Background/Aim: Dentist-technician communication is crucial for a successful working relationship in the healthcare setting that contributes to positive health outcomes. However, the interruption of the dentist and the technician communication can harm the dental care procedure. Besides, mistakes in dental laboratories can be both expensive and time-consuming. Materials and Methods: This research project focused on paper-based communication in the dental laboratory setting and its impact on the quality of prosthesis of zirconium crowns. The study's findings suggested that ineffective communication between technicians and dentists resulted in poor quality of zirconium crowns. The study adopted quantitative analysis, which entails a purposive non-probabilistic sampling of dentists targeted located in Texas. Statistical measurements such as Pearson's correlation employed in determining the association between the independent and dependent variables. Results: The research results suggest a significant positive correlation between inter-professional communication in the dental care setting and prosthetic treatment quality. The results also underscore the effectiveness of prep guides in improving the communication between the technicians and the dentist. Conclusions: The researchers conclude that adherence to prep guides contributes to the quality of zirconium crowns and productive communication among dentists and technicians.

IntroductionInsufficient physical activity is a significant contributor to non-communicable disease amongst the global population. Insufficient physical activity is directly linked with reduced cardiorespiratory fitness (CRF). CRF is as strong a predictor of mortality as well-established risk-factors such as smoking, hypertension, dyslipidaemia, and type 2 diabetes mellitus, however, it remains the only major risk factor not routinely assessed in primary health care settings. The aim of this review was to assess the validity and reliability of existing submaximal tests of CRF which can be employed in a standard medical consultation for the estimation of CRF and physical function in adults.MethodsA systematic review of the scientific literature was undertaken to find all studies reporting the reliability and/or validity of submaximal tests of CRF and physical function. Studies published up to 12 January 2023 were included in the search of the Medline, Embase, Cinahl, SPORTdiscus, Cochrane library, Informit Health and Web of Science databases. Risk of bias was assessed using the JBI critical appraisal checklist for analytical cross-sectional studies. Data including reliability of the submaximal protocols as measured by test-retest Pearson’sr(r) or Intraclass co-efficient (ICC); and validity as measured by the correlation between the submaximal protocol results and the graded exercise test results (r) was extracted. Meta-analyses were performed to determine the overall mean r of the correlation coefficients.ResultsIn total 1754 studies were identified. Following screening, 143 studies including 15,760 participants were included. All clinical tests included in meta-analysis demonstrated strong reliability. The Siconolfi step test (r=0.81), Incremental shuttle walk test (r=0.768) and 1- minute sit-to-stand test (r=0.65) demonstrated strongest validity following meta-analysis.ConclusionBased on the validity of the tests outlined, these can be used as an acceptable method of estimating VO2peak in a broad population, without the cost and access issues of formal GXT.

Purpose Metastatic breast cancer (MBC) patients are living longer. However, symptom burden remains a significant issue. Technology-based interventions may be a solution. The purpose of this study was to test a virtual assistant for addressing symptoms in MBC using the Amazon Echo Show with Alexa. Methods In this partial crossover randomized trial, the immediate treatment group was exposed to the intervention, called Nurse AMIE (Addressing Metastatic Individuals Everyday) for six months. The comparison group was unexposed for the first 3 months, then exposed for 3 months. The randomized controlled trial (RCT) during the first 3 months allowed for evaluation of intervention effects on symptoms and function. The partial crossover maximized exposure to the intervention for evaluation of feasibility, usability, and satisfaction. RCT outcome data were collected at baseline and 3 months. Feasibility, usability, and satisfaction data were collected throughout the first 3 months of intervention exposure. Results Forty-two MBC patients were randomized (1:1). Participants were 53±11 years old and 4±7 years from diagnosis with metastatic disease. No significant effects on psychosocial distress, pain, sleep disturbance, fatigue (vitality), quality of life, or chair stands were noted, despite high levels of acceptability (51%), feasibility (65%), and satisfaction (70%). Conclusion A high level of participant acceptability, feasibility, usability, and satisfaction all suggest further research on this platform is warranted. The lack of statistically significant effects on symptoms, quality of life, and function may be the result of small sample size. Approximately 50% of transitioning service members report difficulty adjusting to civilian life. However, there is limited research exploring factors that influence adjustment for former Australian Defence Force (ADF) members. The aim of this study was to investigate the influence of demographic and service-related characteristics, trauma exposure, health, and participation in meaningful occupations on adjustment for former ADF members. One hundred and ninety-eight former ADF members completed a voluntary, online survey containing validated self-report measures for adjustment, health, and exposure to combat and military sexual trauma. Participation in meaningful occupations was assessed using open-ended questions and a rating scale for frequency of participation. A more difficult adjustment was reported by participants who had completed operational service, reported exposure to combat and/or military sexual trauma, had poor physical health and were discharged for medical reasons. Other characteristics associated with a difficult adjustment included emotional distress, involuntary discharge, age category 30–49 years, final rank of Senior Noncommissioned Officer/Warrant Officer or below, and discharge 6–8 years previously. Employment, voluntary work and care, and social and community interaction were associated with an easier adjustment. Screening tools that consider health, age, deployment type, final rank, type of discharge and exposure to combat or military sexual trauma may be helpful to identify and refer high risk individuals to employment, rehabilitation or transition support programs.

e21577 Background: Response rates to PD1 inhibitors are high in unresectable locally advanced or metastatic cSCC. However, the management of cSCC patients with LN-PNI is challenging given that standard measures of disease burden and treatment response are poorly suited to PNI. This study aims to quantify radiological and symptomatic PD1 inhibitor response in cSCC LN-PNI. Methods: Patients with biopsy or radiologically confirmed cSCC LN-PNI treated with PD1 inhibitors between September 2017 and April 2021 were enrolled in this retrospective observational study. Radiological disease control (RDC) was defined as non-progressive radiological changes (including absence of proximal extension of PNI) ± partial or complete resolution on MRI or CT. A detailed symptomatic assessment was performed at every clinic visit. Progression free survival (PFS) was calculated from the time of initiating treatment to radiological disease progression (RDP). Survival was calculated using the Kaplan-Meier method and Cox regression was used to determine the effect of early symptomatic response (≤6 wks) and RDC at first staging scan (≤12 wks) on PFS. Results: Four Australian hospitals enrolled 31 cSCC LN-PNI patients (median age 75 years; M: F = 3:1), of which 29 (93.5%) had trigeminal and 9 (29.0%) had facial nerve involvement. Symptoms were present in 29 (93.5%) patients at baseline, including neuropathic pain in 24 (77.4%) and motor weakness (facial palsy or ophthalmoplegia) in 10 (32%). 16 (51.6%) patients had prior surgery or radiotherapy, but none received prior systemic therapy for LN-PNI. RDC on first restaging (6-12 wks) was observed in 28 (90.3%) patients and symptomatic improvement was seen in 23 (79.3%) amongst the 29 symptomatic at presentation. At a median follow-up of 18.7 months, patients with RDC on first restaging scan had a 24-month PFS of 84.2% versus 25% with RDP (p < 0.001, HR = 0.086, 95% CI = 0.018 to 0.403). Symptomatic response at 6 wks was associated with superior 12-month PFS (94.7% versus 55.6%, p < 0.01, HR = 0.068; 95% CI 0.008 to 0.584). There was high concordance (89.6%) between symptomatic and radiological control (Cohen’s Κ = 0.61, χ2 = 8.0023, p-value = 0.0047). Ocular toxicity was seen in 5 patients (16.1%), with corneal ulceration in 2 patients which was higher than expected but otherwise, toxicity profile was consistent with PD-1 inhibitors. Conclusions: These results demonstrate high response rates of PD1 inhibitors in patients with cSCC LN-PNI with acceptable toxicity and durability of response. Early symptomatic improvement within 6 wks and RDC on the first staging scan were significant markers of durable treatment response. PD1 inhibitors are active in patients with LN-PNI and should be considered for the treatment of cSCC with LN-PNI.

Sociologists increasingly use insights from dual-process models to explain how people think and act. These discussions generally emphasize the influence of cultural knowledge mobilized through automatic cognition, or else show how the use of automatic and deliberate processes vary according to the task at hand or the context. Drawing on insights from sociological theory and suggestive research from social and cognitive psychology, we argue that socially structured experiences also shape general, individual-level preferences (or propensities) for automatic and deliberate thinking. Using a meta-analysis of 63 psychological studies (N = 25,074) and a new multivariate analysis of nationally representative data, we test the hypothesis that the use of automatic and deliberate cognitive processes is socially patterned. We find that education consistently predicts preferences for deliberate processing and that gender predicts preferences for both automatic and deliberate processing. We find that age is a significant but likely nonlinear predictor of preferences for automatic and deliberate cognition, and we find weaker evidence for differences by income, marital status, and religion. These results underscore the need to consider group differences in cognitive processing in sociological explanations of culture, action, and inequality.
