2. The evaluation of the effects of the Mitotic Arrest Deficiency Protein 2 (MAD2) and the miR-433 microRNA on chemoresistance and cancer prognosis

Abstract

Chemoresistance is a major obstacle in the treatment of high-grade serous ovarian cancer (HGSOC) hence identification of molecular markers of chemoresistance is of crucial importance. The primary focus of this thesis is to investigate the prognostic potential of the miR-433 microRNA target, mitotic arrest deficiency 2 protein (MAD2). In order to do this, MAD2 expression was evaluated in three HGSOC cohorts and was found not to correlate with survival. An association between MAD2 expression in neoadjuvant treated HGSOC patients and survival was evident. MAD2 levels therefore may be of prognostic value in this subgroup of patients. We previously described miR-433 as a critical cell cycle regulator and mediator of cellular senescence. In an attempt to determine whether miR-433 infers a survival advantage under chemotherapeutic selective pressures, a miR-433 overexpressing cell line was developed. Here, we have shown that higher miR-433 expression was associated with an increased resistance to platinum, which was unrelated to loss of MAD2 expression. Moreover, cells which remained viable following exposure to high doses of chemotherapy expressed higher levels of miR-433. Our data suggests that chemotherapy may not be driving the transcriptional up-regulation of miR-433 but rather selecting a population of cells with high miR-433 expression that escape chemotherapeutic kill. IHC analysis by our group of the miR-433 target, histone deacetylase 6 (HDAC6), previously, confirmed that its expression was significantly associated with a decrease in overall survival in HGSOC patients. In this thesis, the inhibition of HDAC6 in HGSOC cells did not attenuate apoptotic responses to paclitaxel. Therefore we suggest that HDAC6 inhibitors should not be recommended in conjuction with paclitaxel for HGSOC treatment. In conclusion, miR-433 and its target proteins play an important role in chemoresistance in HGSOC and could be measured or manipulated for potential therapeutic benefit.

9. K-12 Education Nonprofit Employees' Perceptions of Strategies for Recruiting and Retaining Employees

Abstract

This qualitative study explored the key reasons individuals who work in K-12 education nonprofit organizations enter the field of K-12 nonprofit education and their motivations for doing so. The purpose of this study was to find new strategies for recruiting and retaining K-12 education nonprofit employees by examining the obstacles that exist to entering this field and strategies for overcoming these obstacles through the perspective of the K-12 education nonprofit employee. The data were collected through online questionnaires completed by 46 K-12 education nonprofit employees and in-depth interviews with 9 K-12 education nonprofit employees. During the interview, participants were also asked to draw or verbally walk through a timeline of significant events that led to their current position. The findings revealed that participants were educated from a variety of educational backgrounds. The main ways participants discovered their passion for the field were through volunteering or community service, by taking a job in the field and then discovering a passion for the field, and by being a participant at a K-12 education nonprofit as a youth. The key motivations for being in the K-12 education nonprofit field were doing meaningful work, having a passion for the cause, and the ability to serve others. The main obstacles for entering the field were low pay, fewer job opportunities, and long work hours. The first strategy for overcoming obstacles in the field was for K-12 education nonprofits to increase public exposure and community awareness of their organizations The second strategy was to better emphasize the rewards and sense of fulfillment the sector brings. The third strategy was to create clearer career paths for K-12 education nonprofit employees, most specifically at the college level. Recommendations are that (a) Educational institutions offer more undergraduate and graduate nonprofit degrees and courses to students; (b) K-12 education nonprofits invest more time and energy in recruiting new volunteers and employees externally by connecting more with the community; and (c) K-12 education nonprofits create a culture that values and promotes self-care, a supportive environment, flexibility, and training opportunities in order to better offset obstacles to entering the field. Abstract

Background: Heroin smokers have high rates of COPD, respiratory morbidity, hospital admission, and mortality. We assessed the natural history of symptoms and lung function in this population over time. Methods: A cohort of heroin smokers with COPD was followed for 18 to 24 months. At baseline and follow-up, respiratory symptoms were measured by the Medical Research Council Dyspnea Scale (MRC) and the COPD Assessment Tool (CAT), and postbronchodilator spirometry was performed. Frequency of health-care-seeking episodes was extracted from routine health records. Parametric, nonparametric, and linear regression models were used to analyze the change in symptoms and lung function over time. Results: Of 372 participants originally recruited, 161 were assessed at follow-up (mean age, 51.0 ± 5.3 years; 74 women [46%]) and 106 participants completed postbronchodilator spirometry. All participants were current or previous heroin smokers, and 122 (75.8%) had smoked crack. Symptoms increased over time (MRC score increased by 0.48 points per year, P <.001; CAT score increased by 1.60 points per year, P <.001). FEV 1 declined annually by 90 ± 190 mL (P <.001). This deterioration was not associated with change in tobacco or heroin smoking status or use of inhaled medications. Conclusions: Heroin smokers experience a high and increasing burden of chronic respiratory symptoms and a decline in FEV 1 that exceeds the normal age-related decline observed among tobacco smokers with COPD and healthy nonsmokers. Targeted COPD diagnostic and treatment services hosted within opiate substitution services could benefit this vulnerable, relatively inaccessible, and underserved group of people.
