1. Individual Differences in Personality Associated with Aggressive Behavior among Adolescents Referred for Externalizing Behavior Problems

Abstract

The present study examined the extent to which individual differences in personality that have been previously associated with aggression in non-clinical subjects (Caprara et al., European Journal of Personality, 27(3), 290–303, 2013, Caprara et al., Developmental Psychology, 50(1), 71–85, 2014) account for aggression among adolescents referred to psychiatric services with diagnosis within the externalizing spectrum (i.e., conduct disorder, oppositional defiant disorder, and attention deficit hyperactivity disorder). In particular a conceptual model was examined in which individual differences in basic traits (i.e., emotional instability and agreeableness), lower order traits (i.e., irritability and hostile rumination), and social cognitive mechanisms (i.e., moral disengagement) account for aggressive behavior. One hundred and nine adolescents (81 males, 74.3%), ranging in age from 11 to 18 (M = 13.83, SD = 1.70) and referred to psychiatric services for the above diagnoses, participated at the study. Adolescents filled in questionnaires measuring the Big Five traits, as well as irritability, hostile rumination, and moral disengagement; their parents filled in the Child Behavior Checklist to assess children’s aggressive behavior. Findings corroborated the posited pattern of relations previously found in non-clinical samples. In accordance with those findings, moral disengagement largely mediated the association between traits and aggressive behavior. The model explained a significant portion of variance in aggressive behavior. As a novelty, findings showed a direct association between emotional instability and aggressive behavior, pointing to the major relevance of emotional disregulation in adolescents referred for externalizing problem behaviors in comparison to non-clinical adolescents.

Abstract

This study was conducted to assess the long-term effect of methylphenidate (MPH) or atomoxetine (ATX) on growth in attention-deficit/hyperactivity disorder (ADHD) drug-naïve children.The study was an observational, post-marketing, fourth phase study.Data on height and weight were collected at baseline and every 6 months up to 24 months.Both ATX and MPH lead to decreased height gain (assessed by means of z-scores); the effect was significantly higher for ATX than for MPH. At any time, height z-score decrease in the ATX group was higher than the corresponding decrease observed in the MPH group, but the difference was significantly relevant only during the first year of treatment. An increment of average weight was observed both in patients treated with MPH and in those treated with ATX. However, using Tanner's percentile, a subset of patients showed a degree of growth lower than expected. This negative effect was significantly higher for ATX than for MPH.We conclude that ADHD drugs show a negative effect on linear growth in children in middle term. Such effect appears more evident for ATX than for MPH.

Abstract

Kyphoscoliotic Ehlers-Danlos syndrome and 17p13.3 microduplication share multiple clinical features such as muscle hypotonia, cleft palate, and growth impairment. This paper describes a patient who was first diagnosed with the duplication and a decade later also with FKBP14 -kEDS. The latter was initially overlooked due to the pathogenic significance attributed to the duplication and to the fact that, at the time of the first diagnosis, this specific form of kEDS had yet to be discovered. The patient's progressive kyphoscoliosis and severe joint laxity were the clinical features that prompted the patient's physiatrist to reassess the genetic work-up. This extreme latency caused inaccurate management in the patient's follow-up program, which ultimately may have resulted in preventable clinical complications. This report underlines the importance of remaining up-to-date with patient status, reviewing old cases, and relying on specialist advice to reach a correct diagnosis.
