Background: Hypertension has earned the moniker "the silent killer" because to its unique characteristic of often remaining asymptomatic for extended periods, in contrast to numerous other ailments. Aim: To determine the Quality of Life (QoL) among 50 years and older hypertensive patients in Saudi Arabia. Methods: A cross-sectional quantitative study using structured interviews was conducted among Saudi hypertensive patients aged 50 years and old. The World Health Organization's Quality of Life (WHOQOL)-BREF to measure the quality of life of patients with hypertension. Binary logistic regression analysis was used to identify predictors of better QoL. Settings: Imam Mohammad ibn Saud Islamic University Medical Center and King Fahad Medical City in Riyadh, Saudi Arabia for the duration between October 2022 and January 2023. Results: A total of 392 patients with hypertension participated in this study. Overall, the mean QoL score for the participating patients was 67.7 (SD:13.4), which presents a moderate QoL. The highest QoL score was observed for the psychological health domain (83.7 (SD:14.9)) and the lowest QoL score was observed for the physical health domain (42.8 (SD:8.8)). Male patients and those with higher level of education (bachelor's degree or higher) were more likely to have better QoL compared to others (p<0.05). Conclusion: Overall, patients with hypertension demonstrated moderate QoL. Male patients and those with higher level of education tend to exhibit a better QoL compared to the rest of hypertensive patients. A multi-center study approach is recommended to confirm our study findings and recommend interventions to improve patients QoL. [ABSTRACT FROM AUTHOR]

The genotoxicity of AuNPs has sparked a scientific debate, with one perspective attributing it to direct DNA damage and another to oxidative damage through reactive oxygen species (ROS) activation. This controversy poses challenges for the widespread use of AuNPs in biomedical applications. To address this debate, we employed four-dimensional atomic force microscopy (4DAFM) to examine the ability of AuNPs to damage DNA in vitro in the absence of ROS. To further examine whether the size and chemical coupling of these AuNPs are properties that control their toxicity, we exposed individual DNA molecules to three different types of AuNPs: small (average diameter = 10 nm), large (average diameter = 22 nm), and large conjugated (average diameter = 39 nm) AuNPs. We found that all types of AuNPs caused rapid (within minutes) and direct damage to the DNA molecules without the involvement of ROS. This research holds significant promise for advancing nanomedicines in diverse areas like viral therapy (including COVID-19), cancer treatment, and biosensor development for detecting DNA damage or mutations by resolving the ongoing debate regarding the genotoxicity mechanism. Moreover, it actively contributes to the continuous endeavors aimed at fully harnessing the capabilities of AuNPs across diverse biomedical fields, promising transformative healthcare solutions. Background The Middle East respiratory syndrome coronavirus (MERS-CoV) is a highly pathogenic zoonotic betacoronavirus and a global public health concern. Better undersetting of the immune responses to MERS-CoV is needed to characterize the correlates of protection and durability of the immunity and to aid in developing preventative and therapeutic interventions. Although MERS-CoV–specific circulating antibodies could persist for several years post-recovery, their waning raises concerns about their durability and role in protection. Nonetheless, memory B and T cells could provide long-lasting protective immunity despite the serum antibodies levels. Methods Serological and flow cytometric analysis of MERS-CoV–specific immune responses were performed on samples collected from a cohort of recovered individuals who required intensive care unit (ICU) admission as well as hospital or home isolation several years after infection to characterize the longevity and quality of humoral and cellular immune responses. Results Our data showed that MERS-CoV infection could elicit robust long-lasting virus-specific binding and neutralizing antibodies as well as T- and B-cell responses up to 6.9 years postinfection regardless of disease severity or need for ICU admission. Apart from the persistent high antibody titers, this response was characterized by B-cell subsets with antibody-independent functions as demonstrated by their ability to produce tumor necrosis factor α (TNF-α), interleukin (IL)-6, and interferon γ (IFN-γ) cytokines in response to antigen stimulation. Furthermore, virus-specific activation of memory CD8+ and CD4+ T cell subsets from MERS-recovered patients resulted in secretion of high levels of TNF-α, IL-17, and IFN-γ. Considering the global trend to confine the COVID‐19 pandemic by applying various preventive health measures, preprocedural mouth rinsing has been proposed to mitigate the transmission risk of SARS‐CoV‐2 in dental clinics. The study aimed to investigate the effect of different mouth rinses on salivary viral load in COVID‐19 patients. This study was a single‐center, randomized, double‐blind, six‐parallel‐group, placebo‐controlled clinical trial that investigated the effect of four mouth rinses (1% povidone‐iodine, 1.5% hydrogen peroxide, 0.075% cetylpyridinium chloride, and 80 ppm hypochlorous acid) on salivary SARS‐CoV‐2 viral load relative to the distilled water and no‐rinse control groups. The viral load was measured by quantitative reverse transcription PCR (RT‐qPCR) at baseline and 5, 30, and 60 min post rinsing. The viral load pattern within each mouth rinse group showed a reduction overtime; however, this reduction was only statistically significant in the hydrogen peroxide group. Further, a significant reduction in the viral load was observed between povidone‐iodine, hydrogen peroxide, and cetylpyridinium chloride compared to the no‐rinse group at 60 min, indicating their late antiviral potential. Interestingly, a similar statistically significant reduction was also observed in the distilled water control group compared to the no‐rinse group at 60 min, proposing mechanical washing of the viral particles through the rinsing procedure. Therefore, results suggest using preprocedural mouth rinses, particularly hydrogen peroxide, as a risk‐mitigation step before dental procedures, along with strict adherence to other infection control measures. Background: There is substantial evidence that most SARS-CoV-2 infections are mild or even asymptomatic, yet they can transmit the virus to others. The current study described the clinical presentation of mild COVID-19 cases isolated in the ministry of health (MOH) quarantines in Riyadh, Saudi Arabia. Methods: A cross-sectional study targeted the SARS-CoV-2 PCR +ve asymptomatic and mild COVID-19 patients isolated in the Saudi MOH quarantines in Riyadh city between July and December 2020. The confirmed COVID-19 patients were enrolled and interviewed by telephones after obtaining the informed consent. Results: The study included 223 patients with a mean age of 32.5± 10.7 years old. The majority were male 156 (70%). Only 27 [12.1%; 95% CI = (8.1– 17.1%)] were asymptomatic. General fatigue was the most common reported symptom, 43.5%, followed by headache with 42.6%, and cough by 38.1%. Anosmia and ageusia were reported by 33.2% and 31.4%, respectively. The least common reported symptoms were vomiting, earache, and nausea with 1.8%, 4.0%, and 7.6%, respectively. Conclusion: The top five clinical manifestations of mild COVID-19 cases were general fatigue, headache, cough, anosmia, and ageusia. Only 12% of confirmed COVID-19 cases were asymptomatic. Hepatitis-B Virus (HBV) infection is a serious health problem that can be prevented by vaccination. Dental Health Care Workers (DHCWs) are at-risk of occupational exposure to HBV infection. This study was aimed to assess the prevalence of HBV and evaluate the immune response to hepatitis B vaccine among DHCWs in Dental Teaching Hospital, Umm Al-Qura University, Saudi Arabia. A cross-sectional study was conducted on 139 DHCWs, 71 males and 68 females. Blood samples were collected and the levels of hepatitis B surface antigen (HBsAg) and hepatitis B surface antibody (anti-HBs) were measured by Chemiluminescent Microparticle Immunoassay. The prevalence of HBV among DHCWs was zero (0.0%). The hepatitis B vaccine was given to 95% of DHCWs. Among the vaccinated participants, 90.1% (n=119) have protective immunity to hepatitis B. An inverse correlation between anti-HBs levels and increasing the duration of vaccination (P < 0.0001) was found. We compared the anti-HBs levels in 28 students who received childhood vaccine and revaccinated at age of 21. The anti-HBs concentration was greater than 10mIU/mL (protected) in 17.9% of those who had childhood vaccine compared to 100% one-year after revaccination. The mean of anti-HBs levels for childhood vaccine was 5.6 mIU/mL and these levels increased significantly to 620 mIU/mL after recent revaccination (P < 0.0001). In conclusion, Hepatitis B vaccine is effective in prevention of HBV infection among DHCWs. Objectives: To highlight the causes of hospitalization among sickle cell diseased (SCD) children in Al-Madinah Al-Munawarah, Saudi Arabia., Methods: A retrospective study conducted at the Maternity and Children's Hospital, Al-Madinah Al-Munawarah, Saudi Arabia. A data of 739 SCD children admitted to the hematology/oncology unit between October 2010 and September 2015 were collected. The collected data were analyzed using an independent t test and a Chi square test as appropriate., Results: Approximately 49% of the studied children were presented by acute painful crisis. Acute chest syndrome was reported in 20.9%. Infection was the cause of admission in 17.5%, and acute anemia was reported in 8.1% of the studied patients. No significant difference of the reported clinical manifestations by patients' gender. Children aged &lt;12 years showed significantly high frequency of acute chest syndrome (ACS) (26.5%), while acute painful crisis (66.4%) was significantly more frequent among children aged ≥12 years., Conclusion: This study revealed high rate of hospitalization of SCD children because of acute painful crisis, ACS, infection, and anemia. These admissions causes could potentially be continuously assessed to minimize the rate of hospitalization.

Objective: To evaluate the prevalence of methicillin resistant Staphylococcus aureus (MRSA) carriage among a cohort of pilgrims during 2004 Hajj season., Methods: Pilgrims attending the 2004 Hajj season were recruited and screened for carriage of MRSA. Standard microbiological techniques were used to screen for the presence of MRSA., Results: Out of 411 individuals screened, 85 (20.6%) were positive for Staphylococcus aureus (S. aureus) of which only 6 (1.46 %) were MRSA. Four individuals (4.6%) had the S. aureus organism in both nasal and axillary swabs, while 7 individuals (8%) had the organism in their axillae only. The other 74 individuals (87.1%) had the organism in their nares only. The 6 MRSA isolates were positive for the mecA gene by polymerase chain reaction method. None of the pilgrims examined had any risk factors for community-acquired methicillin resistant S. aureus (CAMRSA). Overall, the prevalence of MRSA in the population of pilgrims examined was found to be low (1.46%) in comparison with most community based studies., Conclusion: A low rate of MRSA carriage was noticed among the screened cohort. Physicians treating patients suspected of S. aureus infection during the Hajj pilgrimage should bear in mind the possibility of community acquired - MRSA and should obtain appropriate samples for bacterial cultures and susceptibility testing so that antimicrobial agents could be introduced when necessary at a later stage.
