1. The Impact of the Mobile Money on People’s Use of Financial Services in Sub-Sahara Africa

Abstract

This paper details the impact of mobile money financial technology on financial inclusion in Sub-Saharan Africa. The research methodology used was descriptive research design with the use of secondary information sources to collect panel data. The research model had Financial Technology as the independent variable and the dependent variable was Financial Inclusion. The analysis of data was done using Stata 15. From the interpretation of the findings using the panel data on the 14 selected Sub-Saharan countries, it was found that the increase in the number of registered mobile money agents per 100,000 adults led to an increase in the total number of people using the financial services per 100,000 adults in Sub-Sahara Africa. Furthermore, the relationship between the registered mobile money agents per 100,000 adults and the total number of people using financial services was proven to be significant at a 5% level of significance.

Abstract

Abstract Intrathecal delivery of autologous culture-expanded adipose tissue-derived mesenchymal stem cells (AD-MSC) could be utilized to treat traumatic spinal cord injury (SCI). This Phase I trial (ClinicalTrials.gov: NCT03308565) included 10 patients with American Spinal Injury Association Impairment Scale (AIS) grade A or B at the time of injury. The study’s primary outcome was the safety profile, as captured by the nature and frequency of adverse events. Secondary outcomes included changes in sensory and motor scores, imaging, cerebrospinal fluid markers, and somatosensory evoked potentials. The manufacturing and delivery of the regimen were successful for all patients. The most commonly reported adverse events were headache and musculoskeletal pain, observed in 8 patients. No serious AEs were observed. At final follow-up, seven patients demonstrated improvement in AIS grade from the time of injection. In conclusion, the study met the primary endpoint, demonstrating that AD-MSC harvesting and administration were well-tolerated in patients with traumatic SCI.

Abstract

Abstract Introduction Effective pain and anxiety management during the perioperative phase remains a challenge for patients undergoing surgeries and other invasive procedures. The current standard of care involves prescribing analgesics to treat these conditions; however, there has been recent interest in applying multimodal strategies that limit the use of these medications. One such modality is meditation, which has been shown to be effective in alleviating various physical and psychological symptoms in other settings. This systematic review aims to assess how current meditative practices affect perioperative pain and anxiety. Methods We conducted a systematic review of randomized controlled trials following the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses guidelines. A comprehensive literature search was conducted using PubMed MEDLINE, Embase, PsycINFO, APA PsycINFO, EBM Reviews, Scopus, and Web of Science for all available dates. Our primary outcomes of interest were patient‐reported pain and anxiety scores using the Visual Analog Scale, the Brief Pain Inventory, the Depression Anxiety Stress Scale, the State‐Trait Anxiety Inventory (STAI), and the Hospital Anxiety and Depression Scale (HADS). For the HADS and STAI scales, only the anxiety and anxiety‐state subgroups were reported, respectively. Results The literature search yielded 1746 articles. A total of 286 full‐text articles were screened, and 16 studies were included in this systematic review. A total of eight studies assessed pain scores after invasive procedures; five reported improvements in pain scores, and three reported no change after meditative practices. Ten studies assessed anxiety outcomes after invasive procedures: nine reported a decrease in overall anxiety levels as a result of meditation practices while one study reported no change in anxiety scores. Conclusion Data from this limited literature suggests that different meditation practices could be effective in alleviating pain and anxiety within the perioperative phase for patients undergoing various types of invasive procedures. Future prospective studies are needed to determine whether routine meditation in the perioperative setting is effective in mitigating perioperative pain and anxiety.

Objectives To investigate the impact of the COVID-19 pandemic as well as concomitant COVID-19 itself on stroke care, focusing on middle cerebral artery (MCA) territory infarctions.Design Registry-based study.Setting We used the National Inpatient Sample (NIS) database, which covers a wide range of hospitals within the USA.Participants The NIS was queried for patients with MCA strokes between 2016 and 2020. Abstract

Although hypothermia has shown to protect against ischemic and traumatic neuronal death, its potential role in neurologic recovery following traumatic spinal cord injury (TSCI) remains incompletely understood. Herein, we systematically review the safety and efficacy of hypothermia therapy for TSCI. The English medical literature was reviewed using PRISMA guidelines to identify preclinical and clinical studies examining the safety and efficacy of hypothermia following TSCI. Fifty-seven articles met full-text review criteria, of which twenty-eight were included. The main outcomes of interest were neurological recovery and postoperative complications. Among the 24 preclinical studies, both systemic and local hypothermia significantly improved neurologic recovery. In aggregate, the 4 clinical studies enrolled 60 patients for treatment, with 35 receiving systemic hypothermia and 25 local hypothermia. The most frequent complications were respiratory in nature. No patients suffered neurologic deterioration because of hypothermia treatment. Rates of American Spinal Injury Association (AIS) grade conversion after systemic hypothermia (35.5%) were higher when compared to multiple SCI database control studies (26.1%). However, no statistical conclusions could be drawn regarding the efficacy of hypothermia in humans. These limited clinical trials show promise and suggest therapeutic hypothermia to be safe in TSCI patients, though its effect on neurological recovery remains unclear. The preclinical literature supports the efficacy of hypothermia after TSCI. Further clinical trials are warranted to conclusively determine the effects of hypothermia on neurological recovery as well as the ideal means of administration necessary for achieving efficacy in TSCI.

Abstract

Most COVID-19 predictive modeling efforts use statistical or mathematical models to predict national- and state-level COVID-19 cases or deaths in the future. These approaches assume parameters such as reproduction time, test positivity rate, hospitalization rate, and social intervention effectiveness (masking, distancing, and mobility) are constant. However, the one certainty with the COVID-19 pandemic is that these parameters change over time, as well as vary across counties and states. In fact, the rate of spread over region, hospitalization rate, hospital length of stay and mortality rate, the proportion of the population that is susceptible, test positivity rate, and social behaviors can all change significantly over time. Thus, the quantification of uncertainty becomes critical in making meaningful and accurate forecasts of the future. Bayesian approaches are a natural way to fully represent this uncertainty in mathematical models and have become particularly popular in physics and engineering models. The explicit integration time varying parameters and uncertainty quantification into a hierarchical Bayesian forecast model differentiates the Mayo COVID-19 model from other forecasting models. By accounting for all sources of uncertainty in both parameter estimation as well as future trends with a Bayesian approach, the Mayo COVID-19 model accurately forecasts future cases and hospitalizations, as well as the degree of uncertainty. This approach has been remarkably accurate and a linchpin in Mayo Clinic's response to managing the COVID-19 pandemic. The model accurately predicted timing and extent of the summer and fall surges at Mayo Clinic sites, allowing hospital leadership to manage resources effectively to provide a successful pandemic response. This model has also proven to be very useful to the state of Minnesota to help guide difficult policy decisions.

Abstract

Background: Neurosurgeons, particularly spine surgeons, have high exposure to vibrations via electric or pneumatic drills and repetitive motion. Although no data exist for the prevalence of carpal tunnel syndrome (CTS) among these surgeons, anecdotal evidence suggests the rate of symptoms is higher than in the general population. Methods: An anonymous questionnaire was developed to assess demographics, practice patterns, presence of CTS symptoms, and treatment (time off, bracing, medication, injections, surgery). The survey was sent via anonymous email link to members of the AANS/CNS Joint Section on Disorders of the Spine and Peripheral Nerve. Results: 101 members responded: 44 reported at least one symptom related to CTS (43.6%). There was no statistically significant relationship between overall or spine case volume, the number of cases performed annually/daily, and CTS symptoms. Respondents working in non-teaching settings were significantly more likely to have CTS symptoms than academic teaching institutions (50.0% v. 45.0%; p = 0.0112). Conclusions: Our survey demonstrated CTS to be more prevalent in spine neurosurgeons (43.6%) than in the general population (1–5%). The lack of significant association between most practice-based metrics and CTS symptoms may indicate that respondents have a minimum case volume that exceeds the amount of vibration exposure/repetitive motion to develop symptoms. The significantly increased prevalence of CTS among neurosurgeons at non-teaching institutions suggests that residents provide operative assistance offsetting the vibration exposure/repetitive motion by attendings. Further research may determine the root cause for the high prevalence of CTS in spine neurosurgeons and devise methods for reducing vibration exposure.
