatin/irinotecan group was treated with 2 cycles of induction chemotherapy followed by concurrent chemoradiotherapy, whereas the carboplatin/paclitaxel group began with chemoradiotherapy followed by 2 additional cycles of chemotherapy. Acute toxicities, response rates, disease control, survival outcomes, and patterns of failure were compared between the groups.Between January 2000 and December 2007, 57 patients were identified for inclusion in the present study (38 cisplatin/irinotecan and 19 carboplatin/paclitaxel). Groups were well-balanced by clinical-, pathologic-, staging-, and treatment-related factors. Thirty-five patients (92%) in the cisplatin/irinotecan group and 18 patients (95%) in the carboplatin/paclitaxel group completed the concurrent phase of chemoradiotherapy. There were no significant differences in hematologic or nonhematologic toxicities between the groups. At a median survivor follow-up of 37.6 months (range: 7.3-59.3 months) for the entire population, 22 patients were alive (16 without evidence of disease). The 3-year overall survival estimates was 19.7% for the cisplatin/irinotecan group versus 56.1% for the carboplatin/paclitaxel group (P = 0.022). Estimated 3-year cancer-specific survivals were 24.6% for the cisplatin/irinotecan group versus 59.3% for the carboplatin/paclitaxel group (P = 0.033).Concurrent chemoradiotherapy with carboplatin/paclitaxel is well-tolerated and provided superior overall and disease-specific survival compared with cisplatin/irinotecan chemoradiotherapy in the present study population. Further investigation is warranted.

The present study defines the limits of the deformed region around A similar to 100 and indicates that the deformation of Rb-97 is essentially the same as that observed well inside the deformed region. It further highlights the power of the Coulomb-excitation technique for obtaining spectroscopic information far from stability. The Rb-99 case demonstrates the challenges of studies with very short-lived postaccelerated radioactive beams.

The neutron deficient 188−198Pb isotopes have been studied in a Coulomb excitation measurement employing the Miniball spectrometer and radioactive beams from REX-ISOLDE, CERN. These isotopes are of particular importance as they lie in a transitional region, where the intruding structures, associated with different deformed shapes, come down in energy close to the spherical ground state. For detailed analysis of the Coulomb excitation data, the understanding of the beam composition is essential. peerReviewed

In-gas-cell laser ionization spectroscopy studies on the neutron deficient 97–101 Ag isotopes have been performed with the LISOL setup. Magnetic dipole moments and mean-square charge radii have been determined for the first time with the exception of 101 Ag, which was found in good agreement with previous experimental values. The reported results allow tentatively assigning the spin of 97,99 Ag to 92 and confirming the presence of an isomeric state in these two isotopes, whose collapsed hyperfine structure suggests a spin of 12 . The effect of the N=50 shell closure is not only manifested in the magnetic moments but also in the evolution of the mean-square charge radii of the isotopes investigated, in accordance with the spherical droplet model predictions.
