Abstract

Blastocystis is the most common gastrointestinal protist found in humans and animals. Although the clinical significance of Blastocystis remains unclear, the organism is increasingly being viewed as a commensal member of the gut microbiome. However, its impact on the microbiome is still being debated. It is unclear whether Blastocystis promotes a healthy gut and microbiome directly or whether it is more likely to colonize and persist in a healthy gut environment. In healthy people, Blastocystis is frequently associated with increased bacterial diversity and significant differences in the gut microbiome. Based on current knowledge, it is not possible to determine whether differences in the gut microbiome are the cause or result of Blastocystis colonization. Although it is possible that some aspects of this eukaryote’s role in the intestinal microbiome remain unknown and that its effects vary, possibly due to subtype and intra-subtype variations and immune modulation, more research is needed to characterize these mechanisms in greater detail. This review covers recent findings on the effects of Blastocystis in the gut microbiome and immune modulation, its impact on the microbiome in autoimmune diseases, whether Blastocystis has a role like bacteria in the gut–brain axis, and its relationship with probiotics.

3. Candida albicans'da bazı biyosidler ve antifungallerin ın vitro etkileşiminin araştırılması

Abstract

Bağışıklığı baskılanmış bireylerin sayısındaki artışla birlikte, fırsatçı mantar enfeksiyonları ve antifungal ilaç kullanımı da artmıştır. Candida albicans, fırsatçı mantar enfeksiyonlarının en sık nedenidir. Ayrıca insanda deri, gastrointestinal ve genitoüriner sistemde normal flora üyesi olarak da yer alabilir. Biyosidler, geniş spektrumlu antimikrobiyal maddelerdir. Antiseptik ve dezenfektan olarak kullanılabilirler, kişisel bakım ürünlerinde, temizlik ürünlerinde bulunabilirler. Bu çalışmada, Candida albicans suşlarında, çok sık kullanılan iki biyosid olan klorheksidin diglukonat ve benzalkonyum klorürün, klinikte sık kullanılan antifungal ilaçlar olan flukonazol ve nistatinle in vitro etkileşimlerinin araştırılması amaçlanmıştır. Çalışmaya 3 flukonazol dirençli, 3 flukonazol duyarlı olmak üzere 6 klinik Candida albicans kökeni ve 1 standart Candida albicans kökeni (ATCC 10231) alınmıştır. Biyosid antifungal kombinasyonları için dama tahtası mikrodilüsyon yöntemi kullanılmış, fraksiyonel inhibitör konsantrasyon (FİK) indeks analizlerine göre in vitro etkileşim sonuçları değerlendirilmiştir. Flukonazol dirençli kökenlerin hepsinde, flukonazol klorheksidin kombinasyonlarında sinerjistik etki görülmüştür. Benzalkonyum klorür duyarlılığı azalmış flukonazol duyarlı 2 kökende, flukonazol benzalkonyum klorür kombinasyonlarında antagonizma görülmüştür. Nistatin içeren kombinasyonlarda etkileşim görülmemiştir. The incidence of opportunistic fungal infections and usage of antifungal drugs have significantly increased due to the raise in the number of immuncompromised patients. Candida albicans is the primary cause of opportunistic fungal infection in humans. Species of Candida are members of the human normal flora which are located in the skin, gastrointestinal tract and genitourinary tract of the human body. Biocides are compounds that exhibits a broad spectrum of antimicrobial activity. They are used for various purposes like antisepsis, disinfection and preservation. This study aimed to determine the in vitro interaction between nystatine and fluconazole, commonly used antifungal agents in the treatment of fungal infections, and the biocides of chlorhexidine digluconate and benzalkonium chloride against clinical isolates of Candida albicans. A total of six clinical isolates (three fluconazole resistant and three fluconazole susceptible ones) of Candida albicans were used in this study. Candida albicans ATCC 10231 strain was included also as a quality control. In vitro interactions were evaluated by the microdilution checkerboard technique and the results were analyzed by fractional inhibitory concentration (FIC) index. In this study, synergistic effects were detected in fluconazole-chlorhexidine digluconate combinations against fluconazole-resistant C. albicans strains. Antagonistic effects were detected in the fluconazole-benzalkonium chloride combinations in fluconazole susceptible and benzalkonium chloride resistant C. albicans strains. 113
