1. Long-term outcomes with new generation prostheses in patients undergoing transcatheter aortic valve implantation

Abstract

Background Transcatheter Aortic Valve Implantation (TAVI) is today the first option for older patients with aortic valve stenosis (AS) at intermediate or high risk for surgery. Constant development of bioprosthetic valves and delivery systems have reduced complications and improved outcomes over the years. The 3rd generation Edwards Sapien 3 Valve (S3) and the 2nd generation Medtronic Evolut R Valve (ER) are currently the most frequently used worldwide. There is a paucity of published data regarding long term outcomes in these new generation TAVI patients. Methods and results In our retrospective, single-center analysis we included patients with severe Aortic Stenosis who underwent transfemoral TAVI with a new generation prosthesis between 2014 and 2016. Peri- and postprocedural outcomes of these patients were analyzed according to the VARC-2 criteria. The study population consisted of 359 patients (mean patient age 82±7 years, 47% male, mean EuroSCORE II 8.0±8). The S3 group included 215 patients, the ER group 144 patients. Median follow-up period was 3.8 years (IQR 3.3 to 4.4 years, maximum follow-up in living patients 5.1 years). Device Success rates where equal in both groups (93.0% vs. 92.4%, p=0.812). We report a 30-day mortality of 2.8% in the S3 group, 2.1% in the ER group, respectively (p=0.674). There was no difference in stroke rate, conversion to open heart surgery, major vascular complications, life-threatening or disabling bleeding or myocardial infarction. Implantation of a new permanent pacemaker was lower in the S3 group (S3: 27.4% vs. ER: 44.5%, p=0.002). While prosthesis mean gradients where higher in the S3 group (12.0 mmHg vs. 8.2 mmHg, p All-Cause Mortality up to 5 years did not show a difference between both patient groups (mean survival S3 3.5 years, ER 3.3 years, p=0.895). The analyzed cohort consisted of patients at intermediate to high surgical risk. Yet, 30-day mortality was very low both in S3 and ER patients. Device success and periprocedural outcomes in both groups were comparable and are in line with previous studies using VARC-2 definitions. Conclusion New generation TAVI valves offer an excellent implant and outcome success rate compared to early transcatheter Aortic valve replacement. Long-term survival was independent of prostheses choice and mainly attributed to pre- and intraprocedural comorbidities and complications. Abstract

In 14 healthy children contrast sonography of the bladder was performed with various solutions prior to voiding cystourethrography. The solutions were introduced into the bladder via a bladder catheter. The contrast effects which occurred with iothalamate injections in the urine-filled bladder, with urine injections in the iothalamate-filled bladder, with injections of 0.9% saline solution, and with air injections into the iothalamate-filled bladder were sonographically demonstrated. In echogenicity and duration the intravesical air contrast was superior to all mixture contrasts in the bladder resulting from injections of the respective fluids.

Abstract

In July 2005, San Diego, California, Trolley officials and riders celebrated completion of the light rail system's eighth extension. The 6-mi (9.7-km) Mission Valley East project closed the gap in the 53-mi (85.3-km) system and provided service to San Diego State University (SDSU). The project allowed for the introduction of the 19-mi (30.6-km) Green Line, which operates between Old Town and Santee on existing, new, and shared tracks. The Green Line is distinctive in a number of ways. It is a nonradial corridor that does not serve downtown San Diego. It directly serves the 35,000-student SDSU campus and surrounding community through the system's first tunnel and underground station. In addition, the line links a variety of regional activity centers along Mission Valley's intense residential and employment corridor. In fall 2005, the Metropolitan Transit System and the San Diego Association of Governments conducted two surveys and a passenger count to help understand more about who rides the Green Line, why they ride it, and where they are going. Survey results indicate that the Green Line has changed the composition and riding habits of transit users, resulting in high proportions of school trips, increased frequency of transit use, high numbers of choice riders, and high percentages of prepaid fare use. The Green Line has increased transit trips at SDSU by over 350%. The surveys also prompted plans for improvements, including operating later night service, pursuing station modifications that would allow the Green Line to extend into downtown, and considering partnerships with SDSU to expand the college semester pass program.

Abstract

The Large Size Telescope (LST) of the Cherenkov Telescope Array (CTA) is designed to achieve a threshold energy of 20 GeV. The LST optics is composed of one parabolic primary mirror 23 m in diameter and 28 m focal length. The reflector dish is segmented in 198 hexagonal, 1.51 m flat to flat mirrors. The total effective reflective area, taking into account the shadow of the mechanical structure, is about 368 m$^2$. The mirrors have a sandwich structure consisting of a glass sheet of 2.7 mm thickness, aluminum honeycomb of 60 mm thickness, and another glass sheet on the rear, and have a total weight about 47 kg. The mirror surface is produced using a sputtering deposition technique to apply a 5-layer coating, and the mirrors reach a reflectivity of $\sim$94% at peak. The mirror facets are actively aligned during operations by an active mirror control system, using actuators, CMOS cameras and a reference laser. Each mirror facet carries a CMOS camera, which measures the position of the light spot of the optical axis reference laser on the target of the telescope camera. Abstract

The Cherenkov Telescope Array (CTA) is a new observatory for very high-energy (VHE) gamma rays. CTA has ambitions science goals, for which it is necessary to achieve full-sky coverage, to improve the sensitivity by about an order of magnitude, to span about four decades of energy, from a few tens of GeV to above 100 TeV with enhanced angular and energy resolutions over existing VHE gamma-ray observatories. An international collaboration has formed with more than 1000 members from 27 countries in Europe, Asia, Africa and North and South America. In 2010 the CTA Consortium completed a Design Study and started a three-year Preparatory Phase which leads to production readiness of CTA in 2014. Abstract

Oxantel pamoate has high activity against the helminth species Trichuris spp. As there is no human pharmaceutical product, we developed tablets optimized for school-aged children containing 400 mg oxantel pamoate enabling the accomplishment of a clinical trial. NMR analysis confirmed the identity of the active ingredient used for the formulation development. Oxantel pamoate showed low bulk density, poor flow properties and high electrostatic adhesion, which made aqueous granulation necessary. We produced granules demonstrating good flowability using different excipients and drug loadings. The granules were compressed successfully into tablets and optimized with respect to disintegration time, which was aimed to be lower than 5 min. The final formulation resulted in tablets with 60% drug load, 172 N hardness and a disintegration time of less than 3.5 min.

Abstract

The Cherenkov Telescope Array (CTA) project aims to implement the world’s largest next generation of Very High Energy gamma-ray Imaging Atmospheric Cherenkov Telescopes devoted to the observation from a few tens of GeV to more than 100 TeV. To view the whole sky, two CTA sites are foreseen, one for each hemisphere. The sensitivity at the lowest energy range will be dominated by four Large Size Telescopes, LSTs, located at the center of each array and designed to achieve observations of high red-shift objects with the threshold energy of 20 GeV. The LST is optimized also for transient low energy sources, such as Gamma Ray Bursts (GRB), which require fast repositioning of the telescope. The overall design and the development status of the first LST telescope will be discussed.

Abstract

The Large Size Telescopes, LSTs, located at the center of the Cherenkov Telescope Array, CTA, will be sensitive for low energy gamma-rays. The camera on the LST focal plane is optimized to detect low energy events based on a high photon detection efficiency and high speed electronics. Also the trigger system is designed to detect low energy showers as much as possible. In addition, the camera is required to work stably without maintenance in a few tens of years. In this contribution we present the design of the camera for the first LST and the status of its development and production.

Abstract

Ground-based gamma-ray astronomy has had a major breakthrough with the impressive results obtained using systems of imaging atmospheric Cherenkov telescopes. Ground-based gamma-ray astronomy has a huge potential in astrophysics, particle physics and cosmology. CTA is an international initiative to build the next generation instrument, with a factor of 5-10 improvement in sensitivity in the 100 GeV-10 TeV range and the extension to energies well below 100 GeV and above 100 TeV. CTA will consist of two arrays (one in the north, one in the south) for full sky coverage and will be operated as open observatory. The design of CTA is based on currently available technology. Abstract

One hundred and ten children, ages 6 days to 14 years, were investigated for vesicoureteral reflux (VUR) using ultrasound before voiding cystourethrography (VCU). Sonographically a VUR was assumed if a retrovesical dilated ureter and/or an increase of the separation of the central renal echo complex (CRC) could be detected. By means of sonography VUR grades III and IV were seen in 100%, grade II in 84% of all cases. There was a false positive rate of 10%. Sonographic reflux study is a sensitive and specific screening and follow-up procedure for VUR.

Abstract

In 14 healthy children contrast sonography of the bladder was performed with various solutions prior to voiding cystourethrography. The solutions were introduced into the bladder via a bladder catheter. The contrast effects which occurred with iothalamate injections in the urine-filled bladder, with urine injections in the iothalamate-filled bladder, with injections of 0.9% saline solution, and with air injections into the iothalamate-filled bladder were sonographically demonstrated. In echogenicity and duration the intravesical air contrast was superior to all mixture contrasts in the bladder resulting from injections of the respective fluids.

15. Long-Term Outcome with New Generation Prostheses in Patients Undergoing Transcatheter Aortic Valve Replacement.

Abstract

The aim of this study was to compare patients with transcatheter aortic valve replacement (TAVR) receiving new generation prostheses SAPIEN 3 (S3, Edwards Lifesc.) and Evolut R (ER, Medtronic Inc.) in terms of periprocedural and long-term outcome. Our retrospective, single-center analysis included 359 consecutive patients with severe aortic stenosis who underwent TAVR with S3 or ER from 2014-2016 (mean age 82 ± 7 years, 47% male, mean EuroSCORE II 8.0 ± 8%, mean follow-up 3.8 years). Device Success was equal (S3 93.0% vs. ER 92.4%, p = 0.812). We report a 30-day mortality of 2.8% in the S3 group, and 2.1% in the ER group ( p = 0.674). There was no difference in stroke, conversion to open surgery, vascular and bleeding complications or myocardial infarction. While prosthesis mean gradients were higher with S3 (12.0 mmHg vs. 8.2 mmHg, p < 0.001), there was a trend to less paravalvular regurgitation (PVR moderate or severe: 1% vs. 3.6%, p = 0.088). All-cause mortality up to 5 years did not show a difference (mean survival S3 3.5 ± 0.24 years, ER 3.3 ± 0.29 years, p = 0.895). Independent predictors of long-term mortality were impaired LVEF, chronic kidney injury, peripheral artery disease, malignant tumor and periprocedural stroke. New generation TAVR valves offer an excellent implant and outcome success rate. Long-term survival was independent of prostheses choice and mainly attributed to comorbidities and complications.

Abstract

Cartilage degeneration after injury affects a significant percentage of the population, including those that will go on to develop osteoarthritis (OA). Like humans, most mammals, including mice, are incapable of regenerating injured cartilage. Interestingly, it has previously been shown that p21 ( Cdkn1a ) knockout (p21 -/- ) mice demonstrate auricular (ear) cartilage regeneration. However, the loss of p21 expression is highly correlated with the development of numerous types of cancer and autoimmune diseases, limiting the therapeutic translation of these findings. Therefore, in this study, we employed a screening approach to identify an inhibitor (17-DMAG) that negatively regulates the expression of p21. We also validated that this compound can induce chondrogenesis in vitro (in adult mesenchymal stem cells) and in vivo (auricular cartilage injury model). Furthermore, our results suggest that 17-DMAG can induce the proliferation of terminally differentiated chondrocytes ( in vitro and in vivo ), while maintaining their chondrogenic phenotype. A new approach based on non-pigmented, stable colored nanoparticles able to migrate upon application of an electrical field (10-60 V) has been developed for the improvement of the color brightness of e-displays. The scientific challenges comprised the development of efficient syntheses of tri- and bifunctional dendrimers including branching points for further extension and individual decoration with dye (yellow, magenta, cyan). The covalent attachment of these scaffolds to silica nanoparticles was performed via hydrosilylation and final in situ charging generated attractive silica shells for the substractive CMY color space model.

18. Neurosarcoidosis: clinical review of a disorder with challenging inpatient presentations and diagnostic considerations.

Abstract

Neurosarcoidosis is frequently on the differential diagnosis for neurohospitalists. The diagnosis can be challenging due to the wide variety of clinical presentations as well as the limitations of noninvasive diagnostic testing. This article briefly touches on systemic features that may herald suspicion of this disorder and then expands in depth on the neurological clinical presentations. Common patterns of neurological presentations are reviewed and unusual presentations are also included. A discussion of noninvasive testing is undertaken, exploring dilemmas that may be encountered with sensitivity and specificity. Drawing from a broad range of clinical clues and diagnostic data, a systematic approach of pursuing a potential tissue diagnosis is then highlighted. Correctly diagnosing neurosarcoidosis is critical, as treatment with appropriate immunosuppression protocols can then be initiated. Additionally, treatment of refractory disease, the trend toward exploring targeted immunomodulation options, and other therapeutic issues are discussed.

Abstract

Purpose: In this prospective study, bone formation in human extraction sockets augmented with Bio-Oss Collagen after a 12-week healing period was quantified and compared to bone formation in unaugmented extraction sockets., Materials and Methods: Selected patients with four-walled extraction sockets were included in this prospective study. After extraction, the sockets were randomly augmented using Bio-Oss Collagen or left to heal unfilled without raising a mucoperiosteal flap. At the time of implant placement, histologic specimens were obtained from the socket and analyzed. Statistical analysis was performed using the Wilcoxon signed-rank test., Results: Twenty-five patients with a total of 39 sockets (20 augmented, 19 unaugmented) were included in the study and the histologic specimens analyzed. All specimens were free of inflammatory cells. The mean overall new bone formation in the augmented sites was 25% (range, 8%-41%) and in the unaugmented sockets it was 44% (range, 3%-79%). There was a significant difference in the rate of new bone formation between the grafted and ungrafted sockets and a significant difference in the bone formation rate in the apical compared to the coronal regions of all sockets, independent of the healing mode., Conclusion: This descriptive study demonstrated that bone formation in Bio-Oss Collagen-grafted human extraction sockets was lower than bone formation in ungrafted sockets. Bone formation occurred in all specimens with varying degrees of maturation independent of the grafting material and was initiated from the apical region.

Abstract

Poly-chlorinated biphenyls (PCBs) have been recognized as a significant contaminant in the Great Lakes ecosystem. Although PCBs are implicated in the reduced survival and reproductive success of several piscivorous bird species, the biological pathway in which PCBs bioaccumulate remains largely unknown. This study investigates the two most likely biological pathways, suggested via research on Great Lakes sport fish, by which PCBs would be acquired by common terns (Sterna hirundo), a piscivorous species of conservation concern. The first proposed pathway is through atmospheric deposition of PCBs which are subsequently acquired by filter-feeding fish (e.g., alewives, Alosa pseudoharengus). An alternative pathway is via the biodeposits of zebra mussels which are consumed by shallow water fish (e.g., round gobies, Neogobius melanostromus). Because common terns breed in near-shore sites where concentrations of zebra mussels are found, as well as forage in more pelagic environments it is possible that either or both pathways may be contributing to their PCB exposure. Field experiments and stable isotope analyses suggest the most likely pathway by which terns are exposed to PCBs is via alewives, similar to how apex predators such as lake trout acquire PCBs. Biodeposits from zebra mussels do not appear to be a significant factor in PCB accumulation in terns. We quantified extremely poor parental attentiveness during incubation. Although we cannot determine whether poor parental attentiveness alone or in combination with PCB contamination led to low hatching success, accumulation of PCBs appears to have significant impacts on the overall reproductive success of common terns.

Abstract

Rationale: Catamenial and endometriosis-related pneumothorax are considered relatively rare entities. Their clinical characteristics and outcome are incompletely known., Objectives: To evaluate the frequencies, clinical characteristics, and outcomes of catamenial and endometriosis-related pneumothoraces occurring in women with no underlying lung disease referred for surgical treatment., Methods: Clinical files of all the women of reproductive age referred to our center during a 6-year period for surgical treatment of spontaneous pneumothorax were retrospectively reviewed. Catamenial pneumothorax (CP) was defined as recurrent pneumothorax occurring between the day before and within 72 hours after the onset of menses. All histologic slides were reviewed to confirm initial diagnoses., Measurements and Main Results: A total of 114 women underwent video-assisted thoracic surgery; 28 women (24.6%) had CP (right-sided in all but one), and diaphragmatic abnormalities (perforations and/or nodules) were observed in 22 of them. Diaphragmatic abnormalities were seen in 21 of 86 patients with non-CP. Histologic examination found endometriosis, mainly diaphragmatic, in 18 of 28 CPs and 11 of 86 non-CPs. A 6-month antigonadotropic treatment was prescribed postoperatively to women with either CP or endometriosis-related pneumothorax. Mean follow-up was 32.7 (+/-18.5) months. Recurrence rates in CP, non-CP but endometriosis-related, and non-CP non-endometriosis-related pneumothoraces were 32, 27, and 5.3%, respectively., Conclusions: Our experience shows that (1) CP and/or endometriosis-related pneumothoraces account for an important percentage of spontaneous pneumothoraces referred for surgery, (2) diaphragmatic abnormalities seem to play a fundamental role in their pathogenesis, and (3) management is difficult because of the high recurrence rate.

Abstract

The link between hypothyroidism and infertility is still a matter of debate. Hypothyroidism can result in cycle disturbances, such as oligomennorhea and functional bleeding. Additionally, several studies have shown that thyroid autoimmunity (detection of anti peroxydase antibodies) may account for the occurrence of repetitive miscarriages. In infertility work-up, screening thyroid function should be specifically recommended for women with clinical hypothyroidism, with a personal, familial history of thyroid or other auto immune diseases (such as type I diabetes) as well as for women with unexplained anovulation or functional bleeding. Moreover, detection of thyroid antibody seems to be worthwhile for the assessment of recurrent miscarriages, due to the potential benefit of thyroid supplementation. In pregnant women, assessment of thyroid function seems specifically crucial to ensure adequate foetal development. Indeed, it has been well established that untreated maternal hypothyroidism may be associated with disturbances of brain development and low intellectual quotient. Additionally, other foetal (growth deficiency, premature birth, low birth weight) as well as maternal (gestational hypertension, pre-eclampsia...) complications have been also reported in pregnant women with untreated hypothyroidism. Consequently, screening of thyroid function should be performed in every woman at risk of thyroid disease. Recent studies even advocate that thyroid screening should be extended to the overall pregnant population. The objective is to adjust L-thyroxin supplementation to maintain serum TSH concentrations below the threshold of 2.5 mUI/l. Finally, iodine deficiency, currently observed in pregnant women, should be prevented by iodine supply prior to conception, during pregnancy and during breast feeding as well.

Abstract

Mono- (3a,b) and binuclear (4a,b) tetradentate NiII complexes of a series of 26-membered macrocyclic salen dimers, [salen(CH2)]2, are prepared in good yield by solvent-controlled reaction with Ni(OAc)2. The mononuclear complex 3b crystallizes in the trigonal space group 3P1(#144), a = 18.2566(2) A, c = 15.9244(2) A, V = 4596.57(8) A3, and Z = 3. Refinement converged with R = 0.054 and Rw = 0.049 for 6852 reflections with I > 2.003 sigma(I). The NiII in complex 3b coordinates in an approximate square planar geometry to one of the two available tetradentate salen sites. Complex 4b crystallizes in the orthorhombic space group P2(1)2(1)2(1)(#19), a = 19.531(2) A, b = 22.891(3), c = 13.373(1) A, V = 5960(1) A3, and Z = 4. The refinement converged with R = 0.067 and Rw = 0.065 for 3752 reflections with I > 2.003 sigma(I). Complex 4b coordinates two distorted square planar, cofacially oriented NiII-salen units held 7.1 A apart by a rigid, syn-folded macrocyclic structure. The solution spectroscopic data and solid-state crystallographic data of 3b and 4b demonstrate the presence of a molecular-sized cavity which shows host-guest properties. Reaction of the flexible 32-membered disalen macrocycle [salen(OCH2CH2O)]2 with Ni(OAc)2 resulted in formation of a binuclear complex, 5. Complex 5 crystallizes in the triclinic space group P1(#1), a = 10.366(4) A, b = 12.170(3) A, c = 10.021(2) A, alpha = 106.29(2) degrees, beta = 91.69(2) degrees, gamma = 68.63(2) degrees, V = 1126.3(5) A3, and Z = 1. The refinement converged with R = 0.052 and Rw = 0.053 for 2385 reflections with I > 2.003 sigma(I). The binuclear complex 5 contains two cofacially oriented, square planar NiII-salen groups lying 3.5 A apart in an anti-folded macrocyclic structure.

Abstract

One hundred and ten children, ages 6 days to 14 years, were investigated for vesicoureteral reflux (VUR) using ultrasound before voiding cystourethrography (VCU). Sonographically a VUR was assumed if a retrovesical dilated ureter and/or an increase of the separation of the central renal echo complex (CRC) could be detected. By means of sonography VUR grades III and IV were seen in 100%, grade II in 84% of all cases. There was a false positive rate of 10%. Sonographic reflux study is a sensitive and specific screening and follow-up procedure for VUR.

Abstract

In 14 healthy children contrast sonography of the bladder was performed with various solutions prior to voiding cystourethrography. The solutions were introduced into the bladder via a bladder catheter. The contrast effects which occurred with iothalamate injections in the urine-filled bladder, with urine injections in the iothalamate-filled bladder, with injections of 0.9% saline solution, and with air injections into the iothalamate-filled bladder were sonographically demonstrated. In echogenicity and duration the intravesical air contrast was superior to all mixture contrasts in the bladder resulting from injections of the respective fluids.
