Abstract

Abstract Nowadays, exome sequencing is a robust and cost-efficient genetic diagnostic tool already implemented in many clinical laboratories. Despite it has undoubtedly improved our diagnostic capacity and has allowed the discovery of many new Mendelian-disease genes, it only provides a molecular diagnosis in up to 25–30% of cases. Here, we comprehensively evaluate the results of a large sample set of 4974 clinical exomes performed in our laboratory over a period of 5 years, showing a global diagnostic rate of 24.62% (1391/4974). For the evaluation we establish different groups of diseases and demonstrate how the diagnostic rate is not only dependent on the analyzed group of diseases (43.12% in ophthalmological cases vs 16.61% in neurological cases) but on the specific disorder (47.49% in retinal dystrophies vs 24.02% in optic atrophy; 18.88% in neuropathies/paraparesias vs 11.43% in dementias). We also detail the most frequent mutated genes within each group of disorders and discuss, on our experience, further investigations and directions needed for the benefit of patients.

Abstract

Abstract Despite the improved accuracy of next-generation sequencing (NGS), it is widely accepted that variants need to be validated using Sanger sequencing before reporting. Validation of all NGS variants considerably increases the turnaround time and costs of clinical diagnosis. We comprehensively assessed this need in 1109 variants from 825 clinical exomes, the largest sample set to date assessed using Illumina chemistry reported. With a concordance of 100%, we conclude that Sanger sequencing can be very useful as an internal quality control, but not so much as a verification method for high-quality single-nucleotide and small insertion/deletions variants. Laboratories might validate and establish their own thresholds before discontinuing Sanger confirmation studies. We also expand and validate 23 copy number variations detected by exome sequencing in 20 samples, observing a concordance of 95.65% (22/23).

Human iPSC line Oex2054SV.4 was generated from fibroblasts of a patient with an optic atrophy ‘plus’ phenotype associated with a heterozygous mutation in the OPA1 gene. Reprogramming factors OCT3/4, SOX2, CMYC and KLF4 were delivered using a non-integrative methodology that involves the use of Sendai virus.

Abstract

La morfología de los eritrocitos urinarios es fundamental en el diagnóstico de la hematuria, y la presencia de eritrocitos dismórfi cos es indicativa de hematuria glomerular. Se evaluó la utilidad del volumen corpuscular medio (VCMu), ancho de distribución del volumen (ADEu) y el patrón de la curva de distribución de volumen de los eritrocitos urinarios (CDVu) obtenidos en un contador hematológico para diferenciar las hematurias glomerulares de las no glomerulares. Fueron evaluadas 50 orinas con hematuria, de diferentes orígenes, por microscopía de contraste de fase (MCF) y en el contador hematológico. La media del VCMu fue 63.0±9.2 fl vs 90.8±11.3 fl (p35% mostraron sensibilidad 79.3% y 86.2% y especifi cidad 100% y 96.2%, respectivamente, para detectar hematuria glomerular. El patrón de la CDVu presentó sensibilidad 86% y especifi cidad 96%. La medida de VCMu, ADEu y la CDVu constituyen un método sencillo y objetivo y podría ser un método alternativo a la MCF.

Abstract

Inherited Retinal Dystrophies (IRDs) are a group of rare diseases with a prevalence of 1:3000-4000 people. They are genetic, primarily affecting retinal photoreceptors and epithelial pigmentary cells, and lead to neurodegeneration and finally apoptosis. In 2021, we published our global results obtained in our registry at the Fundación Jiménez Díaz University Hospital (Madrid, Spain) from 1991 to August 2019. Now, we aimed to update these results until August 2022. Thus, we conducted a retrospective hospital-based cross-sectional study on 4.794 IRD-affected unrelated families from all the Spanish autonomous communities. Families were classified into three phenotypic categories: a) “NON-RP” for cone-dominated phenotypes, b) “RP” (retinitis pigmentosa) for primary rod involvement, and c) “syndromic IRD” when visual plus extra-ocular symptoms are present. Molecular studies included: single-gene studies, clinical exome, whole exome or whole genome sequencing. Overall, 62% (2962/4794) of the families were genetically characterized, in which 1.997 different likely causative variants (5.064 different alleles) were identified in 188 genes. The most common phenotype encountered was RP (59% of families, 2465/4794). Regarding the types of causative alleles, missenses were the most frequent (51%), followed by truncating (nonsense, frameshift, indels and splicing; 44%), while copy number variations were only 2%. The most recurrently mutated genes were PRPH2, ABCA4 and RS1 in autosomal dominant (AD), autosomal recessive (AR) and X-linked (XL) NON-RP families, respectively; RHO, USH2A and RPGR in AD, AR and XL for non-syndromic RP; and MYO7A, USH2A and BBS1 in syndromic IRD. Pathogenic variants ABCA4:p.Arg1129Leu and USH2A:p.Cys759Phe were the most frequent. Our study provides the overall genetic landscape for IRD in Spain, reporting the largest cohort ever presented and a high number of causal genes involved in these diseases. Furthermore, our findings have important implications for genetic diagnosis and counseling, but also for possible therapeutic management

Abstract

Background and Objectives Stargardt disease produces lipofuscin accumulation predisposing to subretinal fibrosis (SRFib) after ocular trauma. Noninvasive imaging techniques allow in vivo assessment. The purpose of this study is to determine the prevalence of SRFib in a cohort of Stargardt patients, the presence of history of ocular trauma, the clinical features and possible genotype-phenotype associations in Stargardt patients with SRFib. Methods We evaluated retrospectively 106 Stargardt patients and analysed the multimodal imaging and the genotype of patients with SRFib. Results Six patients exhibited SRFib, three of them with history of ocular trauma. Multimodal imaging showed extensive SRFib principally in the temporal midperipheral retina with no fluid associated. SRFib was better defined by short wavelength autofluorescence and spectral domain optical coherence tomography and appeared clinically stable over time. There was no particular genotype associated to SRFib. Conclusion SRFib occurs in a significant percentage of patients with Stargardt disease and can be diagnosed through multimodal imaging regardless the history of trauma, further sustaining the importance of an appropriate imaging in such patients. No genotype-phenotype association has been established, supporting the traumatic etiology in half of cases. The remaining cases may be classified as idiopathic or have a minimal trauma occurring early in life that may be not recalled by the patients.

Abstract

Treatment for hepatocellular carcinoma (HCC) has experienced major advancements since the last update of the official Barcelona Clinic Liver Cancer (BCLC) prognosis and treatment strategy published in 2018. Advancements in the field have emerged in all areas, but in this manuscript, we present those that have primed a change in the strategy and comment why some encouraging data in select interventions are still considered immature and in need of further research to gain their incorporation into an evidence-based model for clinicians and researchers. Finally, we describe the critical insight and expert knowledge that is needed to take clinical decisions in individual patients so that the recommendation takes into account all the needed parameters to allow a personalised approach.

Abstract

The fundamental building blocks of the proton, quarks and gluons, have been known for decades. However, we still have an incomplete theoretical and experimental understanding of how these particles and their dynamics give rise to the quantum bound state of the proton and its physical properties, such as for example its spin. The two up and the single down quarks that comprise the proton in the simplest picture account only for a few percent of the proton mass, the bulk of which is in the form of quark kinetic and potential energy and gluon energy from the strong force. An essential feature of this force, as described by quantum chromodynamics, is its ability to create matter-antimatter quark pairs inside the proton that exist only for a very short time. Their fleeting existence makes the antimatter quarks within protons difficult to study, but their existence is discernible in reactions where a matter-antimatter quark pair annihilates. In this picture of quark-antiquark creation by the strong force, the probability distributions as a function of momentum for the presence of up and down antimatter quarks should be nearly identical, since their masses are quite similar and small compared to the mass of the proton. In the present manuscript, we show evidence from muon pair production measurements that these distributions are significantly different, with more abundant down antimatter quarks than up antimatter quarks over a wide range of momentum. These results revive interest in several proposed mechanisms as the origin of this antimatter asymmetry in the proton that had been disfavored by the previous results and point to the future measurements that can distinguish between these mechanisms., 10 pages, 5 figures, 5 tables; Matches published version

Abstract

International audience; Experimental studies of the collisions of heavy nuclei at relativistic energies have established the properties of the quark–gluon plasma (QGP), a state of hot, dense nuclear matter in which quarks and gluons are not bound into hadrons1–4. In this state, matter behaves as a nearly inviscid fluid5 that efficiently translates initial spatial anisotropies into correlated momentum anisotropies among the particles produced, creating a common velocity field pattern known as collective flow. In recent years, comparable momentum anisotropies have been measured in small-system proton–proton (p+p) and proton–nucleus (p+A) collisions, despite expectations that the volume and lifetime of the medium produced would be too small to form a QGP. Here we report on the observation of elliptic and triangular flow patterns of charged particles produced in proton–gold (p+Au), deuteron–gold (d+Au) and helium–gold (3He+Au) collisions at a nucleon–nucleon centre-of-mass energy $\sqrt {s_{{\mathrm{NN}}}} = 200$ GeV. The unique combination of three distinct initial geometries and two flow patterns provides unprecedented model discrimination. Hydrodynamical models, which include the formation of a short-lived QGP droplet, provide the best simultaneous description of these measurements.

13. Diagnóstico genético de la epidermólisis bullosa: recomendaciones de un grupo español de expertos

Abstract

Resumen La epidermolisis bullosa (EB), enfermedad genetica de fragilidad mucocutanea rara y devastadora, es clinica y geneticamente heterogenea. Se caracteriza por la aparicion de ampollas inducidas por contacto/friccion o de forma espontanea. La EB se clasifica en 4 tipos: simple, juntural, distrofica y sindrome de Kindler y en 30 subtipos. Esta genodermatosis esta causada por defectos en proteinas implicadas en la adhesion dermoepidermica, con al menos 19 genes caracterizados hasta el momento y mas de 1.000 mutaciones identificadas, que explican la complejidad de su diagnostico. El diagnostico molecular de la EB es el ultimo paso de un proceso laborioso que se inicia con la recogida de una historia clinica detallada y la toma de una biopsia cutanea, que incluya una zona de despegamiento entre la dermis y la epidermis inducida, en el momento de la recoleccion. Dicho despegamiento permite establecer el plano de rotura por mapeo antigenico y, en el mejor de los casos, un unico gen candidato en el que realizar la busqueda de las mutaciones patogenicas. Finalizado el diagnostico molecular, se esta en condiciones de ofrecer al paciente un asesoramiento genetico adecuado (patron de herencia, riesgo de recurrencia y opciones de diagnostico prenatal y preimplantacional) y los consecuentes programas preventivos, asi como un pronostico clinico razonable que facilite su acceso a opciones terapeuticas y de rehabilitacion especificas. Por ultimo, el diagnostico molecular es imprescindible para la participacion de los pacientes en ensayos clinicos, de gran importancia en una enfermedad como la EB, que no tiene cura. El objetivo de la presente guia es difundir el procedimiento de diagnostico de la EB tal y como se esta llevando a cabo en nuestro laboratorio y, asi, evitar diagnosticos clinicos suboptimos o incompletos. Las recomendaciones recogidas son fruto de nuestra experiencia de mas de 10 anos de diagnostico molecular de EB en Espana.

Abstract

Epidermolysis bullosa (EB) is a rare genetic disease that causes mucocutaneous fragility. It comprises a clinically and genetically heterogeneous group of disorder characterized by spontaneous or contact/friction–induced blistering. EB is classified into 4 types–simplex, junctional, dystrophic, and Kindler syndrome–and 30 subtypes. The disease is caused by defects in proteins implicated in dermal-epidermal adhesion. At least 19 genes have been characterized and more than 1000 mutations identified, thus rendering diagnosis complex. Molecular diagnosis of EB is the last stage of a laborious process that starts with a detailed clinical history compilation and careful procurement of a skin fresh biopsy that includes an area where the epidermis detaches from the dermis. The detachment area makes it possible to establish the cleavage plane by antigen mapping and, in the best scenario, to identify a single candidate gene to search for pathogenic mutations. The results of the molecular diagnosis enable the physician to provide appropriate genetic counseling (inheritance pattern, risk of recurrence, and options for prenatal and preimplantation diagnosis) and implement subsequent preventive programs, as well as to establish a reasonable clinical prognosis facilitating access to specific therapy and rehabilitation. Lastly, molecular diagnosis is essential for the participation of patients in clinical trials, a critical issue given the current incurable status of EB. The present guidelines aim to disseminate the procedure for diagnosing EB in our laboratory and thus avoid suboptimal or incomplete clinical diagnoses. The recommendations we provide are the result of more than 10 years’ experience in the molecular diagnosis of EB in Spain.

The primary physics program uses the Drell–Yan process to probe antiquark distributions in the target nucleon. The spectrometer consists of a target system, two dipole magnets and four detector stations. The upstream magnet is a closed-aperture solid iron magnet which also serves as the beam dump, while the second magnet is an open aperture magnet. Each of the detector stations consists of scintillator hodoscopes and a high-resolution tracking device. The FPGA-based trigger compares the hodoscope signals to a set of pre-programmed roads to determine if the event contains oppositely-signed, high-mass muon pairs.

Abstract

Asymmetric nuclear collisions of p+Al, p+Au, d+Au, and ^{3}He+Au at sqrt[s_{NN}]=200 GeV provide an excellent laboratory for understanding particle production, as well as exploring interactions among these particles after their initial creation in the collision. We present measurements of charged hadron production dN_{ch}/dη in all such collision systems over a broad pseudorapidity range and as a function of collision multiplicity. A simple wounded quark model is remarkably successful at describing the full data set. We also measure the elliptic flow v_{2} over a similarly broad pseudorapidity range. These measurements provide key constraints on models of particle emission and their translation into flow.

Abstract

Recently, multiparticle-correlation measurements of relativistic p/d/He3+Au, p+Pb, and even p+p collisions show surprising collective signatures. Here, we present beam-energy-scan measurements of two-, four-, and six-particle angular correlations in d+Au collisions at sNN=200, 62.4, 39, and 19.6 GeV. We also present measurements of two- and four-particle angular correlations in p+Au collisions at sNN=200 GeV. We find the four-particle cumulant to be real valued for d+Au collisions at all four energies. We also find that the four-particle cumulant in p+Au has the opposite sign as that in d+Au. Further, we find that the six-particle cumulant agrees with the four-particle cumulant in d+Au collisions at 200 GeV, indicating that nonflow effects are subdominant. These observations provide strong evidence that the correlations originate from the initial geometric configuration, which is then translated into the momentum distribution for all particles, commonly referred to as collectivity. © 2018 American Physical Society. Abstract

During 2015, the Relativistic Heavy Ion Collider (RHIC) provided collisions of transversely polarized protons with Au and Al nuclei for the first time, enabling the exploration of transverse-single-spin asymmetries with heavy nuclei. Large single-spin asymmetries in very forward neutron production have been previously observed in transversely polarized p+p collisions at RHIC, and the existing theoretical framework that was successful in describing the single-spin asymmetry in p+p collisions predicts only a moderate atomic-mass-number (A) dependence. In contrast, the asymmetries observed at RHIC in p+A collisions showed a surprisingly strong A dependence in inclusive forward neutron production. The observed asymmetry in p+Al collisions is much smaller, while the asymmetry in p+Au collisions is a factor of 3 larger in absolute value and of opposite sign. The interplay of different neutron production mechanisms is discussed as a possible explanation of the observed A dependence.

Abstract

Asymmetric nuclear collisions of $p$$+$Al, $p$$+$Au, $d$$+$Au, and $^{3}$He$+$Au at $\sqrt{s_{_{NN}}}=200$ GeV provide an excellent laboratory for understanding particle production, as well as exploring interactions among these particles after their initial creation in the collision. We present measurements of charged hadron production $dN_{\rm ch}/d\eta$ in all such collision systems over a broad pseudorapidity range and as a function of collision multiplicity. A simple wounded quark model is remarkably successful at describing the full data set. We also measure the elliptic flow $v_{2}$ over a similarly broad pseudorapidity range. These measurements provide key constraints on models of particle emission and their translation into flow., Comment: 368 authors, 7 pages, 4 figures. v2 is the version accepted for publication by Physical Review Letters. Abstract

We report a measurement of e+e− pairs from semileptonic heavy-flavor decays in p+p collisions at sNN=200 GeV. The e+e− pair yield from bb¯ and cc¯ is separated by exploiting a double differential fit done simultaneously in dielectron invariant mass and pT. We used three different event generators, pythia, mc@nlo, and powheg, to simulate the e+e− spectra from cc¯ and bb¯ production. The data can be well described by all three generators within the detector acceptance. However, when using the generators to extrapolate to 4π, significant differences are observed for the total cross section. These difference are less pronounced for bb¯ than for cc¯. The same model dependence was observed in already published d+A data. The p+p data are also directly compared with d+A data in mass and pT, and within the statistical accuracy no nuclear modification is seen.

Abstract

Recently, multiparticle-correlation measurements of relativistic p/d/^{3}He+Au, p+Pb, and even p+p collisions show surprising collective signatures. Here, we present beam-energy-scan measurements of two-, four-, and six-particle angular correlations in d+Au collisions at sqrt[s_{NN}]=200, 62.4, 39, and 19.6 GeV. We also present measurements of two- and four-particle angular correlations in p+Au collisions at sqrt[s_{NN}]=200 GeV. We find the four-particle cumulant to be real valued for d+Au collisions at all four energies. We also find that the four-particle cumulant in p+Au has the opposite sign as that in d+Au. Further, we find that the six-particle cumulant agrees with the four-particle cumulant in d+Au collisions at 200 GeV, indicating that nonflow effects are subdominant. These observations provide strong evidence that the correlations originate from the initial geometric configuration, which is then translated into the momentum distribution for all particles, commonly referred to as collectivity.

Abstract

The cross section and transverse single-spin asymmetries of μ- and μ+ from open heavy-flavor decays in polarized p+p collisions at s=200 GeV were measured by the PHENIX experiment during 2012 at the Relativistic Heavy Ion Collider. Because heavy-flavor production is dominated by gluon-gluon interactions at s=200 GeV, these measurements offer a unique opportunity to obtain information on the trigluon correlation functions. Abstract

We report the first measurement of the fraction of J/ψ mesons coming from B-meson decay (FB→J/ψ) in p+p collisions at s=510 GeV. The measurement is performed using the forward silicon vertex detector and central vertex detector at PHENIX, which provide precise tracking and distance-of-closest-approach determinations, enabling the statistical separation of J/ψ due to B-meson decays from prompt J/ψ. Abstract

We present the first measurements of long-range angular correlations and the transverse momentum dependence of elliptic flow $v_2$ in high-multiplicity $p$$+$Au collisions at $\sqrt{s_{_{NN}}}=200$ GeV. A comparison of these results with previous measurements in high-multiplicity $d$$+$Au and $^3{\rm He}$$+$Au collisions demonstrates a relation between $v_2$ and the initial collision eccentricity $\varepsilon_2$, suggesting that the observed momentum-space azimuthal anisotropies in these small systems have a collective origin and reflect the initial geometry. Good agreement is observed between the measured $v_2$ and hydrodynamic calculations for all systems, and an argument disfavoring theoretical explanations based on momentum-space domain correlations is presented. The set of measurements presented here allows us to leverage the distinct intrinsic geometry of each of these systems to distinguish between different theoretical descriptions of the long-range correlations observed in small collision systems.

Abstract

Epidermolysis bullosa (EB) is a rare genetic disease that causes mucocutaneous fragility. It comprises a clinically and genetically heterogeneous group of disorder characterized by spontaneous or contact/friction-induced blistering. EB is classified into 4 types-simplex, junctional, dystrophic, and Kindler syndrome-and 30 subtypes. The disease is caused by defects in proteins implicated in dermal-epidermal adhesion. At least 19 genes have been characterized and more than 1000 mutations identified, thus rendering diagnosis complex. Molecular diagnosis of EB is the last stage of a laborious process that starts with a detailed clinical history compilation and careful procurement of a skin fresh biopsy that includes an area where the epidermis detaches from the dermis. The detachment area makes it possible to establish the cleavage plane by antigen mapping and, in the best scenario, to identify a single candidate gene to search for pathogenic mutations. The results of the molecular diagnosis enable the physician to provide appropriate genetic counseling (inheritance pattern, risk of recurrence, and options for prenatal and preimplantation diagnosis) and implement subsequent preventive programs, as well as to establish a reasonable clinical prognosis facilitating access to specific therapy and rehabilitation. Lastly, molecular diagnosis is essential for the participation of patients in clinical trials, a critical issue given the current incurable status of EB. The present guidelines aim to disseminate the procedure for diagnosing EB in our laboratory and thus avoid suboptimal or incomplete clinical diagnoses. The recommendations we provide are the result of more than 10 years' experience in the molecular diagnosis of EB in Spain.

Abstract

Objective The aims of this review are to describe the main characteristics for the CT diagnosis of isolated caecal ischaemia (ICA) and give details of the differential diagnosis with other conditions with a similar clinical picture. Material and methods A retrospective study was conducted to review the CT findings of 4 patients diagnosed with ICA in our hospital. The parameters recorded to analyze their characteristics in the CT were: maximum thickness of the cecum wall, the appearance of the peri-cecum fat, presence of free fluid, signs of caecal or portal pneumatosis, the appearance of the caecal appendix, and general signs of the presence of vasculopathy. Results In all cases it was recorded that there was a thickening of the walls of the blind loop with an abrupt transition between the caecal wall and the walls of the ascending colon wall. In all cases the caecal thickening had a characteristic image in the central area. Signs of caecal pneumatosis were observed in two cases. All of them had an appendix with normal characteristics. Conclusions The combination of caecal wall thickening with oedematous characteristics, with no changes in the appendix, ileum and colon, suggests the diagnosis of caecal ischaemia, particularly with the presence of pneumatosis.

Abstract

Summary Introduction Iron-deficient erythropoiesis attributable to nutritional deficiency is the most common cause of anemia in early childhood. Beckman Coulter has proposed new parameters: red blood cell size factor (RSf) and low hemoglobin density (LHD%). The aims of the present communication were to establish reference ranges (RR) for RSf and LHD% in a healthy pediatric population; to evaluate clinical utility of these parameters in the diagnosis of iron-deficiency anemia (IDA); and to compare RSf and LHD% with RET-He. Methods Two hundred healthy infants and 51 with IDA were analyzed on Coulter LH750 and Sysmex XT2000i analyzers. Results RR for RSf in children aged 0.5–6 years, 82.2–102.0 and 83.7–103.1 fl in the group aged 6-18 years. RR for LHD% was 1.1–9.0%. ROC analysis for RSf and LHD% in the diagnosis of IDA showed an AUC 0.8460 and 0.8654, respectively. The best RSf value to detect a restricted erythropoiesis was 89fl, sensitivity 92%, specificity 81%. LHD% optimal cutoff point was 6.0%, sensitivity 80%, and specificity 88%. Good correlation was observed between these parameters with Ret-He, r = 0.888 and r = −0.790, respectively. Conclusion RSf and LHD% could be useful tools in the screening of IDA. These parameters can be obtained in the course of routine blood counts, with no additional cost or need for more blood sampling.

Resumen El cancer de recto es una neoplasia con elevada incidencia en nuestro medio. Las distintas opciones terapeuticas que podran aplicarse en cada caso van a depender fundamentalmente de la extension que presente el tumor en el momento del diagnostico. Por esto, los radiologos tenemos un papel central en su tratamiento. En el presente texto se discuten la tecnica radiologica optima, algunos aspectos anatomicos fundamentales y la semiologia aceptada en la interpretacion de los distintos estudios de imagen que se realizan a estos pacientes, ademas de su rendimiento, y una vision general del pronostico y de las distintas opciones terapeuticas. Su conocimiento es muy util para participar en equipos formados por diferentes especialistas y comprender la trascendencia de la informacion aportada por la interpretacion de los estudios que realizamos a estos pacientes.
