The National Air Emission Inventory and Research Center (NAIR) has refined emissions estimation methods to enhance the accuracy and reliability of national statistics on air pollutant emissions. The center estimated 2020 national emissions by applying 23 items identified to have been improved from the improvement research and re-estimated the national emissions from 2016 to 2019 to secure the coherence of national annual emissions. This study compares national emissions of the past years before and after the re-estimation and analyzes the major causes of changes in 2020 national emissions compared to those of 2019. The re-estimation of national emissions from 2016 to 2019 revealed the following change rates for each substance: CO, − 5.5 to 5.8%; NOx, − 3.9 to 1.8%; SOx, − 14.7 to − 0.6%; PM2.5, − 31.5 to − 27.0%; VOCs, − 1.3 to 1.1%; NH3, − 15.4 to 14.3%; and BC, − 4.9% to 5.5%. National air pollutant emissions in 2020 were as follows: CO, 711,399 tons; NOx, 929,227 tons; SOx, 180,157 tons; PM2.5, 58,558 tons; VOCs, 990,629 tons; and NH3, 261,207 tons. It turned out that the year-on-year reduction rates of emissions were as follows: CO, − 5.5%; NOx, − 11.1%; SOx, − 23.9%; PM2.5, − 4.9%; VOCs, − 2.0%; NH3, − 2.9%; and BC, − 11.6%. National statistics on air pollutant emissions serve as basic data for establishing, implementing, and evaluating national atmospheric environment policies to improve air quality. It is necessary to establish effective atmospheric environment policies based on highly accurate emission statistics to improve air quality. To this end, it is imperative to conduct ongoing research including studies on improving the national air pollutant emissions estimation method as well as on verifying emissions using air quality modeling and satellite observation data. In this study, the 2021 National Air Pollutant Emissions Inventory in the Republic of Korea was estimated. Spatially, Gyeonggi-do had the highest emissions of PM-2.5, NOx, and VOCs due to its dense populations and heavy traffic, while Ulsan and Chungcheongnam-do had the highest emissions of SOx and NH3 from production process in their large national industrial complexes. Abstract According to the 2018 National Air Pollutant Emissions Inventory (NEI), air pollutant emissions in the Republic of Korea comprised 808,801 tons of CO, 1,153,265 tons of NOX, 300,979 tons of SOX, 617,481 tons of TSP, 232,993 tons of PM10, 98,388 tons of PM2.5, 15,562 tons of black carbon (BC), 1,035,636 tons of VOCs, and 315,975 tons of NH3. As for national emission contributions to primary PM2.5 and PM precursors (NOX, SOX, VOCs, and NH3), major source categories were the road sector for NOX, the industry sector for SOX and PM2.5, and the everyday activities and others sector for VOCs and NH3. In the case of emissions by region, the largest amount of NOX was emitted from the Seoul Metropolitan Areas (SMA; Seoul, Incheon, and Gyeonggi-do, hereafter SMA) and the largest amounts of SOX, PM2.5, VOCs, and NH3 were from the Yeongnam region. A 3D chemical transport modeling system was used to examine the uncertainty of the national air pollutant emissions based on the National Emission and Air Quality Assessment System (NEAS). Air quality was simulated using CAPSS 2018, and the simulation data were compared with observed concentrations to examine the uncertainties of the current emissions. These data show that emissions from five si (cities) (Pohang, Yeosu, Gwangyang, Dangjin, and Ulsan) need to be improved. Most of all, it is necessary to examine the emissions from places of business that use anthracite, which is the major PM2.5 emission source, as fuel in these areas.

Abstract According to the 2017 National Air Pollutant Emissions Inventory (NEI), air pollutant emissions in the Republic of Korea comprised 817,420 metric tons (hereafter tons) of CO, 1,189,800 tons of NOx, 315,530 tons of SOx, 592,582 tons of TSP, 218,476 tons of PM10, 91,731 tons of PM2.5, 15,555 tons of black carbon (BC), 1,047,585 tons of VOCs, and 308,298 tons of NH3. Emissions of the 13 first-level emission source categories, which constitute the NEI, were estimated and, based on their characteristics, the emission source categories were grouped into five sectors (energy, industry, road, non-road, and everyday activities and others). In addition, the contributions of primary PM2.5 and its four precursors (NOx, SOx, VOCs, and NH3) to the 2017 NEI were assessed in this study. The emission contributions of NOx to the NEI were 36.5% for the road sector, which was the highest of those of all the air pollutants for this sector; NOx emissions for this sector were 4.2% lower than those in the previous year. The emission contributions of SOx and PM2.5 to the NEI were higher than those of the other air pollutants for the industry sector; SOx and PM2.5 emissions for this sector decreased by 9.8% and 19.7%, respectively, compared with those in the previous year. The emission contributions of VOCs and NH3 to the NEI were 65.3% and 83.9% for the everyday activities and others sector, respectively, higher than those of the other air pollutants for this sector; VOCs and NH3 emissions for this sector increased by 0.8% and 2.9%, respectively, compared with those in the previous year. A three-dimensional (3D) chemical transport modeling system was used to validate the emission estimates. These data suggest that simulated SOx emissions were overestimated in areas with dense large-scale industrial complexes, such as Jeollanam-do, Gyeongsangbuk-do, and Ulsan, and that simulated NOx emissions were overestimated in Seoul, Incheon, and Jeollanam-do.

Objectives : This study aims to discuss air quality policy improvement that reflect regional characteristics through analyzing recent PM2.5 concentration, air pollutant emission sources and those contributions to annual PM2.5 concentration in Chungcheong region (Daejeon Metropolitan City, Sejong Metropolitan Autonomous City, the Province of Chungcheongnam-do, and Chungcheongbuk-do) in South Korea. In addition, we identified the characteristics of the PM2.5 pollution at the level of fundamental local government, and demonstrated the number of vulnerable population exposed to high level of PM2.5 concentration in order to propose policy implications in Chungcheong region. Methods : Based on the national emissions estimates (CAPSS: Clean Air Policy Support System) and air quality modelling system, major sectors/sources of air pollutants emission and national contributions of PM2.5 concentrations in Chungcheong region were analyzed. Furthermore, the study identified the number of people exposed to the higher PM2.5 concentrations (>25 μg/m3) by the measurement data and demographics available in 2019. Results and Discussion : The national air pollutants emissions in Chungcheong region were emitted from Chungnam (about 59% of NOx emission volume, 89% of SOx, 70% of NH3, 54% of VOCs, 79% of PM2.5, and 68% of TSP respectively), mainly from industry, domestic, energy, and road sector. According to the results of the air quality modelling, Chungcheong region also had the largest contribution on the average annual PM2.5 concentration in South Korea (27%). Chungnam emitted the largest emission volume of air pollutants, mainly from industry and power generation sectors (especially in Dangjin, Seosan, and Boryeong), while Asan, Yesan, Hongseong, and Cheongyang were classified as the areas with higher PM2.5 concentrations (>25 μg/m3), showing a gap between the areas with large emission volume and high concentration. Chungbuk and Sejong had higher annual PM2.5 concentration due to the influence of external sources and their geographical characteristics. The largest vulnerable population (over 65 years old and under 18 years old) exposed to high PM2.5 concentrations annually lived in Cheongju. Chungbuk had about 40% more air pollutant emission volume than Chungnam, but about 17% more vulnerable population. Conclusions : At the current stage of “master plan” in Chungcheong region, it is important to mitigate air pollutants emissions on the basis of the local emissions characteristic at the level of fundamental local government (such as industry sector in Dangjin, Seosan, and Danyang/ Domestic buring in Cheongju, Cheonan, and Daejeon/ power generation in Boryeong, Taean and Dangjin/ road in Daejeon, Cheongju, and Cheoan). In addition, Chungbuk requires management of the areas with higher PM2.5 concentration such as Goesan, Boeun, Okcheon, and Yeongdong located outside “air control zone”. To reduce high level of PM2.5 concentration in Chungcheong region, cooperation with neighboring local governments such as Gyeonggi Province is crucial, and policy solutions are needed between the stakeholders to resolve the disparity issues between areas with larger emission volume and higher PM2.5 concentration.

In 2016, air pollutant emissions in the Republic of Korea were 795,044 metric tons (hereafter tons) of CO, 1,248,309 tons of NOx, 358,951 tons of SOx , 611,539 tons of TSP, 233,085 tons of PM10, 100,247 tons of PM2.5, 16,401 tons of BC, 1,024,029 tons of VOCs, and 301,301 tons of NH3. Including energy production, thirteen emission sources, which comprise the national air pollutant emission inventory, were classified by their characteristics into five sectors (Energy, Industry, Road, Non-road, and Everyday Activities and Other Emission Sources) to analyze their relative contributions to the national emissions. Specifically, their contributions by pollutant were as follows: NOx (11.0%), SOx (21.9%), PM2.5 (3.2%), VOCs (0.8%), NH3 (0.5%) from the energy sector; NOx (20.2%), SOx (59.7%), PM2.5 (42.1%), VOCs (24.3%), and NH3 (14.4%) from the industry sector; NOx (36.3%), SOx (0.1%), PM2.5 (9.7%), VOCs (4.6%), and NH3 (1.7%) from the road sector; NOx (24.8%), SOx (11.5%), PM2.5 (14.3%), VOCs (4.0%), and NH3 (0.04%) from the non-road sector; and NOx (7.6%), SOx (6.7%), PM2.5 (30.6%), VOCs (66.3%), and NH3 (83.4%) from the everyday activities and other emission sources sector. The data we calculate are used as official national emissions data for the establishment, implementation, and assessment of national atmospheric environment policy to improve air quality. As critical and necessary materials, the data are also utilized on a wide range of studies on policies such as customized regional particulate matter reduction measures. Thus, it is crucial to estimate highly reliable national emissions by enhancing the emissions factors and inventory and to establish a scientific emissions testing system by using air quality modeling and satellite data.

Background: With high-efficient water-use and drought tolerance, broomcorn millet has emerged as a candidate for food security. To promote its research process for molecular breeding and functional research, a comprehensive genome resource is of great importance. Results: Herein, we constructed a BAC library for broomcorn millet, generated BAC end sequences based on the clone-array pooled shotgun sequencing strategy and Illumina sequencing technology, and integrated BAC clones into genome by a novel pipeline for BAC end profiling. The BAC library consisted of 76,023 clones with an average insert length of 123.48 Kb, covering about 9.9-fold of the 850 Mb genome. Of 9216 clones tested using our pipeline, 8262 clones were mapped on the broomcorn millet cultivar longmi4 genome. These mapped clones covered 308 of the 829 gaps left by the genome. To our knowledge, this is the only BAC resource for broomcorn millet. Conclusions: We constructed a high-quality BAC libraray for broomcorn millet and designed a novel pipeline for BAC end profiling. BAC clones can be browsed and obtained from our website (http://eightstarsbio.com/gresource/JBrowse-1.16.5/index.html). The high-quality BAC clones mapped on genome in this study will provide a powerful genomic resource for genome gap filling, complex segment sequencing, FISH, functional research and genetic engineering of broomcorn millet. In 2015, air pollutant emissions in the Republic of Korea were 792,776 metric tons of CO, 1,157,728 metric tons of NOx, 352,292 metric tons of SOx, 604,243 metric tons of TSP, 233,177 metric tons of PM10, 98,806 tons of PM2.5, 15,934 metric tons of BC, 1,010,771 metric tons of VOCs, and 297,167 metric tons of NH3. Among major emission source categories, the main emission sources and the contributions to emissions, by pollutant, were as follows: road transport (31.0%), biomass burning (29.3%), and non-road transport (17.1%) for CO; road transport (31.9%), non-road transport (26.3%), and manufacturing industry (14.6%) for NOx; industrial processes (29.9%), energy production (25.9%), and manufacturing industry (24.2%) for SOx; fugitive dust (67.6%) manufacturing industry (20.1%) for TSP; fugitive dust (47.0%) and manufacturing industry (30.4%) for PM10; manufacturing industry (36.8%), fugitive dust (17.5%), and non-road transport (14.3%) for PM2.5; road transport (42.0%) and non-road transport (39.6%) for BC; solvent use (54.9%) and industrial processes (18.1%) for VOCs; and agriculture (77.8%) and industrial processes (13.3%) for NH3. The data we calculate can be used as official national emissions data for the establishment, implementation, and assessment of air quality-related policy, such as measures to deal with particulate matter, as well as for related modeling and other research.

Abstract Background Health services have not provided adequate support for young people with long term health conditions to transfer from child to adult services. National Institute of Health and Care (NICE) guidance on transition has been issued to address these gaps. However, data are often sparse about the number of young adults who might need to transition. Using Attention Deficit Hyperactivity Disorder (ADHD) as an exemplar, this study used an existing surveillance system and a case note review to capture the incidence of the transition process, and compared and contrasted the findings. Methods The Child and Adolescent Psychiatry Surveillance System (CAPSS) was used to estimate the incident transition of young people with Attention Deficit Hyperactivity Disorder (ADHD) from child to adult services. This involves consultant child and adolescent psychiatrists from the United Kingdom (UK) and Republic of Ireland (ROI) reporting relevant young people as they are seen in clinics. In parallel, a case note review was conducted using the Maudsley Biomedical Research Centre (BRC) Clinical Records Interactive Search (CRIS). The study period ran for twelve months with a nine month follow up to see how the transition proceeded. Results CRIS identified 76 cases in the study period, compared to 18 identified using surveillance via CAPSS. Methodological issues were experienced using both methods. Surveillance issues; eligibility criteria confusion, reporting errors, incomplete questionnaires, difficulties contacting clinicians, and surveillance systems do not cover non-doctors and psychiatrists who are not consultants. Case note review issues using CRIS included the need for researchers to interpret clinical notes, the availability and completeness of data in the notes, and data limited to the catchment of one particular mental health trust. Conclusions Both methods demonstrate strengths and weaknesses; the combination of both methods in the absence of strong routinely collected data, allowed a more robust estimate of the level of need for service planning and commissioning.

Background: Evidence suggests that investing in specialist eating disorders services for young people with anorexia nervosa could have important implications for the NHS, with the potential to improve health outcomes and reduce costs through reductions in the number and length of hospital admissions. Objectives: The primary objectives were to evaluate the costs and cost-effectiveness of alternative community-based models of service provision for young people with anorexia nervosa and to model the impact of potential changes to the provision of specialist services. Design: Observational surveillance study using the Child and Adolescent Psychiatry Surveillance System. Setting: Community-based secondary or tertiary child and adolescent mental health services (CAMHS) in the UK and the Republic of Ireland. Participants: A total of 298 young people aged 8–17 years in contact with CAMHS for a first episode of anorexia nervosa in accordance with Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, diagnostic criteria. Interventions: Community-based specialist eating disorders services and generic CAMHS. Main outcome measures: Children’s Global Assessment Scale (CGAS) score (primary outcome) and percentage of median expected body mass index (BMI) for age and sex (%mBMI) (secondary outcome) were assessed at baseline and at 6 and 12 months. Data sources: Data were collected by clinicians from clinical records. Results: Total costs incurred by young people initially assessed in specialist eating disorders services were not significantly different from those incurred by young people initially assessed in generic CAMHS. However, adjustment for baseline covariates resulted in observed differences favouring specialist services (costs were lower, on average) because of the significantly poorer clinical status of the specialist group at baseline. At the 6-month follow-up, mean %mBMI was significantly higher in the specialist group, but no other significant differences in outcomes were evident. Cost-effectiveness analyses suggest that initial assessment in a specialist service has a higher probability of being cost-effective than initial assessment in generic CAMHS, as determined by CGAS score and %mBMI. However, no firm conclusion can be drawn without knowledge of society’s willingness to pay for improvements in these outcomes. Decision modelling did not support the hypothesis that changes to the provision of specialist services would generate savings for the NHS, with results suggesting that cost per 10-point improvement in CGAS score (improvement from one CGAS category to the next) varies little as the percentage of participants taking the specialist or generic pathway is varied. Limitations: Follow-up rates were lower than expected, but the sample was still larger than has been achieved to date in RCTs carried out in this population in the UK, and an exploration of the impact of missing cost and outcome data produced very similar results to those of the main analyses. Conclusions: The results of this study suggest that initial assessment in a specialist eating disorders service for young people with anorexia nervosa may have a higher probability of being cost-effective than initial assessment in generic CAMHS, although the associated uncertainty makes it hard to draw firm conclusions. Although costs and outcomes were similar, young people in specialist services were more severely ill at baseline, suggesting that specialist services were achieving larger clinical effectiveness gains without the need for additional expenditure. The results did not suggest that providing more specialist services would save money for the NHS, given similar costs and outcomes, so decisions about which service type to fund could be made with reference to other factors, such as the preferences of patients and carers. Future work: Data on measures of quality of life capable of generating quality-adjusted life-years are needed to confirm the cost-effectiveness of specialist services. Abstract This paper has aimed to evaluate the air pollution emissions from maritime transportation arrive in/depart from Port of Incheon using two different approaches; fuel-based (Tier 1) and activity-based (Tier 3) methodologies. So far, Tier 1 or 2 level of estimation approaches, based on the fuel consumption provided by national fuel supply statistics, have been reasonably supported for environmentalists and policy makers at national level of emission inventory analysis, even for the global analysis. When considering the local air quality such as in a port area, however, the fuel based approach would not be properly estimating the shipping air emission sector due to the characteristic of maritime transportation. In this study, the fuel-based air emission inventory provided by Clean Air Policy Support System (CAPSS), South Korea was compared and analyzed with the activity-based emission estimation produced by Portal Air Quality Management System (PAQman©). As a result, there discovered a significant gap in the air pollution emissions between fuel-based and activity-based estimations. The findings in this study implies the necessity and significance of local/regional level focused implementation for air quality improvement and regulatory framework especially in a port-city region e.g. Incheon. Although this paper still contains some limitations and assumption described, the results are confident enough to emphasize the application of the activity-based shipping emission estimation approach to discover the potential limitation, which has been unknown and not listed on the national air emission inventory using fuel-based approach. Emissions of ozone precursors (NOx and VOCs) and photochemical ozone creation potentials (POCPs) of VOC emission sources were investigated in the largest port city (i.e., Busan), Korea during the year 2011. This analysis was performed using the Clean Air Policy Support System (CAPSS) national emission inventory provided by the National Institute of Environmental Research (NIER), Korea. For NOx, the emissions from off-road mobile sources in Busan were the most dominant (e.g., 31,202 ton yr-1), accounting for about 60% of the total NOx emissions. The emission from shipping of off-road mobile sources (e.g., 24,922 ton yr-1) was a major contributor to their total emissions, amounting to 47% of the total NOx emissions due to the port-related activities in Busan. For VOCs, the emission source category of solvent usage was predominant (e.g., 36,062 ton yr-1), accounting for approximately 82% of the total VOC emissions. Out of solvent usages, the emission from painting was the most dominant (22,733 ton yr-1), comprising 52% of the total emissions from solvent usages. The most dominant VOC species emitted from their sources in Busan was toluene, followed by xylene, butane, ethylbenzene, n-butanol, isopropyl alcohol, and propane. The major emission sources of toluene and xylene were found to be painting of coil coating and ship building, respectively. The value of POCP for the off-road mobile source (61) was the highest in ten major activity sectors of VOC emissions. Since the POCP value of ship transport of off-road mobile source (72) was also high enough to affect ozone concentration, the ship emission can play a significant role in ozone production of the port city like Busan.

Korea has experienced dramatic development and has become highly industrialized and urbanized during the past 40 years, which has resulted in rapid economic growth. Due to the industrialization and urbanization, however, air pollutant emission sources have increased substantially. Rapid increases in emission sources have caused Korea to suffer from serious air pollution. An air pollutant emissions inventory is one set of essential data to help policymakers understand the current status of air pollution levels, to establish air pollution control policies and to analyze the impacts of implementation of policies, as well as for air quality studies. To accurately and realistically estimate administrative district level air pollutant emissions of Korea, we developed a Korean Emissions Inventory System named the Clean Air Policy Support System (CAPSS). In CAPSS, emissions sources are classified into four levels. Emission factors for each classification category are collected from various domestic and international research reports, and the CAPSS utilizes various national, regional and local level statistical data, compiled by approximately 150 Korean organizations. In this paper, we introduced for the first time, a Korean national emissions inventory system and release Korea’s official 2007 air pollutant emissions for five regulated air pollutants.

Emissions inputs for use in air quality modeling of Korea were generated with the emissions inventory data from the National Institute of Environmental Research (NIER), maintained under the Clean Air Policy Support System (CAPSS) database. Source Classification Codes (SCC) in the Korea emissions inventory were adapted to use with the U.S. EPA’s Sparse Matrix Operator Kernel Emissions (SMOKE) by finding the best-matching SMOKE default SCCs for the chemical speciation and temporal allocation. A set of 19 surrogate spatial allocation factors for South Korea were developed utilizing the Multi-scale Integrated Modeling System (MIMS) Spatial Allocator and Korean GIS databases. The mobile and area source emissions data, after temporal allocation, show typical sinusoidal diurnal variations with high peaks during daytime, while point source emissions show weak diurnal variations. The model-ready emissions are speciated for the carbon bond version 4 (CB-4) chemical mechanism. Volatile organic carbon (VOC) emissions from painting related industries in area source category significantly contribute to TOL (Toluene) and XYL (Xylene) emissions. ETH (Ethylene) emissions are largely contributed from point industrial incineration facilities and various mobile sources. On the other hand, a large portion of OLE (Olefin) emissions are speciated from mobile sources in addition to those contributed by the polypropylene industry in point source. It was found that FORM (Formaldehyde) is mostly emitted from petroleum industry and heavy duty diesel vehicles. Chemical speciation of PM2.5 emissions shows that PEC (primary fine elemental carbon) and POA (primary fine organic aerosol) are the most abundant species from diesel and gasoline vehicles. To reduce uncertainties in processing the Korea emission inventory due to the mapping of Korean SCCs to those of U.S., it would be practical to develop and use domestic source profiles for the top 10 SCCs for area and point sources and top 5 SCCs for on-road mobile sources when VOC emissions from the sources are more than 90% of the total.

Emissions of ozone precursors (NOx and VOCs) and photochemical ozone creation potentials (POCPs) of VOC emission sources were investigated in the largest port city (i.e., Busan), Korea during the year 2011. This analysis was performed using the Clean Air Policy Support System (CAPSS) national emission inventory provided by the National Institute of Environmental Research (NIER), Korea. For NOx, the emissions from off-road mobile sources in Busan were the most dominant (e.g., 31,202 ton yr-1), accounting for about 60% of the total NOx emissions. The emission from shipping of off-road mobile sources (e.g., 24,922 ton yr-1) was a major contributor to their total emissions, amounting to 47% of the total NOx emissions due to the port-related activities in Busan. For VOCs, the emission source category of solvent usage was predominant (e.g., 36,062 ton yr-1), accounting for approximately 82% of the total VOC emissions. Out of solvent usages, the emission from painting was the most dominant (22,733 ton yr-1), comprising 52% of the total emissions from solvent usages. The most dominant VOC species emitted from their sources in Busan was toluene, followed by xylene, butane, ethylbenzene, n-butanol, isopropyl alcohol, and propane. The major emission sources of toluene and xylene were found to be painting of coil coating and ship building, respectively. The value of POCP for the offroad mobile source (61) was the highest in ten major activity sectors of VOC emissions. Since the POCP value of ship transport of off-road mobile source (72) was also high enough to affect ozone concentration, the ship emission can play a significant role in ozone production of the port city like Busan. Abstract: In this study, the emission inventories of NO x , which is a major air pollutant of South Korea were compared and analyzed. The two bottom-up emission inventories, Clean Air Policy Support System (CAPSS) and Regional Emission inventory in ASia (REAS), which are the latest emission inventories about the air pollutant emissions about South Korea were compared to find out the trend of NO x emission during 1996–2005. Also, these two emission inventories were compared with the top down NO x emissions estimated from satellite observations to validate the amount of NO x emitted from South Korea. The total NO x emission trends, sectoral and regional comparisons were carried out. The trend of the top down estimated NO x emission was similar to CAPSS and REAS. However, the magnitudes of the top down estimated NO x emission were usually closer to those of CAPSS than those of REAS. The NO x emissions from transportation sector of REAS were larger than that of CAPSS, and this corresponded to the difference of total amount of NO x emission between CAPSS and REAS. By comparing the differences of the ratios of the vehicle kilometers traveled (VKT) and emission factors (EFs), it was identified that most of the difference between CAPSS and REAS was due to these factors for diesel vehicles in REAS. Columnar alteration with prominent snouts and secretions (CAPSS) is routinely described on breast core needle biopsies (CNBs); however, its significance and appropriate treatment is unknown. This study evaluated the incidence of cancer (in situ/invasive) in core biopsies and in subsequent surgical biopsy specimens after an initial core biopsy identifying CAPSS.Using our medical center''s pathology database, we retrospectively identified lesions described as CAPSS on breast core needle biopsies performed between January 1998 and August 2003. The specimens were subdivided into CAPSS lesions with and without atypical features. We also identified lesions described as atypical ductal hyperplasia.Overall, 10 cancers were associated with 135 (7.4%) CAPSS lesions without atypia, and 11 (18.3%) were associated with 60 CAPSS lesions with atypia (P = 0.023). CAPSS lesions with atypical features had a slightly higher rate of coexisting cancer on initial biopsy than CAPSS without atypical features (7% vs 12%, P = 0.320). Specimens showing atypical ductal hyperplasia on initial biopsy had a greater rate of coexisting cancer than CAPSS with or without atypical features (P <0.0001).We support the existing recommendation that a patient with a CNB showing CAPSS with atypical features undergo surgical biopsy. Columnar cell lesions (CCL) of the breast range from the innocuous CAPSS (Columnar alteration with prominent apical snouts and secretions) to DCIS. Their precancerous potential poses diagnostic and therapeutic riddles, especially for those with mammographic microcalcifications. To document the incidence of columnar cell lesions in the breast, to study their morphologic spectrum and to study the significance of their association with benign and malignant breast lesions. We studies 100 consecutive breast specimens. These specimens included biopsies, lumpectomies and mastectomies. Columnar cell lesions were classified as per Schnitt criteria. Associated lesions in the adjacent tissue were also studied. Of the 100 samples (43 malignant and 57 benign), 37 cases showed CCL: CAPSS 23, columnar cell hyperplasia-11 and Columnar cell hyperplasia with atypia-3. The commonest associated lesion was fibrocystic change (n=54). Their incidence in benign and malignant lesions was 42% and 30% respectively, with no statistical significance in the difference. Lesions with atypia were seen only in malignant cases. Columnar cell lesions are common, and often overlooked. These lesions are seen in association with both benign and malignant breast lesions. It is only the presence of atypia in CCL that warrants a special mention and necessitates follow up, and resection if indicated. [ABSTRACT FROM AUTHOR]

Columnar cell alteration in the breast encompasses a spectrum of pathologic changes ranging from simple columnar cell change to more complex columnar cell hyperplasia with and without atypia to in situ carcinoma, often with a micropapillary architecture. For reasons that remain unclear, the columnar cell lesions are associated with tubular carcinomas and lobular carcinoma in situ. Background: Health services have not provided adequate support for young people with long term health conditions to transfer from child to adult services. National Institute of Health and Care (NICE) guidance on transition has been issued to address these gaps. However, data are often sparse about the number of young adults who might need to transition. Using Attention Deficit Hyperactivity Disorder (ADHD) as an exemplar, this study used an existing surveillance system and a case note review to capture the incidence of the transition process, and compared and contrasted the findings.Methods: The Child and Adolescent Psychiatry Surveillance System (CAPSS) was used to estimate the incident transition of young people with Attention Deficit Hyperactivity Disorder (ADHD) from child to adult services. This involves consultant child and adolescent psychiatrists from the United Kingdom (UK) and Republic of Ireland (ROI) reporting relevant young people as they are seen in clinics. In parallel, a case note review was conducted using the Maudsley Biomedical Research Centre (BRC) Clinical Records Interactive Search (CRIS). The study period ran for twelve months with a nine month follow up to see how the transition proceeded.Results: CRIS identified 76 cases in the study period, compared to 18 identified using surveillance via CAPSS. Methodological issues were experienced using both methods. Surveillance issues; eligibility criteria confusion, reporting errors, incomplete questionnaires, difficulties contacting clinicians, and surveillance systems do not cover non-doctors and psychiatrists who are not consultants. Case note review issues using CRIS included the need for researchers to interpret clinical notes, the availability and completeness of data in the notes, and data limited to the catchment of one particular mental health trust.Conclusions: Both methods demonstrate strengths and weaknesses; the combination of both methods in the absence of strong routinely collected data, allowed a more robust estimate of the level of need for service planning and commissioning.
