Abstract

IntroductionThe current study aimed to investigate the rates of anxiety, clinical depression, and suicidality and their changes in health professionals during the COVID-19 outbreak.Materials and methodsThe data came from the larger COMET-G study. The study sample includes 12,792 health professionals from 40 countries (62.40% women aged 39.76 ± 11.70; 36.81% men aged 35.91 ± 11.00 and 0.78% non-binary gender aged 35.15 ± 13.03). Distress and clinical depression were identified with the use of a previously developed cut-off and algorithm, respectively.Statistical analysisDescriptive statistics were calculated. Chi-square tests, multiple forward stepwise linear regression analyses, and Factorial Analysis of Variance (ANOVA) tested relations among variables.ResultsClinical depression was detected in 13.16% with male doctors and 'non-binary genders' having the lowest rates (7.89 and 5.88% respectively) and 'non-binary gender' nurses and administrative staff had the highest (37.50%); distress was present in 15.19%. A significant percentage reported a deterioration in mental state, family dynamics, and everyday lifestyle. Abstract

IntroductionDuring the COVID-19 pandemic various degrees of lockdown were applied by countries around the world. It is considered that such measures have an adverse effect on mental health but the relationship of measure intensity with the mental health effect has not been thoroughly studied. Here we report data from the larger COMET-G study pertaining to this question.Material and methodsDuring the COVID-19 pandemic, data were gathered with an online questionnaire from 55,589 participants from 40 countries (64.85% females aged 35.80 ± 13.61; 34.05% males aged 34.90±13.29 and 1.10% other aged 31.64±13.15). Anxiety was measured with the STAI, depression with the CES-D and suicidality with the RASS. Distress and probable depression were identified with the use of a previously developed cut-off and algorithm respectively.Statistical analysisIt included the calculation of Relative Risk (RR), Factorial ANOVA and Multiple backwards stepwise linear regression analysis RESULTS: Approximately two-thirds were currently living under significant restrictions due to lockdown. For both males and females the risk to develop clinical depression correlated significantly with each and every level of increasing lockdown degree (RR 1.72 and 1.90 respectively). The combined lockdown and psychiatric history increased RR to 6.88 The overall relationship of lockdown with severity of depression, though significant was small.ConclusionsThe current study is the first which reports an almost linear relationship between lockdown degree and effect in mental health. Our findings, support previous suggestions concerning the need for a proactive targeted intervention to protect mental health more specifically in vulnerable groups.

Abstract

IntroductionThere are few published empirical data on the effects of COVID-19 on mental health, and until now, there is no large international study.Material and methodsDuring the COVID-19 pandemic, an online questionnaire gathered data from 55,589 participants from 40 countries (64.85% females aged 35.80 ± 13.61; 34.05% males aged 34.90±13.29 and 1.10% other aged 31.64±13.15). Distress and probable depression were identified with the use of a previously developed cut-off and algorithm respectively.Statistical analysisDescriptive statistics were calculated. Chi-square tests, multiple forward stepwise linear regression analyses and Factorial Analysis of Variance (ANOVA) tested relations among variables.ResultsProbable depression was detected in 17.80% and distress in 16.71%. A significant percentage reported a deterioration in mental state, family dynamics and everyday lifestyle. Persons with a history of mental disorders had higher rates of current depression (31.82% vs. 13.07%). At least half of participants were accepting (at least to a moderate degree) a non-bizarre conspiracy. The highest Relative Risk (RR) to develop depression was associated with history of Bipolar disorder and self-harm/attempts (RR = 5.88). Suicidality was not increased in persons without a history of any mental disorder. Based on these results a model was developed.ConclusionsThe final model revealed multiple vulnerabilities and an interplay leading from simple anxiety to probable depression and suicidality through distress. This could be of practical utility since many of these factors are modifiable. Future research and interventions should specifically focus on them.

Epidemiological characteristics of TBI at a national level are absent for most LMICs including Georgia. This study aimed to establish the registries and assess causes and outcomes in TBI patients presenting to two major trauma hospitals in the capital city –Tbilisi.Patients and Methods: The prospective observational study was conducted at Acad. O. Gudushauri National Medical Center and M. Iashvili Children’s Central Hospital from March, 1 through August, 31, 2019. Patients of all age groups admitted to one of the study hospitals with a TBI diagnosis were eligible for participation. Collected data were uploaded using the electronic data collection tool –REDCap, analyzed through SPSS software and evaluated to provide detailed information on TBI-related variables and outcomes using descriptive statistics.Results: Overall, 542 hospitalized patients were enrolled during the study period, about 63% were male and the average age was 17.7. The main causes of TBI were falls (58%) and struck by or against an object (22%). The 97% suffered from mild TBI (GCS 13– 15). Over 23% of patients arrived at the hospital more than 1 hour after injury and 25% after more than 4-hours post-injury. Moderate and severe TBI were associated with an increased hospital length of stay. Mortality rate of severe TBI was 54%.Conclusion: This study provides important information on the major epidemiological characteristics of TBI in Georgia, which should be considered for setting priorities for injury management.Keywords: traumatic brain injury, injury epidemiology, registry, Georgia

16. Molecular markers of the progression of conjunctival neoplastic epithelial lesions and its correlation with P16 and HPV expression

Abstract

Epithelial lesions of the conjunctiva include both benign and transitional malignant and malignant lesions., Therefore, it is important to identify molecular markers of malignant progression of these lesions.The aim of our study was to study molecular markers determining the risk of progression of epithelial neoplastic processes of the conjunctiva. The following markers were detected by standard immunohistochemical methods: Ki67, PHH3, Bcl2, P16, HPV16/18, P53, P63 and CK7.A squamous-epithelial index was also derived by evaluating standard hematoxylin-eosin (H&E)-stained fluorescence and the P63/CK7 ratio.The results of the study showed that during the progression of intraepithelial neoplasia of the conjunctiva, the content of squamous epithelium and, accordingly, the squamous-gland index increases significantly.In addition, according to proliferative and apoptotic characteristics, conjunctival intraepithelial lesions can be divided into two groups, namely, low-grade intraepithelial neoplasia and high-grade intraepithelial neoplasia.

21. Distribution of tumor-associated macrophages and M1/M2 polarization in different types and grades of ovarian tumors

Abstract

Ovarian cancer represents one of the major causes of gynaecologic cancer mortality worldwide. Tumor-associated macrophages (TAM) are associated with the development and progression of ovarian cancer. TAMs are mainly represented by two types M1 and M2. We investigated the distribution of total, M1, M2 macrophages, tumor cell/macrophage ratio and M1/M2 ratio in mucinous and serous ovarian tumors. The study results showed that total TAM count is significantly higher in serous ovarian tumors, compared to mucinous tumors and the highest infiltration rate is detected in high-grade serous ovarian carcinomas. In addition, the number of M2 macrophages is significantly increased in higher-grade serous ovarian carcinomas. The evaluation of tumor/cell macrophage ratio could be used as an objective measure of macrophage infiltration in ovarian tumors.

22. Immunohistochemical phenotype of fallopian tubes in patients with different grades of serous ovarian carcinoma

Abstract

During recent years, there is an accumulating evidence that high grade ovarian carcinoma is developed from the fallopian tube epithelial lesions. However, is not yet completely understood and still represents the subject of investigation. We investigated the immunohistochemical phenotype of matched fallopian tubes from the patients with different types and malignancy grades of ovarian carcinoma. Matched fallopian tubes from ovarian cancer patients were available in 260 cases, including mucinous borderline tumor (n=25), mucinous carcinoma (n=15), serous borderline tumor (n=90), low grade serous carcinoma (n=72) and high grade serous carcinoma (n=48). Immunohistochemical investigation included markers of proliferation (Ki67), apoptosis (Bcl2, p53), hormone receptors (ER, PR), epithelial differentiation (CK7), mesenchymal differentiation (vimentin, calretinin) and stem cells (CD44). The results indicate that the presence of fallopian tube carcinoma in situ is significantly correlated with the presence of high grade ovarian serous carcinoma (r=0.44, p

Abstract

The subject we address in the article covers one of the most important periods in the history of Colchis. The ancient period, which, according to the written records and archeological materials, seems to be quite active and varied in Colchis, is attracting more and more interest of scholars every year. Contacts with the ancient world date back to Greek colonization, when Greeks and Colchians coexisted side by side. The Romans began expressing their interest in the Caucasus, including Colchis, from the 1st century BC as the area seemed attractive to them as a protective barrier from the tribes coming from the north, but also for them it was a potential market for trade and economic activities. Like in other countries conquered by the Romans, they began to set up garrisons in Colchis to make it easier for them to fight the piracy that prevailed in Colchis at the time and at the same time to make it easier to control the northern frontier. The fact is that the process, which aimed at the deployment of Roman garrisons and the conquest of new territories, began from the 1st century AD and continued until the 3rd century, until the Romans settled up in the most extreme point in Colchis for them at that time, Bichvinta. The process of “appropriation” of territories may have spread deeper into central Colchis, as well as into other eastern provinces, had it not been for the problems of the Roman Empire that eventually led to its partition. To confirm the written sources we can cite the archaeological discoveries that date from the 19th century to the present day, and every year new discoveries are being recorded, which complement not only the history and archaeology of Colchis, but also the history of Rome and the world in general.

36. Georgia's Illiberal Forces: Political Polarization against Democracy

Abstract

This article explores how illiberal forces are structuring in Georgia and what this evolution reveals about the weakness of Georgia’s democratic institutions and liberal values. The analysis starts with a discussion of how political polarization creates the context for illiberal groups to undermine democracy. Drawing on theoretical and empirical inquiry, I suggest the causal mechanism of how weak democratic institutions enable the emergence of illiberal groups through a personalized party system. Then, I address the lack of genuine democratic commitment on the part of Georgia’s main political actors. Further, I probe for the movements that are gradually changing the grassroots dynamics by promoting conservative values, and their ties to Russia. The concluding section discusses the politicization of gender issues and the polarization of political space as two concomitant trends that frustrate the search for democratic consensus in Georgia.

Abstract

In the digital era, technological advances have brought innovative opportunities. Artificial intelligence is a real instrument to provide automatic routine tasks in different fields (healthcare, education, the justice system, foreign and security policies, etc.). AI is evolving very fast. More precisely, robots as re-programmable multi-purpose devices designed for the handling of materials and tools for the processing of parts or specialized devices utilizing varying programmed movements to complete a variety of tasks.1 Regardless of opportunities, artificial intelligence may pose some risks and challenges for us. Because of the nature of AI ethical and legal questions can be pondered especially in terms of protecting human rights. The power of artificial intelligence means using it more effectively in the process of analyzing big data than a human being. On the one hand, it causes loss of traditional jobs and, on the other hand, it promotes the creation of digital equivalents of workers with automatic routine task capabilities. “Artificial intelligence must serve people, and therefore artificial intelligence must always comply with people’s rights,” said Ursula von der Leyen, President of the European Commission.2 The EU has a clear vision of the development of the legal framework for AI. In the light of the above, the article aims to explore the legal aspects of artificial intelligence based on the European experience. Furthermore, it is essential in the context of Georgia’s European integration. Analyzing legal approaches of the EU will promote an approximation of the Georgian legislation to the EU standards in this field. Also, it will facilitate to define AI’s role in the effective digital transformation of public and private sectors in Georgia.

50. A 6 years trends in hospitalization: retrospective study of Traumatic Brain Injury

Abstract

Introduction Traumatic Brain Injury (TBI) is a significant public health issue globally, and the mortality and morbidity burden is particularly high in low- and middle-income counties (LMICs). The WHO predicts a disproportionately large increase of TBI burden in LMICs. The aim of this study was to identify trends in hospitalization associated with TBI in Georgia from 2015-2020. Methods The study was designed in the framework of the project INITIatE: International Collaboration to Increase TBI Surveillance in Europe, funded by the US National Institutes of Health (NIH/NINDS R21NS098850). The surveillance database of National Center for Disease Control and Public Health of Georgia was used for the study. Results During the study period a total 51 147 patients were admitted in hospitals throughout the country. In 2015-2019 the hospitalization increased and highest number of cases was in 2019 (n = 11779; 23,0%), in 2020 hospitalization decreased in comparison with the previous year (n = 9228; 18,0%). The highest number of burn injuries (n = 22963; 45,0%) occurred in the capital of Georgia (Tbilisi). Among hospitalized patients about 61,0% were males (n = 31162) and 39,0% females (n = 19985), retrospectively with ratio 1,6:1. The modal age of hospitalized patients was 25-44 and the highest hospitalization was in the age group of 15-24. 92,5% of cases were unintentional. The leading cause of Traumatic Brain Injury in all years were falls with some variations (57%-71%), followed by road traffic injuries (12%-25%). The average of LOS was 3 days, the highest LOS was 702 days. 1,6% (n = 805) of patients died. The most common mechanism of fatal injuries were falls. Conclusions The study provide important information about trends in hospitalization, size of the TBI problem, which is crucial for elaborating relevant policy and establishing priorities in order to reduce the burden of Traumatic Brain Injury in Georgia, as well to identify directions for further TBI related research. Key messages • Epidemiological data are essential for designing relevant preventive programs. • Prevention is a key component of public health efforts to reduce TBI burden.
