9. Basiliximab vs. No Induction Therapy in Kidney Transplant Recipients with a Low Immunological Risk Profile Receiving Tacrolimus/Mycophenolate/Steroids Maintenance Immunosuppression.

- Abstract
Background: Induction therapy with basiliximab is recommended in kidney transplant (KT) recipients with a low immunological risk (LIR) profile. Whether basiliximab is associated with a decreased risk of acute rejection (AR) and graft loss is controversial. Methods: In our institution, LIR patients (absence of anti-HLA antibodies before KT) are inducted with basiliximab in case of living-donor KT, while deceased-donor KT recipients receive no induction. Maintenance immunosuppression is similar, including a combination of tacrolimus (Tac), mycophenolate (MPA) and steroids. In this single-center retrospective study, we included all adult LIR patients who underwent KT between 1 January 2015 and 31 December 2022. Results: Of the 471 patients included, 354 received no induction and 117 received basiliximab. The median (IQR) number of HLA A-B-DR mismatches was 3 (2–3) and 2 (2–4) in the no induction group and the basiliximab group, respectively. The cumulative incidences in the no induction group vs. the basiliximab group of acute rejection and graft loss over 5 years post-KT were similar at 8.9% vs. 7.8% (p = 0.8) and 8.5% vs. 4.2% (p = 0.063), respectively. In multivariable Cox regression analysis, delayed graft function emerged as an independent risk factor for acute rejection (hazard ratio [HR] 2.75, 95% confidence interval [CI] 1.23–6.13, p = 0.014) and graft loss (HR 9.32, CI 4.10–21.1, p < 0.001). Conclusions: Basiliximab did not provide any advantage in terms of rate of acute rejection and graft survival within 5 years post KT compared with a strategy without induction therapy in patients with a low immunological risk profile receiving triple maintenance immunosuppression Tac/MPA/steroids. BACKGROUND Since its outbreak in early 2020, the COVID-19 pandemic has diverted resources from non-urgent and elective procedures, leading to diagnosis and treatment delays, with an increased number of neoplasms at advanced stages worldwide. The aims of this study were to quantify the reduction in surgical activity for indeterminate thyroid nodules during the COVID-19 pandemic; and to evaluate whether delays in surgery led to an increased occurrence of aggressive tumours. METHODS In this retrospective, international, cross-sectional study, centres were invited to participate in June 22, 2022; each centre joining the study was asked to provide data from medical records on all surgical thyroidectomies consecutively performed from Jan 1, 2019, to Dec 31, 2021. Patients with indeterminate thyroid nodules were divided into three groups according to when they underwent surgery: from Jan 1, 2019, to Feb 29, 2020 (global prepandemic phase), from March 1, 2020, to May 31, 2021 (pandemic escalation phase), and from June 1 to Dec 31, 2021 (pandemic decrease phase). The main outcomes were, for each phase, the number of surgeries for indeterminate thyroid nodules, and in patients with a postoperative diagnosis of thyroid cancers, the occurrence of tumours larger than 10 mm, extrathyroidal extension, lymph node metastases, vascular invasion, distant metastases, and tumours at high risk of structural disease recurrence. Univariate analysis was used to compare the probability of aggressive thyroid features between the first and third study phases. The study was registered on ClinicalTrials.gov, NCT05178186. FINDINGS Data from 157 centres (n=49 countries) on 87 467 patients who underwent surgery for benign and malignant thyroid disease were collected, of whom 22 974 patients (18 052 [78·6%] female patients and 4922 [21·4%] male patients) received surgery for indeterminate thyroid nodules. L’année 2022 a été celle du retour progressif à la normale après la pandémie COVID-19. Une occasion d’optimiser les processus parfois mis à mal et de mettre à jour les procédures. Une opportunité de se lancer dans de nouveaux défis : réfléchir à l’acquisition de nouveaux équipements pour les années à venir, à l’implémentation de nouvelles techniques plus sensibles, plus rapides, mieux adaptées aux besoins des prescripteurs. Au sein du Laboratoire d’hématologie spéciale, et en particulier dans le secteur de l’hémostase spécialisée, l’automatisation du dosage de la protéine ADAMTS13 était indispensable pour le diagnostic et le suivi biologique des patients présentant un purpura thrombotique thrombocytopénique immun (PTTi). Le décours de cette affection, peu fréquente, peut être rapidement péjoratif en l’absence d’un diagnostic précoce et d’une prise en charge adaptée, guidée par le dosage d’ADAMTS13. Ce dernier permet également d’identifier les patients à risque de rechute qui doivent ainsi bénéficier d’un traitement préventif. La détermination des groupes sanguins érythrocytaires s’effectue la plupart du temps sans difficulté, par des techniques sérologiques. Cependant, dans certaines situations, il est nécessaire de recourir à des techniques de biologie moléculaire. Le Laboratoire d’immunohématologie – biologie moléculaire a implémenté une technique de PCR en temps réel qui permet de réaliser un génotypage érythrocytaire très étendu et de détecter des variants antigéniques passés inaperçus en technique sérologique. Il s’agit d’un outil indispensable pour la détermination de groupes sanguins chez des patients polytransfusés et pour la sélection de poches pour des personnes ayant un groupe sanguin variant ou en délétion. On sait depuis plus de 50 ans que le système HLA joue un rôle central dans l’immunité humaine et qu’il constitue aussi l'un des principaux obstacles à la transplantation d’organe. L’implication des anticorps anti-HLA dans le rejet précoce et tardif est indiscutable. Le Laboratoire d’immunologie leuco-plaquettaire a implémenté en 2021 un nouveau test pour une identification plus spécifique des anticorps anti-HLA cliniquement plus pertinents, permettant de faciliter l’attribution d’un greffon. Durant cette année 2022, les dossiers des patients en attente d’une greffe rénale et qui présentaient des anticorps anti-HLA ont été revus et des contrôles ont été réalisés sur d’anciens sérums en utilisant la nouvelle technique. Ceci a permis de retirer des spécificités anticorps « interdites » chez plus de la moitié des patients augmentant ainsi leur chance de bénéficier d’un greffon compatible. Six patients ont reçu un greffon rénal portant un antigène HLA précédemment répertorié comme interdit avec l’ancienne technologie et retiré après la révision. Actuellement, ces patients ont, tous, montré un greffon fonctionnel sans complication immunologique. Ces résultats seront présentés au congrès de l’EFI (European Foundation for Immunogenetics) en avril 2023. Bien que la transfusion soit une procédure thérapeutique courante et bénéfique pour le patient, elle peut s’accompagner de réactions transfusionnelles. Le diagnostic de celles-ci est parfois flou et malaisé. Le secteur Hémovigilance de la transfusion a dessiné de nouveaux algorithmes décisionnels à partir de la symptomatologie majeure présentée par le patient et du composant sanguin incriminé dans la réaction. Ces algorithmes ont pour but d’orienter vers un diagnostic et, pour ce faire, de déterminer les contrôles et les tests de laboratoire qui doivent être réalisés dans chaque situation. Ces algorithmes ont fait l’objet d’une publication dans la revue de la Société Francophone de Transfusion Sanguine. Bien que des prémices le laissaient suspecter déjà en 2021, la situation de l’approvisionnement en concentrés érythrocytaires de groupe sanguin O négatif s’est encore fortement aggravée durant l’année 2022. La Banque de sang, en concertation avec la direction médicale et le Comité de transfusion, a dû y faire face en mettant en place différentes actions pour tenter d’endiguer l’« hémorragie ». Ceci fait l’objet d’une procédure particulière : mise en place d’indicateurs très stricts et surveillés de façon très étroite, priorisation des indications transfusionnelles, distribution unitaire exclusive (en dehors des situations hémorragiques), décision médicale de transfuser des concentrés érythrocytaires Rh positifs à des receveurs Rh négatifs. Diverses évaluations très précises de cette situation sont encore en cours et seront soumises prochainement à publications. Une action concertée a été mise en place au sein des hôpitaux du réseau et des initiatives sont également prises au niveau national. Finalement, l’année 2022 a également été celle de la publication d’une nouvelle directive européenne et de nouveaux standards EDQM (European Directorate for the Quality of Medicines) définissant désormais le sang comme SoHO (Substance of Human Origin), au même titre que les cellules et les tissus. C’est la raison pour laquelle, il a été décidé que la Banque de sang et le laboratoire d’immunologie érythrocytaire rejoignaient le Département des Banques le 1er janvier 2023.

- Abstract
Background. Pregnancies in women who underwent kidney transplants are at high risk compared with the general population. Methods. In this study, we aimed to retrospectively assess the obstetrical complications, delivery outcomes, and impact of pregnancy on kidney allograft function in a single-center cohort of kidney transplant recipients (KTRs). We provide data regarding the long-term evolution of children. Results. Thirty-two KTRs underwent a total of 57 pregnancies between 1994 and 2010. Fourteen pregnancies (24 %) did not survive caused by miscarriages (n = 9), stillborn (n = 1), ectopic pregnancies (n = 2), and medical abortion (n = 2). Live birth occurred in 76% of pregnancies. Delivery was by cesarean in 66%. The mean gestational age was 30.45 § 11.3 weeks and 65% of newborns were premature. A low birth weight <2500g was noted in 46%. Obstetric complications were de novo hypertension in 4%, pre-eclampsia in 9%, and gestational diabetes in 2%. The 5- and 10-year post-delivery death-censored graft loss rates were 3.1% and 12.5%, respectively. Data on 21 children were collected via a self-questionnaire. After a median follow-up time of 17 years, they appeared in good medical and psychological health. None of them suffered from chronic disease (especially uronephrological condition) or was taking chronic medication. Conclusions. Long-term evolution of children born to women who underwent kidney transplants seems favorable. Pregnancies in KTRs are successful in two-thirds of cases but are at increased risk of prematurity, delivery by cesarean, and low birth weight.

- Abstract
Neutralizing monoclonal antibody treatments have shown promising preliminary results in kidney transplant recipients infected with SARS-CoV-2. However, their efficacy in kidney transplant recipients infected with Omicron variant has not been reported yet. Single-center retrospective study. We included all consecutive kidney transplant recipients treated with monoclonal antibodies for SARS-CoV-2 infection (positive polymerase chain reaction on nasopharyngeal swab) between June 10 2021 and January 14 2022. Forty-seven kidney transplant recipients were included. All patients had symptoms evolving for ≤ 7 days and no oxygen therapy need at monoclonal antibody infusion. Symptoms at diagnosis were mainly cough (n=25 [53%]) and fever (n=15 [32%]). Eighty-three percent of the cohort (n=39) had been vaccinated with at least 2 doses before infection, of whom 77% (n=30) had demonstrated a vaccine-induced humoral response. They were treated with either casirivimab-imdevimab (n= 16 [34%]) or sotrovimab (n= 31 [66%]) a median of 2 (ranges 0-6) days after the onset of symptoms. Except for one mild allergic reaction during casirivimab-imdevimab infusion, no side effects were reported. Median viral loads at admission (day 0) and 7 days after mAb infusion were 2,110,027 (ranges 1000-153,798,962) copies/mL and 1,000 (ranges 0-10,000,000) copies/mL, respectively. Genotypes were available for 22 kidney transplant recipients (47%). Omicron, Delta, and Gamma variants were identified in 13 (59%), 8 (36%), 1 (5%) patients, respectively. In kidney transplant recipients infected with Omicron variant, the median viral loads at day 0 and day 7 were 752,789 (ranges 4000-12,859,300) copies/mL and 1,353 (ranges 0-1,211,163) copies/mL, respectively. Two kidney transplant recipients required hospitalization immediately after sotrovimab perfusion for oxygen therapy that was weaned in 3 days allowing patients' discharge. - Abstract
The aim of this study was to evaluate the effect of a recipient's obesity on posttransplant complications and patient and graft survival. A single-institution, retrospective study was performed on obese renal transplant recipients (BMI ≥ 30 kg/m, n = 102) from January 2010 to December 2018, matched with non-obese recipients (BMI < 30 kg/m, n = 204). For comparison, for every obese patient we selected 2 nonobese patients with a similar age, sex, and period of transplantation. The comparative analysis included patient and graft survival as primary outcomes and graft function and postoperative complications as a secondary outcome. Recipient demographics were comparable in both groups except for diabetic nephropathy in obese patients (P = .0006). Obesity was strongly related to a poorer patient survival (risk ratio [RR] = 2.83 confidence interval [CI] 95% 1.14-7.04; P = .020) but there was no observed difference in graft survival (P = .6). While early graft function was inferior in the obese population (RR = 2.41; CI 95% 1.53-3.79; P = .00016), during late follow-up, no statistically significant differences were observed between both groups (P = .36). Obese recipients had a significantly higher risk of delayed graft function (RR = 1.93; CI 95% (1.19-3.1), P = .0077), heart infarction (RR = 7; CI 95% 1.68-29.26; P = .0042), wound infections (RR = 8; CI 95% 1.96-32.87; P = .0015), diabetes aggravation (RR = 3.13; CI 95% 1.29-7.6; P = .011), and surgical revision for eventration (RR = 8; CI 95% 1.22-52.82; P = .026) when compared with nonobese recipients. Despite the inferior early kidney graft function in obese recipients, there was no difference observed at the long-term follow-up. However, recipient obesity demonstrated a negative effect on patient survival and postoperative complications.

- Abstract
Nirmatrelvir-ritonavir (NR) has demonstrated its efficacy to decrease the risk of progression to severe COVID-19 in high-risk patients. However, evidence in infected kidney transplant recipients (KTRs) is lacking. Moreover, NR has significant and potentially harmful interactions with calcineurin inhibitors (CNIs). In this single-center retrospective study, we included all KTRs treated with NR from April 28 to June 3, 2022. A standard management strategy of CNIs dose adaptation (discontinuation of tacrolimus 12 hours before the start of NR and administration of 20% the cyclosporine dose) and laboratory follow-up was applied. Fourteen patients were included. Compared to day-0 (day before NR initiation), day-7 plasma creatinine concentrations and SARS-CoV-2 viral loads were similar (p = 0.866) and decreased (p=0.002), respectively. CNIs trough concentrations at the end of the treatment were satisfactory, with nevertheless high individual variability. After a median follow-up time of 34 days, no death nor viral pneumonia were observed. However, 2 patients experienced early SARS-CoV-2 infection relapses (at day-10 and day-21) associated with an increase in SARS-CoV-2 viral loads. NR can be used in KTRs but requires a strict protocol of drug adaptation. We observed two cases of early relapse after NR treatment that need further investigations.

- Abstract
Aims/hypothesis: The rapid remission of type 2 diabetes by a diet very low in energy correlates with a marked improvement in glucose-stimulated insulin secretion (GSIS), emphasising the role of beta cell dysfunction in the early stages of the disease. In search of novel mechanisms of beta cell dysfunction after long-term exposure to mild to severe glucotoxic conditions, we extensively characterised the alterations in insulin secretion and upstream coupling events in human islets cultured for 1–3 weeks at ~5, 8, 10 or 20 mmol/l glucose and subsequently stimulated by an acute stepwise increase in glucose concentration. Methods: Human islets from 49 non-diabetic donors (ND-islets) and six type 2 diabetic donors (T2D-islets) were obtained from five isolation centres. After shipment, the islets were precultured for 3–7 days in RPMI medium containing ~5 mmol/l glucose and 10% (vol/vol) heat-inactivated FBS with selective islet picking at each medium renewal. Islets were then cultured for 1–3 weeks in RPMI containing ~5, 8, 10 or 20 mmol/l glucose before measurement of insulin secretion during culture, islet insulin and DNA content, beta cell apoptosis and cytosolic and mitochondrial glutathione redox state, and assessment of dynamic insulin secretion and upstream coupling events during acute stepwise stimulation with glucose [NAD(P)H autofluorescence, ATP/(ATP+ADP) ratio, electrical activity, cytosolic Ca2+ concentration ([Ca2+]c)]. Results: Culture of ND-islets for 1–3 weeks at 8, 10 or 20 vs 5 mmol/l glucose did not significantly increase beta cell apoptosis or oxidative stress but decreased insulin content in a concentration-dependent manner and increased beta cell sensitivity to subsequent acute stimulation with glucose. Islet glucose responsiveness was higher after culture at 8 or 10 vs 5 mmol/l glucose and markedly reduced after culture at 20 vs 5 mmol/l glucose.
