Abstract

Introduction The population is ageing and two thirds of older people in the UK have multiple long-term health problems. These patients consult with GPs frequently. They live with a burden of illness and treatments, poor quality of life and associated healthcare costs. Ensuring high-quality personalised care for these patients is challenging. Potential benefits of shared decision-making (SDM) have been acknowledged. However, there are few existing interventions designed with the aim of facilitating SDM in this field. Aim The aim of VOLITION is to facilitate the involvement of older people living with multiple long-term conditions in decision-making about their healthcare during UK general practice consultations. Methods An Intervention Mapping (IM) framework was adopted as a means of systematically applying existing literature, new data and relevant theory in the development, refinement and planning for the evaluation of VOLITION. Patient and public involvement was central and expert stakeholder opinion was obtained. A Cochrane review of similar interventions was published, and VOLITION components were subsequently drafted. Focus groups with patients and GPs enabled the refinement of behaviour-change objectives. A mixed-methods exploratory-explanatory study informed plans to implement VOLITION in the context of remote vs. face-to-face consultations. Findings VOLITION consists of i) a prompt to patients, inviting them to express their personal preferences for involvement and ii) training for GPs, delivering a responsive patient-tailored approach to SDM, tackling challenges experienced by GPs when implementing SDM in practice. Discussion This project addresses recommendations from the UK National Institute for Health and Care Excellence, for SDM research targeting patient groups who often believe in the decisional authority of the health care practitioner. VOLITION adheres to NHS England's universal personalised care plan, to empower patients in the management of their healthcare and to train health care practitioners in SDM. Conclusion A full, definitive trial of VOLITION is planned. An effective intervention, designed using end-user perspectives, has the potential to influence high-quality patient-centred care for older people living with MLTC.

Abstract

The attention in the West, especially in the United States, now accorded Islam and those who conduct themselves according to its precepts betrays woeful ignorance of both. As Graham Fuller has persuasively argued in his recent book, "A World Without Islam", Western culture owes much to Islam as well as to Muslims and would be greatly impoverished had Islam never come to be or were it to pass away. The riches lost would derive as much from the religion as from the civilization to which it has given rise. This article considers both, the emphasis being on Islam and its genesis and development as part and parcel of a larger Arabic cultural tradition. (Contains 14 footnotes.)

34. Infants' Attention to Object Structure in Early Categorization.

Abstract

Examined infants' categorization using object manipulation tasks that involved objects that were models of animals, vehicles, or furniture. Objects were normal, had anomalous moving parts (such as a dog with wheels), or had different textures. Found that 14- to 22-month olds attended to the parts and structural configuration of objects, but not to their texture. (BC)

35. Infants' Use of Object Parts in Early Categorization.

Abstract

Two experiments used object-manipulation tasks to examine whether one- to two-year-olds form superordinate-like categories by attending to object parts. Findings indicated that 14- and 18-month-olds behaved systematically toward categories with different, but not matching, parts. Without part differences, none formed superordinate categories. Younger infants categorized by parts rather than by category membership, suggesting a perceptual basis for categorization. (Author/KB)

Abstract

This 1945 National Education Association yearbook focuses attention on the post-World War II problems of rural schools and encourages national, state, and local leaders to lay plans for strengthening rural education programs. The articles examine social and economic problems faced by rural Americans, the implications of these problems for rural schools, and some practical steps for improvement. Part 1 of the document presents goals for rural America, concerning rural work, living standards and services, and rural-urban relationships. Part 2 presents basic economic and social data, identifies major problems affecting rural education, and discusses establishment of standards for rural education and other services. Considering farm size, efficiency, and crop types as factors, agriculture can be made to earn a living for rural families. Education's role lies in placing the right farmer on the right farm. Other factors affecting rural education include rural incomes, taxation, and population trends. Part 3 discusses specific problems faced by rural schools, including curriculum, teacher recruitment, funding, organization, and administration. The goals of rural education involve guidance programs, health care, and vocational education. Standards of teacher competency should be understood as part of rural efforts to attract and hold good teachers. School administration needs should be assessed in the context of community size, school attendance, and system organization. Rural education also implies certain needs for student transportation, buildings and equipment, and community education activities. Federal and state funding is important as a means of equalizing educational opportunities for all rural students. (TES)

Abstract

It is of interest from time to time to make studies or have studies made of the various ranks of the superintendency. One type of the superintendency, which is all in all the oldest and is found in every State, is the county superintendent of schools. He exists in more than 3,300 places. The return accompanying this blank was able to get a line on him from more than 2,000 places. It showed his age and marital status, educational experience, and the grades in which he had previously taught, the degrees held, and the like. In October, 1928, the Office of Education sent out to each county school superintendent or comparable officer in the United States an inquiry blank seeking certain information. This list included, besides the officers specifically designated as "county superintendent," the "division superintendent" of Virginia (outside those in charge of city systems), the "district superintendent" of New York, the "deputy superintendent" of Nevada, and the "union superintendent" of the New England States. This bulletin contains the following: (1) Letter of transmittal; (2) The adequacy of the data; (3) Sex, age, and marital condition; (4) The professional equipment of the county superintendent; (5) In-service growth of county superintendents; (6) Salaries; (7) Some aspects of the working conditions of county superintendents; and (8) Some implications of foregoing data.
