The aim of this study was to compare clinical characteristics and adipose/liver tissue histology analysis in HIV-infected and HIV-uninfected subjects undergoing bariatric surgery.This was a cross-sectional study of HIV-infected subjects undergoing single-port sleeve gastrectomy with prospective enrolment and frequency age (±5 years), sex, and body mass index (BMI, ± 5 kg/m2) matched on HIV-uninfected subjects.This study was conducted at a single clinical site at Pitié-Salpêtrière hospital-Paris-France comprising 19 HIV-uninfected and 21 HIV-infected subjects with plasma VL20 copies/mL, all with a BMI40 kg/m2 or35 kg/m2 with comorbidities. Histology of subcutaneous and visceral abdominal adipose tissue (SCAT/VAT) and liver biopsies was collected during single-port sleeve gastrectomy. Outcomes included anthropometric characteristics, comorbidities, cardiovascular parameters, adipose tissue, and liver histology.The age of HIV-infected participants was (median, interquartile range IQR) 48 y (42-51), with 76.2% females, a BMI of 41.4 kg/m2 (37.3-44.4), an antiretroviral duration of 16 y (8-21), current integrase strand transfer inhibitor (INSTI)-based regimen in 15 participants and non-INSTI regimen in 6 participants, and a CD4 count of 864/mm3 (560-1066). The age of controls was 43 y (37-51), with 78.9% females and a BMI of 39.2 kg/m2 (36.3-42.6). Anthropometric characteristics, comorbidities, and cardiovascular parameters did not differ according to HIV status and INSTI treatment. The number of macrophage crown-like structures in SCAT was lower in INSTI-treated participants than in HIV-uninfected participants (P = 0.02) and non-INSTI-treated HIV-infected subjects (P = 0.07). Hepatic steatosis and liver disease severity global score were lower in INSTI-treated participants than in non-INSTI-treated HIV-infected participants (P = 0.05 and P = 0.04, respectively).HIV-infected and HIV-uninfected subjects undergoing bariatric surgery presented a similar profile regarding anthropometric measures, cardiovascular parameters, and comorbidities. However, INSTI-treated participants presented milder SCAT and liver alterations than non-INSTI-treated participants.

Objectif : Au CHU d'Angers, hopital pavillonnaire, les transports intrahospitaliers medicalises, dits « tertiaires », reposent sur le Smur du SAMU 49. Cette organisation semble provoquer des evenements indesirables organisationnels (EIO) car le medecin de l'equipe ne peut assurer ni une autre intervention urgente, ni un renfort en regulation. L'objectif de cette etude est donc d'objectiver le retentissement reel d'une telle organisation.Materiel et methodes : Nous avons realise une etude observationnelle prospective de 120 jours ou etaient releves les EIO et les periodes sans renfort en regulation possible. Les EIO etaient definis par les carences, les retards d'engagement et les desengagements d'interventions Smur. Le critere de jugement principal etait la comparaison de frequence de survenue des EIO entre les periodes avec ou sans tertiaire en cours. Le critere de jugement secondaire etait la comparaison de frequence d'impossibilite de renfort en regulation entre les periodes avec ou sans tertiaire en cours.Resultats : Nous avons releve 156 EIO. La survenue d'EIO etait significativement plus elevee en periode avec tertiaire en cours (5,3 %) comparativement a celle sans tertiaire (1 %) (p : La realisation des transports tertiaires par le Smur du Samu 49 impacte directement son organisation et son activite. Au decours de cette evaluation, des mesures correctives ont ete mises en place dont l'efficacite devra etre evaluee.

To elucidate the catalytic action of Zn(II) in the exchange reactions taking place in epoxy-acid vitrimers, extended X-ray absorption fine structure (EXAFS) spectroscopy has been used as well as infrared (IR) spectroscopy on vitrimers. The presence of Zn(II) in vitrimers is revealed by characteristic alterations in both the hydroxide groups and the ester carbonyl bond structures. An EXAFS analysis showed that the zinc cation is coordinated by four oxygen atoms, two of them being negatively charged, i.e. alkoxide groups to satisfy the electroneutrality, whereas IR analysis demonstrated the lengthening of the ester carbonyl bond. The catalytic effect of Zn2+ can be stated as follows: (1) shifting the alcohol/alkoxide equilibrium towards the more nucleophilic alkoxide form; (2) activating the carbonyl bonds by increasing the electrophilicity of the carbon atom; (3) bringing reacting centres close to each other. Zn2+ coordinated in the epoxy-based network therefore appears as a good option to catalyze exchange reactions by transesterification and afford vitrimer properties.

Acrylate formulations can spontaneously generate surface patterns by UV-curing in open-air. This phenomenon, designated as self-patterning, occurs if there is a balance, at the film surface, between oxygen inhibition and polymerization rates. Indeed, to counterbalance the continuous oxygen diffusion at the film surface, polymerization kinetics has to be very fast. It can be achieved using acrylate monomers which are characterized by very high propagation rate constants. However, if the polymerization kinetics is too fast (for example using very high UV intensities) oxygen inhibition is strongly reduced at the surface, preventing self-patterning. Finally, highly ordered and long-range morphologies, such as hexagons, peanuts or lamellae, can be obtained. Their type and size can be tuned with easy-to-handle parameters such as the crosslinker concentration and the film thickness.

Thymine-functionalized telechelic poly(propylene glycol) (PPG) chains of 460–4000 g mol−1 present a long-range ordered lamellar microphase separation due to thymine crystallization. This organization is lost if the difference of polarity between the H-bonding units and the chain is not sufficiently high to induce segregation, as with a poly(ethylene glycol) (PEG) chain. Moreover, the mesoscopic order is gradually lost with the longest PPG chains i.e. with the smallest volume fractions of supramolecular units limiting their clusterization/crystallization. Methylated thymine (MeThy) end-functionalized PPG presents the same lamellar mesoscopic organization, proving that Thy/Thy H-bonding self-association does not drive their crystallization. Infrared spectroscopy evidences that the amide functions linking MeThy motifs to PPG chains interact with each other through H-bonds, forming either a random or an aligned pattern, just like protein secondary structures. On cooling from the melt, disordered H-bonded amides align and pre-order the supramolecular units just prior to the disorder–order transition driven by the MeThy crystallization. The close packing, necessary to induce crystallization, is no longer possible with bulky neo-pentyl functionalized thymines (tBuCH2Thy), preventing mesoscopic organization. Hence, the pattern formed by H-bonded amide links plays a major role in the long-range organization of the telechelic supramolecular polymer in helping supramolecular units to crystallize. Indeed, replacing the amide by an ester link suppresses their crystallization and thus the mesoscopic order.

Since its co-receptors were discovered, HIV tropism is defined by the type of co-receptor used to infect its host cells : R5 for viruses using only CCR5, X4 for viruses using only CXCR4, and R5/X4 or dual for viruses using either CCR5 or CXCR4. Tropism prediction actually made is via recombinant phenotypic assays, which are labour-intensive, long-lasting and expensive, available in only a few highly qualified laboratories. Prescription of CCR5-antagonists requires screening for the presence of X4 variants before starting therapy. With development of this new class of antiretrovirals there is a need for genotypic tests, based on V3 loop gp 120 env gene sequencing and its interpretation with an algorithm. As for resistance tests 10 years ago, these genotypic tests will be easier to introduce into routine clinical practice. These genotypic tropism assays will help to select patients before CCR5-antagonist prescription and to follow patients treated by this class of antiretrovirals, looking for a potential switch of tropism during therapy.

This paper introduces a generic and opened multi-agent platform that provides a powerful scientific equipment for collective research on self-organized systems. A general thought on the mutation of object model towards the agent model is presented. The paper details the construction of the platform upon generic models of environment and agent. Then we present the extension of the toolkit to web-based technologies, and its suitability for collective research and remote works.

Resume En Europe, la transmission verticale du virus de l'hepatite C est asymptomatique, et se produirait dans 0 a 25 % des cas avec viremie maternelle. La seule mesure preventive est de deconseiller l'allaitement. La faible charge virale, l'absence de replication ancienne et la non-integration genomique sont des conditions favorables a un essai de sero-prophylaxie chez le nouveau-ne par des anticorps neutralisants d'origine humaine. Les obstacles a un essai randomise et controle sont nombreux : - Epidemiologiques : rarete du recrutement; risque faible; transmission in utero possible par les meres les plus viremiques; risque de contamination horizontale du nourrisson au foyer. - Methodologiques : randomisation complexe des groupes (genotypes, PCR quantitative, hepatite maternelle, modes d'accouchement et d'allaitement). - Ethiques : benefice individuel direct non etabli; l'allaitement par des meres viremiques serait-il autorise en l'absence de seroprophylaxie ? Risque de fvoriser l'emergence de variants viraux chez le nourrisson; origine et selection des plasmas-source.

To evaluate the contribution of CD4 T cells from blood and gut compartments to the HIV-1 reservoir, we directly quantified cell-associated HIV DNA in isolated rectal (R-) and peripheral blood (PB-) memory CD4 T cells from 11 successfully long-term treated patients. Proportion of activated (CD25(+); CD69(+); and HLA-DR(+)) and CCR5 expressing CD4 T cells were markedly higher in rectal tissue compared with blood. However, HIV-1 infection levels of R- and PB-memory CD4 T cells did not significantly differ (medians: 4000 and 2100 copies per million cells) after effective long-term viral control, suggesting that each of these 2 compartments does not contribute in a similar fashion to the total HIV reservoir.

International audience; Abstract. Natural hazards, due to climate change, are particularly damaging in urban areas because of interdependencies of their networks. So, urban resilience has to face up to climate risks. The most impacting phenomenon is the urban heat island (UHI) effect. The storage capacity of heat is depending on shapes of buildings, public spaces, spatial organization, transport or even industrial activities. So, adaptive strategies for improving urban climate could be possible in different ways. In the framework of the French project Resilis, this study characterises urban vulnerability and resilience in terms of energy needs of buildings and outside urban comfort according to the IPCC carbon dioxide emission scenarios B2 and A2 for the period 2050–2100 for 10 French cities. The evolutions of four climate indicators in terms of heating and cooling needs and number of hours when the temperature is above 28 °C are then obtained for each city to analyse climate risks and their impacts in urban environment.

One or both of two co-receptors, CCR5 (R5) and CXCR4 (X4), are used by HIV-1 to enter into host cells. The glycoprotein 120 (gp120) V3 sequence is correlated with the R5 and X4 phenotype. CCR5 inhibitors are specifically active against R5 viruses, suggesting the need to determine tropism before the use of these antagonists. A comparison of the position-specific scoring matrices (PSSM) and Geno2pheno algorithms based on the V3 loop gp120 sequences and previously described to be correlated to the R5 or X4 phenotype was carried out.V3 envelope (env) genes from 83 plasma samples were amplified and sequenced, and 69 sequences were analysed with the PSSM and Geno2pheno algorithms.These two algorithms were concordant in 86.5% of cases. The Geno2pheno algorithm gave a tropism result more frequently than the PSSM algorithm, but R5X4 or X4 viruses were less frequently detected by the Geno2pheno algorithm. R5X4 or X4 tropism was predicted in 29.9% of samples. There was more R5X4 co-receptor use in the antiretroviral-treated group than in the antiretroviral-naïve group.It is advisable to run a validated co-receptor use prediction tool before using co-receptor antagonists. If genotyping methods are considered, the PSSM and Geno2pheno algorithms are complementary and both are necessary. The association between predicted co-receptor use and virological response to co-receptor antagonists needs to be thoroughly evaluated.

To assess the peak systolic velocity in the middle cerebral artery (PSV-MCA) in the prediction of fetal anemia in case of severe red-cell alloimmunization.A prospective study, from January 2003 to April 2006, of 47 consecutive pregnancies with severe alloimmunization. Fetal surveillance was based on titration and dosage of antibodies, ultrasound scans, and doppler for PSV-MCA measurement up to twice a week. A fetal blood sampling and in utero transfusion was performed in case of increase in PSV-MCA above 1.5 multiples of the median (MoM), and/or signs of hydrops on ultrasound. Severe fetal anemia was defined by fetal hemoglobin below 0.55MoM for gestational age. Analyses performed included the correlation between PSV-MCA and fetal hemoglobin, the value of PSV-MCA in the prediction of severe fetal anemia, and the determination of adequate threshold for intervention based on ROC curve analysis.Four hundred and eighty-five PSV-MCA were performed in 47 high-risk pregnancies, of which 125 were coupled with hemoglobin measurement by fetal blood sampling. There is a significant negative correlation between PSV-MCA and fetal hemoglobin (R2=0.6545 ; p0.0001). Based on all prospective data, the negative predictive value of PSV-MCA was 97.8 %, sensitivity was 86.7 %, with a false positive rate of 12.2%. Area under the ROC curve was 0.85 (IC 95 %, 0.742-0.927 ; p0.0001), suggesting an excellent value of this test. When switching the threshold for intervention from 1.5 to 1.6MoM, the positive predictive value increased, without decrease in sensitivity or negative predictive value.This study confirms the correlation between PSV-MCA and fetal hemoglobin. It allows a decrease of invasive procedures in the follow-up of pregnancies with severe red-cell alloimmunization.

There are numerous studies describing the neuroprotective effects of Ginkgo biloba extract EGb 761 on patients with disturbances of vigilance, memory and cognitive functions associated with aging and senility. Describing the pattern of gene expression in EGb 761-treated human hNT neurons may elucidate the molecular pathways leading to the neuroprotection. We used cDNA macroarrays including genes implicated in the antioxidant and stress responses to define the transcriptional effects of EGb 761 (250 microg/ml, 24 hr) on human hNT neurons. Seven genes were identified whose expression was strongly modified by the EGb 761 treatment. Three groups are distinguished: genes encoding transcription factors (increase of NF-kappaB p65 subunit and zinc finger protein 91 mRNAs, and decrease of c-myc transcripts), genes involved in antioxidant defenses (increase of the CuZn SOD mRNAs, and decrease of glutathione reductase and glutathione S-transferase pi mRNAs) and genes involved in stress responses (up-regulation of HSP70 transcripts). Consistent with the modulation of mRNAs by EGb 761, the enzymatic activities of glutathione reductase and glutathione S-transferase were decreased. Surprisingly, CuZn SOD activity was decreased despite increased abundance of the mRNAs; furthermore MnSOD activity was unmodified, and thus the effect of EGb 761 was specific to CuZn SOD. These results support the idea that modulation of target genes and transcription factors may be involved in the neuroprotective action of EGb 761.

The necessity of saving blood products has established the practice of exchange transfusion (ET) with reconstituted blood in newborns. The aim of this retrospective study was to evaluate the indications and the practice of this technique at the Perinatal Hemobiology Centre (Paris, France).The records of intervention allowed us to review the etiologic categories for neonates having undergone exchange transfusion with reconstituted blood, the dosages used (bilirubin, hemoglobin), and the other main parameters of ET.Sixty ETs were performed in 48 newborns between the 1st July 1996 and the 1st July 1998. Twenty-seven with Rh hemolytic disease had 39 ETs (19 for hyperbilirubinemia, 12 for anemia, and eight for both), whereas ten out of 12 repeated ETs were indicated for hyperbilirubinemia (six of these cases were in newborns weighingor = 2500 g and after a volume exchangeor = 1 blood mass [range 0.72-1.0] at the last ET). Twenty-one cases showed other diseases: six of them had anemia, nine had hyperbilirubinemia, and seven showed disseminated coagulopathy. The tolerance of ET was poor in 24% infants in this group.The volume of 1.3 blood mass for ET is sufficient for the majority of cases with hyperbilirubinemia, allowing transfusional savings in comparison with the previous recommendation of two blood volumes. Exact labeling of the content of units of packed red cells and plasma is essential to fulfill the volume and hematocrit requirements in every case.

Vertical transmission of the HCV infection is asymptomatic, occurring in 0-25% of infants born to viremic mothers in Europe. Nowadays, the only preventive measure in to advise against breastfeeding. Favourable conditions for a seroprophylaxis trial in neonates at risk are the low viral charge and the absence of former replication or integration. Several impediments to a randomized-controlled trial should be considered: Epidemiological: paucity of recruitment; low risk of transmission; possible antenatal transmission in cases of high maternal viremia; risk for intrafamilial transmission. Methodological: complex randomization of the study groups (genotyping, quantitative PCR, activity of mothers' diseases, modes/durations of delivery and feeding). Ethical: a direct individual benefit is not clearly established; should viremic mothers be allowed to breast feed their babies in the absence of prophylaxis? the risk of the emergence of mutants or quasi-species of the transmitted hepatitis C virus in neonates; the origin and selection of seropositive plasma donors.

The review of 15 cases of cardiac involvement demonstrate the cardiac tropism of the human parvovirus B19. Ten of these cases were collected from fetuses during second trimester of maternal-fetal infections. In situ hybridisation detects viral DNA sequences in the nucleus of infected myoblasts. Myocarditis is the most frequent histological damage. Cardiac failure, secondary to myocarditis, may occur in the absence of fetal anaemia. When the fetus is deeply anaemic, like usually in cases of hydrops, damages of the cardiac tissues might hamper the reactional increase of cardiac output; therefore, they might account for the poor prognosis of parvovirus B19 fetal hydrops in the second trimester of pregnancy, despite transfusional therapy attempts in the third trimester.

The diagnosis of the risk for vertical infection with the virus of hepatitis C rests on the finding of specific antibodies in the mother. It is rare to see clinical signs of infection in the newborn. More frequently asymptomatic transmission of the viral genome occurs. As it is important to watch newborn infants at risk it is recommended that pregnant women who have a risk factor should have serum screening.

HBsAg was detected in 152 pregnant women among 6,605 (2.3%) screened in the prenatal clinics of four hospitals representative of the Paris metropolitan area. In 98% of cases, HBsAg positivity indicated chronic HBV carrier status. Among patients born out of continental France (47% of screened women, 79% of positive women) relative risk of chronic infection was 6 in Asians, 5.5 in Africans, and 4 in French women born in non-continental France. No significant difference in medical history was seen between HBsAg-positive and HBsAg-negative patients, in any of the birthplace groups. In women born out of continental France, number of children and crowding in the home were correlated with HBsAg-positivity; these correlations were not found in French women born in continental France. In non-African, non-Asian women, screening on the basis of medical, social and familial criteria (simulated in this study) would not be effective. Routine screening for HBsAg in pregnancy is advocated. The cost of the prevention of each case of perinatally acquired chronic HBV infection by routine screening followed by prophylactic treatment of a risk neonates was estimated at 180,000 French Francs (35,000 dollars). This approach is the only means of preventing the long-term life-threatening complications of chronic HBV infection in the 600 neonates born each year in France to HBsAg-positive mothers.

The HBs antigen (AgHBs) was detected in 152 out of 6605 (2.3 percent) pregnant women who attended four representative maternity clinics in the Paris region. In 98 percent of the cases this finding reflected chronic hepatitis B virus infection. Among women born outside France (47 percent of the women tested, 79 percent of the AgHBs positive women), the relative risk was 6 for Asiatics, 5.5 for Africans and 4 for French women born in overseas departments or territories. Whatever the women's geographical origin, studies of their medical history revealed no significant difference between AgHBs positivity and AgHBs negativity. Overcrowding and multiparity correlated globally with the presence of AgHBs, but this correlation was absent in French women born in France. In non-African and non-Asiatic women detection guided by medical and socio-familial criteria would not be efficacious. The authors recommend systematic detection of AgHBs in pregnant women and estimate at about 180,000 french francs the cost of prevention for each case evolving toward the vital complications of chronic hepatitis B virus infection, an outcome which in the long term may affect 600 individuals born each year and who had contracted the infection during the perinatal period.

A study to evaluate the safety and immunogenicity of a recombinant hepatitis B vaccine containing the S and pre-S2 antigens (GenHevac B Pasteur) was conducted in healthy newborn infants. All infants received 20 micrograms of vaccine within 24 hours after birth, at 1 and 2 months with a booster injection at month 12. The vaccine was administered alone in 19 infants born to low risk mothers, i.e. surface antigen (HBsAg)-negative and antibody to the core antigen (Anti-HBc)-positive mothers. The vaccine was administered in combination with 100 IU hepatitis B immune globulin (HBIg) at birth and one month in 18 infants born to high risk mothers, i.e. HBsAg positive mothers. In the group not receiving HBIg, the anti-HBs seroconversion rate at the 10 mlU/ml threshold was 50% one month after the first injection. In both groups, the anti-HBs seroconversion rates were 100% one month after the third injection and above 85% one month after the second injection. After booster injection, more than 90% of the infants had an anti-HBs titre above 1,000 mlU/ml that will probably provide them with adequate protection for several years. The kinetics of the anti-pre-S2 response was similar to those of the anti-HBs response and 100% infants in both groups had seroconverted one month after the second injection of the vaccine. The side effects were scarce, all mild and transient. Given the protective role of the anti-pre-S2 antibodies, the precocity of the response to pre-S2 antigen may therefore increase the effectiveness of hepatitis vaccination.(ABSTRACT TRUNCATED AT 250 WORDS)

HBs Ag carrier rate among pregnant women is 1.5% in our hospital near Paris, six times higher than French national prevalence among women. 78% of them are of extra-metropolitan origin (especially from the African continent), 80% have a low socioeconomic status. Their newborns are at risk of "mother-to-infant" transmission of the virus HB. The risks, mechanisms and consequences of such a transmission are studied. For these reasons, it seems reasonable to perform routine prenatal testing for HBs Ag carriage. We have successively carried out two prophylactic methods. From June 1980 to October 1982, prevention with specific immunoglobulins (HBIG) was used: ten infants received at birth 0.5 ml, and 0.1 ml/kg at 1, 2, 4, 6 months of age. At 6 months, they all were safe of liver damage, HBs Ag and HBe Ag negative. Eleven infants received only the first HBIG injection. From October 1982 to September 1983, fifteen carrier women were identified at the time of delivery, and all accepted the sero-vaccination procedure for the newborn: HBIG (1 ml) and vaccine before 48 hours of life and at one month, vaccine alone at two months. The babies were controlled (9 at 4 months, 3 at 12 months) and all were safe of liver damage, HBs Ag and HBe Ag negative, with HBs antibody mean value of 240 m I.U./ml. We are grateful to the collaboration of the Departments for Mother and Infant Protection in our region--according to the usual chronology of the visits--for these satisfactory performance and results of the sero-vaccination.

From June 1980 to October 1982, prevention with specific immunoglobulins (HBIG) was used: ten infants received at birth 0.5 ml, and 0.1 ml/kg at 1, 2, 4, 6 months of age. At 6 months, they all were safe of liver damage, HBs Ag and HBe Ag negative. Eleven infants received only the first HBIG injection. From October 1982 to September 1983, fifteen carrier women were identified at the time of delivery, and all accepted the sero-vaccination procedure for the newborn: HBIG (1 ml) and vaccine before 48 hours of life and at one month, vaccine alone at two months. The babies were controlled (9 at 4 months, 3 at 12 months) and all were safe of liver damage, HBs Ag and HBe Ag negative, with HBs antibody mean value of 240 m I.U./ml. We are grateful to the collaboration of the Departments for Mother and Infant Protection in our region--according to the usual chronology of the visits--for these satisfactory performance and results of the sero-vaccination.

The elastic bulk effect was measured during a 2 MeV electron irradiation of pure nickel at 18 K. A value (δM/M)/C = − 70 was obtained. Background: Monoclonal antibodies (mAbs) targeting the spike of SARS-CoV-2 prevent severe COVID-19. Omicron subvariants BQ.1.1 and XBB.1.5 evade neutralization of therapeutic mAbs, leading to recommendations against their use. Yet, the antiviral activities of mAbs in treated patients remain ill-defined., Methods: We investigated neutralization and antibody-dependent cellular cytotoxicity (ADCC) of D614G, BQ.1.1 and XBB.1.5 in 320 sera from 80 immunocompromised patients with mild-to-moderate COVID-19 prospectively treated with mAbs (sotrovimab, n=29; imdevimab/casirivimab, n=34; cilgavimab/tixagevimab, n=4) or anti-protease (nirmatrelvir/ritonavir, n=13). We measured live-virus neutralization titers and quantified ADCC with a reporter assay., Findings: Only Sotrovimab elicits serum neutralization and ADCC against BQ.1.1 and XBB.1.5. As compared to D614G, sotrovimab neutralization titers of BQ.1.1 and XBB.1.5 are reduced (71- and 58-fold, respectively), but ADCC levels are only slightly decreased (1.4- and 1-fold, for BQ.1.1 and XBB.1.5, respectively)., Interpretation: Our results show that sotrovimab is active against BQ.1.1 and XBB.1.5 in treated individuals, suggesting that it may be a valuable therapeutic option., Competing Interests: Declaration of interest T.B. and O.S. have a pending patent application for an anti-RBD mAb not used in this study (PCT/FR2021/070522).
