Abstract

Gout is a chronic urate crystal deposition disease, and must not be managed only during gouty flares which are one of the symptomatic manifestations of the disease. Gout has been defined as a progressive metabolic disease characterized by symptomatic hyperuricemia and deposition of monosodium urate (MSU) crystals in joints and soft tissues due to an imbalance in uric acid uptake, synthesis or excretion. Several transporters playing a role in reabsorption and secretion have been identified. Hyperuricemia is defined by the 6.0 mg/dL (36 micromol/dL) cutoff, used in the latest recommendations issued by the European League Against Rheumatism (EULAR). About half the 19 000 participants whose baseline urate level was above > 10.0 mg/dL did not develop gout, according to a recent sudy of cohorts. These data establish a role for factors other than the urate level in the development of gout, such as substances that inhibit or facilitate crystal formation and/or genetic and environmental factors (e.g., fructose intake) that modulate the inflammatory response to MSU deposits. The pathological process of gout involves both innate and adaptive immune responses. Gout development is associated with the presence of monosodium urate (MSU) crystals which serve as a “danger signal” affecting certain immune cells, cytokine production, and effector molecule expression, triggering an immune response. Different cell subsets, cytokines, pattern recognition receptors (PRRs) and the inflammasome have had noticeable effects on the pathogenesis of gout. The early phase of acute gouty arthritis is characterized by an auto-amplification loop of necrosis and inflammation triggered by MSU crystals. The later phase of gouty arthritis is characterized by the resolution of symptoms despite persistence of MSU crystals. Numerous immuno regulatory pathways as well as aggregated neutrophil extracellular traps contribute to this process. Chronic gout is characterized by tophis formation ; that is, masses of MSU crystals and NETs encapsulated by a foreignbody reaction involving giant cells, macrophages, and fibroblasts that contribute to the destruction of surrounding tissues., La goutte est une maladie chronique à dépôts de cristaux d’urate monosodique (UMS), et ne doit pas être prise en charge uniquement durant les épisodes d’accès aigu qui sont l’une des manifestations symptomatiques de la maladie. La goutte a été définie comme une maladie métabolique progressive caractérisée par une hyperuricémie symptomatique et des dépôts de cristaux d’UMS dans les articulations et les tissus mous en rapport avec un déséquilibre entre l’apport, la synthèse et l’excrétion des urates. Plusieurs transporteurs de réabsorption et de sécrétion des urates ont été identifiés. L’hyperuricémie est définie par un seuil de 60 mg/L (360 micromol/L), utilisé dans les dernières recommandations de l’EULAR (European League Against Rheumatism). Environ la moitié de 19 000 participants dont l’uricémie de base était supérieure à 100 mg/L n’ont pas développé de goutte selon une étude récente de cohortes. Ces données montrent l’existence de facteurs autres que l’uricémie dans la survenue de la goutte, comme les substances qui inhibent ou facilitent la formation de cristaux et/ou des facteurs génétiques ou environnementaux (comme la prise de fructose) qui modulent la réponse inflammatoire aux dépôts de cristaux d’UMS. Le processus pathologique de la goutte implique à la fois des réponses de l’immunité innée et adaptative. Le développement de la goutte est associé à la présence de cristaux d’UMS qui constituent un signal de danger, entraînant la participation de certaines cellules immunitaires, la production de cytokines, l’expression de molécules effectrices, déclenchant une réponse immune. Différents sous-types de cellules, de cytokines, de récepteurs appelés PRR (pattern recognition receptor), et l’inflammasome ont des effets notables dans la pathogénie de la goutte. La phase précoce de l’accès goutteux est caractérisée par une boucle d’auto-amplification de nécrose et d’inflammation déclenchées par les cristaux d’UMS. La phase tardive de l’accès goutteux est caractérisée par la résolution des symptômes malgré la persistance de cristaux d’UMS. De nombreuses voies de régulation du système immunitaire et les agrégats de NETs (neutrophil extracellular traps) contribuent à ce processus. La goutte chronique est caractérisée par la formation de tophis : la masse de cristaux et les agrégats de NETs sont encapsulés par une réaction à corps étranger impliquant des cellules géantes, des macrophages et des fibroblastes contribuant à la destruction des tissus environnants.
