7. Schools for an Information Age. Reconstructing Foundations for Learning and Teaching.

Abstract

Prospective and experienced educators are invited to envision new schools for the emerging information age by this book, which rests on the belief that information technologies will generate new ways for people to make sense of their lives. It is argued that the narrative that made sense of industrial progress has been broken, and that it is no longer unquestionable that academic achievement will bring higher income and a satisfying life. Schools may change their missions and may begin to teach for cooperation rather than for competition to reflect new social goals. The first chapter introduces socially constructed meaning systems and considers the views of social scientists and educators about current prospects for education. Chapter 2 summaries alternative accounts of industrial-era schooling, and Chapter 3 examines the dysfunctional tensions evident in teachers' roles and school structures. Chapters 4 through 6 present contemporary accounts of school dilemmas in the social structure. The final four chapters advance possibilities for participatory learning in light of current educational structures and the changes technology will bring. Abstract

The problem of mixed convection from horizontal surfaces in a porous medium saturated with a power-law-type non-Newtonian fluid is investigated. The transformed conservation laws are solved numerically for the case of variable was heat flux conditions. Results for the details of the velocity and temperature fields as well as the Nusselt number have been presented. The viscosity index ranged from 0.5-1.5.

Abstract

Congenital heart block (CHB) is a rare disorder that may be associated with a high morbidity and even mortality, with a risk of death both in utero and during infancy. Women with serum titres of anti-Ro and/or anti-La antibodies carry a risk of CHB of 1–5% in their offspring, with a recurrence risk of approximately 20%. We present a case of a 36-year-old female with a pregnancy complicated by congenital heart block. Autoimmune profiling at booking showed she was positive for lupus anticoagulant and anti-Ro antibodies. A fetal echocardiogram at 21 + 3 showed complete heart block. She was monitored throughout the remainder of her pregnancy with serial growth scans, cardiovascular profiling, and BPP scoring. She had a normal vaginal delivery at term to a female infant.

Abstract

Building organizational linkages between schools and other organizations requires many levels of support, trust, volunteered efforts, and complex understandings of social realities. Drawing on documented accounts and the authors' experiences, this volume presents definitions of interactive partnerships, barriers to school improvement and the culture of schools, concepts for understanding interorganizational developments, modal developmental stages of a partnership, interests and purposes of outside organizations, cooperation and conflict within collaborative endeavors, teacher development and empowerment through interactions with others, interorganizational strategies and structures, leadership styles and assessments for collaborations, and school improvements and future societies. Part 1, "A School Improvement Perspective," argues that partnerships between schools and other organizations have empowered teachers and improved schools. Part 2, "Studies of Partnerships," presents a detailed study of a school-university project and scenarios of school interactions with parent/community groups, businesses, and human service agencies. Part 3, "Evolving Understandings," is devoted to common dynamics and processes found in partnerships from the distinct standpoints of teachers, organizations, leaders, and the larger society. Over time, interactive processes between schools and outside organizations foster evolving understandings of education and better futures. An index is included. (163 references) (MLH)

Abstract

BACKGROUND The potential benefit of dual antiplatelet therapy beyond 1 year after a myocardial infarction has not been established. We investigated the efficacy and safety of ticagrelor, a P2Y(12) receptor antagonist with established efficacy after an acute coronary syndrome, in this context. METHODS We randomly assigned, in a double-blind 1: 1: 1 fashion, 21,162 patients who had had a myocardial infarction 1 to 3 years earlier to ticagrelor at a dose of 90 mg twice daily, ticagrelor at a dose of 60 mg twice daily, or placebo. All the patients were to receive low-dose aspirin and were followed for a median of 33 months. The primary efficacy end point was the composite of cardiovascular death, myocardial infarction, or stroke. The primary safety end point was Thrombolysis in Myocardial Infarction (TIMI) major bleeding. RESULTS The two ticagrelor doses each reduced, as compared with placebo, the rate of the primary efficacy end point, with Kaplan-Meier rates at 3 years of 7.85% in the group that received 90 mg of ticagrelor twice daily, 7.77% in the group that received 60 mg of ticagrelor twice daily, and 9.04% in the placebo group (hazard ratio for 90 mg of ticagrelor vs. placebo, 0.85; 95% confidence interval [CI], 0.75 to 0.96; P = 0.008; hazard ratio for 60 mg of ticagrelor vs. placebo, 0.84; 95% CI, 0.74 to 0.95; P = 0.004). Rates of TIMI major bleeding were higher with ticagrelor (2.60% with 90 mg and 2.30% with 60 mg) than with placebo (1.06%) (P < 0.001 for each dose vs. placebo); the rates of intracranial hemorrhage or fatal bleeding in the three groups were 0.63%, 0.71%, and 0.60%, respectively. CONCLUSIONS In patients with a myocardial infarction more than 1 year previously, treatment with ticagrelor significantly reduced the risk of cardiovascular death, myocardial infarction, or stroke and increased the risk of major bleeding. Abstract

Background: Cardiovascular (CV) disease is the leading cause of morbidity and mortality in patients with type 2 diabetes mellitus (T2DM). Furthermore, patients with T2DM and acute coronary syndrome (ACS) have a particularly high risk of CV events. The glucagonlike peptide 1 receptor agonist, lixisenatide, improves glycemia, but its effects on CV events have not been thoroughly evaluated. Methods: ELIXA (www.clinicaltrials.gov no. NCT01147250) is a randomized, double-blind, placebo-controlled, parallelgroup, multicenter study of lixisenatide in patients with T2DM and a recent ACS event. The primary aim is to evaluate the effects of lixisenatide on CV morbidity and mortality in a population at high CV risk. The primary efficacy end point is a composite of time to CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for unstable angina. Data are systematically collected for safety outcomes, including hypoglycemia, pancreatitis, and malignancy. Results: Enrollment began in July 2010 and ended in August 2013; 6,068 patients from 49 countries were randomized. Of these, 69% are men and 75% are white; at baseline, the mean ± SD age was 60.3 ± 9.7 years, body mass index was 30.2 ± 5.7 kg/m2, and duration of T2DM was 9.3±8.2 years. The qualifying ACS wasamyocardial infarctionin83% and unstableangina in 17%. The study will continue until the positive adjudication of the protocol-specified number of primary CV events. Conclusion: ELIXA will be the first trial to report the safety and efficacy of a glucagon-like peptide 1 receptor agonist in people with T2DM and high CV event risk. © 2015 Elsevier Inc. Abstract

SUMMARY: Introduction: Sickle cell disease in pregnancy carries a high risk of maternal and fetal adverse outcomes. The use of prophylactic transfusions to reduce the risk of sickle complications is controversial. Current UK standards do not recommend the routine use of transfusion for sickle pregnancy. We examined transfusion episodes during sickle pregnancies in a single centre over an 11‐year period. Methods: We conducted a retrospective observational study of all pregnancies in patients with sickle cell disease who attended the joint obstetric/haematology clinic over an 11‐year period. All pregnancies were managed according to a local protocol, which did not recommend routine transfusion. Results: A total of 38 pregnancies (HbSS 22, HbSC 13, Hb S/beta thalassaemia 3) were included, with a mean age at booking of 29 years. A total of 61% of pregnancies required on‐demand or emergency transfusion during the course of pregnancy or post‐partum. Women requiring a transfusion during pregnancy had a higher mean number of hospital admissions in the previous year (1·11 vs 0·15, P = 0·057), a significantly lower mean steady‐state haemoglobin (85·0 vs 99·6 g L−1, P = 0·003) and a significantly lower mean haemoglobin at the pregnancy booking visit. (86·1 vs 99·5 g L−1, P = 0·02). Conclusion: In sickle pregnancies assigned to standard management in a single centre, a high proportion of women required on‐demand transfusion. Possible pre‐pregnancy factors predictive of a need for transfusion include lower baseline haemoglobin and number of hospital admissions in the previous 12 months. [ABSTRACT FROM AUTHOR]
