10. Impact of elevated transmitral mean gradient on cardiovascular mortality after transcatheter mitral valve implantation.

Abstract

Transcatheter mitral valve implantation (TMVI) has emerged as an alternative to surgery in high risk patients with failing mitral bioprosthesis or annuloplasties. The presence of an elevated mean gradient > 5 mmHg after the procedure has been defined as an unsuccessful procedure but the impact and outcomes of an elevated mean gradient after TMVI have not been studied. In all, 175 patients undergoing TMVI from 2010 to 2024 were included. Patients were classified in three groups according to mean gradient after the procedure: group 1 < 5 mmHg, group 2 between 5 and 10 mmHg and group 3 ≥ 10 mmHg. The median age was 69 (52.5–80) years. Indications for TMVI were bioprosthesis failure in 131 patients (74.9%) and ring annuloplasty failure in 44 patients (25.1%). 155 (88.6%) patients were in NYHA class III or IV. Fifty patients were in group 1, 111 in group 2 and 14 in group 3, without any difference in baseline characteristics (except for age [ P = 0.02]) or procedural findings. At 30 days, the rates of all-cause mortality (respectively 2.6%, 0% and 0%, P = 0.3), rehospitalization for heart failure (respectively 7,7%, 7.4% and 20%, P = 0.62), paravalvular leak (4.6%, 16.8%, 20%, P = 0.41), LVOT obstruction (respectively 0%, 2.1% and 10%, P = 0.48) or valve thrombosis (respectively 11.6%, 4.2% and 0%, P = 0.48) were similar in the three groups. At a median follow-up of 3 years, 43 patients have died (35 for cardiovascular reasons) with a cumulative rate of all-cause mortality at 1, 2 and 3 years of 18.7%, 19.2% and 25.4% respectively. An elevated mean gradient was associated with increased risk of cardiovascular mortality with a HR of 1.16 (1.03–1.31, P = 0.02, Fig. 1). An improvement of NYHA class was observed in the two groups with mean gradient < 10 mmHg at one month and one year (P < 0.01) compared to baseline without any difference for the group 3. At one year, median systolic pulmonary arterial pressure also significantly improved in group 1 (from 50 mmHg (43–65) in baseline to respectively 45 mmHg (36–55), P < 0.01), in group 2 (from 55 mmHg [45–70] in baseline to 41 mmHg [35–55], P < 0.01]) without improvement in group 3 (from 55 mmHg (50–62) to 51 mmHg (35–58), P = 1). An elevated mean gradient post-TMVI ≥ 10 mmHg was associated with cardiovascular mortality up to 3 years and without significant functional improvement. Abstract

Long-term results of transcatheter mitral valve replacement (TMVR) using balloon-expandable valves remain uncertain. This study aims to evaluate the long-term outcomes and durability of bioprosthetic heart valves after TMVR in patients undergoing valve in valve (ViV), valve in ring (ViR) and valve in mitral annular calcification (ViMAC) procedures. All patients undergoing TMVR for bioprosthesis or ring failure or severe annular calcifications in our center were included. A ballon expandable transcatheter aortic valve (SAPIEN XT or SAPIEN 3) was used in all cases. Patients were followed at 3 months, 1 year and yearly thereafter by means of clinical visits or phone contacts. The primary outcome was a combined criteria of death and mitral re-intervention (mitral valve replacement, heart transplantation, TMVR in TMVR). A total of 200 patients underwent TMVR between may 2010 and august 2023, with 121 (60,5%) ViV, 45 (22,5%) ViR and 34 (17%) ViMAC procedures. The median age of the population was 70 years. Over a median follow-up period of 2.6 years [IQR: 0.7–6.7], the composite outcome of death or mitral reintervention occurred in 102 patients: 84 (82%) patients had died, 12 (12%) required mitral surgical replacement, 2 (2%) underwent heart transplantation and 4 (4%) required TMVR in TMVR. This outcome occurred more frequently in the ViR and ViMAC group than in the ViV group (Fig. 1) (P < 0.0001). Regarding echocardiographic findings, there was a significant reduction in mean transmitral valve gradient after the procedure with a median of 6.0 versus 10.3 mmHg before. The mean gradient remained initially stable with a slight increase during the longer follow up period (Fig. 2a). The median gradient was of 7 mmHg at 1.3 and 5 years 8 mmHg at 7 years and 9 mmHg at 9 years. A similar trend was observed with the assessment of the mitral valve area showing a slight decrease over the years. Reoccurrence of higher-grade (3–4+) mitral regurgitation was infrequent at last follow-up (Fig. 2b). Abstract

Background Prosthetic paravalvular leaks (PVLs) are associated with congestive heart failure and haemolysis, for which the standard treatment is open-heart surgery with the attendant risks to the patient. Transcatheter closure has emerged as an alternative. Patient selection criteria for the best option are needed. We aimed to identify predictors of clinical success after transcatheter PVL closure. Purpose We aimed to identify predictors of clinical success after transcatheter PVL closure. Methods Consecutive patients referred to 24 European centres for transcatheter PVL closure in 2017–2019 were included in a prospective registry (Fermeture de Fuite ParaProthétique, FFPP). Clinical success was absence of any of the following within 1 month: re-admission for heart failure, blood transfusion, open-heart valvular surgery, and death. Results We included 216 symptomatic patients, who underwent 238 percutaneous PVL closure procedures on the mitral (64.3%), aortic (34.0%), or tricuspid (1.7%) valve. The prosthesis was mechanical in 53.3% and biological in 45.3% of procedures. All patients were symptomatic with heart failure, haemolytic anaemia, and the association of both conditions in 48.9%, 7.8% and 43.3%. One, two and three PVL were addressed during the same procedure in 69.6%, 26.6% and 3.8% respectively. Abstract

OBJECTIVES The interference of a transtricuspid cardiac implantable electronic device (CIED) lead with tricuspid valve function may contribute to the mechanism of tricuspid regurgitation (TR) and poses specific therapeutic challenges during transcatheter tricuspid valve intervention (TTVI). Feasibility and efficacy of TTVI in presence of a CIED is unclear. BACKGROUND Feasibility of TTVI in presence of a CIED lead has never been proven on a large basis. METHODS The study population consisted of 470 patients with severe symptomatic TR from the TriValve (Transcatheter Tricuspid Valve Therapies) registry who underwent TTVI at 21 centers between 2015 and 2018. The association of CIED and outcomes were assessed. RESULTS Pre-procedural CIED was present in 121 of 470 (25.7%) patients. The most frequent location of the CIED lead was the posteroseptal commissure (44.0%). As compared with patients without a transvalvular lead (no-CIED group), patients having a tricuspid lead (CIED group) were more symptomatic (New York Heart Association functional class III to IV in 95.9% vs. 92.3%; p = 0.02) and more frequently had previous episodes of right heart failure (87.8% vs. 69.0%; p = 0.002). No-CIED patients had more severe TR (effective regurgitant orifice area 0.7 ± 0.6 cm2 vs. 0.6 ± 0.3 cm2; p = 0.02), but significantly better right ventricular function (tricuspid annular plane systolic excursion = 16.7 ± 5.0 mm vs. 15.9 ± 4.0 mm; p = 0.04). Overall, 373 patients (79%) were treated with the MitraClip (Abbott Vascular, Santa Clara, California) (106 [87.0%] in the CIED group). Among them, 154 (33%) patients had concomitant transcatheter mitral repair (55 [46.0%] in the CIED group, all MitraClip). Procedural success was achieved in 80.0% of no-CIED patients and in 78.6% of CIED patients (p = 0.74), with an in-hospital mortality of 2.9% and 3.7%, respectively (p = 0.70). At 30 days, residual TR ≤2+ was observed in 70.8% of no-CIED and in 73.7% of CIED patients (p = 0.6). Symptomatic improvement was observed in both groups (NYHA functional class I to II at 30 days: 66.0% vs. 65.0%; p = 0.30). Survival at 12 months was 80.7 ± 3.0% in the no-CIED patients and 73.6 ± 5.0% in the CIED patients (p = 0.30). CONCLUSIONS TTVI is feasible in selected patients with CIED leads and acute procedural success and short-term clinical outcomes are comparable to those observed in patients without a transtricuspid lead.

Abstract

Background Aspecific scoring systems are used to predict the risk of death postsurgery in patients with infective endocarditis ( IE ). The purpose of the present study was both to analyze the risk factors for in‐hospital death, which complicates surgery for IE , and to create a mortality risk score based on the results of this analysis. Methods and Results Outcomes of 361 consecutive patients (mean age, 59.1±15.4 years) who had undergone surgery for IE in 8 European centers of cardiac surgery were recorded prospectively, and a risk factor analysis (multivariable logistic regression) for in‐hospital death was performed. The discriminatory power of a new predictive scoring system was assessed with the receiver operating characteristic curve analysis. Score validation procedures were carried out. Fifty‐six (15.5%) patients died postsurgery. BMI >27 kg/m 2 (odds ratio [ OR ], 1.79; P =0.049), estimated glomerular filtration rate OR , 3.52; P IV ( OR , 2.11; P =0.024), systolic pulmonary artery pressure >55 mm Hg ( OR , 1.78; P =0.032), and critical state ( OR , 2.37; P =0.017) were independent predictors of in‐hospital death. A scoring system was devised to predict in‐hospital death postsurgery for IE (area under the receiver operating characteristic curve, 0.780; 95% CI, 0.734–0.822). The score performed better than 5 of 6 scoring systems for in‐hospital death after cardiac surgery that were considered. Conclusions A simple scoring system based on risk factors for in‐hospital death was specifically created to predict mortality risk postsurgery in patients with IE .
