INTRODUCTION: The authors aimed to evaluate the outcomes of free flap reconstructions performed for intraoral region in a single center.METHODS: Eleven free flaps have been performed for defects of the head and neck over a 4-year period. The patients were evaluated in terms of defects, flaps performed, complications and hospital stays.RESULTS: Seven patients were reconstructed with radial forearm free flap, one patient was reconstructed with free transverse rectus abdominis musculocutaneous flap, two patients were reconstructed with free vertical rectus abdominis musculocutaneous flap and the remaining one was reconstructed with free fibula osteomusculocutaneos flap. None of the flaps required reexploration for anastomotic thrombosis or hematoma. No flap failure occurred. We did not encounter any mortality or major morbidity. The average hospital stay was 21.1 days with a range from 11 to 25 days. DISCUSSION AND CONCLUSION: Multidisciplinary approach should be encouraged for the reconstruction of intraoral defects. The versatility of free tissue transfers allows reconstruction of most complex defects; although it carries significant risks, including longer operative times, donor site morbidity, recipient site complications and microsurgical failure, and longer hospital stays., GİRİŞ ve AMAÇ: Yazarlar, tek bir merkezde intraoral bölge için gerçekleştirilen serbest flep rekonstrüksiyonlarını değerlendirmeyi amaçlamışlardır.YÖNTEM ve GEREÇLER: Baş boyun bölgesindeki defektlerin rekonstrüksiyonu için 4 yıllık süre içinde onbir tane serbest doku nakli gerçekleştirildi. Hastalar, defekt lokalizasyonu, uygulanan flepler, komplikasyonlar ve hastanede yatış süreleri açısından değerlendirildi.BULGULAR: Yedi hasta serbest radial önkol flebi ile, iki hasta serbest vertikal rektus abdominis muskulokütan flebi ile, bir hasta serbest transvers rektus abdominis muskulokütan flebi ile bir hasta da serbest fibula osteomuskulokütan flebi ile rekonstrükte edildi. Fleplerin hiçbirinde anastomozda tromboz veya hematom nedeniyle reeksplorasyon yapılmadı. Flep kaybı görülmedi. Mortalite veya major morbidite ile karşılaşılmadı. Ortalama hastanede yatış süresi 21.1 gündü (11-25 gün). TARTIŞMA ve SONUÇ: Ağız içi defektlerinin rekonstrüksiyonu için multidisipliner yaklaşım desteklenmelidir. Uzun ameliyat süresi, donör alan morbiditesi ve mikrocerrahi başarısızlığı gibi riskleri olmasına rağmen serbest doku nakilleri birçok kompleks defektin onarımına olanak sağlamaktadır.

Amaç: Çalışmamızda glottik kanser nedeniyle vertikal parsiyel larenjektomi (VPL) ve suprakrikoid parsiyel larenjektomi (SCPL) uygulanan hastalar retrospektif olarak incelenmiş ve bu iki cerrahi yöntemin onkolojik sonuçları araştırılmıştır. Gereç ve Yöntem: Kulak Burun Boğaz kliniğinde 2001- 2008 yılları arasında larenks kanseri tanısıyla primer olarak transservikal yöntemle VPL veya SCPL uygulanan 40 hasta incelemeye alındı. Bulgular: Aspirasyon VPL yapılan hastaların %10’unda, SCPL yapılan hastaların %30’unda gözlendi. VPL ve SCPL’nin lokal kontrol oranları %80 ve %85 olarak saptandı. Larenks korunma oranı VPL ile %80, SCPL ile %90 olarak saptandı. Üç ve 5 yıllık hastalıksız sağkalım oranları VPL ve SCPL için sırası ile %79- %74 ve %85- %75 olarak tesbit edildi. Sonuç: Gerek SCPL gerekse VPL glottik tümörlerin tedavisinde kullanılan, düşük rekürrens ve yüksek larenks korunma oranlarına sahip, güvenilir cerrahi yöntemlerdir. VPL ile kıyaslandığında SCPL’nin komplikasyon oranı yüksektir. Objective: We retrospectively analyzed the patients who underwent vertical partial laryngectomy (VPL) and supracricoid laryngectomy (SCPL) for glottic cancer and the oncologic results of these surgical techniques. Material and Methods: Forty patients who underwent VPL and SCPL for glottic cancer at Otolaryngology Clinic between 2001-2008 were included in the study. Results: Aspiration occurred in 10 %of the VPL patients and 30 %of the SCPL patients. Local control rates of VPL and SCPL were 80 %and 85 %respectively. Larynx preservation rate was 80 %for VPL and 90 %for SCPL. Three and 5-year disease free survival rates of VPL and SCPL were 79 %- 74 %and 85 %- 75 %respectively. Conclusion: Both VPL and SCPL are reliable surgical techniques that have low recurrence and high larynx preservation rates for the treatment of glottic cancer. When compared with VPL, SCPL has increased complication rates.

Bu çalışmanın amacı apex partis petrosa lezyonlarını drenaj için kullanılan transmastoid infralabirentin yolun cerrahi anatomisini ortaya koymaktır. Diseksiyonlar sırasında gözlenen ilişkiler radyolojik olarak da değerlendirilmiştir.Otuzyedi temporal kemikte (30 kadavra tarafı ve 7 kuru kemik) mastoidektomi yapılıp orta kulak açılarak infralabirentin, subkohlear ve apex partis petrosa hücreleri açılarak transmastoid infralabirentin yol tüm temporal kemik boyunca ortaya konmuştur. Mastoid bölgeden cochlea?nın konumu tarif edilmiştir. Buna göre cochlea?nın en inferior kesimi canalis semicircularis posterior?un en inferior kesimi ile aynı aksiyel plandadır ve n. facialis?in mastoid segmentinin proksimal kesiminin 8,43 mm derininde yer alır. İnfralabirentin yolun a. carotis interna?nın horizontal parçasına paralel oluşturulabilmesi için yolun cochlea?nın inferior ve medialinden, meatus acusticus internus?un anterior ve inferior?undan geçmesi gerekir. Yolun koronal ve aksiyel plan ile olan açısı 420 ve 300 olarak ölçüldü. A. carotis interna?nın vertikal ve horizontal kesimleri arasında oluşan açının promontoryuma göre 7 mm?den fazla posterior?da yerleşmesi durumunda yol ile a. carotis interna?nın yakın temasının oluştuğu gözlendi. Sonuçlarımız esas olarak orta kulak yerleşimli ve cochlea ile yakın ilişkide olan bulbus jugularis varlığında işlemin uygulanamadığını gösterdi. Bulbus jugularis?in esas olarak mastoid bölgede yerleştiği ve n. facialis?e temas etmediği vakalarda ise bu yöntem uygulanabilmektedir.Anahtar Sözcükler: Apex partis petrosa, anatomi, drenaj, infralabirentin yaklaşım, kolesterol granulom The aim of the study is to demonstrate the surgical anatomy of transmastoid infralabyrinthine approach which is used in drainage of the lesions of apex partis petrosa. The relationships observed during dissections were also collaborated with the radiological findings. Mastoidectomy and middle ear dissection were performed in 37 temporal bones (30 cadaver sides and 7 bones) and by opening the infralabyrinthine, subcochlear and apex partis petrosa air cells, the whole tract of transmastoid infralabyrinthine approach was demonstrated. The topography of cochlea was described from the mastoid view. According to our results, in surgical position, the most inferior part of cochlea was at the same axial plane with the most inferior part of canalis semicircularis posterior and located 8,43 mm deep to proximal part of the mastoid segment of n. facialis. In order to perform a tract parallel to horizontal part of a. carotis interna, the tract should pass inferior and medial to cochlea and anterior and inferior to meatus acusticus internus. The mean coronal and axial angle of the route was 420 and 300 respectively. The tract was seen in close proximity with a. carotis interna if the angle between vertical and horizontal segments of a. carotis interna located more than 7 mm posterior to promontoryum. Our results demonstrated that the procedure could not be performed if bulbus jugularis mainly located in the middle ear and in close relation with cochlea. Kırıkkale Üniversitesi Tıp Fakültesi Kulak Burun Boğaz ve Baş Boyu Cerrahisi Anabilim Dalı'nda 01.01.2016 – 01.04.2018 tarihleri arasında baş dönmesi şikayeti ile kliniğe başvuran çocuk hastaların dosyaları incelenerek seröz otitis media (SOM) saptanan ve seröz otit saptanmayan hastalar belirlenerek bu iki hasta grubunun video head impulse testi (vHIT) sonuçlarının değerlendirilmesi amaçlandı.Bu amaçla belirttiğimiz tarih aralığında hastaneye başvuran ve 30 adet SOM saptanan ve 30 adet SOM saptanmayan hastaların vestibülooküler refleks (VOR) kazanç ilişkileri vHIT ile değerlendirildi. SOM saptanan hastalar kulak zarı muayene bulgularına göre de kendi içinde değerlendirildi.SOM saptanan 30 hastanın 15'i nin kız (%50) ve 15 'inin erkek (%50), SOM saptanmayan 30 hastanın 15'inin kız (%50) ve 15'inin erkek (%50) olduğu belirlendi. Çalışma grubunun yaş ortalaması 8.26, kontrol grubunun yaş ortalaması 8.2 olarak belirlendi. Çalışma grubu olarak kabul edilen hastaların 24 tanesinin kulak zarı mat olarak izlenirken, 6 tanesinin kulak zarı muayenesi retrakte olarak izlendi. Grup 1 hastaların 12 tanesinde sol posterior kanallarda covert sakkat saptandı, 6 tanesinde sağ anterior kanallarda ve 4 tanesinde sağ horizontal kanallarda sakkat saptandı. Sol posterior kanal VOR kazanç ortalamalarında istatistiksel olarak anlamlı azalma izlendi. Sağ anterior ve sağ horizontal kanal VOR kazanç ortalamalarında istatistiksel olarak anlamlı azalma izlendi.Çalışmaya dahil edilen hastalarımızın yaş grubuna göre bakıldığında en sık baş dönmesi nedenleri olarak Vestibüler Migren ön plana çıkmaktadır. Benign paroksismal pozisyonel vertigo ve ataksi, postravmatik vertigo ve anksiyete bozukluğuna bağlı vertigo da diğer nedenler olarak karşımıza çıkmaktadır. Çocuklarda vertigo her yaş grubunu etkilemekte, farklı yaş gruplarına göre farklı şikâyetlere yol açmaktadır. Ebeveynlerden alınan anamnez de tanı koymada oldukça önemli yer tutmaktadır. Bizim çalışmamıza göre SOM' un vestibüler sistemi kısmen etkilediği fakat bu etkinin, vestibüler sistemin bütün kanallarında görülemediği gözlemlendi. Sonuçları istatistiksel olarak anlamlı düşünebilmek için daha fazla sayıda hasta üzerinde daha fazla çalışma gerektiğini düşünmekteyiz. Bu şekilde vHIT seröz otitli çocuklarda vestibüler sistemi değerlendirmek için önemli bir tanı aracı olarak kullanılabilir.Anahtar kelimeler: Çocuklar, seröz otitis media, vertigo, vestibülookuler refleks, vHIT, The aim of the study is to investigate the effect of serous otitis media on vestibular system in children complaining of vertigo by evaluation of files of the the patients with serous otitis media and evaluation of files of the the patients without serous otitis media in Kırıkkale University Mediacl Faculty Otorhinolaringology Department between dates 01.01.2016-01.04.2018 and evaluation and matching of video head impuls test results of the groups.For this goal, 60 patients applied to our clinic in these dates are evaluated. The diagnosis of 30 patients' was serous otitis media (Group 1) and 30 patients had different diagnoses other than serous otitis media (Group 2). We evaluated vestibuloocular reflex gain results of each groups by video head impulse test (vHIT). Group with serous otitis media also evaluated according to ear drum examination findings.Group 1 is consisted of 15 girls (%50) and 15 boy patients (%50). Group 2 is consisted of 15 girls (%50) and 15 boy patients (%50). Mean age of Group 1 was 8.26, mean age of Group 2 was 8.2. İn study group patients 24 patients ear draum examination was opaque and 6 patients ear drum examination was retracted. In Group 1, covert saccades is discovered in 12 of left posterior semisircular canals of 30 patients, 6 covert saccades is discovered in right anterior canals and 4 covert saccades was discovered in right horizontal canals. A statistifically signicant decraese was determined in left posterior canal vestibuloocular reflex gain results. Also statistifically signicant decraese was determined in right anterior and right horizontal canal vestibuloocular reflex gain results.When we look to the age groups of the our children, most common reason for vertigo complaints was vestibular migraine. Other diagnoses were, benign paroksysmal positionel vertigo, ataxia, postraumatic vertigo and anxiety disorders. Vertigo effects every age groups in children and causes different complaints according to different ages. Anamnesis from parents is very important in differential diagnosis. According to our study, serous otitis media effect vestibular system partially but does not effect all the canals. In order to find statistically significant results, new researches with much more hasta groups should be made. With new researches, vHIT can be used as a valuable diagnostic device in vestibular system in children with serous otitis media.Key words: Children, serous otitis media, vertigo, vestibuloocular reflex, vHIT 107
