Abstract

The lymph node (LN) is an intriguing site not only for inducing protective effector immunity but also for inducing tolerance against peripherally encountered antigens such as tissue-specific self-antigens that are regionally drained and through draining lymph nodes (DLNs). The dual functions of DLNs in immunity are attributable at least in part to fibroblastic reticular cells (FRCs), which are a major population of the nonhematopoietic compartment in the LN. In this issue of theJCI, Li, Zhao, and colleagues investigated DLNs in the transplantation setting. The authors demonstrated that, following skin transplantation, the donor mast cell-mediated senescence in FRCs was associated with collagen 1 deposition in DLNs. Systemic administration to mice of FRCs that were expanded ex vivo decreased DLN fibrosis and strengthened the effect of anti-CD40L in prolonging heart allograft survival. Abstract

Launching in 2023, NASA’s Europa Clipper mission will place a spacecraft into a long, looping orbit around Jupiter to perform a series of close flyby investigations of its moon Europa. Strong evidence suggests that Europa hosts an ocean of liquid water beneath its icy crust that could harbor conditions favorable for life. The spacecraft is currently under development by the Jet Propulsion Laboratory (JPL) and the Applied Physics Laboratory (APL) and nearing completion of its Detailed Design Phase. A Mechanical Pumped Fluid Loop (MPFL) architecture known as the Heat Redistribution System (HRS) has been baselined to provide power efficient thermal control of the sensitive components within the Avionics, Radio Frequency (RF), and Propulsion Modules. Principally, the HRS harvests waste heat from the onboard dissipating equipment and distributes it to a 1.4 m diameter by 3 m tall cylindrical structure that cocoons and keeps the propulsion subsystem warm. Numerous technical issues had to be resolved in order to securely attach nearly 100 m of HRS tubing to the spacecraft, which is predicted to experience high launch loads. This paper will review the relevant environments and mission constraints as well as outline the stress and fatigue analysis approach taken to verify the detailed mechanical design of the HRS.

Abstract

The feasibility and effectiveness of kidney paired donation (KPD) for the management of patients with incompatible donors were determined. Evidence suggests that recipients with incompatible live donors, even those with rare blood type combinations, can receive transplants through KPD with graft survival rates equivalent to directed, compatible liver donor transplants.

Abstract

A monoclonal antibody called daclizumab in combination with other immunosuppressive drugs may reduce acute graft rejection in patients who have received kidney transplants. Immunosuppressive drugs decrease immune response to transplanted tissue, but can increase the risk of infection and cancer. Researchers treated 260 patients with daclizumab or placebo, in addition to standard immunosuppressive therapy. Acute rejection of the new kidney was documented in 22% of patients who received daclizumab and 35% of those receiving the placebo. Both groups had similar rates of infection and cancer.

Abstract

Laparoscopic sleeve gastrectomy (LSG) is the most common bariatric surgery performed for morbid obesity. Leaks of the vertical staple line can occur in up to 7% of cases and are difficult to manage. Endolumenal vacuum (EVAC) therapy and fistulojejunostomy (FJ) have separate documented uses to heal these complicated leaks. We aim to show the benefit of using EVAC with FJ in the treatment of LSG staple line leaks. The patient presented with an LSG chronic leak. EVAC therapy was initiated but failed to close the fistula after 101 days. EVAC therapy was abandoned, and FJ was performed to resolve the leak. Postoperatively, no leak was encountered requiring any additional procedures. Based on our findings, we conclude that EVAC therapy facilitates in resolving leaks that restore gastrointestinal continuity and maintain source control. It promotes healing and causes reperfusion of ischemic tissue and fistula cavity debridement.

46. Progression to total occlusion is an underrecognized complication of the medical management of carotid disease

Abstract

One of the most frequently performed but controversial vascular operations is carotid endarterectomy, a procedure that removes the cholesterol-plaque thickened inner lining of the carotid artery (the arteries that provide the main blood supply to the brain.) The value of surgery, rather than medical treatment, for carotid artery disease has been questioned. Little attention has been given to the potential complication of medical treatment, which is progression of the disease to total occlusion of the carotid artery. This may occur without symptoms or with only mild neurologic symptoms. A study was undertaken to study this further. Of 993 patients who underwent carotid angiography (serial X-ray examinations of a the carotids used to define the size and shape of the vessels), 44 patients had an occlusion of the internal carotid artery. A careful review of their previous medical records revealed that 64 percent of the patients previously had symptoms on the same side as the current internal carotid artery occlusion. These symptoms had occurred an average of 30 months (with a range of 2 to 120 months) prior to diagnosis of occlusion. At the time of these prior symptoms, only eight patients (28 percent) had undergone vascular evaluation (either noninvasive or angiographic); all had been placed on medical antiplatelet medication. These findings suggest that a serious complication of medical management of symptomatic carotid artery disease is total occlusion of the internal carotid artery. 47. Angiographic localization of spinal cord blood supply and its relationship to postoperative paraplegia

Abstract

Surgical repair of long segments of the aorta (main systemic artery) carries the risk of paraplegia, paralysis in the lower part of the body. This paraplegia may be a consequence of lack of adequate blood supply to the spinal cord during surgery. The great radicular artery (GRA) is a major component of spinal cord circulation. There is considerable anatomic variation in the point cord may be altered by the development of vascular disease such as atherosclerosis (the presence of cholesterol plaque formation on the inner layer of the artery wall) and mural thrombus (clots along the wall of the artery). Various techniques have been developed to protect spinal cord circulation during aortic surgery, but these do not identify the origin of the blood supply to the spinal cord. A report is presented of 47 patients who underwent catheterization of the thoracic intercostal and lumbar arteries to attempt to identify the origin of spinal cord blood supply. The origin was identified in 26 patients (55 percent); 21 of these patients underwent aortic aneurysm repair. In 12 patients this artery could be included in the anastomosis (surgical connection of blood vessels); all 12 patients survived, one ended up paralyzed. In nine patients this was not able to be done and seven of the nine patients died or were paralyzed. Sixteen of the 19 patients in whom the blood supply was not able to be identified had successful surgery; 12 of these survived. It is concluded that selective intercostal angiography is safe and may add to our understanding of spinal cord complications of aortic aneurysm surgery. Abstract

Current therapeutic strategies for diabetic foot ulcer (DFU) have focused on developing topical healing agents, but few agents have controlled prospective data to support their effectiveness in promoting wound healing. We tested a stem cell mobilizing therapy for DFU using a combination of AMD3100 and low-dose FK506 (tacrolimus) (AF) in streptozocin-induced type 1 diabetic (T1DM) rats and type 2 diabetic Goto-Kakizaki (GK) rats that had developed peripheral artery disease and neuropathy. Here, we show that the time for healing back wounds in T1DM rats was reduced from 27 to 19 days, and the foot wound healing time was reduced from 25 to 20 days by treatment with AF (subcutaneously, every other day). Similarly, in GK rats treated with AF, the healing time on back wounds was reduced from 26 to 21 days. Further, this shortened healing time was accompanied by reduced scar and by regeneration of hair follicles. We found that AF therapy mobilized and recruited bone marrow-derived CD133+ and CD34+ endothelial progenitor cells and Ym1/2+ M2 macrophages into the wound sites, associated with enhanced capillary and hair follicle neogenesis. Moreover, AF therapy improved microcirculation in diabetic and neuropathic feet in GK rats. This study provides a novel systemic therapy for healing DFU. [ABSTRACT FROM AUTHOR]
